Conduct a “Windshield Survey” in a section of your community. Instructions for the survey can be found in Stanhope and Lancaster (2016) on page 416, Table 18-6. As you notice, conducting a Windshield Survey requires that you either walk around or drive around a particular section of the community and take notes about what you observe. A Windshield Survey cannot be conducted by reviewing websites or Google Earth only. It requires actually taking a look at the selected area of the community. This survey should be focused on the problem and population you have selected for your practicum project. If you choose, for example, obesity among Hispanic schoolchildren, you might want to locate a section of the community where many Hispanic children live, or you might want to conduct the Windshield Survey around where Hispanic children attend school. If Hispanic children are not found in a specific section of your community (e.g., Chinatown in San Francisco or Harlem in New York), then you may select the section of the community where you live or work but pay particular attention to your practicum population and practicum problem as you conduct a survey of the community as viewed through the eyes of the public health nurse.

By Day 7 of Week 3

Submit a 3- to 4-page paper including:

  • Introduction to the community, including the name of the community and any interesting or historical facts you would like to add about where you live
  • Photographs of the selected area of the community that serve as evidence of your observations and hypotheses
  • Windshield Survey findings, including a description of the section of your community that you chose to survey
  • Description of the Vulnerable Population and Available Resources
    • Demographics of the vulnerable population
    • What social determinants create their vulnerable status?
    • What community strengths exist to assist this population?
  • Conclusions based on Nursing Assessment of the Community
    • Based on what you have found, what conclusions can you draw about your community and your selected population for your practicum?
  • Select at least 5 scholarly resources to support your assessment. Websites may be included but the paper must include scholarly resources in its development.

For this Assignment, review the following:

  • AWE Checklist (Level 4000)
  • BSN Program Top Ten Citations and References
  • Walden paper template (no abstract or running head required)

  APA format with citations.

  No Plagiarism.

As part of the class on Health Policy and Politics, you are required to write a letter to their legislator. The letters to the legislator will be evaluated for content, form, and clarity.

This assignment will help the student become an active participant in the policy-making arena, which directly affects the nursing profession and our country at large. Included are suggestions for effective written communication to your legislator. The importance of letter writing should not be underestimated as it has been rated by Congress as the number one means of communicating with the legislator. Letters that are written to the legislator are usually reviewed by staff and the various view points are tabulated. Letters with significant opinions or information are directed to the legislator for individual consideration. It is important to note that form letters and post cards receive the least amount of attention from the legislator. Therefore, it is most appropriate to create a letter which is thoughtful and reflects an informed opinion.

You (the student) are required to create a letter to the legislator in your home district related to a current senate/house bill. You will choose a current health care legislative issue and draft a well written letter to your legislator discussing your opinion regarding the bill.Search http://mlis.state.md.us/#bill to find a health care related bill of interest to you. Attach a brief summary of the bill along with your letter.

When writing your letter, always refer to the issue by its bill name and number. You can look up the name and address of your local legislator at http://mlis.state.md.us. The following page outlines is a list of specific do’s and don’ts that you should consider when writing to your legislator. projet 1

While treating the chronically ill, a major challenge is developing a plan of care that addresses the specific needs of a patient and a caregiver. You need to be in close touch with patients and their support group, family and peers, to come up with an ideal plan.

In this course project, you are going to develop a plan of care for a chronic illness group of your choice using the Healthy People 2020 Topics. While executing the tasks of this project, remember that while you need to give a general overview of the biomedical considerations of the case, the focus should always be on the psychosocial elements. Your perspective in this care plan should be the patient’s goals rather than those of the medical team.

Each week, you will complete a part of this project. You will submit a final completed project in Week 5. Be sure to take the time to carefully complete each of the weekly assignments and then put it all together in Week 5.

Ensure that you save a copy of this course project after you have submitted it at the end of this course. You are expected to resubmit this project along with the other course projects at the end of the Registered Nurse (RN) to Bachelor of Science in Nursing (BSN) program. File Transfer Protocol (FTP) details will be provided in the Capstone course.

Identifying a Group

Identify an area of chronic illness of specific interest to you and that is represented as a Healthy People 2020 topic (healthypeople.gov). Explain your choice and your interest in it. Prepare a questionnaire utilizing your knowledge of health and illness, with the aim of acquiring all information you need from patients to prepare a plan of care for the specific illness group. Submit your information in a 4- to 5-page Microsoft Word document. In addition to your questionnaire, be sure to include the following details in your paper.

  • Chronic Illness of interest
  • Morbidity and comorbidity of the disease
  • Impact of the chronic illness and patient morbidity and the affect to overall health of nation
  • Healthy People 2020 goals and objectives for specific illness group

Support your responses with examples.

On a separate references page, cite all sources using APA format.

Promoting Health Care Quality-  Quality Improvement Issue

To help you move forward with your Course Project, this week you identify a quality issue that you would like to address and submit a description of this issue for Instructor feedback.   Your proposed issue should reflect a genuine need for improvement within your selected health care organization. Through your Course Project, you will analyze this issue and propose strategies to improve outcome(s), including the redesign of a related process.

To prepare: •Review the six quality improvement aims from the Institute of Medicine (IOM) presented in Chapter 3 of the Sadeghi, Barzi, Mikhail, and Shabot text.

•Conduct a search of the literature to help you identify a specific issue that warrants attention and action to promote quality improvement.

•Consider what you have surmised thus far through your analysis of the organization that you selected. What have you noticed that could help you identify a quality-related need for improvement? For instance:

•What has captured your attention in meetings, reports, and/or daily activities?

•Have you noticed discrepancies between activities in the organization and recommendations in the research literature, quality standards, and/or the organization’s stated policies and procedures?

