Weighing the Evidence

When conducting original research, the final step researchers must complete is weighing the evidence and interpreting the meanings of their data, statistics, and analyses. This is the culmination of the research process in which all of the research methods and designs can be synthesized into a meaningful conclusion. In this stage, researchers should formulate explanations for what their data indicates, determine whether the data answers their initial research question, identify areas of uncertainty, and consider directions for further research.

In this Discussion, you focus on one of the research articles that you identified for Part 2 of the Course Project (Literature Review). You then explore the process of how the researchers generated conclusions based on their data, consider other possible interpretations of their data, and formulate ideas for further research.

To prepare:

  • Review this week’s Learning Resources, focusing on how researchers find meaning in their data and generate sound conclusions. Pay particular attention to Table 2 in the article, “Study Design in Medical Research.”
  • Revisit the 5 articles that you identified in Part 2 of the Course Project. Select one to consider for the purpose of this Discussion.

1) Fee For Service is where the payer has a fee schedule with a set reimbursement amount for each service covered. Cost Based is for facilities that receive payments throughout the year until the cost report comes out. Then the payments that have been received are deducted from the remaining amount. Prospective Payment inpatient hospital reimbursement was based on the DRG data that already appeared on the claim. Ambulatory Surgical Centers are based on only procedures that are performed. Skilled Nursing Facilities payment based on the acuity or illness of the patient has been used.

2) Medical necessity is where you have a procedure done for a diagnosis and when it is sent to claims it can either be approved or denied. If it is denied that means that the insurance company doesn’t believe the procedure or tests run for that diagnosis was necessary. This impacts payment a great deal because if a patient has insurance, but the insurance company will not pay for the procedure or test that were done that means that the patient has to pay out of pocket. This can be very difficult for patients that live on a fixed income.

3) Payment methods now have changed coding for the better. Everything that we do now as a society is done electronically now, this means that every claim that is filed it is done electronically. Everything can be filed and stored electronically and still meet HIPAA’s requirements for patient privacy and safety.

Cost based details the cost of running a business and receiving payments. A Prospective Payment System (PPS) is a method of reimbursement in which Medicare payment is made based on a predetermined, fixed amount. The payment amount for a particular service is derived based on the classification system of that service. CMS uses separate PPSs for reimbursement to acute inpatient hospitals, home health agencies, hospice, hospital outpatient, inpatient psychiatric facilities, inpatient rehabilitation facilities, long-term care hospitals, and skilled nursing facilities. Explain medical necessity and how it impacts payment. Medical necessity refers to a decision by your health plan that your treatment, test, or procedure is necessary for your health or to treat a diagnosed medical problem. Most health plans will not pay for healthcare services that are not considered medically necessary. A common example of a non-medically necessary procedure is a Botox. The physician was Richard Washington, MD. Dr. Washington is an orthopedic surgeon. His nurse practitioner is Judy Gouda, RN, NP. Dr. Washington reviewed the consent with Mrs. Carpenter prior to surgery. Joseph Alsoff, LPN, witnessed the consent and Mr. Carpenter was present. Joseph does not remember the doctor ever mentioning that death could be a result of the surgery. The recovery room nurse is Elizabeth Adelman, RN. The respiratory therapist is David Casler, LRT. The nurse on the post-surgical unit was Kelly Wheeler, RN. The supervising nurse was Mrs. Scale, RN, MS. The patient had an epidural catheter for a post-operative pain management following an episode of hypotension in the recovery room which was treated with Ephedrine. Judy Gouda made rounds on the patient in the recovery room after the hypotensive event and vital signs were stable. The patient, Mrs. Carpenter, was placed on a medical surgical nursing unit with the epidural. The nurse, Kelly, was assigned to the patient and had not worked on that unit before, but had worked in post-acute critical care units. The nurse’s assignment was to provide patient care on the entire floor for that shift. There was also an LPN, Joseph, on the unit. It was a busy day on the unit. Mrs. Carpenter was not the only post-operative patient. Kelly assessed the plaintiff upon admission, checked the IVs, asked if the patient was in pain, noted that the patient was responsive and understood where she was, and was stable. She then left to care for other patients. The licensed practical nurse, Joseph Alcoff, had been working on the unit for several years. It had been rumored that Joseph was an alcoholic. There was no evidence that he had been drinking on the unit. Approximately an hour after the patient arrived on the unit, she was unable to tolerate respiratory therapy that was ordered and she became nauseated and vomited. David Casler administered the respiratory therapy. According to Kelly, the registered nurse, 10 minutes after the vomiting episode, Joseph Alcoff, the LPN, found the patient blue and unresponsive and called a code. Joseph is the only person other than the physician that carries his own liability insurance. The hospital also has malpractice insurance. The code team responded, along with Kelly, the registered nurse. Mrs. Carpenter was intubated and cardiac resuscitation was initiated. The patient responded to resuscitative efforts and she was transferred to the intensive care unit. Subsequently, Mrs. Carpenter did not do well, was unresponsive, and declared brain dead and taken off the respirator. She did not have a DNR in place. There is a conflict in testimony between Joseph the LPN and Kelly the RN. Joseph indicated that Kelly found the plaintiff to be unresponsive after the vomiting episode and called the code. The record is not clear as to when the vital signs and epidural site were assessed. Kelly said she did a motor and sensory level assessment and they were fine — it is not charted though. The time elapsed between the vomiting episodes and finding the patient is in dispute. The final diagnosis was anoxia encephalopathy due to the time lapse between CPR being initiated. The patient was eventually extubated, breathed independently for a period of time, and then subsequently expired. The vital signs ordered by the physician were hourly. The hypotensive episode in the recovery room had not been reported to the registered nurse. The risk manager is Susan Post, JD, who works in collaboration with the quality assurance director Amy Green. Amy had noted when doing chart reviews over the last 3 months prior to this incident that the vital signs taken in the recovery room were not charted, not done, or not reported to the units. She was in the process of collecting data from the different units on this observation. She also noted a pattern of using float nurses to several postoperative units. Prior to this incident, the clinical nurse specialist, Michael Parks, RN, MS, CNS, was consulting with Susan Post and Amy Green about the status of staff education on these units and what types of resources and training was needed.

