Factors that influence the cost of health care include inflation, changes in population demography, technology, and intensity of services. The aging population is expected to affect health services more than any other demographic factor.

Jefferson County is rural area with several small towns scattered throughout the large county. More than half of the population are over the age of 65 and have incomes below the poverty level. You are a community health nurse specialist hired to help Jefferson County prepare for the increased need for health services.

You decide to implement a program that would open several nursing clinics operated by nurse practitioners and registered nurses. The nursing clinics will specialize in geriatric care and be located in easily accessible areas for Jefferson County residents. The nurse practitioners will provide primary care and referral services, and the registered nurses will provide health promotion education.

Consents for treatment, payment, and release of records are routine components of health care delivery of services, and nurses should be familiar with the role of these types of consent in their daily work. The information in this unit presents considerations for consents for participation in research and more detailed considerations for the protection of vulnerable populations, such as minors, inmates, pregnant women, institutionalized individuals and so on.

Review the U.S Department of Health and Human Services (DHHS) Web site regarding HIPAA and other regulations pertinent to health information privacy. Review the information for covered entities (include provider organizations and clinicians).

For this discussion, you will:

  • Discuss two key differences between consents that might be seen in your own workplace on a routine basis and the specific consent process required in research studies involving human participants.
  • Conclude your post with new information that you have earned about research protocols, protection of vulnerable populations and the protection of human subjects in research endeavors.

Evolving Practice Of Nursing And Patient Care Delivery Models

As the country focuses on the restructuring of the U.S. health care delivery system, nurses will continue to play an important role. It is expected that more and more nursing jobs will become available out in the community, and fewer will be available in acute care hospitals.

  1. Write an informal presentation (500-700 words) to educate nurses about how the practice of nursing is expected to grow and change. Include the concepts of continuity or continuum of care, accountable care organizations (ACO), medical homes, and nurse-managed health clinics.
  2. Share your presentation with nurse colleagues on your unit or department and ask them to offer their impressions of the anticipated changes to health care delivery and the new role of nurses in hospital settings, communities, clinics, and medical homes.
  3. In 800-1,000 words summarize the feedback shared by three nurse colleagues and discuss whether their impressions are consistent with what you have researched about health reform.
  4. A minimum of three scholarly references are required for this assignment.

Evaluation Project

  1. The expression “easier said than done” can describe the challenges nurses face when actually implementing an evaluation. Throughout their planning and research efforts, nurses may identify a variety of ways to evaluate the implementation and use of a new HIT system; however, without a well-stipulated evaluation methodology plan detailing how the evaluation will actually occur, meaningful results will not be obtained. The gap between thinking about an evaluation and actually applying it can be quite large. A thoughtfully developed evaluation methodology plan helps close this gap by providing a blueprint for conducting an evidence-based evaluation. The methodology elaborates on specific steps, time frames, methods and responsibilities necessary to successfully translate the plan into practice. Nurse informaticists must take the time to generate evaluation methodologies that reflect accurate research designs and that promote data collection and data analysis.

In this part of your Evaluation Project, you determine the evaluation methodology necessary for answering your PICO question. In your methodology, you identify a research design and data collection and analysis methods.

To prepare:

  • Review your PICO question. Does implementing a new unified acute and ambulatory Electronic Health Record (EHR) system within the hospital, compared to when they are not used improve the quality of healthcare for the patients through documentation?
  • Consider the goal and viewpoint you developed for your Evaluation Project. Reflect on the types of information you would need to fulfill the goal and answer the PICO question. Think about the “who,” “what,” “when,” “where,” and “how” of your evaluation. This is where you determine the specific details of your evaluation, such as the number of people involved, the time frame for conducting the evaluation, the types of information you hope to gather, and how you will obtain the data.
  • Reflect on the research design that would be most appropriate for answering the PICO question you developed. Common options include experimental designs, quasi-experimental designs, and surveys. Consider how you could effectively measure the success of your evaluation and how you would present your results.

HEALTH CARE COSTS BENEFIT

Imagine you have been asked to deliver a two-part informative workshop at a local health care expo by your Director of Community Outreach. Before he approves your workshop, you have to submit a proposal for each workshop that discusses the content you would like to present.

The first half was geared to the health consumer while this second half of the workshop is more detailed and provides health care administrators with better understanding of financial and economic tools used in the health care industry, particularly cost-benefit analysis. It is important that your audience understands possible challenges in the cost of health care services as well as what impact consumers and health care reform have on the availability of health care services. You also will discuss a specific population within health care (e.g., people with diabetes, people with mental health, etc.) and their needs.

Proposal Part ll

Write a 1,050- to 1,400-word paper in which you present the content you would like to discuss in the workshop.

Include the following in your paper:

  • Analyze the use of cost-benefit analysis in health care.
    • What cost-benefit techniques are used by providers of health care services? 
    • What impact does the mix of fixed costs vs. variable costs have on a cost-benefit analysis scenario? Is it better to have more fixed costs or less?
  • Analyze common methods and tools used to address financial and economic challenges in the health care industry.
    • What is the role of health insurance companies in managing the cost and efficiency of the delivery of health care services?
    • How do billing and reimbursement processes affect providers of health care services as well as organizations/individuals paying for the services?
  • Analyze the changes in health care services to meet the needs of an ever-changing population. For this bullet, select a population to concentrate on (e.g., people with diabetes, people with mental health etc.).
    • What are the challenges of delivering health care services to your selected population? What are the ethical implications associated with the challenges?
    • What are the new types of health care services that have evolved to support your population?
    • What impact does the increase of preventive care services have on the health of your population?
  • Analyze the drivers and barriers for health care reform.
    • Currently, what role does the government play in the delivery of health care services to consumers?
    • What are the challenges and/or problems in providing/requiring health insurance for all consumers?

