Organizational Foundations

As you strive to grow in your leadership skills and abilities, you will find that the context in which you work influences your motivation and areas of focus. In a similar vein, your commitment to developing professionally can contribute toward organizational effectiveness.

To that end, it is critical to recognize the importance of organizational culture and climate. In particular, through this week’s Learning Resources, you may consider several questions: How do an organization’s mission, vision, and values relate to its culture? What is the difference between culture and climate? Moreover, how are culture and climate manifested within the organization?

For this Discussion, you explore the culture and climate of your current organization or one with which you are familiar. You also consider decisions and day-to-day practices and the way they relate to the organization’s mission, vision, and values.

To prepare:

• Review the information related to planning and decision making in health care organizations presented in the textbook, Leadership Roles and Management Functions in Nursing: Theory and Application. Consider how planning and decision making relate to an organization’s mission, vision, and values, as well as its culture and its climate.
• Familiarize yourself with the mission, vision, and values of your organization or one with which you are familiar. Consider how the statements and actions of leaders and others within the organization support or demonstrate the organizational mission, vision, and values. In addition, note any apparent discrepancies between word and deed. Think about how this translates into expectations for direct service providers. Note any data or artifacts that seem to indicate whether behaviors within the organization are congruent with its mission, vision, and values.
• Begin to examine and reflect on the culture and climate of the organization. How do culture and climate differ?
• Why is it important for you, as a master’s-prepared nurse leader, to be cognizant of these matters?

Review the following case and respond to the questions below as a nurse case manager who is doing a safety assessment in the home:

Mrs. F., an 84 year old woman, has lived at this home all of her married life (60 years). Her husband had a recent stroke and is now at a local rehab/skilled nursing facility. The stroke was mild but did affect his right side so he has trouble walking and eating. Mrs. F. still drives and visits him twice daily bringing him food to eat and takes care of him while she is there. Plans are that he will be in the facility for another week and then he will come home. Mrs. F. has arthritis and walks with a limp. She does not use any assistive devices because those are “for sissies”. She was recently diagnosed with COPD as she was a smoker for many years. She believes that she is well and can still do anything around the house particularly take care of her husband. They have 7 children who all live in the general area and want to help as much as they can. Mrs. F. discourages their help because she believes she and her husband can do “just fine”. The house is a two story and quite old. It has not been updated for many years. There are a lot of old antiques and the house is crowded with many “memories” such as photos, crafts, rugs, and small pieces of furniture Mrs. F.’s husband made.

1. In your assessment, identify any safety risks for Mrs. F. in her home and those that may be a challenge when her husband returns from Rehab.
2. Determine what might be done to manage these risks and make the home safe.
3. Describe any other safety issues in this situation and what might be done about them.
4. Explain how you will work with Mrs. F. to discuss the risks and how you can encourage her to make her home and life safer. What obstacles might you encounter in doing this? How will you overcome them?

Collaboration is a critical component of quality improvement. It is also a central tenet of your endeavors as a scholar-practitioner in this Walden University Master’s program. To succeed in your role as a nurse leader-manager, it is important to engage in ongoing development of collaborative skills and to help foster a culture that values collaboration.

In Weeks 1 and 2 of this course, you analyzed your strengths and considered how to leverage these strengths for effective leadership. In a similar vein, this week you evaluate your strengths and areas for growth with regard to interdisciplinary collaboration.

To prepare:

• Review the competencies described in the Interprofessional Education Collaborative report and in Chapter 9 of the Hickey and Brosnan text.
• As you reflect on the competency statements outlined for each domain, think about which ones reflect your strengths. Which competency statements point to areas for growth?
• Consider the benefit of working with interdisciplinary teams for addressing quality improvement initiatives.
• Conduct a search of the literature and select at least one article that illustrates the benefits of interprofessional collaboration for nurse leader-managers as it relates to improving quality.

Note: Think about a potential connection between the concepts presented throughout this course (i.e. systems-level change, quality, etc.), and a Practicum Project that you might focus on in NURS 6600. Consider how you might use interprofessional collaboration in your practicum setting.

By Day 3

Post an analysis of your strengths and areas for growth related to interprofessional collaboration competencies. Explain the benefits of interprofessional collaboration for meeting quality improvement goals. Support your response with examples from current literature.

Discussion: The Nurse’s Role in Promoting Quality Health Care

As you have noted in this course and in your practice, nurses have an integral role in promoting a just culture focused on patient safety in their health care organizations. Determining resources to prevent error and improve patient safety can often be a battle, and this scenario illustrates how one organization makes strides to make sure resources are available in the interest of patient safety. As you review the scenario, consider the nurse’s role in promoting a just culture in your practice setting.

By Day 3

Post a description of the role of the staff nurse for promoting quality in a high-performing health care organization. Define the organizational culture of your health care organization, or one with which you are familiar, and identify the leadership style of the CEO and CNO in your organization. Explain how your senior leadership supports—or does not support—changes that require financing/budget to support patient safety. Describe the strategies you might be able to implement to move your health care organization to a high-performing organization.

Support your response with references from the professional nursing literature. Your posts need to be written at the capstone level (see checklist)

Practice Experience: Developing a Quality Improvement Plan

Storyboard Instructions

The Practice Experience Project Storyboard is a brief, visual summary of a completed quality improvement initiative. The storyboard highlights key aspects of a quality improvement effort by documenting the Practice Experience Project from beginning to end. 

The course template uses a PowerPoint format to complete this assignment. You only need to provide two slides, no more. The first slide is the summary of your project; the second slide is the reference page. You will need to choose the most pertinent information from your practice experience and Capstone Paper to complete the Storyboard. No voiceover is required.  

The completed storyboard is submitted for grading and also serves as the main post for the Week 6 Practice Experience discussion. 

Heritage Assessment

The RN to BSN program at Grand Canyon University meets the requirements for clinical competencies as defined by CCNE and AACN using nontraditional experiences for practicing nurses. These experiences come in the form of direct and indirect care experiences in which licensed nursing students engage in learning within the context of their hospital organization, their specific care discipline, and their local communities.
