Geriatric patients have many nutritional and hydration concerns that impact their health and ability to acquire sufficient nutrients. Advanced practice nurses evaluating these patients must be able to account for all barriers that prevent elders from obtaining adequate nutrition, including medical conditions, transportation, finances, physiologic changes, and functional abilities. When evaluating patients, it is important to consider how they eat, what their diet consists of, and whether they have any special dietary needs that are not being met. Assessment tools, such as the Lawton Instrumental Activities of Daily Living (IADL) Scale, are an integral part of this evaluation process as they help providers identify potential obstacles for patients. In this Discussion, you assess a patient at your current practicum site and consider strategies for improving any nutrition or hydration issues.

To prepare:

  • Review this week’s media presentation, as well as Chapters 28 and 29 of the Flaherty and Resnick text.
  • Assess a patient using tools for inpatient and long-term patient care, such as the Lawton IADL Scale.

Note: You should coordinate this opportunity with the Preceptor at your practicum site.

  • Consider whether nutrition and/or hydration might be impacted by the patient’s functional abilities. Reflect on whether the patient is able to go out and get food to eat, cook meals, safely use the stove, etc.
  • Consider the patient’s diet and whether they have any special dietary needs due to medical conditions, such as congestive heart failure, end-stage kidney disease, diabetes, oral health issues, etc. Reflect on whether or not the patient is attempting to compensate for a medical issue and thus creating a deficiency or excess in his or her diet.
  • Based on your patient assessment, think about strategies for improving any nutrition issues that might have presented (e.g., nutritional supplements, community resources such as Meals on Wheels, referral to a nutritionist or dietician, etc.).

By Day 3

Patient Outreach would like you to select a group and create a lesson plan that will help educate them and help them better understand how to manage or prevent illness or disease. Consider characteristics of culture, ethnicity, and social background that might affect the target group.

The population you choose could be a subset such as the elderly, an ethnic minority, a lower socio-economic group, or one that you have identified from your own observations. She is very devout Catholic and grew up in a strongly devout family. Her church family is very important to her. Her mother and siblings live out of state. All of her family prays regularly for her along with their church and friends.

The patient was very sick as a child requiring frequent hospitalizations. Although her parents were repeatedly told that the patient would soon die from pneumonia, she always pulled through. This caused her family to note her strong-willed nature as a child. Critical Appraisal   

           As advanced practice nurses, it is vital to critically appraise research studies.  Barr-Walker (2017) discusses that critical appraisal is the practice of systematically analyzing research to evaluate its trustworthiness, value, and pertinence in a specific situation (p. 71).  When critically appraising quantitative studies, it is imperative to assess three factors.  

Quantitative Studies 

Validity, reliability, and applicability are three essential components of quantitative studies that must be assessed.  Melnyk and Fineout-Overholt (2015) discuss that the validity of a study addresses whether the results of the specific study were acquired from valid scientific resources (p. 93).  If confounding variables are identified in the study, one can conclude that the findings are in jeopardy.  Melnyk and Fineout-Overholt (2015) explain that quantitative studies utilize statistics to release their conclusions (p. 96).  As a provider, one must evaluate the statistics in order to gain an understanding of the reliability of the reviewed study.  Melnyk and Fineout-Overholt (2015) point out that the primary considerations are the breadth of the intervention’s effect and how accurately that effect was predicted.  Combined, these decide the reliability of the study findings (p. 97).  In order to properly assess for accountability, providers must exhaustingly evaluate if the quantitative study is directly applicable to the specific patient population.  Melynyk and Fineout-Overholt (2015) discuss that each study addresses explicit questions that, when addressed, help providers in critically appraising the study in order to determine its worth in the practice setting (p. 107).  

Although each of these three factors are vital components to assess, applicability is the most important.  If the quantitative study results cannot be directly applied to the specific patient population, the results can simply not be utilized.  Melynk and Fineout-Overholt (2015) emphasize that analysis of quantitative study results requires the provider to consider the clinical significance of the findings (p. 93).  Consequently, if the quantitative study findings are not directly applicable to one’s specific practice setting, the study results lack pertinence.  

Conclusion 

LoBiondo-Wood and Haber (2014) explain that it is essential to initially examine the level of evidence that is supported by the design and how the promising strength and quality of the findings can be utilized to improve or alter practice (p. 177).   As advanced practice nurses, it is key to consistently critically appraise studies in order to boost patient outcomes.  Furthermore, consistent appraisal of quantitative studies will assist in expanding one’s clinical knowledge base.

                                           Initial Post

The  research study’s focus was on the interventions to improve medication  adherence in people with multiple chronic conditions.  The population of  the study consisted of people over 70 years of age and having 3-4  chronic conditions.  A well-defined PICOT question helps to identify the  best available evidence to influence treatment (Echevarria, 2014).  The  PICOT question (P) The research study addressed medication adherence of  individual over the age of 70, (I) research involving eight studies  related to medication nonadherence, (C) Identified effective  interventions to improve medication adherence, and (O) evidence-based  treatment that assists in medication adherence would delay disease  progression.  

Design

             The design was a systematic review model by Cochrane Collaboration to  search, retrieve and appraise the quality and synthesis of the finding  of the RCT’s.  Out of the 248 abstracts only 97 were selected to be used  in full test and after an independent review by two separate reviewers,  only eight were chosen.  According to Polit, The Consort Checklist was  used to obtain the highest scores that could be used to generalize  results to similar groups.  “Consolidated Standards of Reporting Trials)  for reporting information for a randomized controlled trial, including a  checklist and flowchart for tracking participants through the trial,  from recruitment through data analysis.” (p. 723). The researchers used  the Intervention Group, Control Group, and Randomized Control Trials to  come up with the results.  

Results

             The researchers came to their conclusion after conducting a quality  assessment of the randomized, control trials examining medication  adherence using the CONSORT Group.  The research study did identify  weaknesses in the study. Several RTC’s had methodological problems, did  not identify all medical conditions, explanation of interventions used  (William, 2008).  

Alternate Conclusion

             There was a wealth of data in this research study that could be used to  explore several different issues related to medication adherence.   Researchers could have drawn a conclusion based on the data they  identified, what chronic conditions are individual most like or less  likely to be compliant with their medications.  They did find that  further research is needed on this subject.

In this week’s media presentation, Dr. Trautman, Dr. Wakefield, and Ms. Coyle discuss how public policies initiated at the national or state level ultimately influence what occurs at the institutional and local level of nursing practice and health care delivery. Health care policy usually is developed to address health care cost, quality, or access, or a combination of the three. Due to the nature of their interaction with patients, nurses are well situated to be effective, knowledgeable advocates for their patients.

In this week’s Discussion, you will examine how current policies impact how you as a nurse provide health care. You will then select an issue of interest and determine how you could advocate for policy in that area.
