Grading Criteria for Clinical Case Decisions

CLINICAL CASE DECISION INSTRUCTIONS

Grading Criteria for Clinical Case Decisions:

The highest potential points for each Clinical Case Decision Question are 15-points.  Students are required to upload their response as a word document attachment to the assignment drop box.

Each uploaded response is worth a possible 15 points.

  1. Your comments should be substantiated and substantive. Postings require two citations from 1 scholarly journal and/or 1 outside textbook. Textbooks assigned to this course may be used for an additional citation only.
  2. Each of the two references must be from a different reference source.
  3. To gain full credit for the assignment, the response must be a full page, but no more than three (3) pages in length, which includes the reference list.
  4. Students are required to use APA format, proper citations, and references.  Students using direct quotes from referenced sources in the body of the paper must include quotations.
  5. Students will be assigned to a question.

Clinical Decision Cases

Begin with a Head-Toe assessment, you decide if it should be treated as episodic with rationale.

You have been assigned two (2) cases to provide analysis. All cases should include the following

a. Pathophysiology and pharmacology of the disease

b. Expected signs and symptoms of the disease.

c. Nursing Diagnosis with a plan of care.

C3. Mrs. Muller is a 78-year-old homemaker who arrives in the emergency department with a history of chest discomfort and indigestion two days ago that lasted about 12 hours. She was severely fatigued after this and within the past 2 hours has become increasingly short of breath. Her initial ECG shows that she recently experienced an anterior AMI. Her skin is cold, and she is very diaphoretic and cyanotic. She is diagnosed with acute heart failure and cardiogenic shock.

a. What will be the intent of her management, and what will be included in her management plan?

C4. John lacerates his radial artery while cutting a bagel. He loses approximately one-fourth of his blood volume before arriving at the hospital.

a. What physiological responses do you expect in response to this blood loss?

Evaluating Patients

Instructions

It is anticipated that the initial discussion response should be in the range of 250-300 words. Response posts must demonstrate topic knowledge and scholarly engagement with peers.

This is not the only criteria utilized for evaluation; substantive content is imperative. All questions in the topic must be addressed. Please proofread your response carefully for grammar and spelling.

Do not upload any attachments.

All responses need to be supported by a minimum of one scholarly resource. Journals and websites must be cited appropriately. Citation and reference must adhere to APA format (6th Ed.).

Classroom Participation

Students are expected to initially address the discussion question by Wednesday of each week. Participation in the discussion forums is expected with a minimum total of three (3) substantive postings (this includes your initial posting and posting to two peers) on three (3) different days per week. Substantive means that you add something new to the discussion, you aren’t just agreeing.

This is also a time to ask questions or offer information surrounding the topic addressed by your peers. Personal experience is appropriate for a substantive discussion and should be correlated to the literature.

All discussion boards will be evaluated utilizing rubric criterion inclusive of content, analysis, collaboration, writing and APA. If you fail to post an initial discussion you will not receive these points, you may however post to your peers for partial credit following the guidelines above.

Due to the nature of this type of assignment and the need for timely responses for initial posts and posting to peers, the Make-Up Coursework Policy (effective July 2017) does not apply to Discussion Board Participation.

Discussion Prompt [Due Wednesday]

Case study:

Bana is a 24-year-old Muslim woman who was recently relocated to the United States as a refugee from Syria, along with her husband and three young children.

She arrives at your office today for a physical with her children and a case manager. Her English is limited, and she relies on her case manager to explain the details of her situation, although the case manager does not speak Arabic and thus cannot interpret for her.

In interviewing Bana with the help of the case manager, you learn that her husband is currently looking for a job and that she works at night cleaning office buildings.

Her last menstrual period was about 8 weeks ago, and she has been experiencing nausea and vomiting in the mornings recently. A pregnancy test proves positive, and you provide some prenatal education to the client.

During the physical, you notice recent bruises around her throat. She is embarrassed by them and indicates that she was assaulted in Syria before flying to the United States several weeks ago.

On questioning further about the assault, Bana appears not to understand and looks at her case manager for help.

  1. What is the biggest barrier you face in providing care to Bana at this point?
  2. What is the priority action for you to take at this time?
  3. What action is most important for you to take to ensure the health and safety of the fetus?
  4. Given Bana’s financial situation, you are concerned about her and her family having adequate nutrition. Which intervention would best address this concern?

Online discussions are intended to be thoughtful exchanges among students that focus on the weekly readings and translating theory to practice. Initial postings are expected to be completed by Wednesday of each week. Response posts are due by Sunday of each week. Minimum expectations are to complete the initial posting and provide a substantive response to the initial posting of one other classmate who posted to a different question. Students earn points weekly for posts which meet (or exceed) minimum posting requirements. Points are deducted daily for late initial posts. Late response posts will not be accepted.

Estimated time to complete: 3 hours

Quantitative Research

Please read carefully instructions. This is a master level writing. See attached the chapters of the book and article to do the  Summary of a Research Report worksheet and data analysis.

When reviewing a research study, you must have the knowledge and skills to accurately read and interpret all parts of a research study. The goal for this assignment is for you to read the entire article, and accurately identify and describe the various sections of the study report. Use the information in chapters from prior weeks as well as this week to help you accurately summarize the study.

Select one quantitative research article from this week’s Electronic Reserve Readings or an article from Appendix A-H of Resource Manual for Nursing Research.

-Complete the Summary of a Research Report worksheet. Fill out the form in its entirety and include the following details in the appropriate sections of the form:

-Identify the type of design that is used in the study.

Using Table 9.4 and Table 9.5 of Nursing Research locate the type of design described in the article.

-Compare the design description in the study to the one in the tables and determine if there is any difference between the two.

-Describe the data analysis to include specific statistical tests and the reason for selecting the specific tests for the study. If there is a test of a hypothesis, what are the results of the data analysis?

Review Box 10.1: Guidelines for Critiquing Design Elements and Study Validity in Quantitative Studies on Nursing Research.

-Develop a data analysis. If there is a test of a hypothesis, what are the results of the data analysis? Do not simply identify a few words by the author(s) referring to the major findings under discussion or conclusions.

Include a full reference for the article you selected using APA guidelines.

Click the Assignment Files tab to submit your Summary of a Research Report worksheet and your data analysis.

Differing Approaches of Nursing Leaders and Managers

DETERMINANTS OF HEALTH AND PUBLIC HEALTH INTERVENTION WHEEL

This week’s graded topics relate to the following Course Outcome (CO).

CO1: Apply principles of nursing theory to the public health system by analyzing determinants of health and the public health intervention wheel. (PO1)

What are some factors that make some people healthy and others ill? Healthy People 2020 identifies five determinants of health that influence the health of individuals and populations. Healthy People 2020 describes them as “a range of personal, social, economic, and environmental factors that influence health status” (U.S. Department of Health and Human Services, 2018, About Determinants of Health, para. 1). Determinants fall into five categories, including (a) policy making, (b) social factors, (c) health services, (d) individual behavior, and (e) biology and genetics.

Go to the Determinants of Health link at https://www.healthypeople.gov/2020/about/foundation-health-measures/Determinants-of-Health (Links to an external site.) Links to an external site., and click on each determinant to read about it.

Choose one of the five determinants other than individual behavior (as we are all aware of how individual behavior impacts health). In your post, describe this determinant and its importance.

Discuss how a community health nurse (CHN) could use one of the 17 intervention categories from the Public Health Intervention Wheel to address this determinant (Nies & McEwen, 2015).
