How public policies initiated at the national or state level ultimately influence what occurs at the institutional and local level of nursing practice and health care delivery

Discussion: Health Care Policy

In this week’s media presentation, Dr. Trautman, Dr. Wakefield, and Ms. Coyle discuss how public policies initiated at the national or state level ultimately influence what occurs at the institutional and local level of nursing practice and health care delivery. Health care policy usually is developed to address health care cost, quality, or access, or a combination of the three. Due to the nature of their interaction with patients, nurses are well situated to be effective, knowledgeable advocates for their patients.

In this week’s Discussion, you will examine how current policies impact how you as a nurse provide health care. You will then select an issue of interest and determine how you could advocate for policy in that area.

To prepare:

Select one public policy that currently is impacting you and your practice(mental health). Consider the following:

What health care driver was the policy designed to address: cost, quality, access, or a combination?

Does the policy appear to be achieving its intended results? On what data are you basing your assumption?

What have been the effects (adverse or positive) of this policy on health care cost, quality, and access?

How is this policy affecting your nursing practice?

Next, select a health care issue—something you see or experience on a daily basis—about which you would like to influence a practice change through the policy process.

Review the Learning Resources, focusing on Kingdon’s Model. How would you utilize this model to guide your policy development?

By Day 3

Post a Brief Description of a Public Policy that is having an impact on your Practice(Mental Health);

Summarize your Analysis of the policy using the bulleted list above as a guide.

Then, Summarize the issue you would like to influence though a change in public policy, and outline

How you would utilize Kingdon’s Model to guide your policy development.

Standards of professional ethics and the principles of diversity and inclusion as applied by health care leaders in real-world situations

Overview

Write an executive report of 4–5 pages that analyzes the role of diversity in an underperforming community program, unit, or department, and outline a plan for providing ongoing training on cultural awareness, inclusion, and sensitivity to current employees.

Be sure you read all of the requirements for this assessment and review the suggested resources to see if they may be useful in completing the assessment.

This assessment provides an opportunity to determine how you can help advance your professional field by developing a strategy for managing diversity that recognizes the synergy achieved by having a diverse and inclusive workforce.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

•Competency 1: Explain the concepts, principles, and characteristics of effective health care leadership. ◦Determine the influence of effective leadership on the development of a diverse and culturally competent workforce.

•Competency 3: Analyze standards of professional ethics and the principles of diversity and inclusion as applied by health care leaders in real-world situations. ◦Outline a plan to recruit, hire, and retain a diverse workforce.

◦Outline a plan for providing employee training on cultural awareness and sensitivity.

Context

The United States continues to reflect the diversity of the world, as the country seeks to maintain its tradition of being a safe haven of freedom and the world’s melting pot. As health care practitioners, leaders, and scholars, it is vital that we embrace, recognize, and celebrate the diversity of our workforces and population.

It is also important for health care organizations to represent the communities that they serve in as many ways as possible. This diversity is represented in gender, race, ethnicity, age, and ability. Wynne (2012) posits that this diversity makes organizations stronger, allows for more productivity, and leads to enhanced job satisfaction. When health care organizations make themselves as diverse and inclusive as possible, they are integrating the community into the organization and the organization into the community, creating a win-win scenario for both. The health care organization is now more in touch with the needs, desires, and fears of the community. At the same time, the community members feel that they have representation in health agencies.

Assessment Instructions

Preparation

Use the following case study as the basis of this assessment.

Imagine you are the manager of a new health care facility satellite office or community outreach center located in a diverse neighborhood. The parent organization opened the facility to address the under-served health care needs of the culturally diverse residents who were not using the services offered at the main facility. The first month of operation saw a very high volume of patients, but since then, the numbers have dropped off drastically. Executive leadership wants to understand why this has happened and what you plan to do about it.

After careful examination of all aspects of the facility, and talking with some of the neighborhood residents, you have determined that the residents do not feel comfortable coming to the facility, because the staff does not represent the diversity of the neighborhood. Staff members were hired for their skill, but their diversity was not considered. Now, you must report to executive leadership what you have learned and how you intend to manage diversity.

Note: Remember, you can submit all, or a portion, of your draft to Smarthinking for feedback, before you submit the final version of your proposal for this assessment. However, be mindful of the turnaround time for receiving feedback, if you plan on using this free service.

Report Requirements

Write an executive report on the need for a diverse, culturally competent staff, including your plans to address that need.

Note: The requirements outlined below correspond to the grading criteria in the Managing Diversity scoring guide. At a minimum, be sure to address each point. In addition, you are encouraged to review the performance level descriptions for each criterion to see how your work will be assessed. You are provided an example report, to illustrate what proficient-level work for this assessment looks like.

Report Format and APA Style

•Use the Executive Report Template, linked in the Resources, to draft your report. You may also organize the content of your report in a format used in your organization. An abstract is not required.

