The threat to patient safety and quality outcome are associated with healthcare-associated infections (HAI) incidents. In nursing evidence-based practices are utilized to provide quality patient care and reduce unexpected outcomes. In my facility the nurses utilize the prevention bundle for the above-mentioned healthcare acquired infection or incident. The CAUTI bundle includes the indication for a foley catheter insertion, foley is no longer used for intake and output outside of the intensive care units the only indication is for urinary retention. Remove as soon as it is no longer necessary. Foley insertion is a two-nurse process to ensure sterile field is maintained, all Cather care is done with soap and water or CHG wipes once a shift or as needed. One of the leading causes of injury related death in the elderly is fall. Falls are very costly to the patient and health care system. Implementation of hourly ensures patients’ safety measures are met. To prevent patient falls, the nursing staff assess that patient’s needs are, which includes the 5 P’s potty, pain, position, personal items within reach and pumps. Evidence based practice shows that there is a reduction in the rate of falls when patients are rounded on frequently. First of all, understanding pathophysiology is vital for us to develop the critical thinking and clinical reasoning that leads to correct judgements in our nursing practices. Through this 12-weeks course of advanced pathophysiology, I have learned the physiological processes, the alterations from these processes, and the scientific concepts related these processes. I especially enjoyed learning through our case studies in weekly discussions. I believe that once I start to having clinicals, it will give me the great opportunity to apply my knowledge that I gained from this course into my clinical practice. I also expect to learn the new skills, to influence advanced nursing knowledge, and to gain experience in a new practice role as a nurse practitioner student (NPS) from the clinical practice (Phillips et al., 2020). As a nurse, I assess and provide care to my patients following the orders I received. Many factors can create a nursing shortage in a region or specialty. The first factor is the lack of professional educators in the nursing specialty (Haddad et al. 2020). Lack of educators makes nursing students avoid enrolling in nursing specialties. The shortage of nursing student graduates in different nursing specialties may be created by the universities and colleges not offering the courses due to the lack of educators. The second factor is high turnover rates in the nursing specialty (Haddad et al. 2020). One or all the other nursing specialties may be affected when there are high nursing turnovers in the healthcare organization, local, state, or national level. For instance, high turnover rates for the registered nurses in a state may lead to the healthcare organizations and facilities not meeting the nursing staffing ratio guidelines established in the region (Haddad et al. 2020). Some of the major causes of the high turnover rates may include poor nursing working conditions and personal reasons (Haddad et al. 2020). The third factor is inequitable workforce distribution. The nursing profession uses different strategies to eliminate the shortages of nurses in the affected specialties. Healthcare organizations install technology systems such as Electronic Medical Record systems to improve the working conditions for the nurses (Haddad et al. 2020). What are the sources of pediatric primary care in the United States? Are these sources sufficient for providing health-care services to the pediatric population? Why or why not?

The pediatric population is offered several different resources of primary care.  These resources include: community and school-based health centers, pediatricians, family practice/primary care clinicians, nurse practitioners, clinics, and the health department (Spencer et al, 2018).  Various programs are available to assist families in gaining access to primary care, preventative care guidelines/schedules, insurance, supplemental nutrition, health care referrals, and nutritional education.  State and federally funded programs/resources include: Medicaid, Women, Infants, and Children (WIC), Children’s Health Insurance Program (CHIP), and Bright Futures (Health Resources & Services Administration, 2021).  Despite the number of resources made available for this population, some children’s health care needs are still not being met.  The Children’s Health Fund estimates that nearly 28% of children in the United States still possess unmet healthcare needs, especially those requiring specialty care such as, pediatric endocrinology or pediatric cardiology (PNHP, 2016).  Pediatric – Week 1 Discussion 2nd REPLY

The pediatric population residing in underserved, rural regions, tend to lack access to health-care services due to a limitation of services.  Approximately 20 percent of Americans reside in rural areas, but hardly one-tenth of physicians practice in these areas (Nielsen et al., 2017).  This ratio is projected to get worse as the federal government reports there will be an estimated deficiency of over 20,000 primary care providers in rural regions (Nielsen et al., 2017).

What are the barriers to children in accessing health-care services in the United States? Why do these barriers exist? 

Sadly, many children are faced with numerous barriers in accessing health-care services in the United States.  These barriers include financial and non-financial barriers.  Financial barriers stem from high copays, high deductibles, high-priced prescription drugs, families reporting failure or difficulty to pay medical bills, and providers declining certain forms of insurance (PNHP, 2016). Non-financial barriers encompass informational and geographical barriers.  Informational and geographical barriers are due to the complexity of information on accessing healthcare and obtaining coverage, parents’ limited proficiency in speaking English, health illiteracy, lack of transportation, and lack of healthcare providers per capita in rural and low-income regions (PNHP, 2016).  These barriers can create a delay in seeking care, leading to more complex illnesses and decreased quality of life.  Pediatric – Week 1 Discussion 2nd REPLY
