Overview

This week’s assignment is also related to the ANA Code of Ethics – Provision 9. You might want to review provision 9 again. Advocating for the profession is important. Writing a letter is one of the ways a nurse can positively promote nursing and its causes. The purpose of this assignment is to write a professional letter to address the image of nursing or to promote a cause for nursing.

This assignment is worth 20 points.

Learning Objectives

· Distinguish the professional responsibilities that accompany the nursing profession.

· Define pressing issues that may arise as a nurse.

Directions

Read the letter writing handout. Then construct a professional letter using one of the two options below. Your letter will consist of proper address, correct salutation/greeting, body of the letter, proper closing, and contact information.

Option A: Write a letter on a current economic or political topic related directly to nursing. Address your letter to the appropriate person – You need to write to a person who is in a position to do something about the problem. For example, you should not write to the President to address a healthcare issue that needs to be passed in congress. You would want to address the correct Senator or representative from your state who will be voting on the issue. This is a professional letter, so letter form, grammar and punctuation is graded, too. This could be to a congressman or woman or to other legislators, to big business or corporations. Remember, there must be a close nursing link to write this letter. You might want to look at a professional organization website in order to look for options.

Option B: Write a letter to defend the image of nursing. Most often nursing is portrayed as less than professional in popular media, TV, advertising, greeting cards, etc.  Choose an example and write a professional letter to the appropriate person to tell them why you think they have portrayed nursing unprofessionally. Tell them how this affects the image of nursing and provide suggestions to change the situation. Look at popular TV shows, e-greetings, traditional cards advertising etc. to find an example. You can even “Google” to find some ideas.

Assignment clarifying your vision

Part- 1 Vision IN HEALTHCARE ADMINISTRATION

A vision is a guiding image of success. It is a description in words that conjures up a picture of the destination of your work. There is one universal rule of planning: You will never be greater than the vision that guides you. The vision statement should require you to stretch your expectations, aspirations, and performance.

Create your own vision statement. Note: The vision    statement should be a sentence or two but no more than a short paragraph. Write the vision statement in a separate MS-Word file.

Part II: College Education

After completing your career-driven vision statement, write at least TWO paragraphs on how your degree from a University will help you to achieve your vision. What skills do you think you can learn at the university to help you achieve success in the career that you described in your vision statement?

Part III: Reflection

In the final part of the assignment, answer the following question in at least TWO paragraphs:

Did you have a career-driven vision statement in the past? How did it, or the lack of, impact your career satisfaction in the past?

How will you make sure that you reduce risks to clients as you consider practice changes?

Consider the topic you have selected to complete your evidence-based practice project. While we are not conducting research here, per se, we are considering implementing a new practice – and with any new practice comes the potential for both benefit and harm to clients. It’s important to think about how practice changes may affect clients – both positively and negatively. So as you get ready to conduct your EBP review, you’ll want to think about how you can make sure to keep your clients safe and your practice ethical.

In an APA format paper, approximately 3 pages in length, address the following questions. (Please note that you will continue to write in the same document you completed for part 1 of this assignment (completed in module 2) – you can keep the same cover page, correcting any errors noted on your feedback. Please correct your reference page as noted from your module 2 submission feedback and update it with your new material.) In addition, although you are submitting your entire project to date (with revisions), your actual grade will be based only on the current section, as addressed below:

  • Discuss what the principle of “respect for persons” means to you.
  • How will you ensure that it is carried out in the implementation of your EBP project? (Remember that you don’t need to know the exact outcome/findings of the project yet – you just need to consider how you will ensure autonomy for clients, and protection of those with diminished autonomy, if that is relevant to the practice setting.)
  • Discuss what the principle of “beneficence” means to you. How will you ensure that it is carried out in the implementation of your EBP project?
  • Discuss what the principle of “justice” means to you. How will you ensure that it is carried out in the implementation of your EBP project?

Consider this brief example as you begin the paper:

In previous modules, we gave this example of a PICO(T) question:

In clients over the age of 65 undergoing elective total hip arthroplasty, is chemical DVT prophylaxis more effective than mechanical DVT prophylaxis alone in preventing the development of DVT in the 30 days following surgery?

So for this assignment you might consider the following:

Respect for persons:

  • Do individuals have the right to refuse DVT prophylaxis if they choose (right to self-determination)?
  • What if they fall into the “diminished autonomy” category (i.e. have dementia, etc.)
  • How does this affect their right to autonomy?

