Caring for a family with a child who has significant developmental delays

The community health RN is caring for a family with a child who has significant developmental delays. The child is 9-years-old and exhibits the development of a 6-month old infant.  She can move her extremities spontaneously, hold her head up and cry out occasionally. She has a gastrostomy tube for her medications and she receives continuous tube feeding via pump.  She was discharged 2 days ago after a 5-day hospitalization for failure to thrive.  During the hospital stay, the child’s tube feeding formula was adjusted to meet her growing needs. The community health RN is monitoring the child after discharge, following up on the child’s weight and the parent’s knowledge of the new feeding formula type, amount, and schedule. Today the child weighs 64 pounds. The child’s current weight represents a 2 pound weight gain since hospital admission.

The RN has chosen the NANDA-I nursing diagnosis of Ineffective health management r/t insufficient knowledge of expected growth and calorie requirements AEB parent states, “I thought the same tube feeding would be enough calories for a long time, I don’t know how to tell if the feeding should be adjusted.”

Initial Discussion Post:

  • Compare and contrast narrative documentation with SOAP/SOAPIE/SOAPIER- style documentation.
  • Create a documentation entry to describe the community health nurse’s first visit based on this scenario using SOAP/ SOAPIE/SOAPIER-style documentation. Students may embellish the scenario with additional patient information or parent’s response to teaching if desired.

Important means by which health care providers strive to improve quality of care

Comparing, or benchmarking, outcomes is an important means by which health care providers strive to improve quality of care. Although this concept applies to all kinds of clinical care facilities, for this Discussion you focus on hospitals so that you can make use of data on the CMS’s “Hospital Compare” website to conduct some basic benchmarking analysis.Using this analysis as a launching point, you then evaluate priorities for improvement and consider how to create a motivating environment that supports significant, lasting change.

To prepare:

  • Go to http://www.medicare.gov/hospitalcompare/search.html
  • Enter search criteria and click “Find Hospitals.” Select a hospital on which to focus for this Discussion.
  • Review all of the data on this hospital, which should include the various Process of Care Measures and the Survey of Patients’ Hospital Experiences (HCAHPS scores).
  • Compare the data for your selected hospital with data from other institutions in the area and across the country, as you think appropriate. (To do this, repeat the steps described above, but this time include these other institutions in your comparison.)
  • Based upon your first selected hospital’s comparative scores, what do you think is the most important area related to patient satisfaction/clinical outcomes to improve, and why? It may be helpful to consider what other steps you would take to get a clearer picture of this quality issue.
  • Think about how you as a leader and manager would communicate change at the point of care (front line) to provide service that improves one or more patient satisfaction/clinical outcomes in your selected hospital.

By Day 3

Post a brief description of the hospital and your selected outcome, including indications of the needed improvement. Then, provide a short, persuasive statement demonstrating how you would communicate and motivate a nurse at the point of care (front line) to provide service that improves the selected outcome. Support your statement with evidence and strategies from the literature.

Nursing Informatics and the Foundation of Knowledge

Assignment: Application: Using the Data/Information/Knowledge/Wisdom Continuum

Have you ever gone online to search for a journal article on a specific topic? It is amazing to see the large number of journals that are available in the health care field. When you view the library in its entirety, you are viewing untapped data. Until you actually research for your particular topic, there is little structure. Once you have narrowed it down, you have information and once you apply the information, you have knowledge. Eventually, after thoughtful research and diligent practice, you reach the level of wisdom—knowledge applied in meaningful ways.

Are there areas in your practice that you believe should be more fully explored? The central aims of nursing informatics are to manage and communicate data, information, knowledge, and wisdom. This continuum represents the overarching structure of nursing informatics. In this Assignment, you develop a research question relevant to your practice area and relate how you would work through the progression from data to information, knowledge, and wisdom.

