During the last 5 weeks, you have explored various body systems: neurological, cardiovascular, respiratory, and hematological. These four systems work together along with other body systems to complete a myriad of functions. For this reason, when disorders occur within one body system, it can create potentially devastating effects throughout the entire body. For instance, Parkinson’s disease is a disorder of the central nervous system, yet its alterations actually affect multiple body systems from the cardiovascular system to the gastrointestinal system. In this Assignment, you examine alterations associated with disorders, as well as the impact of the alterations on multiple body systems.

To Prepare

· Reflect on how patient factors such as genetics, gender, ethnicity, age, and behavior might impact the pathophysiology of the alterations you identified, as well as the diagnosis and treatment of your selected disorder. 

· Construct a mind map for the disorder you selected. Include the epidemiology, pathophysiology of alterations, risk factors, and clinical presentation, as well as the diagnosis and treatment of the disorder.

Will is a 68-year-old male with a history of hypertension. Eight months ago, he started regular dialysis therapy for ESRD. Before that, his physician was closely monitoring his condition because he had polyuria and nocturia. Soon it became difficult to manage his hypertension. He also lost his appetite, became weak, easily fatigued, and had edema around his ankles. Will debated with his physician about starting dialysis, but she insisted, before the signs and symptoms of uremia increased, the treatment was absolutely necessary.

  1. What is the difference between azotemia and uremia?
  2. Two years ago, Will’s physician told him to decrease his protein intake. In spite of what the physician ordered, Will could not stop having chicken, beef, pork, or eggs at least once a day. Why did his physician warn him about his diet?
  3. Will’s feelings of weakness and fatigue are symptoms of anemia. Why is he anemic?

Policy And State Boards Of Nursing

Within the far-reaching and multi-layered realm of policy and reform, government at the state level plays an essential role. Consider the federally enacted PPACA’s individual mandate which sought to increase the number of consumers who receive insurance coverage and, therefore, greater access to care. In a system that is already stretched beyond capacity and confronting a nursing shortage, how can the health care system meet this increased demand? Since state boards of nursing determine scope of practice, it is important to stay up to date and current with the policies and regulations that are created by the state board of nursing.

Creating a Welcoming Workplace for the Older Worker

The nursing workforce is aging rapidly. In fact, Baby Boomers constitute the largest group of nurses currently practicing. This aging workforce poses two significant ramifications for nurse managers. First, due to the volatility and uncertainty in the world and national economy, many Baby Boomers have shifted their retirement plans and are staying in the workforce. This is requiring creative responses on the part of employers. Conversely, as these highly experienced and knowledgeable nurses do begin to exit the active workforce, experts anticipate a “brain drain” that will affect all levels of health care environments.

In this week’s Assignment, you critically assess your current organization or one with which you are familiar to determine why older workers may stay in or leave their positions. In addition, you suggest strategies that could help to retain and further engage this specific population of nurses.

To prepare

  • Review the article, “From Veterans to Nexters: Managing the Multi-Generational Nursing Workforce.” Reflect upon the key ideas: the challenges and implications of managing a multi-generational workforce, common generational differences, interventions for increasing generational motivation, and strategies for retaining the older nurse.
  • Reflect on your current organization or one with which you are familiar and determine the demographic breakdown of employees based on age. Then, consider how this specific work environment might present difficulties for older workers and think about factors that might cause these employees to leave your setting.
  • Use this week’s Learning Resources, as well as your own scholarly research, to identify strategies that could engage and retain this population of nurses.

Submit a 3- to 5-page paper in which you do the following:

  • Identify the demographic breakdown of employees in the organization. If you are unable to access exact demographical data, provide your best estimate for each of the following age ranges: 20–30, 30–40, 40–50, 50–60, and 60–70, and 70+.

Advocating Through Policy

As noted by Dr. Stanley and Dr. White in this week’s media presentation, professional nurses should be engaging in advocacy efforts to improve health and nursing practice through involvement in the policy process at the institutional, local, state, or federal levels. This array of possibilities for involvement provides opportunities for all nurses, regardless of time, or other possible constraints. Successful policy making is a collaborative effort, and one that commands mutual respect from all involved. Your involvement in policy making can lead to expanded opportunities as both a nurse leader and as a respected member of an interprofessional health care team.

Note: This Discussion provides a forum for discussing advocacy opportunities and honing your presentation skills in a small group setting.

To prepare:

  • Reflect      on the insights offered by Dr. Stanley and Dr. White on engaging in      advocacy through the policy process.
  • Identify      a practice issue that is of interest to you and that could benefit from      advocacy efforts through the policy process.
  • Consider      the stakeholders and any special interest or professional organizations      that would support your issue.
  • Develop a short, yet      persuasive PowerPoint (up to 3 slides IN APA FORMAT) as follows:

The authors studied the dissemination of information on the Patient Safety and Quality Improvement Act (PSQIA), a federal act that affords protection to those reporting medical errors. They found medical literature to be inadequate in this regard, and as a result, medical personnel were uninformed on their legal protections. This lack of information has become a barrier to policy implementation.

The authors study the compliance to renal-care policies by health care professionals. They conclude with the necessity for nurses to support evidence-based protocols as well as to obtain continuing education on new protocols.

McCracken, A. (2010). Advocacy: It is time to be the change. Journal of Gerontological Nursing, 36(3), 15-17. 

The author proposes that nurses, as patient advocates, need to be more involved in the making of health care policy instead of reacting to policies that are constantly changing. The article provides a guide to help organize initial policy efforts.

Nannini, A., & Houde, S. C. (2010). Translating evidence from systematic reviews for policy makers. Journal of Gerontological Nursing, 36(6), 22–26.

