Calculating Descriptive Statistics

There are two major classes of statistics: descriptive statistics and inferential statistics.

Descriptive statistics are computed to reveal characteristics of the sample data set and to

describe study variables. Inferential statistics are computed to gain information about

effects and associations in the population being studied. For some types of studies,

descriptive statistics will be the only approach to analysis of the data. For other studies,

descriptive statistics are the fi rst step in the data analysis process, to be followed by inferential

statistics. For all studies that involve numerical data, descriptive statistics are

crucial in understanding the fundamental properties of the variables being studied. Exercise

27 focuses only on descriptive statistics and will illustrate the most common descriptive

statistics computed in nursing research and provide examples using actual clinical

data from empirical publications.

MEASURES OF CENTRAL TENDENCY

A measure of central tendency is a statistic that represents the center or middle of a

frequency distribution. The three measures of central tendency commonly used in nursing

research are the mode, median ( MD ), and mean ( X ). The mean is the arithmetic average

of all of a variable ’ s values. The median is the exact middle value (or the average of the

middle two values if there is an even number of observations). The mode is the most

commonly occurring value or values (see Exercise 8 ).

The following data have been collected from veterans with rheumatoid arthritis ( Tran,

Hooker, Cipher, & Reimold, 2009 ). The values in Table 27-1 were extracted from a larger

sample of veterans who had a history of biologic medication use (e.g., infl iximab [Remicade],

etanercept [Enbrel]). Table 27-1 contains data collected from 10 veterans who had

stopped taking biologic medications, and the variable represents the number of years that

each veteran had taken the medication before stopping.

Because the number of study subjects represented below is 10, the correct statistical

notation to refl ect that number is:

n 10

Note that the n is lowercase, because we are referring to a sample of veterans. If the

data being presented represented the entire population of veterans, the correct notation

is the uppercase N. Because most nursing research is conducted using samples, not populations,

all formulas in the subsequent exercises will incorporate the sample notation, n.

Mode

The mode is the numerical value or score that occurs with the greatest frequency; it does

not necessarily indicate the center of the data set. The data in Table 27-1 contain two

ITS VERY IMPORTANT TO MEET THE COMPETENCES; PLS READ THE COMPETENCES !

Investigate your organization’s compliance rate with one specific core measure of your choice and write a 2-3-page (maximum) executive summary analyzing the financial and regulatory impact of organizational compliance with your chosen measure. Include recommendations for improving performance on the measure.

In order to promote safe, caring, patient-centered environments, nurse leaders must both conceptually understand and be proficient at performing a wide variety of leadership skills. Effective nursing leaders develop and maintain awareness of issues that emerge at many levels within and outside of their organizations. In addition to developing self-assessing competencies necessary for their roles, nurse leaders must also acquire organizational and systems leadership skill sets.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

  • Competency 4: Policy, Finance, and Regulations – Understand the scope and role of policy, finance, and regulatory environments in relationship to individual and population outcomes.
    • Compare the compliance rates of a specific measure for different organizations.
    • Explain the importance of compliance in terms of policy, regulation, and finance.
  • Competency 5: Communication – Communicate effectively with all members of the health care team, including interdepartmental and interdisciplinary collaboration for quality outcomes.
    • Write coherently to support a central idea with correct grammar, usage, and mechanics as expected of a nursing professional.
  • Competency 6: Organizational and Systems Management – Apply knowledge of organizational behavior, nursing theory, and systems (micro- and macro-) as appropriate for the scope and role of one’s own practice.
    • Explain strategies to overcome described organizational barriers to change.
  • Competency 9: Professional Role – Incorporate the qualities, skills, behaviors, and knowledge required to function as a patient advocate, practice high-quality care, assess and evaluate patient outcomes, and provide leadership in improving care.
    • Identify the stakeholders affected by changes to a specific compliance measure and describe changes that would improve compliance of a specific measure.

Assessment Instructions

For this assessment, write an executive summary analyzing the financial and regulatory impact of organizational compliance with a specific core measure of your choice. The summary must include recommendations for improving performance on the measure.

You must be able to explain the financial implications associated with the measure, as well as its importance in terms of policy, regulation, and finance. This is another good opportunity to consult with a key stakeholder within the organization. Talking to someone who works closely with the organization’s finances can help you develop a deeper understanding of the financial aspects of compliance. You also need to suggest ways your organization can improve compliance and how you would implement changes and overcome obstacles.

