To help you move forward with your Course Project, this week you identify a quality issue that you would like to address and submit a description of this issue for Instructor feedback.

Your proposed issue should reflect a genuine need for improvement within your selected health care organization. Through your Course Project, you will analyze this issue and propose strategies to improve outcome(s), including the redesign of a related process.

To prepare:

  • Review the six quality improvement aims from the Institute of Medicine (IOM) presented in Chapter 3 of the Sadeghi, Barzi, Mikhail, and Shabot text…… (SIX ITEMS ARE : SAFETY, PATIENT CENTERED CARE, EFFICIENCY, EFFECTIVENESS, EQUALITY AND TIMELINESS)
  • Conduct a search of the literature to help you identify a specific issue that warrants attention and action to promote quality improvement.
  • Consider what you have surmised thus far through your analysis of the organization that you selected. What have you noticed that could help you identify a quality-related need for improvement? For instance:
    • What has captured your attention in meetings, reports, and/or daily activities?
    • Have you noticed discrepancies between activities in the organization and recommendations in the research literature, quality standards, and/or the organization’s stated policies and procedures?
    • If so, what do you think could be the reason(s) for variations or gaps between what you have observed and what is recommended?
  • Identify a quality improvement issue that you would like to investigate for this project.

In this course, you develop a comprehensive Course Project: Promoting Health Care Quality. To initiate this project, this week you examine the systems and structures of an organization with which you are familiar. This analysis will serve as a foundation for developing a plan to address a quality improvement issue.

Section 1: Organizational Systems and Structures Evaluation

When a problem related to health care quality and patient safety arises, there can be an inclination to focus on a specific event rather than to examine the larger context surrounding it. Analyzing the systems and structures within an organization provides a foundation for generating a fuller understanding of how and why the event occurred and for developing strategies to address the underlying issues. Perhaps more importantly, such an analysis can be used to proactively identify potential challenges and improve organizational systems and structures in order to promote positive outcomes.

In the first section of your Course Project, you analyze the systems and structures of a health care organization with which you are familiar.

To prepare:

  • Identify a health care organization with which you are familiar; one that you will be able to sufficiently analyze to complete the full scope of the Course Project. (Refer to the Course Project Overview as needed.) This may be the same organization that you addressed in this week’s Discussion.
  • Select two or more of the following frameworks:
    • Learning organizations
    • Complex adaptive systems (CAS)
    • Clinical microsystems
    • Good to great
    • The 5 Ps

You may use the same frameworks that you addressed in this week’s Discussion.

  • With these frameworks in mind, analyze the systems and structures of the organization. Be sure to research and consider:

    • Strategic plan, goals, and objectives, if possible
    • Key operational processes and patterns
    • Information technology use
    • Organizational priorities and investments, as indicated by financial data

In Week 4, you will receive instructions for integrating an analysis of organizational culture into development of Section 1 of your Course Project. Once you have completed your evaluation, you will write a 3- to 5-page paper, which is due by Day 7 of Week 4.

My specialty area is telemetry and I love the elderly population. I am working towards my master’s degree for family nurse practitioner and would like to specialize in gerontology. Nursing theory, in my opinion, is important to the foundation of nursing. By giving nurses a sense of identity, Nursing theory can help patients, managers and other healthcare professionals to recognize the unique contribution that nurses make to the healthcare service. Providing a definition of nursing theory also helps nurses to understand their purpose and role in the healthcare setting (Colley, 2013). Because the nursing profession has had a hard time proving itself, having a strong foundation based on theory will provide us with the backing we need to prove to naysayers that what nurses do goes beyond wifely duties or those of a non-licensed caregiver. Nursing theory provides a systematic explanation for describing and predicting the direction of the nursing profession.

Health History of a Native American Male

            Native American history is deeply imbedded in American history as they are the first known settlers to the America’s before the British came. However, faced with many challenges throughout history, these settlers, and their families have a unique history worth sharing. According to the website http://indianyouth.org/, the way of life of American Indians differs on each reservation which is many and varied around the United States of America. While there are differences, there are similarities among the American Indian community; Notably, the high poverty rate as there have been reports of up to 85% among tribes, stagnation educational growth, and communities reporting gainfully full-time males at about 33% (http://indianyouth.org/).

            As a 23-year old Native American male with a family history of diabetes, hypertension, and alcoholism which are modifiable at an early age with a change in lifestyle behaviors and diet. He is currently complaining of anxiety; while the reason for anxiety was not listed as one of the behaviors that he is concerning is his lifestyle which is smoking “pot” (marijuana) and drinking alcohol. According to the Center for Disease Control and Prevention (CDC) smoking marijuana and drinking alcohol can cause an altered thought process as the brain functionalities become impaired thus affecting mood hence the feeling of anxiety.

            With concerned presentation and lifestyle, spiritually has now become a displeasing factor for this patient as he believes illicit smoking drugs and drinking alcohol will bar him from heaven.  It may be safe to assume that the use of these drugs is not for medicinal purpose. According to Ball, Dains, Flynn, Solomon & Stewart (2015) Native American has a more holistic approach to life; Therefore, with this possible feeling of imbalance (anxiety) this male client may think he is being punished.

            It is important to understand that the world is a “melting pot” in that there are many different people from different ethnic and cultural background. Healthcare providers have to operate with the understanding that opinions, lifestyle, circumstances of others encountered are different. Therefore, non-judgmental and unbiased practices should be the goal (Bell et al. 2015 p.22).  Debiasi & Selleck (2017) reminds practicing nurse the importance of proving competent care to the patient through training and assessment as this is a strong self-analysis and the ability to make self-improvement.

How can we help you?

            While there are many questions worth exploring, these are crucial questions that will give a greater understanding and knowledge of the patient health history and ways in which the provider can fully understand how to treat the patient. The final question allows the provider to determine if the patient would like to manage his anxiety; to stop smoking marijuana and drinking alcohol; or, both. Sometimes patient wants help with the symptoms and not cure. As the ball., et al. explained while we are there for the patient the goal is to work with the patient on their terms and not by force as forcing treatments can cause noncompliance and missed follow-up appointments.
