Organizational Structures and Leadership

In most health care settings, it is unlikely that you would hear the terms “ad hoc” or “matrix” as you walk down the hallway. Although it is helpful for any organization to delineate pathways of responsibility and authority in an organizational chart, the lived experience of these structures is most apparent through the inquiries and behaviors people share everyday.

In your own workplace, you may find yourself wondering, who should I turn to when I have a practice dilemma? or Where can I go to learn more about this issue? These questions speak to the intricacies of formal and informal organizational structure and leadership. Organizational Structures and Leadership

To prepare:

  • Review the information presented in Chapter 12 of the course text. Focus on the information about formal versus informal structure as well as the types of organizational structures.
  • Consider the overall structure or hierarchy of your organization or one with which you are familiar. Which organizational structure best describes your organization—line (or bureaucratic), ad hoc, matrix, service line, or flat? Note: It is possible to have a combination of structures in one organization. Is decision making centralized or decentralized in this organization?
  • What is the role of committees, task forces, and councils in the organization, and who is invited to join? Consider how this relates to formal and informal leadership.
  • Reflect on how decisions are made within a specific department or unit. Which stakeholders provide input or influence the decision-making process? Assess this in terms of formal and informal leadership.
  • To support your analysis, consider your own experiences and investigate these matters by speaking with others at the organization and reviewing available documents. Be sure to consider how the concepts of formal and informal structure and leadership relate to one another and are demonstrated in the organization and in the particular department or unit. Organizational Structures and Leadership

By Day 3

Post a depiction of your organization’s formal structure, indicating whether it is best described as line, ad hoc, matrix, service line, flat, or a combination. Describe how decisions are made within the organization and within one department or unit in particular, noting relevant attributes of centralized/decentralized decision making. Explain the influence of formal and informal leadership on decision making within this department or unit.

MOBILIZATION PLAN

Create an 8–10-slide mobilization plan PowerPoint presentation (with detailed speaker’s notes) for a mobilization plan by your health care organization to commit 20 nurses to participate in a 4-month-long multinational effort to treat patients exposed to a highly contagious virus in a hot zone in Africa.

Nursing leaders must incorporate approaches that are inclusive and respectful of all stakeholders in the health care workforce, patient population, and the larger community that is served by the organization. Communication among members of teams, between departments and service lines, within large networks, with strategic business partners, and with patients, families, or support persons requires awareness of various facets of effective communication and cultural competence. A mobilization plan for an international medical mission requires careful planning of organizational structure roles, power distribution, and team member empowerment. MOBILIZATION PLAN

Preparation

Use Internet to help you complete this assessment. For help using PowerPoint, refer to the Microsoft Tutorials and other resources located in the Suggested Resources and left navigation menu of the courseroom.

This assessment is based upon the scenario below. The scenario is very limited in detail; where more detail is needed, incorporate any assumptions you make to flesh out the scenario. It is intended to assess your ability to communicate your approach to a challenge by evaluating issues of organization, leadership, safety, quality improvement, multiculturalism and diversity.

Your approach should be very conceptual and high-level.

Scenario

Your health care organization has recently committed 20 nurses to participate in a 4-month-long multinational effort to treat patients exposed to a highly contagious virus in a “hot zone” in Africa. The director of your organization has asked you, because of your previous medical mission experience, to outline nursing-related plans for preparing for the mobilization and present this information at an upcoming staff meeting. MOBILIZATION PLAN

Deliverable: Mobilization Plan PowerPoint Presentation

Create an 8–10-slide PowerPoint presentation (with detailed speaker’s notes) of your mobilization plan. It should be targeted toward members of the hospital’s administrative staff, nurses, and the physicians who will also participate in this medical mission.

Use bullet points and phrases on the slides.

The narrative, or explanation for each slide, should be in the speaker’s notes section.

The mobilization plan should address the following:

Identify the major stakeholders within the health care system that would be affected by the mobilization plan.

Analyze how the mobilization effort will impact staffing patterns and nursing care at the hospital. MOBILIZATION PLAN

Describe the medical mission team’s organizational structure of the mission team and how power is distributed.

Include one slide of an organizational diagram for the mission.

Describe the roles (in the speaker’s notes).

