Case Study: Fetal Abnormality

Jessica is a 30-year-old immigrant from Mexico City. She and her husband Marco have been in the U.S. for the last three years and have finally earned enough money to move out of their Aunt Maria’s home and into an apartment of their own. They are both hard workers. Jessica works 50 hours a week at a local restaurant and Marco has been contracting side jobs in construction. Six months before their move to an apartment, Jessica finds out she is pregnant.

Four months later, Jessica and Marco arrive at the county hospital, a large, public, nonteaching hospital. A preliminary ultrasound indicates a possible abnormality with the fetus. Further scans are conducted and it is determined that the fetus has a rare condition in which it has not developed any arms, and will not likely develop them. There is also a 25% chance that the fetus may have Down syndrome.

Dr. Wilson, the primary attending physician, is seeing Jessica for the first time, since she and Marco did not receive earlier prenatal care over concerns about finances. Marco insists that Dr. Wilson refrain from telling Jessica the scan results, assuring him that he will tell his wife himself when she is emotionally ready for the news. While Marco and Dr. Wilson are talking in another room, Aunt Maria walks into the room with a distressed look on her face. She can tell that something is wrong and inquires of Dr. Wilson. After hearing of the diagnosis, she walks out of the room wailing loudly and praying aloud.

Marco and Dr. Wilson continue their discussion, and Dr. Wilson insists that he has an obligation to Jessica as his patient and that she has a right to know the diagnosis of the fetus. He furthermore is intent on discussing all relevant factors and options regarding the next step, including abortion. Marco insists on taking some time to think of how to break the news to Jessica, but Dr. Wilson, frustrated with the direction of the conversation, informs the husband that such a choice is not his to make. Dr. Wilson proceeds back across the hall, where he walks in on Aunt Maria awkwardly praying with Jessica and phoning the priest. At that point, Dr. Wilson gently but briefly informs Jessica of the diagnosis, and lays out the option for abortion as a responsible medical alternative, given the quality of life such a child would have. Jessica looks at him and struggles to hold back her tears.

Jessica is torn between her hopes of a better socioeconomic position and increased independence, along with her conviction that all life is sacred. Marco will support Jessica in whatever decision she makes, but is finding it difficult not to view the pregnancy and the prospects of a disabled child as a burden and a barrier to their economic security and plans. Dr. Wilson lays out all of the options but clearly makes his view known that abortion is “scientifically” and medically a wise choice in this situation. Weight management is based on the relationship between the number of calories consumed (calories in) and the number of calories burned (calories out). On any given day, one of three things can happen:

Weight management is accomplished by the deliberate application of one of these situations over a period of time. For example, if your goal is to lose weight, you will need to consistently take in fewer calories than you use. Your weight management strategy is the actual steps you intend to take to accomplish your personal goals.

Each year, 43% of American women and 28% of American men try to lose weight (Serdula et al., 1999). One of the Healthy People 2020’s goals is to reduce the obesity population to 31%, but the rate is rising instead of falling. Obesity is a worldwide problem, with 65% of the world’s population living in countries where obesity-related health problems kill more people than underweight health issues (World Health Organization, 2011). Despite the rise in obesity, many people strive for unrealistic thinness in response to pressure from peers and a society obsessed with appearance.

Just as excessive body fat can be unhealthy, too little body fat can also compromise health. The minimum level of body fat needed to maintain good health varies among individuals and depends on gender and genetics. Infertility, depression, and early death are among the possible outcomes of excessive, and unsafe, weight loss.

At the beginning of my career as a nurse, I started working as a licensed practical nurse (LPN) at a nursing home. I used to work at least 72 hours a week with the same residents. Initially, when I witnessed death at work, it was very difficult for me because I had become attached to residents emotionally. I used to interact with the residents almost every day of my life, and I treated them like family. Later, after I had experienced many deaths, I had learned to be more compassionate, able to accept and tolerate delays without becoming anxious or frustrated, and I made sure I was providing the best care of the residents during the last stages of their lives. I have decided to make sure they have the respect, compassion, and dignity that they deserve until their last breath. I make sure that they are comfortable, clean, and dry. The most important thing is that they are not suffering.  I have held their hands after my shift and stayed at the bedside for the patient and the family. I understand now that it is not about me, it is about them. She had a similar episode about eight months ago for which she did not seek care because of lack of health insurance. She is currently sexually active with one partner and has had two partners in the past year. She is not using any type of contraception. On physical exam you note a temperature of 38º C, a regular pulse of 100, and a BP of 110/65. Her abdomen is diffusely tender in both lower quadrants. Week 1: Topic Research/Selection and Literature Review

Part 1: Topic Research and Selection

Begin this process by researching what health care organizations are doing or attempting to do to increase profitability. Remember, profitability can be improved from many different angles. A nonexclusive list of potential ways would be adding additional services, decreasing costs, increasing the amount of services that are provided, or implementing a quality improvement program that qualifies for incentive monies. Check trade journals or professional discussion boards, or reach out to existing health care managers.

Part 2: Literature Review

Perform a literature review of the solution that you have decided to apply to your organization. The review will encompass several articles, and at least 1 of the articles must be peer-reviewed. Look at current material (within the last 2 years). Remember, a literature review includes a summary of the information that you found that is relevant to your topic as well as an APA reference for each resource that you reviewed.

Identifying what topic to perform research on should be accomplished quickly because you will need sufficient time to perform your literature review. Review a minimum of 10 documents.
