STRATEGIC PLANNING IN HEALTH CARE ORGANIZATIONS

Course Project: Developing a Strategic Plan

Section 5: Proposed Core Team of Change Champions

In Week 4, you examined the benefits of stakeholder involvement and the negative consequences that may arise if this is not sufficiently addressed during strategic planning. This week you have been considering how to leverage the influence of change champions and facilitate team building to promote the successful implementation of a proposed change.

How would you create a core team of change champions for the change you are proposing through your Course Project?

To prepare:

Reflect on the results of your SWOT analysis (submitted in Week 7) and other elements of your strategic plan.

Think about how effective stakeholder involvement would promote the successful adoption and implementation of your proposed change.

Consider the following:

What strategies would help you to develop a core team to generate buy-in for this change?

Which stakeholders should be included in the process as change champions? How and when should they become involved?

Begin to write a 1- to 2-page description of your core team of change champions.

Section 6: Evaluation Plan

Creating an evaluation plan is a critical part of developing a strategic plan. How would evaluation allow you to determine the cost benefits and effectiveness of your proposed change?

To prepare:

Reflect on the elements of your Course Project that you have developed thus far. Review the information on evaluation presented in the Learning Resources, and reflect on any insights you have gained from this week’s Discussion. (Week 11 Discussion)

How would you evaluate the implementation of your strategic plan? Develop an evaluation plan that includes a timeline for evaluation. Be sure to incorporate refreezing.

To complete:

Write a 1- to 3-page explanation of your evaluation plan. Include the timeline for evaluation, and explain how you will incorporate refreezing.

Note: This will be submitted as part of your Course Project this week.

Section 7: Executive Summary

As your development of a strategic plan comes to a conclusion, it is time to develop foundational pieces for your Course Project, including an executive summary.

An executive summary provides an overview of your proposed change and allows your target audience to become acquainted with the essential elements of the strategic plan. Your executive summary should be informative and persuasive, delineating the business case for your proposed change.

To prepare:

Review the information in the Learning Resources related to developing an executive summary.

Craft an executive summary that appeals to your target audience and presents key information related to your strategic plan. Be sure to describe the unmet need you are addressing and convey the urgency or significance of this problem. Clearly state your proposed change and substantiate your recommendation.

To complete:

Write an executive summary for your strategic plan.

Note: This will be submitted as part of your Course Project this week.

Section 8: References

The inclusion of a reference list not only allows you to credit others’ work appropriately but also provides the information readers would need to read resources of interest to them and learn more about the underpinnings of your strategic plan.

To prepare:

Develop and/or refine the list of the resources cited in your strategic plan.

To complete:

Create your reference list to accompany your strategic plan.

Comprehensive Course Project: Developing a Strategic Plan

Over the past several weeks of this course, you have been developing elements of a strategic plan. As you prepare to submit your Course Project, reflect on what you have learned throughout this process. Also think about how you would like to convey the results of your analysis and planning to your target audience.

Your Course Project must include information related to each section of the Course Project assigned in Weeks 6–11; however, you have some flexibility in terms of how you present it. You may wish to compare strategic plans from various groups and organizations and then develop a format that best suits your plan.

Designing and Measuring Outcomes

Designing and Measuring Outcomes

As noted in previous courses of the DNP program, it is essential to design measurable outcomes that can guide the development, implementation, and evaluation of an initiative. For this Discussion, you are asked to examine professional standards and other factors that require consideration as you develop outcomes for your proposed evidence-based change. Think about the various groups that are invested in—or that may be affected by—this change. What measures would stakeholders need in order to help determine if the change aligns with organizational priorities and is successful in achieving its intended outcomes? Why is it important to establish accountability around this initiative?

To prepare:

  • Look at outcomes established for at least one      evidence-based change currently underway or previously accomplished within      your practicum setting. What lessons can be learned to support the      development of outcomes for your proposed change?
  • What outcomes would you identify for your proposed      change? How are these tied to AACN Essentials? What other standards or      guidelines should be considered? What interdisciplinary factors need to be      considered as you craft these outcomes? Think about other clinical and      administrative groups, as well as patients.
  • How will the outcomes be measured?
  • Be prepared to support your Discussion postings with      evidence from the literature and specific examples related to your      practicum setting.