Assess how the organizational structure empowers team members.

Provide 1–2 examples of how team members will have power.

Identify key actions needed to assure quality of care and safety for mission patients and personnel.

Evaluate potential power issues that may arise when dealing with a multinational contingent.

(Be generic; that is, do not address individual nationalities, races, et cetera.)

Consider interactions with health care personnel from other countries.

Examine potential multicultural and diversity issues that mission personnel may encounter.

Consider that the indigenous population may be hostile to treatment.

Outline possible training requirements to improve cultural competencies of the personnel.

Additional Requirements

Written communication: Written communication should be free of errors that detract from the overall message.

APA formatting: Resources and in-text citations should be formatted according to current APA style and formatting.

Length: Presentation should be 8–10 slides. Include speaker’s notes on each content slide.

References: Include a minimum of three peer-reviewed resources on the final slide (in APA format).

General formatting: Choose an appropriate theme if using a template. MOBILIZATION PLAN

Global Warming

According to World Health Organization (WHO), global warming is the primary issues for concern. Noise pollution, overcrowding, traffic jams are some factors that have led to this problem. Lack of privacy is another factor in the modern times that is impacting people in ways that haven’t been examined yet. We also see a wide variance in weather patterns, such as severe hurricanes and drought conditions in the U.S.

Based on your understanding of the topic, create a report in a Microsoft Word document answering the following questions:

  • Examine one weather condition over the past two years in the U.S. which drastically affected the population. How can the community better prepare their families for such severe conditions?
  • Do you agree with the statement that countries should be held accountable for their contribution to climate change? Why or why not?
  • List some of the issues that might occur as the world’s population increases? Factor in water, food, and hazardous waste into your comments. Suggest ideas to address or avoid these issues.

According to the CDC website, violence is attributed for approximately fifty thousand deaths each year and results in over 2.5 million injuries. Homicide and suicide are the second and third leading causes of death, respectively, among US population aged fifteen to thirty four years.Global Warming

Hospital emergency departments treat an average of fifty five people for injuries every minute. The worst after effect of the sudden population explosion across the globe is the rise in violence.

  • How have violent injuries affected a community? What steps have communities taken to decrease overall violent crimes?
  • What steps can the federal or state governments take to help support communities affected from random or consistent violent acts?
  • What can parents do in their homes to help educate children about risks and preventative accidents to help keep them safe? What role should parents take to reduce family violence?

Identifying and Engaging Stakeholders for Strategic Planning

Identifying and Engaging Stakeholders for Strategic Planning

Identifying internal and external stakeholders and building productive relationships should be an intentional and well-conceived part of the strategic planning process. As a nurse leader-manager involved in strategic planning, it is important that you consider who could be impacted by a proposed change and how the change may affect individuals’ roles, responsibilities, and relationships. It is also essential to think about which stakeholders may be able to offer expertise, capital, and/or sway to help exercise the strategic plan and generate buy-in. Identifying and Engaging Stakeholders for Strategic Planning

In this Discussion, you identify stakeholders who can assist with carrying forward a strategic plan.

Your Instructor has assigned you to a small group for this Discussion. Craft your stakeholder description independently; through the collegial exchange that follows, you will offer each other suggestions for refinement. As in Weeks 2 and 3, use this small group Discussion to delve deeply into the concepts being presented as they are foundational to this course.

To prepare:

Reflect on a planned change effort you have experienced.

Consider the following questions:

  • How do you think stakeholder involvement may have impacted the outcome of this planned change?
  • Were the right stakeholders involved? Did the stakeholders help to carry out the plan as needed?
  • Should other stakeholders have been engaged? If so, how could relationships with additional stakeholders have been cultivated to better support the plan?
  • How does reflecting on this instance of planned change inform your perspective on what is needed to engage stakeholders to successfully promote other strategic changes?

Review the Mountain View Health Center case study, and reflect on the focus of your postings in the Week 2 Discussion 2 and the Week 3 Discussion.

Conduct additional research as necessary to strengthen your understanding of the process of involving stakeholders in strategic planning and to deepen your thinking about the organization. For instance, you may research organizations with similarities to Mountain View, and examine information related to stakeholder involvement.