This may seem like an enormous undertaking, but you should approach it as if you were to present this information to your supervisor or manager. Keeping in mind that people in these positions do not have time to read a 10–12-page report, you would typically give them only an executive summary of your analyses and recommendations. That means that you will need to be very clear and very concise with the information you choose to include in the executive summary. Say as much as possible with as few words as possible.

Directions

Select a core measure and investigate your organization’s compliance rate with the measure. In your executive summary, address the following:

  1. Compare the compliance rates of your organization with those of other organizations.
  2. Explain why the measure is important in terms of policy, regulation, and finance.
  3. Describe changes that could be made that would improve compliance with the measure.
  4. Identify the stakeholders who would be affected by the changes.
  5. Identify obstacles you would expect to encounter and explain strategies to overcome these obstacles.

Additional Requirements

  • Format: Include a title page and references page. As this is an executive summary, do not use APA style and formatting. To determine the appropriate format, it may be helpful for you to review the documents used in your organization. How do they look? What kind of language is used in memos? These can help guide your development of this executive summary. Certainly, feel free to use bullet points as appropriate.
  • Length: Write a 2–3-page (maximum) summary of your information, not including the title page and references page.
  • References: You must still reference scholarly resources to support your work. Use at least three current scholarly or professional resources.
  • Font: Use 12-point, Times New Roman font.

(turnit in report requested)

Instructions:

As you have learned, the population segment aged 65 and older is rapidly aging in the United States. As a result, new ethical challenges are emerging in the field, specifically in long-term care (i.e. skilled nursing facilities including those with post-surgical short-term rehabilitation services).

One area of concern is the challenge of providing adequate healthcare services to a growing (and aging) population with limited resources. This dilemma is expected to increase as the population continues to increase. As a result, the demand for medical services increases; however, with finite healthcare resources, providing services to everyone who requires care is not possible.

This will present a unique challenge for nursing home administrators because they are responsible to ensure that resident needs (including receiving healthcare services) are being met. Determining who should receive the necessary care that is available will be an interdisciplinary challenge indeed.

Scholars have posited a variety of allocation methods; however, these principles have not been without controversy. One of the most recent principles introduced is the Complete Lives System. This method is unique, because unlike its predecessors that relied on a unilateral perspective, the Complete Lives Systems takes a multi-allocation system approach.

In this unit, you will combine your previous knowledge obtained throughout the course and apply clinical reasoning to evaluate moral action and its relevance to ethical-decision making in long-term care. Using ethical decision-making, you will determine the best method of allocating limited medical resources to a growing and aging population. By the end of this unit, you will be able to analyze the effect ethics has on day-to-day operations as well as long-term policy and procedure in a healthcare environment.

Please review the following resources and using specific information from these resources, your course materials, and additional research, address the tasks posed below.

Three individuals in town require a hip surgery, followed by extensive rehabilitative therapy. Physical and occupational therapy will be provided at the skilled nursing facility where the individual will reside until able to return home. Below are details of each individual.

  • Donna Mueller is a 58-year old disabled widow with no children. She spent her career as a homemaker and taking care of her husband. Donna requires surgery after falling at home while washing windows. Prior to the accident, Donna was relatively independent, although a stroke a few years prior left her partially paralyzed. She lives alone and has minimal company over.
  • Steve McDonald is 40-year old unemployed musician. He has a teenage son who lives with the son’s mother in a neighboring town. At the age of 19, Steve left college to pursue a career as a drummer. For the past two decades, Steve has worked odd jobs while pursuing his dream. Although his perseverance has not landed him national attention, he has performed with a few, known bands in the area. Recently, while exiting stage left from a nightclub performance, Steve tripped over the dark stairs and fractured his hip, ultimately leaving him unemployed as he is currently unable to perform.
  • Chris Snider is a 73-year old entrepreneur and business owner. Never married, Chris spent his career in the fast-pace, produceor perish industry of nanotechnology where he employs over 5,000 workers ranging from maintenance technicians to senior research engineers. Chris requires surgery after being innocently hit with a runaway golf cart on the back 9 with his visiting, foreign affiliates. Chris was completely independent prior to the accident. He lives alone; however, his business requires travel.