•Your report should be 4–5 double-spaced pages in length, not including the title page and reference page.

•Apply correct APA formatting to all in-text citations and references.

•Use Times New Roman, 12-point font.

Writing

As you begin composing your report, consider the purpose of the report, the appropriate tone and style, and the expectations of the executive leaders who will be reading it.

•Be clear and concise in your reporting.

•Express your main points, arguments, and conclusions coherently.

•Use correct grammar and mechanics.

•Be sure to support your claims and arguments with credible evidence from 3–5 current, scholarly or professional sources.

•Proofread your writing.

Report Content

•Outline a plan to recruit, hire, and retain a more diverse workforce. Note: Your outline should be a high-level overview of a plan, not a detailed plan. Other courses in your program will go into human resource practices and diversity in depth. In drafting your plan, consider the following questions: ◦How does a leader develop a diverse workforce?

◦Where will you look for more diverse employees? Will you post job ads?

◦What will the interviewing and hiring process look like? Who will do the interviewing? Who will make the final decisions?

◦What kind of incentives will you offer diverse employees to retain them? What do you need to know about the culture and values of diverse employees before developing a retention plan?

◦How will you address issues related to diversity that result in conflict?

•Outline a plan for the ongoing training and professional development of existing staff in cultural awareness and sensitivity. Consider how your plan can: ◦Improve communications and interpersonal skills.

◦Help overcome bias and resistance to change.

•Explain the benefits, to both the organization and the community it serves, of having a diverse and culturally competent workforce. ◦Explain the relationship between health care organizations and communities.

◦Explain the role that diversity plays in providing culturally competent care and access for the under-served in a community.

•Determine the influence of effective leadership on the development of a diverse and culturally competent workforce. ◦Explain the importance of cultural competence for leaders in an organization.

•Explain how the academic and research skills you develop as a practitioner-scholar can serve you in your role as an effective health care leader. ◦Explain how those skills might contribute to your credibility and effectiveness as a leader.

Discussion: The Impact of ExDiscussion: The Impact of External Factors

Consider the following scenario:

Linda has been a nurse educator at LiveWell Medical Center for about 3 years. Since taking over the position, she has received many accolades for the new evidence-based practices she has shared in nurse trainings. Linda is now looking to turn her attention to patient education. She hopes to initiate bimonthly sessions that will help current and recently discharged patients to better manage their health and/or cope with difficult health issues. At Linda’s next meeting with LiveWell’s board of trustees, she confidently shares her proposal for this new program. At once, she is taken aback at their dismissive responses. “That sounds great Linda, but we simply do not have time to implement something of that caliber here. And how do you know if patients would even be interested in such a service?”

Over the next hour, Linda contemplates these comments and realizes that, despite her best intentions, there was some truth to their remarks. Linda had initially been inspired to create this program after reading about a large rehabilitation center in San Antonio, Texas. The center had revolutionized their outpatient process, helping to demonstrate strong commitment to the community. Though it worked well in San Antonio, Linda had not conducted any preliminary research to learn if it could be effective in their small town of Pinedale, Wyoming.

What external factors should Linda have investigated before trying to develop such a program? In addition, how could these factors continue to impact her program if the LiveWell board of trustees approves her idea?

To prepare:

  • Review this week’s readings, specifically Chapter 3, “Needs Assessment: The External and Internal Frame Factors” (pp. 46-54), in the Keating text and Chapter 5, “Forces and Issues Influencing Curriculum Development,” in the Billings course text. Reflect on the various external factors that can impact the way curriculum is developed, assessed, and evaluated. Then, consider whether certain factors influence institutions (academic settings) more or less than they do agencies (clinical settings).
  • Select one external factor from the following list to further explore for your Discussion posting:
    • Community
    • Population demographics and trends
    • Political climate and body politic
    • Societal patterns
    • Health care system and health needs of the population
    • Characteristics of the setting
    • Need for the program
    • Nursing profession
    • Financial support
  • Consider how this factor might impact curriculum development, assessment, and evaluation in your own academic or clinical setting or in one with which you are familiar. In addition, conduct research to explore how this factor might impact nursing education as a whole. Discussion: The Impact of External Factors

The accountability of the nurse leader for decisions that affect health care delivery and patient outcomes

Write a 3 page evidence-based health care delivery plan for one component of a heart failure clinic.

Nursing within an organization is a critical component of health care delivery and is an essential ingredient in patient outcomes (Kelly & Tazbir, 2014). The concern for quality care that flows from evidence-based practice generates a desired outcome. Without these factors, a nurse cannot be an effective leader. It is important to lead not only from this position but from knowledge and expertise.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

· Competency 2: Explain the accountability of the nurse leader for decisions that affect health care delivery and patient outcomes.