By Day 3 of Week 8

Post a cohesive response that addresses the following:

By tomorrow Wednesday 4/17/19  2pm (America/New York time), write a minimum of 550 words essay in APA format with at least 3 references (see list or required readings below). Include the level one headers as numbered below:

1) Describe three outcomes for your proposed change. For each outcome, describe the evaluation criterion/criteria that is/are measurable.

2) Explain how these outcomes relate to multiple sets of standards, as well as differing groups’ needs and priorities. Why is intra- and interdisciplinary collaboration important as you design these outcomes?

3) How do the measurable outcomes for your proposed change establish accountability for you as a leader?

Measurable Patient-Centered Practice Problem Related To Quality Or Safety

Identify a measurable patient-centered practice problem related to quality or safety and relevant to your practice setting that you will also focus on in your Capstone Paper and post a brief description of the problem and an explanation of why you selected it. Explain how the conversation you had with the key leader in your practice setting impacted your decision to address this particular practice problem. Be sure to support your practice problem with the literature that indicates the relevance of this problem for nursing practice. Provide evidence from your practice area and describe the data that is available.

You will not be collecting any data during the practice experience project; you will use data already available to you in your workplace or other practice setting. Think about the different kinds of data that is shared with you in your work area. For example, data may include patient satisfaction scores, medication error rates, fall, CLABSI or CAUTI rates. Analyzing the data available to you is how you will identify that a quality problem exists. Data identifies potential areas for improvement and monitors the effectiveness of any changes. It is important to obtain baseline data before beginning a quality improvement project and to analyze results during and at the end of a project.

Note: The practice problem must be related to patient outcomes. (Staffing cannot be your main practice problem for the completion of the Practice Experience Project and Capstone Paper.)

Note: If you use the same practice problem that was presented in the Week 1 Discussion on quality theories, keep in mind that you must be much more specific in this post and explain how it is relevant to your setting and nursing practice.

What are special tests used to differentiate type of Tendon tears in the knee

Case Studies

1- Knee Injury

A 15-year-old gymnast has noted knee pain that has become progressively worse during the past several months of intensive training for a statewide meet. Her physical examination indicated swelling in and around the left knee. She had some decreased range of motion and a clicking sound on flexion of the knee. The knee was otherwise stable.

Studies and Results:

Routine laboratory values: Within normal limits (WNL)

Long bone (femur, fibula, and tibia) X-ray: No fracture

Arthrocentesis with synovial fluid analysis

Appearance: Bloody (normal: clear and straw-colored)

Mucin clot: Good (normal: good)

Fibrin clot: Small (normal: none)

White blood cells (WBCs) <200 WBC/mm3 (normal: <200 WBC/mm3)

Neutrophils <25% (WNL)

Glucose 100 mg/dL (normal: within 10 mg/dL of serum glucose level)

Magnetic resonance imaging (MRI) of the knee Blood in the joint space. Tear in the posterior aspect of the medial meniscus. No cruciate or other ligament tears

Arthroscopy Tear in posterior aspect of medial meniscus

Diagnostic Analysis

The radiographic studies of the long bones eliminated any possibility of fracture. Arthrocentesis indicated a bloody effusion, which was probably a result of trauma. The fibrin clot was further evidence of bleeding within the joint. Arthrography indicated a tear of the medial meniscus of the knee, a common injury for gymnasts. Arthroscopy corroborated that finding. Transarthroscopic medial meniscectomy was performed. Her postoperative course was uneventful.

Critical Thinking Questions

1. One of the potential complications of arthroscopy is infection. What signs and symptoms of joint infection would you emphasize in your patient teaching?

2. Why is glucose evaluated in the synovial fluid analysis?

3. What are special tests used to differentiate type of Tendon tears in the knee ? Explain how they are performed (Always on boards)

2-Testicular Cancer

A 21-year-old male noted pain in his right testicle while studying hard for his midterm college examinations. On self-examination, he noted a “grape sized” mass in the right testicle. This finding was corroborated by his healthcare provider. This young man had a history of delayed descent of his right testicle until the age of 1 year old.

