How middle-range theory can help you in your everyday environment as a nurse

PURPOSE:

It is important to understand how middle-range theory can help you in your everyday environment as a nurse. In the previous assignment, you identified a practical problem that emerged from the evidence in the extant literature or professional practice. In this assignment, you will explore and apply middle-range theory to solve the specific problem that you identified in the previous assignment.

DIRECTIONS:

Consider the problem that you described in the previous assignment and the instructor feedback about that assignment.

Write a paper (1,000 to 1,500 words) that describes how middle-range theory can be applied to the identified problem. The paper should include the following:

  1. A brief summary      of the problem.
  2. A description of      a middle-range theory that could be applied to the problem. Is this      middle-range theory appropriate to your identified problem?
  3. A brief      discussion of the middle-range theory’s origins.
  4. A discussion of      how the middle-range theory has been previously applied.
  5. A discussion of      the application of the middle-range theory to the identified problem. How      would your practice change by incorporating this theory?

Prepare this assignment according to guidelines found in the APA Style Guide, located in the Student Success Center. An abstract is not required.

This assignment uses a rubric. Please review the rubric prior to beginning the assignment to become familiar with the expectations for successful completion.

The treatment the patient received for her ovarian cancer

Unit VI Case Study Worksheet Directions: Below is a case study presentation of a patient with a condition covered in this chapter. Read the case study and answer the questions below. Some questions will ask for information not included within this chapter. Use your textbook, a medical dictionary, Be sure to use APA formatting for all citations and references.

Patient is seen by obstetrician today for moderate vaginal bleeding without any cramping or pelvic pain. Patient is 23 years old. She is currently estimated to be at 175 days of gestation. Amniocentesis at 20 weeks indicated male fetus with no evidence of genetic or developmental disorders. This patient is multigravida but nullipara with three early spontaneous abortions without obvious causes. She was diagnosed with cancer of the left ovary 4 years ago. It was treated with a left oophorectomy and chemotherapy. She continues to undergo full-body CT scan every six months, and there has been no evidence of cancer since that time. Pelvic ultrasound indicates placenta previa with placenta almost completely overlying cervix. However, there is no evidence of abruptio placentae at this time. Fetal size estimate is consistent with 25 weeks of gestation. Fetal monitoring revealed that the FHR is strong with a rate of 130 beats/minute. The placenta appears to be well attached on ultrasound, but the bleeding is cause for concern. With the extremely low position of the placenta, this patient is at very high risk for developing abruptio placentae. She may require early delivery by cesarean section if bleeding increases. She will definitely require C-section at onset of labor.

1. Describe in your own words the treatment this patient received for her ovarian cancer. What procedure does she continue to have every six months?

2. This patient had an amniocentesis at 20 weeks of gestation. Describe this procedure and why it is performed.

3. What occurred during this patient’s first three pregnancies? Describe what terminated the pregnancies.

 4. Describe what fetal monitoring is. What is FHR?

5. This patient has placenta previa. What procedure discovered this condition? The physician, however, is much more concerned about abruptio placentae. Explain why.

6. How will this infant need to be delivered? Describe this procedure.

Treatment plans for children and adolescents diagnosed with schizophrenia

                                             Early Onset Schizophrenia

Children and adolescents with schizophrenia have more difficulty functioning in academic or work settings, and significant impairment usually persists into adulthood. They may have speech or language disorders and in some cases borderline intellectual functioning. These individuals are more likely to complete suicide attempts or die from other accidental causes. Schizophrenia is characterized by positive and negative symptoms. Positive symptoms include hallucinations, delusions, and behavior disturbance. Negative symptoms include blunted affect and attention, apathy, and lack of motivation and social interest.

Early-onset schizophrenia is a rare and severe mental illness in which children interpret reality abnormally. There are a range of problems with cognitive functioning, behavior, and emotions. Perceptions may be distorted and children or their parents may report that they have difficulty distinguishing reality. This is a diagnosis that is difficult to confirm in the early stages.

                                            The Assignment (2 pages)

· Compare at least two evidence-based treatment plans for adults diagnosed with

schizophrenia with evidence-based treatment plans for children and adolescents

  diagnosed with schizophrenia.

· Explain the legal and ethical issues involved with forcing children diagnosed with

schizophrenia to take medication for the disorder and how a

Psychiatric Mental Health Nurse Practitioner (PMHNP) may address those

  issues.

Health Insurance and Economy

When dealing with public health policy and laws, we need to consider  various aspects of the economy. Concepts of efficiency can be utilized  when deciding which goods and services should be produced and who should  receive them.

The three types of efficiencies are allocative, production, and  technical. The two types of health economics are positive and normative.  Positive Economics deals with “what is” or the current status that is  linked to a clear standard of measurement. Normative economics deals  with “what ought to be” and is subject to interpretation with the moral  or ethical aspects of a policy. Your lecture has more details.

Using South University Online Library or the Internet, search the  article, “The Interplay of Public Health Law and Industry  Self-Regulation: The Case of Sugar-Sweetened Beverage Sales in Schools.”

On the basis of the article and your understanding on the topic,  write a 3- to 4-page essay in a Microsoft Word document on the three  types of economic efficiencies and both positive and normative economics  as it relates to regulating sugar-sweetened beverage sales in schools.

Support your statements with at least two other scholarly sources,  correctly formatted in APA style. This assignment requires a title page,  an introduction, a body, a conclusion, and a separate reference page.

Submission Details:

  • Support your responses with reasoning and examples.
  • Cite any sources in APA format.