The article cites geronotological nurses as examples of those who are able to translate research into policy briefs that can be clearly understood by policy makers. Geronotological nurses are in this unique position because of their clinical experience and educational background.

Paterson, B. L., Duffet-Leger, L., & Cuttenden, K. (2009). Contextual factors influencing the evolution of nurses’ roles in a primary health care clinic. Public Health Nursing, 26(5), 421-429.

This article provides details on a study conducted in a nurse-managed clinic related to the changing roles of nurses. The authors found that nurses, in response to social, political, and economic forces, became involved in advocacy for the clinic through political action, government funding issues, and media relations roles.

Sistrom, M. (2010). Oregon’s Senate bill 560: Practical policy lessons for nurse advocates. Policy, Politics, & Nursing Practice, 11(1), 29-35. doi: 10.1177/1527154410370786

The author uses the efforts by a nurse advocate in lobbying for an Oregon bill related to healthy food in public schools to illustrate nurse advocacy and policy making. The bill, developed in response to childhood obesity, did not immediately become law. The author concludes with the importance of considering the political environment when creating successful policy.

Nurses have always been advocates at the patient-level of care, but the authors of this article promote the need for all nurses to become advocates at the policy level as well. They discuss factors that have kept nurses from getting involved with policy making and they provide strategies to resolve these challenges.

Wyatt, E. (2009). Health policy advocacy: Oncology nurses make a difference. ONS Connect, 24(10), 12-15. 

The author presents information on two nurses who have become health care policy advocates—one as a policy maker and one as an elected legislator. Both have been able to use their perspectives from their nursing careers to affect health policy.

Zomorodi, M., & Foley, B. J. (2009). The nature of advocacy vs. paternalism in nursing: Clarifying the ‘thin line.’ Journal of Advanced Nursing, 65(8), 1746-1752.

The authors attempt to distinguish the concepts of advocating for a patient and paternalism, or overriding a patient’s wishes. They provide clinical examples to illustrate the differences between these concepts, and they conclude with strategies to use in practice.

Social Technologies In The Workplace

As a nurse manager, it is imperative to understand how social technologies can impact productivity, peer-to-peer relationships, and patient safety within the workplace. Collaborating with HR to understand policies and the organization’s expectations related to the use of social technologies allows nurse managers to more effectively and appropriately integrate today’s social platforms while mitigating the occurrence of inappropriate behaviors.

For this week’s Discussion, your Instructor will assign you to one of the scenarios below. You will then use that scenario to investigate the social, ethical, and legal ramifications of social technologies.

Scenario One: You have recently been promoted to charge nurse for the day shift in your department. As a result of this promotion, your former peers are now reporting directly to you. You have been working in your setting for five years and consider many of these peers to be your friends. The way that one of your friends posts on social media sites has always bothered you. Many times, her comments are inappropriate, discussing her negative feelings about “ignorant superiors” and “annoying patients.” You also know that she frequently accesses these sites for extended periods of time while patients are waiting to be helped. Now that she reports directly to you, how do you address this? If your workplace does not have a formal policy on social media use, how can you adapt her behaviors to align with the expectations of nursing professionalism?

 Post an explanation of the possible social, ethical, and/or legal ramifications of your assigned scenario. Explain the policy your current or past workplace has on the use of social technologies and how effective you believe the policy is. If your workplace does not have a policy, explain what the accepted practices or expectations are for your setting. Has lack of policy led to any problems? Topic 1: Community-Based Nursing

Nursing services play an important role in caring for patients and families in the community and providing them the support they need. For patients with problems ranging from chronic health conditions to need of elder care, nursing has proved itself an important part of the healthcare team. In fact, many people now prefer sending their elderly parents to a daycare for nursing, rather than to a long-term care facility.

For this topic, interview a community-based nurse or nurse practitioner who sees individuals in the home setting. Ask the following questions:

  • What are your job responsibilities?
  • Does a correlation exist between your job description and your practice?
  • Are you involved in policy formulation that affects client services?
  • Do you belong to a professional organization related to your current job position?

Topic 2: Culturally Competent Care

A broad range of cultures exists in the world today. Nursing professionals often interact with people from cultural backgrounds that differ from their own.

  • Share an experience where you cared for a patient in the healthcare setting who was from a culture different from your own. How did you accommodate this patient’s needs? How would you apply the Purnell Model to future cultural encounters?
  • Which one of the twelve domains of culture from the Purnell Model do you feel is most important? Why?

Conflict Resolution Paper Guidelines

Purpose

The purpose of this assignment is to learn how to identify and effectively manage conflicts that arise in care delivery settings resulting in better management of patient care, including appropriate delegation. You will gain insight into conflict management strategies and develop a plan to collaborate with a potential nurse leader about the conflict and its impact in a practice setting.

  1. Read Finkelman (2016), Chapter 13: Improving Teamwork: Collaboration, Coordination, and Conflict Resolution, section on Negotiation and Conflict Resolution, pp. 324-333.
  2. Observe nurses in a care delivery setting. Identify a recurring conflict with the potential to negatively impact patient care. Decide if delegation was an issue in the conflict. This should be from your practice setting or prelicensure experiences.
  3. Provide details of what happened, including who was involved, what was said, where it occurred, and what was the outcome that led you to decide the conflict was unresolved.
  4. Identify the type of conflict. Explain your rationale for selecting this type.
  5. Outline the four stages of conflict, as described in our text, and how they relate to your example.
  6. Propose strategies to resolve the conflict. Search scholarly sources in the library and the Internet for evidence on what may be effective.