Studies and Results

Routine laboratory studies: Within normal limits (WNL)

Ultrasound the testicle: Solid mass, right testicle associated with calcifications

HCG (human chorionic gonadotropin): 550mIU/mL (normal: <5)

CT scan of the abdomen: Enlarged retroperitoneal lymph nodes

CT scan of the chest: Multiple pulmonary nodules

Diagnostic Analysis

At semester break, this young man underwent right orchiectomy. Pathology was compatible with embryonal cell carcinoma. CT directed biopsy of the most prominent pulmonary nodule indicated embryonal cell carcinoma, compatible with metastatic testicular carcinoma. During a leave of absence from college, and after banking his sperm, this young man underwent aggressive chemotherapy. Repeat testing 12 weeks after chemotherapy showed complete resolution of the pulmonary nodules and enlarged retroperitoneal lymph nodes.

Critical Thinking Questions

1. What impact did an undescended testicle have on this young man’s risk for developing testicular cancer?

2. What might be the side effects of cytotoxic chemotherapy?

3. What was the purpose of preserving his sperm before chemotherapy?

4. Is this young man’s age typical for the development of testicular carcinoma?

The role of the family in the care and health of patients

Read chapter 11 of the class textbook and review the attached PowerPoint presentation.  Once done work on the following exercises;

  1. Write a  summary on the role of the family in the care and health of patients.
  2. Include a discussion of the dynamics of family and culture related to health beliefs and community health.
  3. Identify a few cultures that have male heads of family who make decisions for all family members.
  4. Write a paragraph on how to establish health screening for hypertension in a community center such as a church or park district.
  5. What are the primary planning issues to consider while planning the health screening? How can you ensure that the screening was effective?
  6. Search for a website that advertises for a health fair or health intervention in a community site. Write a one-paragraph summary.

As stated in the syllabus present your assignment in an APA format word document, Arial 12 font attached to the forum in the discussion tab of the blackboard titled “Week 11 discussion questions” for grading and in Turnitin to verify originality.   If you don’t post your assignment in any of the required forums you will not get the points.  A minimum of 2 evidence-based references besides the class textbook must be used.  You must post two replies to any of your peers sustained with the proper references and make sure the references are properly quoted in your assignment. A minimum of 900 words is required.  Please make sure to follow the instructions as given.

The importance of critically appraising research evidence

Realtors rely on detailed property appraisals—conducted using appraisal tools—to assign market values to houses and other properties. These values are then presented to buyers and sellers to set prices and initiate offers.

Research appraisal is not that different. The critical appraisal process utilizes formal appraisal tools to assess the results of research to determine value to the context at hand. Evidence-based practitioners often present these findings to make the case for specific courses of action.

In this Assignment, you will use appraisal tools to conduct a critical appraisal of published research. You will then present the results of your efforts.

To Prepare:

  • Review the Resources and consider the importance of critically appraising research evidence.
  • Reflect on the four peer-reviewed articles you selected in Module 2 and analyzed in Module 3.
  • Review and download the Critical Appraisal Tools document provided in the Resources.

The Assignment (Evidence-Based Project)

Part 4A: Critical Appraisal of Research

Conduct a critical appraisal of the four peer-reviewed articles you selected and analyzed by completing the Critical Appraisal Tools document. Be sure to include:

  • An evaluation table
  • A levels of evidence table
  • An outcomes synthesis table

Part 4B: Critical Appraisal of Research

Based on your appraisal, in a 1-2-page critical appraisal, suggest a best practice that emerges from the research you reviewed. Briefly explain the best practice, justifying your proposal with APA citations of the research.

The assessment process as related to individuals and families

Assessment is a structured approach to gathering and analyzing information about the client system and their story. How is the assessment process different when the client system is an individual or family versus a community made up of individuals and families?

Assessment involves gathering the needed information in order to determine appropriate intervention. Thorough assessment allows social workers to identify the problems that need to be addressed. Complex social problems often mean that both the micro and macro level factors contribute to social work cases.

