1.      Kevin, a 39-year-old unemployed homeless male who has paranoid schizophrenia, was brought to the psychiatric hospital by the police.  Citizens called the police because Kevin was in the street directing pedestrians and traffic in opposition to the traffic lights and verbally abusing everyone who did not follow his directions.  Kevin is known to the police since he is often homeless, and states that his family does not want him.  Kevin also has a history of poly substance abuse with alcohol, heroin, and crack cocaine, and he has been jailed for public intoxication several times.  The nursing assessment reveals that Kevin has not been taking his prescribed psychotropic medications for 3 weeks.  Kevin states that he does not have any money, and he does not remember where to go for mental health care (Learning Objectives: 2)

In order to find the best information on a topic, not only should you develop a question and search for resources, but you should also know how to analyze the value of the resources that you identify. There are different ways to evaluate resources, such as using the hierarchy of evidence, which you explored in Week 4 of this course. Another way to evaluate resources is to consider the appropriateness of the research design. Understanding how research designs contribute to the quality of a study is essential for being able to analyze resources when conducting a literature review or locating evidence for practice.

In this Discussion, you consider the different research designs and evaluate how these designs have been used to research a specific topic. You also consider strategies for selecting an appropriate research design.

To prepare:

Review the information in the course texton quantitative research designs. Focus on the information in Box 9.1, “Guidelines for Critiquing Research Designs in Quantitative Studies” located on page 210 of the course text.

Attention Deficit Hyperactivity Disorder

ADHD (attention deficit hyperactivity disorder) is a prevalent pediatric psychiatric illness marked by age-inappropriate characteristics of attention deficit, restlessness, and aggression. Incidence and prevalence

  • According to CDC researchers, 6.1 million children aged 2 to 17 years in the United States were diagnosed with attention deficit hyperactivity disorder (ADHD) in 2016, which is consistent to prior figures.

Methylphenidate

  • Methylphenidate (MPH), sometimes known as Ritalin or Concerta, is an FDA-approved medicine that is recommended for treatment of ADHD.

Side effects

  • Appetite inhibition, stomach pain, sleeplessness, and migraine are some of the most prevalent side effects of stimulants, but they are normally endurable enough to proceed taking the drug (Shier et al., 2012).

Bupropion

  • Bupropion is an off-label dopamine and norepinephrine reuptake inhibitor (NDRI) that has been demonstrated to be a viable non-stimulant option for the management of ADHD in teenagers, with multiple reports of excellent results.

Side effects

  • Drowsiness, constipation, vomiting, loss of weight, myalgia, migraine, nervousness, sweating, anorexia, tinnitus, and high blood pressure are all frequent Bupropion side effects, but most of them go away on their own.
  • Because of the lack of evidence of effectiveness and safety, as with any antidepressant, it is critical to evaluate for suicidal thoughts and educate caregivers of the danger so they can keep an eye on their children. Study Guide for ADHD Disorder
  • Suicide is a significant concern for teenagers using antidepressants (Kweon & Kim, 2019).

Nonpharmacological treatments

  • Parent training in behavior management (PTBM), which fosters parent-child psychotherapy and assists parents improve their parental practices and create a better connection with their kids, is one of the most prevalent behavioral therapies.
  • Parents are trained to notice troublesome activities in their kids and to use nonphysical tactics such as commercial breaks to prevent undesired behaviors while rewarding good actions with good focus and appreciation (Shrestha et al., 2020).

Pertinent patient education considerations

  • Parents whose children suffer from ADHD are educated on how to support the child as well as foster their development by;
  • Keeping in touch with the child’s teacher on a frequent basis.
  • Maintaining a daily plan that includes schoolwork, food, and outside activities at regular intervals.
  • Making schedule alterations ahead of time rather than at the last minute.
  • Reducing the number of distractions in the child’s surroundings.
  • Ensuring the youngster eats a balanced, healthful diet that includes lots of fiber and other minerals.
  • Ascertain that the youngster receives adequate rest.
  • Good conduct should be praised and rewarded.
  • Provide the youngster with clear and consistent rules.