Beneficence:

  • How will this project maximize benefits and minimize harms?
  • What might be the benefits of changing practice or staying with the same practice?
  • What would be the risks?
  • How will you make sure that you reduce risks to clients as you consider practice changes?

Justice:

  • How will you make sure that there is fairness in the distribution of burdens and benefits?
  • Will you make sure that the findings are distributed to all clients?
  • What about on other units? Remember, we should make sure that everyone has the opportunity to benefit from research findings, when applicable

The project must be typewritten, double spaced and very limited in length (maximum 12 pages).

The project must be typewritten, double spaced and very limited in length (maximum 12 pages).

Part I (25%)

A NP researcher randomly sampled 100 women aged 50-65 years and measured their minutes of exercise in the past week, BMI, and depression. Depression was measured using a Likert type scale consisting of 20 items. The summation score ranged from 20 to 100 and the higher the score, the higher the level of depression. The Pearson correlation coefficients (r’s) are summarized in the following table. For the analyses, statistical significant level was set at α=0.05.

Table 1: correlation among minutes of exercise, BMI and depression

1. Write a research and null hypotheses regarding the relationship between exercise and depression.

2. Based on the test statistics in table 1, what is your conclusion regarding your research hypothesis? (Hint: discuss both the magnitude and direction of the relationship).

3. What proportion of variance is shared by minutes of exercise and depression among women 50-65 years of age?

4. For the relationship between minutes of exercise and BMI,

a. what was the estimated power of the statistical test? (Using the power table on page 202, table 9.1, Polit 2010).

b. What was the risk that a type II error was committed?

5. If -0.20 is a good estimation of population correlation, what sample size would be needed to achieve power of 0.80 at a significance α=0.05?

PART II. (25%)

Using the “N6208 Final Project Data”,

a). select two variables with nominal or ordinal level measurements, and perform the descriptive statistics (frequency and percentage). [Please select only dichotomous variables from the following list: poverty, smoker, PoorHealth].

b). perform the bi-variate descriptive statistics using crosstabulation.

c). Hand calculate the ARs, ARR, RR, and OR. Show all your calculations.

d). Perform a chi-square analysis.

e). Using APA format, write a full report with the following sections:

1. Introduction: Describe your research question and hypothesis. Include the variables, measurement levels, the bivariate research question, and the hypothesis [for example, the event of adverse risk (using your variable name here, for instance, alcohol usage) will be higher/or lower in the risk exposed group (i.e., marijuana use) compare to the non-exposed group (non-users of marijuana)].

2. Method: Include the sample description (sample size, eligibility criteria) and statistical methods used for data analysis. (The sample information can be found in “Polit Dataset Description” in SPSS Data Sets folder).

3. Results: Include frequencies and percentages for the two variables, crosstabulation results, risk indexes (ARs, ARR, RR, and OR), and chi-square test results. Include a summary table for the results and write your interpretation.  (Attach SPSS outputs).

4. Discussion: Write a report including summary and interpretation of the findings reported in the previous sections relative to the research questions you posed in your introduction.

Part III. (50%)

Run a one-way ANOVA using the dataset “N6208 Final Project Data”. The Dataset contains 462 cases from the original PolitDatasetA. Two variables will be used for this analysis: Satisfaction and Houseproblem.

The variable Houseproblem is created using the variable housprob, a summary index of eight variables about current housing problems for the women in this sample—for example, whether or not they had their utilities cut off, had vermin in the household, had unreliable hear, and so forth. The variable housprob is a count of the total number of times the women said “yes” to these eight questions. The variable housprob is recoded into Houseproblem based on number of housing problems. The coding for Houseproblem is: 1=no housing problems, 2=one housing problem, and 3= two or more housing problems.

Satisfaction measures the overall satisfaction with material sell-being. This variable is a summated rating scale variable for women’s responses to their degree of satisfaction with four aspects of their material sell-being—their housing, food, furniture, and clothing for themselves and their children. Each item was coded from 1 (very dissatisfied) to 4 (very satisfied), so the overall score for the four items could range from a low of 4 (4 X 1) to 16 (4 X 4). Higher score indicates greater satisfaction. This scale has an internal consistency Cronbach’s alpha of 0.90. The content validity and construct validity have been established in previous research.

