What does it take to be an effective health advocate? As a nurse, you have many opportunities to advocate for patients and populations, whether formally or informally. Being an advocate involves more than knowing how to lobby or to whom to write letters. It requires passion and compassion, commitment and courage.

In this Discussion, you will consider the attributes of an effective advocate for population health and/or the nursing profession. You will analyze those attributes that help nurses be a powerful force in improving the quality of health care and in this case especially, the needs of returning veterans and their families.

Per JAMA (2018) she is at increased risk for IPV because of her age and her recent assault which could have been perpetrated by her husband, we are unsure because she does not understand what we are asking her. What is the priority action for you to take at this time?My priority would be to get a medical interpreter and then proceed with the visit. I would question her further about her assault in Syria and if it included rape. The timing of her pregnancy is close to the time she was assaulted, and it should be explored. It could create another stressor for her and her husband that just adds to their financial issues and adjusting to life in a new country.What action is most important for you to take to ensure the health and safety of the fetus?Per the CDC (2012a) refugees should have a complete H&P with comprehensive labs that include a CBC, UA, and glucose. Diseases that are easily transmittable such as TB, malaria, lead, parasites, and HIV/AIDS should be tested for as well, but HIV/AIDS is no longer tested for everyone, but I should be done as part of Bana’s pregnancy care. She is at increased risk of transmission if she was raped during her assault (CDC, 2012a). Testing for STIs should be done as part of her pregnancy care as well. Given Bana’s financial situation, you are concerned about her and her family having adequate nutrition. For many patients, treatment often resulted in the amputation of limbs or even death. Today, treatment options for bacterial infections typically have a more positive prognosis. Due to the various types of infections presented in patients, it is essential to be able to identify the underlying cause of the infection—whether bacterial or viral—before recommending drug treatments. This will help you identify whether or not an antimicrobial agent would be appropriate and which specific agent would target the infection. Relying solely on the classic features of a disease may be misleading. That’s because the clinical presentation of a disease often varies: the symptoms and signs of many conditions are non-specific initially and may require hours, days, or even months to develop.

Generating a differential diagnosis; that is, developing a list of the possible conditions that might produce a patient’s symptoms and signs — is an important part of clinical reasoning. It enables appropriate testing to rule out possibilities and confirm a final diagnosis.

This case portrays a poor patient outcome after a misdiagnosis.

Case scenario

A previously healthy 35-year-old lawyer presents to a primary care office with a chief complaint of chest pain and a non-productive cough. The pain started suddenly 2 hours prior to coming to the office while the patient was sitting at his desk. The patient describes the pain as sharp in nature, constantly present but made worse with inspiration and movement, and with radiation to the base of the neck. His blood pressure in the right arm and other vital signs are normal.

On physical examination the only findings of note are chest wall tenderness and a faint cardiac murmur. The ECG in the office is normal. The patient is observed for an hour in the office and assessed. He is diagnosed with viral pleurisy and sent home on non-steroidal analgesics.

The following day the patient collapses at home and cannot be resuscitated by the paramedic service. An autopsy reveals a Type 1 aortic dissection with pericardial tamponade.