Legal and ethical considerations

  • The process of establishing restrictions for children’s consent to treatment, particularly mental health care, is in disarray.
  • Confidentiality is a challenge since, in certain situations, data may be disclosed without agreement due to worries regarding child safety, lawbreaking, or danger to others.
  • Hyperactivity often causes ethical difficulties between beneficence/nonmaleficence and fairness for professionals. Before a diagnosis, instructors may inform therapists that the kid will not be permitted to return to school until treated, or that a diagnosis is necessary before special schooling services are made accessible. Victims with hyperactive impulsivity may seek vengeance privately or officially via the courts.

Strategies

A nursing portfolio is a tremendous asset to building confidence in your career. It preserves your educational, clinical, and professional achievements. It is a documentation of your skills and nursing experiences. One strategy to adopt when creating and enhancing a nursing portfolio is to chronologically detail the history of your professional career (Davis, 2015). Placing the events in order shows clinical development, experience, and growth. Another strategy is to list certifications, credentials, and any continuing education that has improved your career as a nurse in a specific field or with a particular population. A final strategy is to utilize the nursing portfolio as a guide and goal for the future years of your career (Davis, 2015). Establishing promotions you want to achieve,  certifications you wish to obtain, licenses you wish to earn, and courses to improve your nursing licensure will guide your career path (Davis, 2015). Listing these goals shows future employers’ drive and determination. Strategies For Academic Portfolios 

Emphasis on Social Change

The Walden University Center for Social Change is meant to be a connective hub that promotes, facilitates, and supports collaborative alliances (Walden University, n.d.). As a nurse, you can view your colleagues, professors, and mentors as your hub. By networking, you promote, facilitate and support future alliances. Walden University’s social change mission focuses on action research and projects that lead to sustainable positive social change. As nurses, we must be active in policy and quality improvement. We can contribute to this positive social change and add to the community by remaining active. Strategies For Academic Portfolios

RE: Discussion – Week 8

According to Casey and Egan, a professional portfolio should be more than just a resume or curriculum vitae. A professional portfolio should be a collection of documents that provides evidence of how an individual has developed personally and professionally. Providing a portfolio to future employers or workplaces contributes to one’s professional development goals by showcasing an individual’s accomplishments and achievements (2010).

One strategy a student may integrate academic activities and accomplishments into one’s professional goals is by sharing clinical experiences (Hannans & Olivo, 2017). For example, when I begin my field experience requirements for the FNP pathway, I can include the facility where I shadowed for PRAC 6552 Advanced Nurse Practice in Reproductive Health Care Practicum. I can include that this practicum requires 160 hours at the site of choice and a requirement to see 100 GYN patients and 25 OB patients. I can also include that I am required to perform internal vaginal exams and breast exams during this practicum. I can also include any electronic documentation systems I used at this field site (Hannans & Olivo, 2017). Strategies For Academic Portfolios

Another strategy a student can include in one’s professional development plan is an expressed interest to join and actively participate in a professional organization. Professional organizations bring nurses together and allow nurses to use their voices to create change and serve the profession’s community (Walden University LLC., 2018). For example, I recently joined the Delaware Coalition of Nurse Practitioners organization as a student. From this membership, I can be kept up to date on any regulation/policy changes to the nursing field, any upcoming job opportunities, etc.

Sharing one’s clinical experiences in a professional development plan contributes to Walden’s emphasis on social change by supporting the development of knowledgeable scholar-practitioners (Walden University, 2022). Walden’s academic requirements contribute to a student’s professional and educational advancement by using comprehensive, constructive academic programs such as the FNP specialization. Joining a professional nursing organization also contributes to Walden’s emphasis on social change by applying one’s ideas and voice to improve human and social conditions (Walden University, 2022).  Strategies For Academic Portfolios