Consider the following questions:

  • Which internal and external stakeholders should be involved in strategic planning related to this case study?
  • What assets and/or perspectives would each of these stakeholders offer to the process?
  • What role would each potential stakeholder play in helping to move forward a strategic plan?
  • When and how should the stakeholders become involved?
  • How would you cultivate relationships with these stakeholders to produce the best possible results?
  • How would the appropriate selection of stakeholders and their involvement promote the likelihood of successful adoption and implementation of a strategic plan? Identifying and Engaging Stakeholders for Strategic Planning

Receiving Aid and Support during Emergencies

Receiving Aid and Support during Emergencies

Superstorm Sandy and Hurricane Katrina raised many questions about the response of the Federal Government, and more specifically, the response of FEMA to natural disasters. FEMA was criticized for the delay in its response in parts of New York City mainly, the citizens of Long Island.

On the basis of you knowledge and understanding of the process of receiving State aid under FEMA and your knowledge about the conditions during Superstorm Sandy, answer the following questions:

  • Do you think FEMA was ready for a disaster such as Superstorm Sandy?
  • What were some of the challenges faced by FEMA with regard to its response to Superstorm Sandy?
  • How do you feel about the evacuation efforts on the part of several hospitals in the wake of Hurricane Sandy?
  • In the wake of Katrina, how did the relationship between the New Orleans Mayor and the Governor of Louisiana slow the flow of FEMA aid?
  • Should politics play a role when there is a disaster looming?
  • List the steps to create a process map showing the various steps required to receive FEMA assistance during an emergency and disaster management (EDM). Receiving Aid and Support during Emergencies

Individuals in the public health sector are often called upon to be the first responders within twenty-four hours of a bioterrorist attack. These first responders must take steps to remain safe while doing their jobs. One way to accomplish this is by using Personal Protective Equipment (PPE). However, in some cases, the first responders may not have adequate training to use the PPE correctly. In other cases, the proper PPE may not be readily available in quantities that are required to protect all first responders. On the basis of your understanding of this topic, answer the following questions regarding the above scenario:

  • What other issues that may occur when using PPE?
  • What are your suggestions to overcome these issues to ensure that all first responders are safe when responding to a bioterrorist attack? How would you implement these suggestions?

Working with other agencies can often help to gather the manpower necessary to treat an influx of patients. In times of disaster, individuals from these agencies may need to act as traditional first responders. However, in case of an outbreak of Ebola hemorrhagic fever, these individuals may be hesitant to potentially put themselves in danger.

On the basis of your understanding of the topic, answer the following questions:

  • What fears or reservations may these individuals from other agencies have about acting as traditional first responders?
  • How would you eliminate these fears in an effort to obtain the man power necessary to treat the influx of patients? Provide suggestions. Be sure to justify your response with factual support.
  • What would you do if you were unable to eliminate these fears? Receiving Aid and Support during Emergencies

The rest of the world has had numerous experiences in dealing with attacks involving chemical weapons. Since the United States has not had any significant experience with attacks involving chemical weapons, the experiences of Europe or Israel can be used as a guide for preparing the healthcare industry in the United States for such events.

Click here to read an article on terrorism attack in London:

On the basis of your readings, answer the following questions:

  • How can the events that occur in Europe and the other parts of the world assist us in securing our healthcare industry in the United States in preparing for attacks involving chemical weapons?
  • In terms of the characteristics of each country such as the population and availability of resources, what are the similarities and dissimilarities between the United States and the United Kingdom? How will these similarities and dissimilarities affect the way the two countries plan for a disaster scenario?

The use of training exercises in preparing for disaster scenarios is an important component of any emergency preparedness plan. Training exercises may be conducted at regularly planned intervals, or they can be conducted at random and without warning. Answer the following questions regarding the training exercises for preparation for an emergency or disaster scenario:

  • What are the advantages of conducting training exercises at regularly planned intervals? Why? What are the advantages of conducting training exercises at random and without warning? Why?
  • Under what circumstances is it most appropriate to conduct training exercises at regularly planned intervals? Why? Receiving Aid and Support during Emergencies
  • Under what circumstances is it most appropriate to conduct unannounced training exercises? Why?
