Cultural Diversity and Teamwork in Healthcare Ethics

Consider the scenarios given below and answer the questions listed after each scenario. Justify your answers with appropriate research and reasoning.

Scenario 1

In the past, paternalism among healthcare providers was an accepted way of practicing medicine. Patients sought care from physicians and accepted their opinions and treatment choices without question. However, in today’s healthcare industry, there has been a shift away from paternalism as patients become active in their treatment choices and work as a team with their healthcare professionals. In the light of these facts, answer the following questions:

  • What do you feel has caused this shift and why?
  • Do you think this shift is for the better? How do you think this team approach to care can affect a patient’s care choices?

Scenario 2

With increasing diversity in the United States, a question has arisen regarding what accommodations should be made to assist the diverse population. Some groups or individuals feel in order to address the increasingly diverse population; we must employ a medical staff that is reflective of a community’s population. For example, if half of the population in a community is Hispanic, the medical office staff should be comprised of Hispanic workers.

  • Do you agree or disagree with this solution for addressing diversity in healthcare and why or why not? How would you address these diversity issues of the Unites States?
  • In the United States healthcare system, disparities such as socioeconomic, racial, and ethnic can be found. What do you feel are the most important disparities that need to be addressed in the healthcare industry? How do the disparities affect individuals in terms of healthcare and what do you feel is the best course of action to remedy each disparity and why?

Organizational Foundations

As you strive to grow in your leadership skills and abilities, you will find that the context in which you work influences your motivation and areas of focus. In a similar vein, your commitment to developing professionally can contribute toward organizational effectiveness.

To that end, it is critical to recognize the importance of organizational culture and climate. In particular, through this week’s Learning Resources, you may consider several questions: How do an organization’s mission, vision, and values relate to its culture? What is the difference between culture and climate? Moreover, how are culture and climate manifested within the organization?

For this Discussion, you explore the culture and climate of your current organization or one with which you are familiar. You also consider decisions and day-to-day practices and the way they relate to the organization’s mission, vision, and values.

To prepare:

  • Review the information related to planning and decision making in health care organizations presented in the textbook, Leadership Roles and Management Functions in Nursing: Theory and Application. Consider how planning and decision making relate to an organization’s mission, vision, and values, as well as its culture and its climate.
  • Familiarize yourself with the mission, vision, and values of your organization or one with which you are familiar. Consider how the statements and actions of leaders and others within the organization support or demonstrate the organizational mission, vision, and values. In addition, note any apparent discrepancies between word and deed. Think about how this translates into expectations for direct service providers. Note any data or artifacts that seem to indicate whether behaviors within the organization are congruent with its mission, vision, and values.
  • Begin to examine and reflect on the culture and climate of the organization. How do culture and climate differ?
  • Why is it important for you, as a master’s-prepared nurse leader, to be cognizant of these matters?

Post  (1) a description of your selected organization’s mission, vision, and values. (2) Describe relevant data, or artifacts, words, and actions of leaders and others in the organization that support, or perhaps appear to contradict the organization’s mission, vision, and values statement. (3) In addition, discuss the organization’s culture and its climate, differentiating between the two. (4) Explain why examining these matters is significant to your role as a nurse leader

Marcus recalls the beginning of his career, when he started as a nurse at Grand View Hospital. He had heard the organization was soliciting proposals from various companies so they could weigh the pros and cons associated with adopting a new health information technology system. He has been curious about the request for proposal (RFP) process ever since. Now, as he looks forward to new professional opportunities, he would like to ensure that he develops the skills and expertise needed to formulate an RFP. 

What are your professional aims? How can you apply what you have learned in your coursework to your practicum setting? How will you leverage your experiences in the practicum to facilitate your development as a nurse leader-manager or informaticist?

In this Discussion, you reflect on your aspirations and consider the transitions that may be required to achieve them. You identify professional development objectives and evaluate opportunities for achieving them through your experiences in the practicum.

To prepare:

  • Think about the professional role changes you have been undergoing or that you may undertake following completion of this MSN program.

You are the manager of an ancillary service department at a large, 500+ bed hospital. Develop a proposal (750-1,200 words) that is directed toward your staff, in which you address the following:

Betsy has type 2 diabetes and high blood pressure.  She visits the doctors often to keep her diabetes and blood pressure controlled.  The doctor regularly checks her blood levels and prescribes level 1 prescriptions to help Betsy control her diabetes and blood pressure.

  • Betsy is considered a controlled diabetic but still utilizes her insurance plan frequently.  
    • Compare the plans provided and determine the best plan for Betsy.  Remember to consider deductibles and general costs for the services she would be using.
  • For a couple of years Betsy was really taking care of herself.  However, after suffering a broken leg and being more inactive, Betsy has gained weight and has not been diligent about controlling her diabetes.  Betsy has found that she is requiring emergency room services and urgent care more often.

Zach is a healthy 30 year old that rarely goes to the doctors and is not on any medications.  He has been offered the following insurance plans and he is considering the plans for use of common alignments (i.e. colds) and for preventative tests.

Zach has recently been hired as a construction worker and was considering changing his health plan.  He realizes that this new job may have more hazards than his last job.
