We also saw when the nursing leader confronted the nurse, the nurse lied several times about the situation, only confessing to going to the break room to get coffee after seeing the videos. 

So, now, consider the aspects we discussed in our post-sim debriefing and make your own conclusion, if you were the immediate supervisor of this nurse, what would your course of action be? Ann is a seventy-seven-year-old grandmother. She has one daughter and three grandchildren. Two years ago, she was diagnosed with moderately advanced Alzheimer’s disease, which causes her to have periods of confusion, frustration, anger, and obsessive thinking. In an emergency evacuation of patients, training plays an important role. Here is an incidence of emergency evacuation of Phillips County Regional Medical Center discussed by Vogt and Sorenson (1999):

Shortly after 1:00 p.m. on Thursday, May 8, 1997, clouds of foul-smelling smoke began pouring from an herbicide and pesticide packaging plant in West Helena, Arkansas. An alert was sounded, employees evacuated, and the West Helena fire department was called. As three firefighters prepared to enter the plant, the chemical compounds exploded, collapsing a solid concrete block wall, and killing all three firefighters. As the odorous smoky cloud drifted away from the plant, authorities ordered residents in a 2-mile area downwind of the plant to evacuate and those in the 2- to 3-mile zone to shelter in . . . The findings indicate that 90% of those that were told to evacuate did so but only 27% of those told to shelter-in-place did so, with 68% opting to evacuate instead. The implications of these findings for emergency managers is that people will likely choose to evacuate when both warnings to evacuate and warnings to shelter are issued to residents in close proximity to each other (Abstract section, para. 1).

The evacuation of the medical center was facilitated by efforts taken 6 months earlier to update the hospital’s evacuation plan and reconfirm support agreements for relocation sites and supplies. At that time of updating the plan, the entire staff had also participated in a mock drill. When the safety officer observed the buildings across the highway being evacuated and started questioning officials on the possibility of also being evacuated, evacuation plans familiar to staff were in place and had been practiced. When the Director of Nursing gave the “Code White” alert (the signal that an evacuation to an off-site facility would follow) staff was amply prepared to move patients (p. 11).

Interviews with staff indicated that all patients that could be discharged were sent home with a physician’s nurse. Another four or five patients (the most seriously ill) were transferred to a hospital about a half hour away. The majority of the patients 11 (approximately 17) in the rehabilitation unit were transferred to a vacant wing of the Crestpark Nursing home accompanied by hospital support staff. Only one maternity patient evacuated to the Phillips College—Community College where the hospital staff had evacuated and opened emergency room services. The evacuation of patients began at 1:40 p.m. and all patients (except for two on ventilators and one being stabilized in the emergency room) were out 55 minutes later. The remaining three patients were evacuated 15 minutes later. A variety of vehicles transported patients—vans, school buses, ambulances, private cars, and mortuary services. As a precaution, National Guard personnel provided security at the evacuated hospital (pp. 11–12).

Introduction

With the increasing development of technology more and more health research is being done. Although many of the research is beneficial, much of it also causes ethical issues. One of the major cases currently being discussed is the effects of researching vaccines on children.

Vaccination Controversy  

Today in the medical world, nothing can be done without informed consent.  Informed consent is defined as “a person knowingly and intelligently, an in a clear and manifest way, gives consent (Fouka &Mantzorou, 2011, pg. 2).” Informed consent is required before blood administration, procedures and surgery as well as many other things. In the case of the pediatric population they are unable to be informed consenters so this makes them a vulnerable population ( Fouka, et al. 2011 pg.7). When it comes to the topic of vaccines, children are usually the subject of the study. “The different opinions about their participation in research can be attributed to their inability to give an informed consent and to their need for further protection and sensitivity from the researcher as they are in a greater risk of being deceived, threatened or forced to participate (Fouka, et al. 2011, pg. 7).”  The problem that many researchers run into is that they are forced to use children because most disease are developed in the pediatric year, but children also experience more adverse effects than adults leading to research controversy (Kulkarni, 2013 para 2).  However, in order to have the best practice you need evidence based practice, which requires valid research (Laureated Education, 2012).