To prepare:

  • Review the information in Figure 6–2 in Nursing Informatics and the Foundation of Knowledge.
  • Develop a clinical question related to your area of practice that you would like to explore.
  • Consider what you currently know about this topic. What additional information would you need to answer the question?
  • Using the continuum of data, information, knowledge, and wisdom, determine how you would go about researching your question.
    • Explore the available databases in the Walden Library. Identify which of these databases you would use to find the information or data you need.
    • Once you have identified useful databases, how would you go about finding the most relevant articles and information?
    • Consider how you would extract the relevant information from the articles.
    • How would you take the information and organize it in a way that was useful? How could you take the step from simply having useful knowledge to gaining wisdom?

Write a 3- to 4-page paper that addresses the following: APA style with refernces

  • Summarize the question you developed, and then relate how you would work through the four steps of the data, information, knowledge, wisdom continuum. Be specific.
    • Identify the databases and search words you would use.
    • Relate how you would take the information gleaned and turn it into useable knowledge.
  • Can informatics be used to gain wisdom? Describe how you would progress from simply having useful knowledge to the wisdom to make decisions about the information you have found during your database search.

Your paper must also include a title page, an introduction, a summary, and a reference page.

How to prevent the spread of these common infections

Prepare two educational handouts (two pages each), one for community members and patients and one for health care providers, that provide information about common bacteria or viruses and how to reduce the risk of infection.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

Preparation

Search the Capella library and the Internet for scholarly and professional peer-reviewed articles on food-borne illness or virus prevention. You will need at least three articles to use as support for your work on this assessment. In addition, look for statistics on cases of food-borne illness or influenza in your community. Sites such as the CDC, WHO, and state or public health departments provide the most current and reliable data.

Directions

Your employer is hosting a Healthy Mind and Body Fair in your community. Your manager has asked for your help in creating educational materials about food-borne illnesses like E.coli or viruses such as influenza. The goal is to provide information about how to reduce the risk of infection. You have been asked to create two informational handouts on the same topic:1.One handout is for participants (community members, patients) that raises awareness about the transmission of common bacteria or viruses and how to prevent the spread of these common infections. Consider health literacy levels when preparing the handout for non-medical populations.2.The second handout is directed at allied health care providers. This material should provide a higher level of information and detail about the subject.

Each handout should cover the following:•Provide information about the illness and transmission.•Provide recent statistics of cases in the community, with sources noted.•Describe the scope of illness (symptoms, severity, adverse outcomes).•Describe both treatment and prevention options, with an emphasis on prevention. •Relay any new research or treatment modalities.•Include information your readers will want to refer to in the future. This includes Web sites or tips for finding additional information or additional resources.

Consider the reader when creating your handouts, and be creative!

Format the handouts as follows:•Break up content into short paragraphs.•Use bullet points and lists whenever possible.•Use sub-headings to isolate different topics within the documents.•Remember, say no more than what is necessary; the urge to say too much can ruin a good handout.

Additional Requirements

Each handout should meet the following criteria:•Contain 500 words (2 pages).•Must be readable, concise, with a logical ordering of ideas.•Include a minimum of three references. (These must be recent, from within the past five years.) The same resources can be used for both handouts.•Provide adequate documentation of ideas and appropriate APA citation of relevant literature.•Assignment submission should include one complete document, prepared as follows: ◦Title page for the participant handout.◦Participant handout.◦Reference page for the participant handout.◦Title page for the allied health care provider handout.◦Allied health care provider handout.◦Reference page for the provider handout.

•Be double spaced in Times New Roman font, 12 point.

Human experimentation that have occurred during the history of medical research

200 words 2 references within 5 years (prefer nursing journals or peer reviewed only)

Henrietta Lackswas an African American woman whose cancer cells were the source of the HeLa cell line, one of the most important cell lines in medical research. HeLa cells are “immortalized cells.” Immortalized cells lines are important because they will reproduce indefinitely under specific conditions. The HeLa immortal cell line was vital for creating the polio vaccine, cloning (i.e., Dolly the sheep), gene mapping (i.e., the Human Genome Project) and more.