. Describe accountability tools and procedures used to measure effectiveness.

· Competency 3: Apply management strategies and best practices for health care finance, human resources, and materials allocation decisions to improve health care delivery and patient outcomes.

. Develop an evidence-based plan for health care delivery.

· Competency 4: Apply professional standards of moral, ethical, and legal conduct in professional practice.

. Apply professional and legal standards in support of a care plan.

· Competency 5: Communicate in manner that is consistent with the expectations of a nursing professional.

. Write content clearly and logically, with correct use of grammar, punctuation, mechanics, and current APA style.

Preparation

Refer to the Internet for supplemental resources to help you complete this assessment.

Instructions

Deliverable: Develop an evidence-based plan for health care delivery.

Scenario:

The hospital where you work has an issue with increased readmissions within 30 days of discharge. After examining the core measures, it was found that heart failure was the most common core measure disease process experiencing the highest rate of readmissions. The leadership team has given your team the charge of developing a nurse-run outpatient heart failure clinic. The purpose of this clinic is to ensure that discharge education is presented to the patient in an orderly, consistent manner and complies with evidence-based practice protocols. Since these patients may be discharged from a variety of areas in the facility, having the heart failure clinic staff take ownership of the process will improve both consistency and compliance. There are cardiologists that interact with the staff and patients, but the day-to-day operations of the clinic are designed and supported by the nurses as they interact with appropriate members of the other health care team disciplines promoting the best care for the heart failure patients.

As a member of the nurse team, you have been asked to develop one component of the clinic.

The hospital leadership established these objectives for the clinic services:

· Evaluate and maximize proper medication therapy.

· Conduct regular diet, exercise, and stress management classes for the patients.

· Monitor physiological indicators for the patients (lab work, weights, vital signs, ECGs).

· Provide a case management system for patients in the program post-discharge.

The overall goals for the heart failure clinic are to:

· Enroll greater than 90 percent of the patients with a primary or secondary diagnosis of HF prior to discharge.

· Facilitate discharge planning to achieve 100 percent compliance with patient education prior to discharge (discharge planning).

· Decrease readmission rates in this population by 5 percent over the next year.

The leadership team has asked you to provide them with an evidence-based plan for one of the components of the clinic. You may use any combination of documents (for example, a spreadsheet or a table) in addition to explanatory information to convey information clearly and succinctly.

Develop one: an Orientation Course Plan, a Discharge Education Plan, or a Care Coordination Plan.

An Orientation Course Plan:

· Develop an evidence-based plan for health care delivery.

· Include a comprehensive schedule of topics, objectives, key points, and patient resources for the orientation course.

. What are the components of an evidence-based education plan?

. How will you know that patients will understand what to do?

. What modalities will you use to deliver information?

. How will you adapt the plan to meet the needs of patients from diverse cultural and language backgrounds?

. Identify specialized and supplementary material needs.

· Apply professional and legal standards in support of a care plan.

. Explain the alignment to the most recent Heart Failure Guidelines and specific professional standards.

. Describe the accountability tools and procedures used to measure effectiveness.

. How will you know if the patient education plan is successful?

. What are the indicators of success or effectiveness?

A Discharge Education Plan:

· Develop an evidence-based plan for health care delivery.

. Develop a discharge plan with objectives and resources, and tools for patients to monitor their progress.

. How will you know that patients understand what to do?

. What modalities will you use to deliver information?

. How will you adapt the plan to meet the needs of patients from diverse cultural and language backgrounds?

· Apply professional and legal standards in support of a care plan.

. Explain the alignment to the most recent Heart Failure Guidelines and specific professional standards.

. Describe accountability tools and procedures used to measure effectiveness.

. How will you know if the discharge plan is successful?

. What are the indicators of success or effectiveness?

Care Coordination Plan:

· Develop an evidence-based plan for health care delivery.

. Develop a procedure for coordinating services.

. Consider the needs of “outliers.” For example, someone with lung disease may need extra resources.

. Who should be on the team?

. When would the team be activated?

. How would it be activated?

. What is the time frame required to coordinate services?

. How would the intervention plan be monitored for effectiveness?

· Apply professional standards in support of a care plan.

· Explain the alignment to the most recent heart failure guidelines and specific professional standards.

· Describe accountability tools and procedures used to measure effectiveness.

. How will you know if the care coordination plan is successful?

. What are the indicators of success or effectiveness?

. How will information be collected or communicated?

Additional Requirements

· Written communication: Written communication should be free of errors that detract from the overall message.

· APA formatting: Resources and in-text citations should be formatted according to current APA style and formatting.

· Length: The report should be 3 pages in content length, double-spaced.

· Font and font size: Times New Roman, 12 point.

· Number of resources: Support your plan with a minimum of three peer-reviewed resources, in addition to professional standards.