•If so, what do you think could be the reason(s) for variations or gaps between what you have observed and what is recommended?

•Identify a quality improvement issue that you would like to investigate for this project.

The instructions for this assignment asks for students to identify a quality improvement issue, and provide a description of the issue they wish to pursue. This can be accomplished in 2 paragraphs.

PAPER proposal for a specific program for older adults

Develop a proposal for a specific program for older adults and/or their families. Assume that your budget is flexible but consider real world limitations to implementing the program.

What is your theoretical and research informed rationale for the program you propose?

Who will be targeted to participate?

Who will staff the program?

What will their qualifications be?

What pre-service and in-service training will they receive?

What obstacles do you anticipate encountering with the development and implementation of the program?

How will you determine the effectiveness of the program?

How will you market the program (advertising, a logo, the name of the program, etc.)?

How will you meet the needs of diverse constituents served by the program?

This must be an original proposal and may not duplicate one prepared for another course. To make sure you cover every important topic.

I suggest you organize your paper with the following subheadings:

Goals of the Program

Factors to consider (market considerations, laws and regulations, financing)

To what extent will your program promote aging in place?

Coordinate services?

Provide new services?

Target different populations?

Integrate services with other agencies or programs

How will you finance your program?

Who benefits from the program

Who will perform the tasks, and what do tasks entail?

Explain how you would treat and/or dress this wound based on guidelines for treatment.

2 page paper, wounds

Skin wounds are sometimes challenging for health care providers to diagnose and treat as many have similar presentations. For advanced practice nurses, being able to identify various types of skin wounds, including whether a wound is a colonization or an infection, is critical because it impacts recommended patient care. In your role, you must be able to evaluate skin wounds, determine the diagnosis, and develop an appropriate treatment and management plan according to current evidence-based guidelines.

To prepare:

  • Review Chapter 45 of the Flaherty and Resnick text, as well as the Burr article in this week’s Learning Resources.
  • Consider how to properly diagnose skin wounds in frail elders, including how to distinguish between a colonization and infection.
  • Select a type of skin wound, such as bumps, bruises, shingles, herpes, bullous pemphigoid, Stevens-Johnson syndrome, etc. Research the guidelines for treatment of the skin wound you selected. Reflect on how you would treat and/or dress this wound.
  • Think about factors that might contribute to the development of the skin wound you selected. Consider strategies for the prevention and improvement of this type of wound.

To complete:

Write a 2- to 3-page paper that addresses the following:

  • Explain how to properly diagnose skin wounds in frail elders, including how to distinguish between a colonization and infection.
  • Describe the type of skin wound you selected.
  • Explain how you would treat and/or dress this wound based on guidelines for treatment.
  • Explain factors that might contribute to the development of the skin wound you selected. Include strategies for the prevention and improvement of this type of wound.

Re:Topic 3 DQ 1

First I would need to find evidence-based research that supported my idea and the need for practice change. Ultimately supporting how this would improve patient care (Huber, 2014). Selling an idea to higher upper management can be intimidating and difficult but not impossible. The key to selling an idea is effectively communicating and making your statement clear, simple, and quick. To sell the idea the presenter must be creative and find common ground that can help relay the message across. The idea must first be presented in an informal manner to see what feedback can be attained before the formal presentation. This type of technique can help the presenter anticipate some questions that might come up when formally selling the idea to upper management. The formal presentation should consist of a power point and handout.

Lab Manager suggests the following when selling an idea to upper management (Lab Manager, 2010).

1. Summarize the idea

2. Define the need for the idea. What problem will it solve?

3. Explain how the idea will make money for the company. Are there other benefits as well?

4. Define the resources needed. What will be needed to develop and commercialize the idea? How much money will it cost? How much time will it take?

5. Request a commitment. Following up on your presentation may be required to get a definite, timely decision on whether it will be funded.

For this Assignment, you apply systems theory to the examination of a problem in a department or a unit within a health care organization. (Note: You may use the same problem you identified for the Discussion as long as it meets the criteria for this assignment.)

To prepare:

· Review the Meyer article, “Nursing Services Delivery Theory: An Open System Approach,” in this week’s Learning Resources. Focus especially on the information presented in Table 1 (p. 2831) and Figure 2 (p. 2833).

· Reflect on your organization or one with which you are familiar. Within a particular department or unit in this organization, identify a problem the staff is encountering.

· Using Table 1 in the Meyer article as a guide, analyze the department or unit, identifying inputs, throughput, output, cycles of events, and negative feedback. Consider whether the problem you have selected relates to input, throughput, output, cycles of events, and/or negative feedback.

· Think about how you could address the problem: Consider what a desired outcome would be, then formulate related goals and objectives, and translate those goals into policies and procedures.

· Research professional standards that are pertinent to your identified problem.

· Reflect on the organization’s mission statement and values. In addition, consider how addressing this problem would uphold the mission and values, while improving the organizational culture and climate. (Depending on the organization you have selected, you may have explored these in the Week 1 Discussion.)

To complete:

Write a 3- to 5-page paper (page count does not include title and reference page) that addresses the following:

· Describe a department or unit within a health care organization using systems theory terminology. Include a description of inputs, throughput, output, cycles of events, and negative feedback.

· Describe the problem you identified within the department or unit using an open- systems approach, and state where the problem exists using the systems theory model (input, throughput, output, cycles of events, or negative feedback).

· Based on this information, explain how you would address the problem as follows:

o Formulate a desired outcome.

o Identify goals and objectives that would facilitate that outcome.

o Translate those goals and objectives into policies and procedures for the department or unit.

o Describe relevant professional standards.

· Explain how your proposed resolution to the problem would uphold the organization’s mission and values and improve the culture and climate.