For this analysis, use the variable Houseproblem as the independent (group) variable and variable Satisfaction as the outcome variable. To run the one-way ANOVA, click Analyze  Compare Means  Oneway. In the opening dialogue box, move Satisfaction into the Dependent List and Houseproblem into the slot for Factor. Click the Options pushbutton, and click Descriptives and Homogeneity of Variance, then continue. Next, click the Post Hoc pushbutton and select LSD. Click continue, then OK, and answer the following questions using compete sentences:

  1. What are the mean levels of       satisfaction in the three groups? Report the mean, SD, minimum, maximum       and sample size in a table.
  2. Write a research question.
  3. Write the research hypothesis (Ha)       and the null hypothesis (Ho).
  4. What was the value of the F statistic and its p-value?
  5. Can the null hypothesis be       rejected?
  6. What were the degrees of       freedom?
  7. According to the LSD test,       were any group means significantly different from any others? If yes,       which ones?
  8. Write a paragraph summarizing all       the results.
  9. Attach the relevant SPSS       printouts.

INTERMOUNTAIN HEALTHCARE: PATIENT SAFETY AND WORKPLACE STRESS IS MY FINAL PROJECT I’M WORKING ON

Begin by identifying an organizational problem within your own workplace healthcare setting or a hypothetical healthcare organization. Propose an initiative that addresses this chosen problem, utilizing evidence-based literature and quality standards. If you choose a problem in your workplace, be sure to utilize data from that healthcare organization; if you have created a hypothetical healthcare organization, you may use a public domain database with instructor permission. As this is a scholarly initiative, this assignment must adhere to all APA requirements and formatting, and include peer-reviewed and evidence-based sources to support any and all claims.  Specifically, the following critical elements must be addressed:  I. What Is the Organizational Problem? A. Provide a contextual basis for the organizational problem that you have chosen. How does this problem fail to meet quality or other regulatory requirements? B. Articulate organizational challenges posed by the problem (e.g., interdepartmental conflicts, communication failure, budgeting issues).

II. Evidence-Based Support A. Provide data that supports the existence of the problem. You may utilize public sources to find data related to your selected problem. B. How has this problem been addressed in the past? What information management systems or patient care technologies have been utilized when addressing this problem? Be sure to use peer-reviewed literature to support your answer. C. Discuss relevant accreditation standards, safety standards, compliance standards, and quality initiatives. How do these standards promote a culture of safety within the department? Be sure to cite the appropriate standards within your answer.  III. Performance Improvement Initiative A. Propose an initiative that will address this problem within the department of your chosen healthcare organization. What specific relevant quality standard will this quality initiative address? B. Discuss the data determinants of success, as related to this initiative. In other words, what type of data will be indicative of a quality outcome?  IV. Implementation of the Plan in the Organization A. What interdepartmental communication channels will be used for plan implementation? B. What manner of data interpretation will be used to communicate the findings within the organization? C. If this initiative was implemented, what do you believe would be the hypothetical effect(s) on patient care outcomes? How will health information systems support those improvements in patient care? D. What do you think the hypothetical effect of the quality or performance initiative will be on the culture of safety within the organization? V. Success of the Performance Improvement Plan A. If this initiative is successful, what would be the financial implications for the healthcare organization? B. How would the existing information management systems contribute to the success of your proposal? C. What organizational processes will permit continued viability of the performance improvement initiative, if it is successful? D. Analyze interdepartmental communication that would be necessary for continued engagement in the proposed initiative.

Guidelines for Submission: Your organizational performance initiative should be 8–10 pages in length; however, the quality of this submission is much more important than the length. All resources must be appropriately cited in APA format.

Primary Task Response: Within the Discussion Board area, write 400–600 words that respond to the following questions with your thoughts, ideas, and comments. This will be the foundation for future discussions by your classmates. Be substantive and clear, and use examples to reinforce your ideas.

Accurate coding and billing are essential to a health care facility and to a physician’s financial survival. Coding is a complex task that ties to charges and revenue generation. Failure to capture all charges associated with a patient encounter can result in significant revenue loss. Medical necessity also plays a vital role in the contract with the facility or physician in terms of receiving payment from the third party payer (e.g., an insurance company).

Part 1

For this Discussion Board, you will define medical necessity, and you will describe the criteria to determine it.

Part 2

Once the patient has an established diagnosis and the services and procedures have been ordered, you will code out this information and place it on the claim for payment.