PHARMACEUTICAL RESEARCH

Before developing a new drug, pharmaceutical companies research products that are currently on the market. In their research process, corporations may ask questions such as, “What are current health needs, and how could a new drug address these needs differently than drugs that are currently available?” or “What new innovations can counteract certain diseases?” Before fully refining the focus of any research, individuals should attempt to determine the current state of affairs in their area of interest.

One of the most common ways to accomplish this is to conduct a literature review. A literature review allows one to take a “snapshot” of the established knowledge in a particular area. Using this snapshot, researchers can determine the merit of their research questions and how they may need to modify their research goals. For this part of your Evaluation Project, you create a Literature Review Summary Table and synthesize the current state of research surrounding the PICO question you developed.

To prepare:

  • Review the Week 6 Learning Resources on procedures for conducting a literature review.
  • Using your approved PICO question and relevant search terms, locate a minimum of five full-text research articles to use in your literature review (Articles must be scholarly and peer-reviewed and no older than 5 years old) See Attached PDF files
  • Read through the articles carefully. Eliminate studies that are not appropriate, and add others to your list as needed. Although you may include more, you are expected to include a minimum of five appropriate articles.

CAPSTONE PROPOSAL PROJECT

An implementation plan prepares students to apply the research they have identified and vetted to the problem or issue they have identified for their Capstone Change Proposal Project. A literature review enables students to map out and move into the active planning and development stages of the Project.

In this assignment students will develop a 750-1,000-word literature review in which to synthesize what is known and unknown about the Project issue selected. Students will use the information from the PICOT Statement and Evaluation Table assignments prior in the course to develop a planned approach to implementing the evidence-based practice solution. The underlying purpose of this assignment is to analyze how the current research supports the PICOT, and to identify what is known and what is not known in the evidence, providing a logical, constructive argument.

MENTAL HEALTH

In completing the case study, students will be addressing the following learning objectives:

Identify common medical conditions that can have accompanying psychiatric complications or symptoms. Describe general nursing implications for clients experiencing psychiatric disorders that are intertwined with other nonpsychiatric illnesses.

1. Carla, a 27-year-old white single mother of two preschool-age children, is in the psychiatric hospital for treatment of anxiety and depression.   Carla has a cardiac condition that requires surgery.  Carla is concerned over the financial cost of the surgery and caretakers for her children while she is in the hospital and undergoing rehabilitation. Carla is also worried that she may be unable to continue in her present employment as a salesperson due to the physical demands of this position.  Carla’s case is representative of many patients with medical conditions who develop psychiatric symptoms.

1.      Kevin, a 39-year-old unemployed homeless male who has paranoid schizophrenia, was brought to the psychiatric hospital by the police.  Citizens called the police because Kevin was in the street directing pedestrians and traffic in opposition to the traffic lights and verbally abusing everyone who did not follow his directions.  Kevin is known to the police since he is often homeless, and states that his family does not want him.  Kevin also has a history of poly substance abuse with alcohol, heroin, and crack cocaine, and he has been jailed for public intoxication several times.  The nursing assessment reveals that Kevin has not been taking his prescribed psychotropic medications for 3 weeks.  Kevin states that he does not have any money, and he does not remember where to go for mental health care (Learning Objectives: 2)

Health Information Portability and Accountability

Chief executive officer (CEO) Beranger wants to know more details about the Health Information Portability and Accountability Act (HIPAA) as she prepares to move the health care organization (HCO) towards a culture of e-Health. She asks that you compose a report of 3–4 pages that includes the following information:

  • Describe the objectives of the HIPAA Security and Privacy Rules
  • Discuss how HIPAA attempts to safeguard protected health information (PHI)

Health Information Systems

Write a six to eight (6) page paper in which you:

  1. Provide at least three (3) various reasons why some health care organizations have been reluctant to implement electronic medical records.
  2. Discuss the essential manner in which the Health Insurance Portability and Accountability Act (HIPAA) impacts patient’s medical records.
  3. Determine the main advantages and disadvantages of the adoption of The HITECH (the Health Information Technology for Economic and Clinical Health) Act for health care professionals. Next, suggest one (1) strategy that the medical staff members could use in order to mitigate the main disadvantages in question.
  4. Review the typical workflow processes within health organizations, and decide the single most significant process that the health organization must eliminate in order to improve the service. Provide a rationale to support your response.
  5. Analyze the primary ways in which the key federal initiatives impact the standards of health care information for patient privacy, safety, and confidentially.
  6. Specify the fundamental advantages of applying an IT system within health care organizations. Predict new IT developments in the health care industry for the next two (2) decades. Justify your response.
  7. Use at least three (3) quality academic resources in this assignment. Note: Wikipedia and similar type Websites do not qualify as academic resources.