The application of evidence-based practice models as a strategy to improve patient safety and other quality dimensions

This week builds on that foundational awareness with a focus on the application of evidence-based practice models as a strategy to improve patient safety and other quality dimensions. In this Discussion, consider how these strategies can sustain practice changes.

To prepare:

Read: Newhouse, R.P. (2007) Diffusing confusion among evidence-based practice, quality improvement and research. JONA 37,432-535 (see attached pdf)

Read: Mazurek Melynk B., Gallagher-Ford, L., English Long, L., & Fineout-Overholt, E. (2014) The establishment of Evidence-Based practice competencies for practicing registered nurses and advanced practice nurses in real world clinical settings: proficiencies to improve healthcare quality, reliability, patient outcomes, and costs. Worldviews on Evidence-Based Nursing 11(1),5-15. (see attached pdf)

We all agree that research is the highest form of reliable evidence, when implemented in an organization that provides the basis for an evidence based practice. FIND a research study published in a peer reviewed journal related to a clinical practice problem that is of interest to you, and which would ultimately contribute to an evidence based practice (See “TeamSTEPPS Improves Operating Room Efficiency and Patient Safety” attached article).

The discussion assignment:

ANSWER the following discussion questions. Include the research study in your post (attach it to the post and post it in doc sharing). See page 11 #15 in Mazurek Melynk, Gallagher Ford, English Long and Fineout-Overholt. In this assignment, you are being asked to critically appraise a single research study for its relevance to a QI practice problem.

Discussion questions:

1) Briefly summarize study findings, conclusions and recommendations. Do you agree with these?

2) As a DNP prepared nurse, would you recommend a change in nursing practice based on the study? Defend and/or justify your decision based on research evaluation principles. In other words, does the evidence generated by this research article signify a need to change nursing practice? If not, why not; if so, why?

Categories of probability interpretations, whose adherents possess conflicting views about the fundamental nature of probability

Module 3 – Case

INTRODUCTION TO HYPOTHESIS TESTING

Assignment Overview

Suppose that a 2012 National Health Interview Survey gives the number of adults in the United States which gives the number of adults in the United States (reported in thousands) classified by their age group, and whether or not respondents have ever been tested for HIV. Here are the data:

Age Group

Tested

Never Tested

18–44 years

50,080

56,405

45–64 years

23,768

48,537

65–74 years

2,694

15,162

75 years and older

1,247

14,663

Total

77,789

134,767

Discuss probability. What is its history? What is the theory of probability? How is it calculated? What are the advantages and disadvantages of using this technique?

  1. Identify and discuss the two major categories of probability interpretations, whose adherents possess conflicting views about the fundamental nature of probability.
  2. Based on this survey, what is the probability that a randomly selected American adult has never been tested? Show your work. Hint: using the data in the two total rows, this would be calculated as p (NT) /( p (NT) + p (T)), where p is probability.
  3. What proportion of 18- to 44-year-old Americans have never been tested for HIV? Hint: using the values in the 18–44 cells, this would be calculated as p (NT) / ( p (NT) + p (T)), where p is probability. Show your work.

Submit your (2-3 pages) paper by the end of this module.

Cultural and systemic considerations that frame the client's presentation

INSTRUCTIONS

The Case of Marisol

Marisol is a 40-year old single female from a tight knit Puerto Rican family. She is employed full time as a data analyst for a technology company. She has steadily progressed in her career by working for relatively small companies where her role allowed her to work relatively independently. Unfortunately, working for small organizations was less stable financially so she has been working for a larger company for the past four years. Interacting with a larger work group has challenged her comfort zone. She prefers to spend time with a network of close friends she has known most of her life whom she visits with one on one. Her family lives in the area, and she sees them frequently. Her parents and older brother have always provided a support system and practical help. She enjoys quiet evenings alone or at the home of her brother and his family. Her friends and family privately wonder why she’s never dated or wanted to travel.

For each case study, you will complete a descriptive diagnosis using tools you select from the list of assessment tools provided later in the assignment. Each case requires the following information to be addressed:

  • Identify presenting concerns from the client’s perspective as described in the video and accompanying narrative. Include relevant cultural and systemic considerations that frame the client’s presentation.
  • Describe what information is still needed to make a differential diagnosis and evaluate how at least one assessment tool, which is listed in the List of Assessment Tools resource, will aid in obtaining that information. The Differential Diagnosis Decision Tree may be helpful to guide this process.
  • Present DSM-5 and ICD-10 codes, including relevant V and Z codes.
  • Provide a descriptive rationale for the DSM diagnosis that best fits the information provided including relevant ICD codes. This should be written in a narrative form using complete sentences. Support your rationale with scholarly sources. Optional readings found in the course syllabus may be particularly relevant.
  • Determine if a medication consultation is appropriate and provide a rationale with support from scholarly sources.

Effective Approaches In Nursing Leadership And Management

Effective Approaches in Leadership and Managment

In this assignment, you will be writing a 1,000-1,250-word essay describing the differing approaches of nursing leaders and managers to issues in practice. To complete this assignment, do the following:

1. Select an issue from the following list: nursing shortage and nurse turn-over, nurse staffing ratios, unit closures and restructuring, use of contract employees (i.e., registry and travel nurses), continuous quality improvement and patient satisfaction, and magnet designation.

2. Compare and contrast how you would expect nursing leaders and managers to approach your selected issue. Support your rationale by using the theories, principles, skills, and roles of the leader versus manager described in your readings.

3. Identify the approach that best fits your personal and professional philosophy of nursing and explain why the approach is suited to your personal leadership style.

4. Identify a possible funding source that addresses your issue. Consider looking at federal, state, and local organizations. For example: There are many grants available through the CDC, HRSA, etc.

5. Use at least two references other than your text and those provided in the course.
