Overview

Create a 3–5-page draft for a strategic plan for your selected organization using a TOWS matrix and portfolio analysis in addition to any other appropriate tool of your choosing.

Note: The assessments in this course are presented in sequence and must be completed in order.

The TOWS Matrix is a decision-making process for developing strategic alternatives. You will be able to better develop a more effective strategic plan based upon an in-depth understand of the current environment.

Note: Assessment 1, Current Environmental Analysis, must be completed before beginning this assessment. You will be revisiting and revising strategies throughout each of the assessments in this course. Each assessment builds on your previous work.

Step One: Critical Thinking Skills Practice

The TOWS Matrix is a decision-making process for developing strategic alternatives. Your ability to think critically is key to organizing your thoughts throughout the strategic planning process.

In preparation for this assessment, you are required to view the Riverbend City: Ruby Lake Evacuation Mission simulation (linked in the Resources). This activity is an ideal setting to practice thoughtful application of critical thinking skills. This media piece addresses multiple competencies, some of which are difficult to practice and assess without a real world or a simulated setting. As you begin your TOWS matrix, keep the following categories of critical thought in mind as you consider the threats, opportunities, strengths, and weaknesses of the Ruby Lake Evacuation scenario.

•Assumptions: What assumptions are being taking for granted by the people in this simulation? What alternative assumptions could be useful in this situation?

•Information: What information did people have and what kind of information were they missing?

•Fairness: How was intellectual and intercultural fairness demonstrated?

•Criteria for Evaluation: What criteria would you use to evaluate the quality of the information, implementation and/or effectiveness?

Complete a practice TOWS Matrix for this scenario before proceeding to Step Two. This practice matrix does not need to be submitted.

Step Two: Complete a TOWS Matrix

Apply the same critical thought process to a real world setting that is relevant for your selected health care organization’s future success. In addition to completing a TOWS matrix, you may use any other appropriate analysis tools to determine alternative strategies.

Include the following as you develop alternative strategies:

•Incorporate the use of new methodologies from the knowledge-economy management approach that might be appropriate for this organization.

•Correlate recent research findings regarding strategy management into your approach.

•Synthesize operational and financial components in your strategic planning process.

•Incorporate your findings from your current environment analysis.

•Determine the organizational structure that best facilitates the strategy—functional, divisional, or matrix.

Step Three: Draft Strategic Plan

Re-examine your work from Assessment 1 and create a high level draft of a strategic plan that incorporates insights from your environmental analysis and the TOWS matrix exercise.The goal is to focus upon application of the concepts and new insight. Keep it brief and substantive.

Preface your draft with the core assumptions regarding environmental opportunities and threats, organizational strengths and weaknesses, and organizational behavior dynamics that are foundational to the strategy.

Include a brief description of the proposed strategy that reflects the choices made regarding:

•Service/product domain.

•Market domain.

•Level of investment in the respective service/product and market domains.

•Positioning approach for achieving competitive advantage.

•Maneuvers that will be used to implement the positioning approach.

•Distinctive competencies or assets that will be relied on to gain and sustain competitive advantage.

Additional Requirements

Submit your selected health organization’s TOWS Matrix from Step Two, with the high-level components of a strategic plan, as an attachment to this assessment.

Deliverable: Develop an evidence-based plan for health care delivery.

Scenario:

The hospital where you work has an issue with increased readmissions within 30 days of discharge. After examining the core measures, it was found that heart failure was the most common core measure disease process experiencing the highest rate of readmissions. The leadership team has given your team the charge of developing a nurse-run outpatient heart failure clinic. The purpose of this clinic is to ensure that discharge education is presented to the patient in an orderly, consistent manner and complies with evidence-based practice protocols. Since these patients may be discharged from a variety of areas in the facility, having the heart failure clinic staff take ownership of the process will improve both consistency and compliance. There are cardiologists that interact with the staff and patients, but the day-to-day operations of the clinic are designed and supported by the nurses as they interact with appropriate members of the other health care team disciplines promoting the best care for the heart failure patients.

As a member of the nurse team, you have been asked to develop one component of the clinic.

The hospital leadership established these objectives for the clinic services:

· Evaluate and maximize proper medication therapy.

· Conduct regular diet, exercise, and stress management classes for the patients.

· Monitor physiological indicators for the patients (lab work, weights, vital signs, ECGs).

· Provide a case management system for patients in the program post-discharge.

The overall goals for the heart failure clinic are to:

· Enroll greater than 90 percent of the patients with a primary or secondary diagnosis of HF prior to discharge.

· Facilitate discharge planning to achieve 100 percent compliance with patient education prior to discharge (discharge planning).

· Decrease readmission rates in this population by 5 percent over the next year.

The leadership team has asked you to provide them with an evidence-based plan for one of the components of the clinic. You may use any combination of documents (for example, a spreadsheet or a table) in addition to explanatory information to convey information clearly and succinctly.

For this assessment, you will select and research a current health care problem or issue faced by a health care organization. Read each portion of the assessment carefully and use the Suggested Resources to help you complete the assessment. This assessment provides an opportunity to apply research skills to a current health care problem or issue.

For this assessment, you will research best practices related to a current health care problem. Your selected problem or issue will be utilized again in Assessment 3. To explore your chosen topic, you should use the first two steps of the Socratic Problem-Solving Approach as described on Campus to aid your critical thinking.

1. View the Assessment Topic Areas media piece linked in the Resources and select one of the health care problems or issues in the media piece to research. Write a brief overview of the selected topic. In your overview: 

. Describe the health care problem or issue.

. Describe your interest in the topic.

. Describe any professional experience you have with this topic.