Conducting research differently

            Researcher who have used children in their research may have not had any other option. Althoug, better practice may to be to involve the children in the research process. They can do this by not only explaining the process and dangers to the parents, but also explaining it to the children. “In the case of a child, every effort should be made to explain to him/her also, in language that is understandable to the child, what the participation means, as regards to potential risks (discomfort, time spent, etc) and benefits. The investigators should document the child’s assent (Kulkarni, 2013 para 23).” Including the child in the informed consent process makes the study more ethical as well as making sure the child is not being threatened or forced into anything. Including the child in the process also helps make it less scary for them. It is important that the child understand what is going on and it be explained to the at their level.

Evidence Based Practice

Although there are many controversies associated with research, it is necessary to find the best practice. In the nursing profession, evidence based practice is essential in knowing what is the best product (Laureate Education, 2012h).

Pulmonary Mucormycosis is a fungal infection that is caused by breathing in fungi spores from mucormycetes, whichis found in organic matter such as soil (CDC. 2015). Mucormycosis can develop in  many areas of the body, but for this question we are focusing on pulmonarymucormycosis. “Pulmonary mucormycosis is the most common type of mucormycosis in people with cancer and in people who have had an organ transplant or a stemcell transplant (CDC.2015.p.3)”.                       My Goals and Walden University Vision and Mission

            In 2011 I graduated from Nursing School.  I knew I wanted work in the Emergency Department.  I had finished my practicum in an ED and was slotted to be hired once I passed my NCLEX.  As life would have it, I wouldn’t be able to take that job until year later.  In the meantime, I toughed it out as a floor nurse, and honed my skills.  I knew that floor nursing was not in my 10-year plan, so I made it a point to get all the experience I could to prepare myself for a career as a nurse. My journey since becoming an RN has been an amazing one.  I have completed Critical Care fellowships, worked beside some of the finest physicians, and now with all my experiences, I need to become a Nurse Practioner.  I am currently working in the Operating Room and have absolutely fallen in love with Orthopedics.  Becoming an Orthopedic Nurse Practioner will allow me to treat patients in clinic as well as assist the surgeon to increase mobility in those requiring surgery.

            I found Walden University during a school fair at my current hospital.  I spoke with a representative who, in my opinion, made the choice very easy.  I did research other online programs, but I always came back to Walden University.  The ability for me to attend a program and still be able to work full time was a must.  I felt like this University had my best interest in mind after reading the School of Nursing’s (SON) vision and mission statement.  The mission statement states “Walden University provides a diverse community of career professionals with the opportunity to transform themselves as scholar-practitioners so that they can effect positive social change.”  (Walden University, 2011a) I feel as though Walden University will assist me to be in control my transformation from Registered Nurse to Nurse Practioner.  

Incorporation of Social Change

            Everyday our society experiences change whether swift or gradual.  I want to promote positive social change by integrating Walden University’s views with my own.  “Positive social change results in the improvement of human and social conditions.” (Walden 2011b, para.1) I want to be part of that change, I want to be part of the improvement, I want to use my knowledge and experience to help improve the way we live together as human beings.  I feel like Walden University will provide me with the tools needed to make this vision a reality.

Once you have received feedback/approval of your selected problem:

• Reflect on the topics you are examining in this course and consider how they relate to your selected problem. Plan to analyze the problem using principles, theories, and concepts related to leadership and management, as well as other pertinent course themes.

• Review evidence-based literature for data related to your selected problem. You will need to use at least five current (not more than five years old), peer-reviewed articles. Your Instructor may provide some guidance for your literature search.

• Synthesize findings from the research literature and/or drawn from other sources of evidence.

• Develop evidence-based recommendations for addressing the problem.

Write a 6-page paper (not including title page, references, or any appendix) that addresses the following:

• Introduction/Statement of the problem (1/2 page)

• Review of the literature for potential solutions to the identified problem

• Synthesis of the evidence as applied to the stated problem.

• Strategic plan for suggested courses of action based on your evidence and including application of a change theory.

• Conclusion/Summary

Reminder: The School of Nursing requires that all papers submitted include a title page, introduction, summary, and references. The Sample Paper provided at the Walden Writing Center provides an example of those required elements (available at http://writingcenter.waldenu.edu/57.htm). All papers submitted must use this formatting.

The completed paper is due by Day 7 of Week 8.