In a 6-8 page paper (excluding title page and reference list), address the following:

  • Considering the aging population and existing challenges, discuss the potential role of allocation principles in the American healthcare system.
  • Discuss the potential implications these principles have on the aging population (consider potential violations of federal antidiscrimination laws and ethical principles about fair treatment).
  • Based on the Complete Lives System, discuss which individual would most likely receive the surgery.
  • Based on your moral compass, ethical standards, and healthcare laws, discuss which individual you would recommend for surgery.
  • Recommend the one allocation principle you would prefer as the primary medical intervention for a growing (and aging) population.

Report your findings noting the following standards:

  • Current APA formatting (e.g. title page, citations, conclusion, reference page, etc.) should be used.
  • Proper grammar, spelling, and punctuation are expected.
  • Plagiarism, self-plagiarism, unoriginal work, and unattributed content is not permitted and will result in action pursuant to the University’s Plagiarism Policy and Procedures.

                                 Discussion: What Can Nurses Do?

Many people, most of them in tropical countries of the Third World, die of preventable, curable diseases. . . . Malaria, tuberculosis, acute lower-respiratory infections—in 1998, these claimed 6.1 million lives. People died because the drugs to treat those illnesses are nonexistent or are no longer effective. They died because it doesn’t pay to keep them alive.

–Ken Silverstein, Millions for Viagra. Pennies for Diseases of the Poor, The Nation, July 19, 1999

Unfortunately, since 1998, little has changed. For many individuals living in impoverished underdeveloped countries, even basic medical care is difficult to obtain. Although international agencies sponsor outreach programs and corporations, and although nonprofit organizations donate goods and services, the level of health care remains far below what is necessary to meet the needs of struggling populations. Polluted water supplies, unsanitary conditions, and poor nutrition only exacerbate the poor health prevalent in these environments. Nurses working in developed nations have many opportunities/advantages that typically are not available to those in underdeveloped countries. What can nurses do to support their international colleagues and advocate for the poor and underserved of the world?

In this Discussion, you will consider the challenges of providing health care for the world’s neediest citizens, as well as how nurses can advocate for these citizens.

                                                  To prepare:

Consider the challenges of providing health care in underdeveloped countries.

Conduct research in the Walden Library and other reliable resources to determine strategies being used to address these challenges.

Using this week’s Learning Resources, note the factors that impact the ability of individuals in underdeveloped nations to obtain adequate health care.

Consider strategies nurses can use to advocate for health care at the global level. What can one nurse do to make a difference?

The Strain of Increasing Patient Populations on Primary Care

One stressor affecting healthcare on a national level is increasing strain that is being placed on primary care providers. In an article published in the journal “Managed Care” titled Primary Care Continues to Feel the Strain (2012), the authors explain that providers in the primary care setting

currently only spend 15 minutes on average with each patient. With the increases in patient needs, these practices simply cannot handle taking on more patients. The demands placed on providers to incorporate preventative care into the treatment of a patient’s current illness or chronic condition are

making it difficult for providers to maintain quality care in such a short visit with a patient.

My current work setting is a primary care office, that provides care to patients on a sliding fee which based on income. We work to serve the uninsured and underinsured population of Eastern Idaho. With five providers currently seeing an average of forty patients a day each, our office is feeling

the strain of increasing patient populations. This strain not only affects the providers, but has added strain to the nursing staff, reception staff and medical billers.

Currently our providers each have their own patient population, where for primary care needs, the patient only sees the one provider. A patient needing refills on his metformin or lisinopril can only be seen by the provider that prescribed that medication currently. The problem this is creating is

the inability to schedule these patients for a follow up before they are out of refills due to a provider’s schedule being booked for weeks out.

The solution that has been implemented to aid in this problem is the use of nursing teams. Each provider has his or her own team of nurses. As the lead nurse for my physician, patient phone calls, questions and refill requests come to me first. We also have a care coordinator that helps to

manage patient referrals and making sure imaging appointments are made and reports are uploaded to the patient chart. In chapter 3 of the course text, it is established that successful teams use each member’s strengths to benefit of the team and to the quality of patient care (Marshall & Broome,

2017).

Our success with these small collaborative teams is the main reason the use of primary care collaboration is being considered. This involves the use of a primary care team, instead of a single provider. Each team including a physician and one or two other providers (either nurse practitioners or

physician assistants). A literature review performed by Norful, de Jacq, Carlino &Poghosyan (2018) identified that in order for this type of arrangement to be successful, the members of the primary care team would be required to not only communicate and respect each other’s medial knowledge

and ability, but also develop a way to align their clinical methods and beliefs about medical care.
