           Developed a pain management education program aimed at addressing the educational needs of home hospice patients and their primary caregivers.  The program targets an overarching problem where pain often goes unrecognized and under treated in hospice patients, contributing to patient and caregiver stress during the end-of-life (Coyne, Mulvenon, & Paice, 2018).

Educational Plan

            The following is our curriculum plan, consisting of seven sessions:

  1. Initial home visit/family meeting: we will establish therapeutic communication and address the goal of pain management.
  2. Identify the methods and tools to assess pain: we will introduce and develop skills utilizing various pain scales.
  3. Understanding different types of pharmacologic analgesic drugs: we will discuss opioid v. non-opioid medications.  We will also address potential limitations to adequate pain management.
  4. Organizational methods to manage medications:  we will instruct on appropriate methods to tracking, storing, and disposing of meds.
  5. Understanding side effects of pain medicine:  we will address common side effects and their treatments.

Program Development Process

            The organizational framework of a program serves to guide knowledge development, prioritize topics, and establish a timeline (Billings & Halstead, 2016).  Our patient education program follows the single-theory framework, where the central theme is caring.  Educational sessions are designed to develop patient and caregiver knowledge, comprehension, and application (Utica College, n.d.) in managing pain, ensuring maximal patient comfort at the end-of-life.  These goals are congruent with the organization’s mission, vision, and philosophy, which include the provision of high-quality care by a team of dedicated health professionals (Suncrest Home Health and Hospice, n.d.).

Esther Jackson is a 56-year-old black female who is 1-day post-op following a left radical mastectomy. During morning rounds, the off-going nurse shares with you during bedside report that the patient has been experiencing increased discomfort in her back throughout the night and has required frequent help with repositioning. She states that the patient was medicated for pain approximately 2 hours ago but is voicing little relief and states that you might want to mention that to the doctor when he rounds later this morning. Week 2: Political, Ethical, and Economical Influences on Health

As you examined in Week 1, the disparity of available health care staffing is varied throughout the world. Often, countries with the greatest disease burden have the fewest health care professionals available to care for their populations. This week extends the premise further to analyze the influence of politics, ethics, economics, environment, and culture on the public’s health in the global arena. Through health care policy and advocacy efforts, nurses act as change agents to improve the quality of care and quality of life for poor and disenfranchised populations.

Ethics in nursing practice and health care is a vitally important issue on the global health care front. A nurse’s responsibility toward a patient or a population can be viewed as an act that supports the preservation of human rights, dignity, and social justice. Nurses can address these needs in several ways, such as through political avenues, and by educating the community on disease prevention and proven methods to safeguard good health. By communicating a population’s vulnerability to specific ailments and diseases, nurses can be found working to promote health, prevent illness, and assist in recovery from many health problems.

This week you will examine how politics and economics influence health and health care. You will also consider ethical dilemmas when promoting health for a population.

The influence of poverty on health is a problem that transcends national borders. This week’s readings discussed some of the political, economic, and ethical issues that lead to health disparities in developed and developing countries. In addition, in this week’s first media presentation, Dr. Mancuso and Dr. Huijer shared insights on working within political systems to bring about positive changes in health care. Think about the importance of nurses in addressing these needs and in bringing about change.

Summary of Results – Clark Healthy Workplace Inventory On the Clark Healthy Workplace Inventory, my present employer received an 84, indicating that it is in good health. Because I work for a large firm that values customer loyalty and health care, I agree with this conclusion. We don’t simply work relentlessly with our patients on a one-on-one basis; we also work well together. This is required in order to succeed.

Healthcare workers must focus on our larger purpose—providing safe, effective patient care while interacting politely with one another—for the benefit of patient safety. Physicians and nurses may clash due to differences in socialization and educational experiences, as well as a perceived power imbalance. Example An uncivil encounter between Alice, a nurse manager, and Kathy, a staff nurse, is shown in the following scenario.

Kathy is unable—and, to be honest, unwilling—to work two shifts. She’d planned to spend time with her family this evening, despite her exhaustion. She has also worked three more shifts this month. She decides that instead of scheduling a meeting with Alice later in the week, she needs to deal with this matter right now.

Here’s an example of how she could apply the DESC model with her boss.

“Alice, I understand the need to cover the unit due to Nicole’s illness,” she says. We’re all in agreement that having enough staff is critical for patient care.”

 

“I’m exhausted, and I’m less prepared to provide safe, high-quality care because I’ve just covered previous shifts,” explain.

 

“I understand that it is your obligation as manager to ensure that we have appropriate workers for the upcoming shift,” you say. But, because I’m unable to work an additional shift today, I’d like to discuss options.”

 

“Let’s work together to discuss alternatives for covering Nicole’s shift,” says the consequence. It’s critical that I have a say in decisions that affect me.”

 

 

General Notes/Comments  

I suppose that if we start listening to one another again, we can transform the world. Simple, honest, human conversation…a chance to speak, be heard, and listen well to each other also be heard, and to listen well to one another rescue the planet in the end.