Mrs. Lacks was the unwitting source of these cells when her tumor was biopsied in 1951 during treatment for cervical cancer at Johns Hopkins Hospital in Maryland. Her cells were then cultured by George Otto Gey, who created the cell line known as “HeLa” (i.e., Henrietta Lacks). Consent was NOT obtained to culture her cells, nor was she (or her family) ever compensated for the use of the cells despite the fact that the HeLa cell line revolutionized modern medicine!

As we examine ethics for nursing research and evidence-based practice, please consider and present examples of human experimentation that have occurred during the history of medical research. Have these projects resulted in beneficial outcomes for society? Can human experimentation be justified when the greater good of society is at stake?

Factors that make some people healthy and others ill

What are some factors that make some people healthy and others ill? Healthy People 2020 identifies five determinants of health that influence the health of individuals and populations. Healthy People 2020 describes them as “a range of personal, social, economic, and environmental factors that influence health status” (U.S. Department of Health and Human Services, 2014, About Determinants of Health, para. 1). Determinants fall into five categories including: (a) policy making, (b) social factors, (c) health services, (d) individual behavior, and (e) biology and genetics.

How can you apply what you have learned in your coursework to your practicum setting

Consider the following scenario:

Marcus recalls the beginning of his career, when he started as a nurse at Grand View Hospital. He had heard the organization was soliciting proposals from various companies so they could weigh the pros and cons associated with adopting a new health information technology system. He has been curious about the request for proposal (RFP) process ever since. Now, as he looks forward to new professional opportunities, he would like to ensure that he develops the skills and expertise needed to formulate an RFP.

What are your professional aims? How can you apply what you have learned in your coursework to your practicum setting? How will you leverage your experiences in the practicum to facilitate your development as a nurse leader-manager or informaticist?

In this Discussion, you reflect on your aspirations and consider the transitions that may be required to achieve them. You identify professional development objectives and evaluate opportunities for achieving them through your experiences in the practicum.

  • Think about the professional role changes you have been undergoing or that you may undertake following completion of this MSN program.
  • Review the information related to professional development and role change in the Learning Resources, and conduct additional research as necessary to address any questions or concerns you may have.
  • Consider the following questions:
    • What types of professional positions interest you? Are they significantly different from the types of positions you have held in the past? If so, how?
    • What challenges are you likely to encounter as you transition into a new role?
    • What resources could help you to manage this change? Consider your inner resources (e.g., drawing on previous experiences, stress management), resources available to you through your relationships with others, and institutional supports.
  • Consider how you could use this Practicum Experience to apply what you have learned and enhance or acquire specialization skills and knowledge, regardless of whether you intend to change roles or stay in your current position for the time being.
  • Review the NURS 6600 Course Outcomes listed in the Syllabus. Determine how your experiences in the practicum could help you to achieve one or more of these outcomes.
  • Review the information in the Introduction to the Practicum (in this week’s Practicum area) and the School of Nursing Practicum Manual as necessary to ensure you have a clear understanding of the practicum requirements.
  • Review the suggestions for developing effective learning objectives provided in the Learning Resources.
  • Think of two or three objectives that could help guide your professional development during your practicum. These objectives, referred to as your practicum professional development objectives, must be:
    • Specific
    • Measurable
    • Attainable
    • Results-focused
    • Time-focused
    • Reflective of the higher-order domains of Bloom’s Taxonomy (i.e., Application level and above)
  • Select one or more practicum professional development objectives to focus on for this Discussion. (You may continue to hone these objectives as you work on this week’s Application Assignment.)
  • Reflect on how you could achieve each objective through your Practicum Experience.

By Day 3

Post an explanation of your professional aspirations and how you intend to use the Practicum Experience to promote career change and/or enhance your performance. Describe at least one objective to facilitate your professional growth, and explain the steps you could take to achieve the objective(s) during your Practicum Experience. Support your response with examples from the literature.