It is important for health care professionals to understand this process to receive accurate reimbursement.

For this part of the discussion, you will be identifying the coding systems that are utilized to support the diagnosis, services, and procedures.

List and discuss the coding system that is utilized to code out the patient’s conditions or diagnosis. Provide an example of a diagnosis with the appropriate code as an example, and respond to the following questions:

  • What is the coding system replacing the diagnostic system?
  • Will it be implemented this year? Explain your answer.

Next, identify what coding system is utilized to capture the services and procedures that the patient has received (both levels). List an example of a service or procedure with the appropriate code as an example, and respond to the following questions:

  • Why is it important to assign the correct codes to the diagnoses, services, and procedures that a patient has received?
  • What are the repercussions of submitting incorrect coding and billing errors to the third party payers?

My Question Discuss why it is important for nurses to understand the basics of these statistical methods. The key concepts of the chapter: Focus on the basic concepts that are important for nurs

My Question

Discuss why it is important for nurses to understand the basics of these statistical methods.

  • The key concepts of the chapter: Focus on the basic concepts that are important for nurses to understand as they review research studies.
  • A description of the statistical methods covered in the chapter, what they measure, and under what circumstances they are used. Identify examples of how the statistical methods have been used in research studies.
  • An explanation of the key statistical tests and how they measure significance (if applicable).

To prepare,  read one of the chapters below and follow the instructions for your discussion,

  • Chapter 16, “Descriptive Statistics”This chapter introduces descriptive statistics, which, as the name implies, describe characteristics of the data. There is an overview of the different levels of measurement—nominal, ordinal, interval, and ratio—and how they are used. The chapter also defines frequency distributions and distinguishes between univariate and bivariate descriptive statistics.
  • Chapter 18, “Multivariate Statistics”In this chapter, the focus shifts to multivariate statistics. These are often used when the data analysis involves three or more variables. The multivariate tests described include simple linear regression, multiple linear regression, analysis of covariance (ANCOVA), causal modeling, and event history analysis.
  • Chapter 19, “Processes of Quantitative Data Analysis”The previous chapters have described a number of different statistical tests used to examine data in a quantitative study; this chapter focuses on how to analyze and interpret the results. It describes the steps in the analysis process and how to determine the credibility of the results.
  • Chapter 24, “Qualitative Data Analysis”The focus of this chapter is qualitative data analysis, which is more difficult in some ways than quantitative analysis, because there are not specific tests that can be used. Instead, much of the analysis is based on the researcher’s ability to identify patterns of behavior or similarities in events. Suggestions are provided for analysis of each of the major types of qualitative studies and how to document findings.

Please provide a 3-4 sentence response to the below discussion question response with in-text citations and references. No title page needed. Original question also below.

Response:

Critical appraisal is the process of evaluating a study to determine its validity, reliability, and applicability to clinical practice (Mazurek Melnyk & Fineout-Overholt, 2019). It is important to understand how to appraise quantitative studies to assess whether the study’s research question methods and results are sufficiently valid to produce useful information (Jack et al., 2010). Validity, reliability, and applicability are all important to consider. For a quantitative study, validity is the extent to which a concept is accurately measured (Heale & Twycross, 2015). There are three types of validity. Content validity refers to whether the instrument adequately covers all the content that it should with respect to the variable (Heale & Twycross, 2015). Face validity is a subset of content validity, and it pertains to experts giving an opinion about whether an instrument measures the concept that it is intended to measure (Heale & Twycross, 2015). Construct validity refers to whether inferences can be drawn about test scores related to the concept being studied (Heale & Twycross, 2015). Reliability for a quantitative study refers to the accuracy of the research instrument (Heale & Twycross, 2015). It relates to consistency of a measure, or if the instrument can be used over and over with the same responses each time (Heale & Twycross, 2015). Finally, applicability to practice is when the findings of the study can fit into contexts outside the study situation and be utilized by researchers and clinicians (Jack et al., 2010). Validity, reliability, and applicability to practice are all important factors to consider when appraising quantitative studies, and arguments can be made that each is the most important measure. Validity is arguably the most important factor to assess because if the measurement instrument is not valid, it does not have to be reliable because it should not be utilized, and if it is not valid, the results should not be applied to practice.

Original Question:

What factors must be assessed when critically appraising quantitative studies (e.g., validity, reliability, and applicability)? Which is the most important? Why?
