The IOM Future of Nursing report calls for an increase in leadership from nurses at all levels. One way nurses demonstrate their role as a leader, is through public policy change. A leader does not always carry an official title or position, but demonstrates leadership through the work and the stance the leader takes to make a change for the good of others. Nurses have been noted by the Gallop poll year after year as the most trusted professionals.

This assignment requires thought about a public policy that is needed or needs to be changed that relates to nursing, health care, or the public. Policy changes can occur by working with members of your legislature, and state or national nurses associations, to introduce a new bill and/or change to a current law in your state or federal government.

Impact of HITECH Legislation on My Organization

I believe the only organizations that could feel they are not affected by HITECH legislation are those who do not accept Medicare or Medicaid. If an organization does accept either of these, then there will be a great motivation to understand and comply with the legislation. The rules are to enforce superior outcomes buy refusing to pay if certain percentages of positive outcomes are not met (McGonigle &Mastrain, 2015). My practice, like most others, feels the impact of this and we strive to ensure quality care. One of the most important issues we face is not ever having venous access infections since we are constantly accessing through ports and IVs. There is a monitoring program and reminders posted around the office regarding safe technique.

Incentives Influence Adoption of HIT/ Impact on Care Quality

HIT has made monitoring easier as well as proof of compliance. If practices comply and provide positive outcomes then there will be a monetary incentive as reward through CMS (McGonigle & Mastrain, 2015). Studies show that physicians are motivated by the incentive but a secondary reward is more positive outcomes for patients (Begum et al., 2013). The same is true in our practice. While we know our digital tracking of infection rates helps us with reimbursement, there is also a great sense of pride that we have almost non-existent infection rates. So it is a win-win situation: the practice makes more money and the patient has better care.

Summary of Chosen Article/Meaningful Use

The article I chose was a study as Mass General Hospital where physicians were given a financial incentive to increase quality measures in meeting Federally mandated HIT meaningful use standards (Torchiana et al., 2013). The article looks at how even low incentives served as a catalyst for better outcomes. To me this s not an indicator that physicians are grasping at even small amounts of money, but rather, if there is an outlined target to follow for better practices, physicians will make the concerted effort to perform. While the financial benefit is nice, I believe the guidance and mindfulness to do better is the real motivation. For example, none of we nurses profit financially from the low infection rate at my practice, but we are very proud of it and fight to keep those statistics!

As a nurse working in a mental health facility, you are responsible for the care of Jane, a young woman in her mid-20s who has been admitted with a diagnosis of paranoid schizophrenia. Jane has a health history of repeated hospitalizations. Her first admission occurred when she was a young teenager. Jane tells you she was always very shy growing up, and that she did not have many friends. However, Jane did play the piano when she was an adolescent and found playing the piano comforting. It was the only engaged activity she participated in during her teenage years. Jane told the nurse that when her mother and father divorced, she became more withdrawn and preferred to remain alone. Further conversation revealed that she did not feel motivated to pursue goals or activities in school. She felt persecuted and interpreted events with no reference to reality. She eventually lost interest in the piano as the symptoms of the disease resulted in being unable to associate letters with the musical notes. She has recently stated she felt little purpose in her life.

For this week’s discussion, I researched advanced care practitioners and palliative care.As of right now I am in critical care, however hospice and palliative care are my passion.Once I am done with the NP program I hope to work in this area.I started my search in the Walden library using the terms ‘nurse practitioner’ and ‘pain’.I received zero results for this search.I then used ‘advance practitioner’ and palliative care’ and received 97 results using this search format.I used the Medline with Full Text data base and checked the box that said peer-review to find an article per this week’s assignment.

Research Experience

The only difficulty I experienced was the lack of results based on my first search terms.I found it interesting that after I changed from Nurse practitioner to advanced practitioner, I received an article that said Nurse Practitioner.One strategy that I used was to think of different names a Nurse Practitioner could be called. In this case, it helped me to find an article that I needed.I do believe that Medline would be beneficial to my colleagues.This database is a well-known and is easy to use and again I would recommend it for research.

Article Evaluation

Evaluating the article was rather easy due to the well written nature of the research.The title of the article allows the reader to understand that they will be reading about nurse practitioner and patients with metastatic pain.The article does discuss pain issues with cancer and how nurse practitioners can help alleviate pain through early intervention.There was no bias or opinions that I could see that could have interfered with the study.There was a great amount of evidenced provided with a detailed analysis of the research process and positive outcomes.Key words such as ‘advanced practitioners’ were not within the study but other keywords were. Overall, I think this is a solid and well written research study.