In this Discussion, you compare differences in the assessment stage at different practice levels.

Answer all 5 questions (No Plagiarism) and Post in APA Format given 3 cites from  Kirst-Ashman, K. K., & Hull, G. H., Jr. (2018). Understanding generalist practice (8th ed.) &  Graybeal, C. (2001). Strengths-based social work assessment: Transforming the dominant paradigm. Families in Society, 82(3), 233–242.

  • 1.Describe the assessment process as related to individuals and families.
  • 2. Briefly identify the types of information it is important to gather.
  • 3.Compare how assessment is similar and dissimilar at the macro level.
  • 4. Explain how you would gather the needed information at the macro level.
  • 5. Identify a situation where a client system (micro or macro) may be reluctant to engage in the assessment process.

A philosophy that supports advanced nursing practice reflecting the values, beliefs, and cultural competencies relative to nursing practice, science, and theory

Bringing It All Together

This Assignment addresses this course outcome:

MN502-4: Develop a philosophy that supports advanced nursing practice reflecting the values, beliefs, and cultural competencies relative to nursing practice, science, and theory.

Now that you have spent the last 8 weeks discussing theory development and exploring models and philosophies that guide advance practice nursing, it is time for you to put it all together. In this Assignment, you will create a presentation using any form of presentation software (e.g., PowerPoint®, Prezi™, Movenote™, Powtoons™, Slidedog™, and so on). Your presentation should bring together the theory that best fits your concept of advance practice nursing, the model that you will use to translate this into practice, and your final philosophy of advance practice nursing related to your role after graduation.

Please remember that your presentation should contain slides that have bullet points. The bullets should number no more than four to six per slide. The bullet point is not written as a complete sentence. It contains key words. You, as the presenter will explain in depth what each bullet point means in the audio portion of the presentation. Slides may contain graphics, but should be uncluttered. Background and font colors should be of sufficient contrast to make reading them easy on the eyes. Attention to colors should be paid so that a person who is colorblind can easily read the presentation.

Assignment Details

The presentation should consist of a series of slides that include:

· Title Slide: A title slide that identifies the title of the presentation and name of the student.

· Introduction Slide: An introduction slide that includes brief information about you and your program track. The slide should also explain to the audience the purpose of the presentation.

· Model Slides: The third and fourth slides will address your chosen model. The bullet points should consist of the specific characteristics of the model. You will need at least one citation that identifies where the model came from. Your audio will expound upon the model and what the characteristics mean. The fourth slide should include why you chose the model and how you will apply it in practice.

· Theory Slides: The next three slides will focus on your chosen theory. The first slide should present your theory and why you chose it. The next slide would present the key concepts of the theory and how it has been used to date in past research no older than 5 years ago. The third slide will discuss how you will use this theory in practice as an advance practice nurse upon graduation.

· Philosophy Slide: The next area will consist of one slide that presents your philosophy of advance practice nursing in the post-graduate role you will assume. The philosophy statement will consist of one to three sentences that distill the essence of what you believe advance practice nursing is and your conception of nursing as a profession. It is written in the first person and present tense. It is a personal statement of your beliefs about the profession of nursing. This should present guiding statements for your future practice in the advanced practice role. An example of a philosophy statement might read this way: Advanced practice nursing takes the art and science of nursing to a level of “other” and “self” interacting together to reach a state of optimal wellness guided with compassion and love for the culture of the “other.”

· Graphical Slide: The next-to-last slide will be a graphical representation of how the model, theory, and your philosophy fit together.

· Conclusion Slide: The last slide will be a conclusion that brings together everything you have presented. It is not a summary that just reiterates what you presented. It is a judgment about what you presented and bring the audience home, ending the presentation on a positive note about the future.

You will need supporting citations for your theory and model slides, and possibly for your philosophy, as appropriate. You will include the citations where appropriate and your very last slide will be a reference slide.

The total slide count should not exceed 15 slides. This does not include the reference slide(s) or the title slide.

1. Organization, documentation, references formatting guidelines must follow APA 6th edition: To view the Grading Rubric for this Assignment, please visit the Grading Rubrics section of the Course Resources.