· Conduct a search for scholarly or academic peer-reviewed literature related to the topic and describe the criteria you used to search for articles, including the names of the databases you used. 

. You will want to access the applicable Undergraduate Library Research Guide related to your degree (found at the NHS Learner Success Lab) for tips to help you in your search.

. Use keywords related to the health care problem or issue you are researching to select relevant articles.

· Assess the credibility of the information sources you find. 

. Determine if the source is from an academic peer-reviewed journal.

. Determine if the publication is current.

. Determine if information in the academic peer-reviewed journal article is still relevant.

· Select four current scholarly or academic peer-reviewed journal articles published during the past three to five years that relate to your topic.

· Explain the relevance of the information sources. 

. Describe how the health care problem or issue is addressed in each source.

. Discuss what kind of contribution each source provides on your selected topic.

· Analyze the scholarly literature or academic peer-reviewed journal articles using the annotated bibliography organizational format. 

. The purpose of an annotated bibliography is to document a list of references along with key information about each one. The detail about the reference is the annotation. Developing this annotated bibliography will create a foundation of knowledge about the selected topic.

. List the full reference for the source in APA format (author, date, title, publisher, et cetera) and use APA format for the annotated bibliography.

. Make sure the references are listed in alphabetical order, are double-spaced, and use hanging indents.

. Follow the reference with the annotation.

· In your annotation: 

. Identify the purpose of the article.

. Summarize the source: 

. What are the main arguments?

. What topics are covered?

· Include the conclusions and findings of the article.

· Write your annotation in a paragraph form. The annotation should be approximately 150 words (1 to 3 paragraphs) in length.

· In a separate paragraph or two at the end of the paper, summarize what you learned from your research. 

· List the main points you learned about.

· Summarize the main contributions of the sources you chose and how they enhanced your knowledge about the topic.

Example Assessment: You may use the assessment example, linked in the Assessment Example section of the Resources, to give you an idea of what a Proficient or higher rating on the scoring guide would look like.

Additional Requirements

1. Length: At least 4 typed, double-spaced pages, not including the title page and reference page.

CONTEXT

Information plays a fundamental role in health care. Providers such as physicians and hospitals create and process information as they deliver care to patients. However, managing that information and using it productively poses an ongoing challenge, particularly in light of the complexity of the U.S. health care sector, with its many diverse settings for care and types of providers and services. HIT has the potential to considerably increase the productivity of the health sector by assisting providers in managing information. Furthermore, HIT can improve the quality of health care and, ultimately, the outcomes of that care for patients.

Preparation

As the senior care coordinator in your organization, your manager has asked you to examine and report on how care coordination can generate cost savings, improve outcomes, enhance the collection of evidence-based data, and improve health care quality. She would like you to compile cost savings data in a well-organized spreadsheet and present your key findings in an executive summary.

Requirements

Determine how care coordination can reduce costs. Compile your cost savings data in a spreadsheet, using Microsoft Excel or suitable application of your choice. Your spreadsheet should contain at least four cost-saving elements that you will discuss in your executive summary. Identify the cost saving element, current costs, and anticipated savings.

If you elect to use an application other than Excel, check with your instructor to avoid potential file compatibility issues. Report key findings from your analysis in an executive summary.

Executive Summary Format and Length

Format your executive summary using APA style.

· Use the APA Style Paper Template, linked in the Required Resources. An APA Style Paper Tutorial is also provided (linked in the Suggested Resources) to help you in writing and formatting your executive summary. Be sure to include:

Discussion: Developing a Balanced Scorecard

How does a group gauge whether the strategic changes they implement are creating sound business results?

The effective use of a balanced scorecard can facilitate alignment between strategic initiatives and day-to-day operations. The balanced scorecard may serve as a valuable tool for monitoring progress and assessing the outcomes of a strategic plan and provide a practical means of communication.

In this Discussion, you examine the use of a balanced scorecard to measure performance related to finances, customer/client satisfaction, internal processes, and learning and growth.

To prepare:

Review the information in the Learning Resources, including this week’s media presentation, focusing on the development of performance metrics for a strategic plan. See Figure 8-6: Design your Balanced Scorecard in the Sare and Ogilvie course text.

With your Course Project in mind, think about how you would develop a balanced scorecard to measure performance, including the processes you would use to gather appropriate data.

Consider how the use of a balanced scorecard reflects the importance of being able to evaluate progress, assess the impact of planned change on performance, and determine if the costs are justified in terms of the outcomes.

2.     Select your interviewee and schedule an interview. This individual must be a Registered Nurse. Job titles of RNs who may be considered include, but are not limited to, Nursing Clinical Information Manager; Super User, Director/Manger Clinical Education; Chief Information Officer; Quality Assurance or Performance Improvement Nurse; Nurse Informaticist; Telenursing Specialist; Nurse Abstractor; Case Manager; Compliance Nurse. If you have any concerns about whether the RN is suitable for this assignment, contact your instructor BEFORE you schedule the interview.

3.     Review all questions (areas of inquiry) on the Interview Form located in Course Resources PRIOR to conducting the interview. You may print the form and take it with you to the interview.

4.     Note there are five required questions to ask the RN.

5.     Note there are four optional questions. You need to select only one of these to ask the RN.

6.     Note there are two follow-up questions that must be answered by you.

Prior to conducting your interview, review two scholarly resources. These resources should guide your understanding of the RN’s role and responsibilities or make you more knowledgeable about GIGO, Interprofessional Communication, or other key concepts in the questions that you may not fully understand. For example, if your interviewee is a telenurse, you would want to review information on this specialty. If you do not know what GIGO means, look it up.

7.     Conduct your interview. The length of your interview will vary but should not exceed one hour.