Workflow Assessment for Health IT Toolkit   Workflow can be defined as a term used to describe the action or execution of a series of tasks in a prescribed sequence (McGonigle, & Mastrian, 2015). 

Workflow tools can help make it easier to redesign workflow and aid in the application of health information technology (U.S Department of Health and Human Services, n.d). 

For the purpose of this discussion three workflow tools were selected which include benchmarking interviews and usability evaluation.  Benchmarking  Benchmarking is a process of evaluating metrics or best practices from other organizations (either related or unrelated to your own) and then applying them to your organization (U.S Department of Health and Human Services, n.d). 

Benchmarking is an important tool that facilitators can use to motivate a practice to engage in improvement work and to help members of a practice understand where their performance falls in comparison to others. Benchmarking can stimulate healthy competition, as well as help members of a practice reflect more effectively on their own performance. Benchmarking can be done by reaching out to peers at other facilities and finding out what they do and if it works well. My facility has many other sister and we are the bigger where we encounter most of the problem. The common system that all we use is the point click care.  What I going to do is to contact all the leader of the entire facility sister, so we can have a meeting to debate about each system in place in each facility. I would want to physically see how the design of their system works and if the charting works well. Because nurse has to be involve in and informatics technology process, I would bring with me 2 to 3 clinical managers nurses to see the charting, so they could also see the new idea and give feedback on their feelings about it. Based on the information gathered I would plan a process to implement the new ideas into the charting system at our facility.  

Interviews provide a means of eliciting information from a group of individuals regarding their opinions, behavior, or knowledge (U.S Department of Health and Human Services, n.d). Here the interview involves 2 to more people and during the interview, data are collected and most of the time, it is the interviewee who provide data Here, I will set up an interview with 2 to 3 leaders form the sister facility where I will ask questions about sensitive issues, to determine users’ needs on the systems and to clarify some of my questions question.  

Usability evaluation  A usability evaluation is conducted to determine the extent to which a system is easy to use or “user friendly.” (U.S Department of Health and Human Services, n.d). Here I will determine if the new system is useful to user and if it is easy for to use it before implementation. Here I will determine the goal of my evaluation, what will be evaluated and how? For that I will choose some main staff from each sister facility who will conduct the survey to the rest of the staff to see how easy it is for them to use the informatics system. Based on the result, I will see what the areas of improvement before the implementation.  The article I will be use the information to improve workflow within your organization is:  Improvement of workflow and processes to ease and enrich meaningful use of health information technology (Singh & al., 2014).  In this article the author develop a self-contained workflow toolkit that uses ‘universal visual language’ and recognizes that most practices lack the required expertise and often need external assistance because the current methods (using esoteric charts or various software packages11 that require long and steep learning curves) are alien to health care workers and are, therefore, perceived to be barriers (Singh & al., 2014). It is a well established fact that, in any setting or industry, better management of work processes leads to improved value. Education and training of stakeholders in this management skill needs appropriate tools. The toolkit particularly addresses the ‘system-based practice’ and ‘practice-based learning and continuing improvement’ (Singh & al., 2014). Base on that, I will educate staff by using that pictorial tool for assessing the current and continuing redesigns of future workflows for achieving staff comfort, efficiency, communication, organization, productivity, effectiveness, safety, and satisfaction of patients and staff, all without having to seek outside help.  According to Caspi (2015), it is clear that informatics nurses will continue to be instrumental players in the analysis, implementation, and optimization of advanced information systems and emerging technologies that aim to improve the quality of patient care, while reducing costs.
