The purpose of this clinical activity is to conduct a virtual community assessment and identify a priority health promotion intervention.  As part of this activity, you will prepare a presentation (zoom or PowerPoint) to be submitted through the assignment link and posted on the Discussion Board for peer review/discussion. If you choose to record your presentation, it should not exceed 15 minutes.

Objectives:

  1. Assess populations in the community for health status and health needs.
  2. Identify and prioritize actual and potential health problems for specified populations.
  3. Formulate healthcare plans addressing the desired population outcomes and appropriate nursing interventions.
  4. Implement theory-based nursing interventions.
  5. Evaluate the effectiveness of nursing interventions toward meeting the desired population outcomes.
  6. Apply theory and research knowledge to address identified population health concerns.
  7. Utilizes critical thinking to identify gaps in the health care of vulnerable populations and identifies potential resources to mitigate those gaps.

  1. Visit the County Health Rankings and Roadmaps website and choose the state and county where you live: http://www.countyhealthrankings.org/
  1. Create a PowerPoint presentation addressing the following assessment data as separate slides within your presentation:
  1. Identify the county where you live and the overall rank in health outcomes.
  2. List three areas where your county ranks better than the state of Texas and three areas where your county ranks worse.
  3. Describe the geographic parameters of your chosen county (Identify: rural vs. urban; prominent landmarks, major roadways, etc.).
  4. Gather and discuss primary This is data that is collected by the person conducting the assessment (you).  Unlike secondary data (discussed below), this data is information that comes directly from a source such as a community informant.  Some ideas for a place to start are the chamber of commerce, a stakeholder at the local health department, or someone else who lives in the county that could provide you with some information related to issues in the county.  If you are unable to speak with an informant on the phone, you can be the informant as someone who lives in the county and can identify yourself as the key informant.  On the slide for this information, you will need to identify the key informant you spoke with and the date of communication.
  5. Gather secondary community assessment data on the identified county.  Start with secondary data sources. Secondary data sources contain information that has already been collected by others such as the County Health Rankings website, the Census, or the local health department.  For secondary assessment data, you can use the internet.
  6. Review the United States Environmental Protection Agency Envirofacts database for potential environmental hazards through an Internet search using https://enviro.epa.gov/. Include a summary of the findings in your own words.
  7. Identify at least 3 community needs and 3 community assets.
  8. Review Healthy People 2030 and link at least 1 of your community needs and 1 of your community assets to a Healthy People 2030 objective. The objective should be identified by the topic name and number (i.e. Access to Health Services AHS-1.1) and there should be a separate objective listed for the need and the asset.
  9. Provide at least 3 recommendations for community level interventions (i.e. health fair, targeted health education, community walks, etc.) based on the assessment data collected and identified in your presentation as well as your experience living in the county.
  10. Identify at least 1 priority problem for your county. Create a slide and list your priority problem, and at least 1 goal and 1 objective (see p. 215 in the course text for information regarding the difference between a goal and an objective) to address the priority problem selected. The objective should be written in the SMART format (specific, measurable, achievable, relevant, and time-bound).  For further information about SMART objectives, please refer to the following resource from the CDC here: https://www.cdc.gov/healthyyouth/evaluation/pdf/brief3b.pdf.
    1. An example of a S-M-A-R=T objective for TB is: In County X, increase the percentage of adult patients with non-rifampin -resistant TB who complete treatment in less than 12 months (as measured by cohort review) from 80% to 90% (the national goal) by 2014.
  11. Review the literature using the online search feature through the University library. Find a research article that supports a health promotion intervention to address the priority problem you have identified.  Discuss how the health promotion intervention could be implemented within your county.  Use at least one evidenced-based reference to support your intervention.
  12. Considering vulnerable populations, discuss how your health promotion intervention impacts at least 1 vulnerable population and how your intervention would address gaps that this population may experience. Vulnerable populations groups of special concern to the nurse include: poor and homeless persons, veterans, pregnant adolescents, migrant workers and immigrants, severely mentally ill individuals, substance abusers, abused individuals and victims of violence, persons with communicable disease and those at risk, and persons who are human immunodeficiency virus-positive, have hepatitis B virus, or have a sexually transmitted disease.   13. Describe at least one method that could be used to evaluate the success of the intervention (see p. 217 of the course text for information about evaluating an intervention).
  14. Reflect upon your strengths from the Strengths Finder Assessment and findings from the community assessment and discuss how you can use your strengths to advocate for community/population health in your community.
  15. Create a reference slide and include all references used in the presentation in APA format.
