Constant change in the health care delivery system requires nurses to be flexible in their role. Flexibility is the key to success during periods of change. Professional nurses need to open themselves up to a variety of ideas in order to arrive at a perspective of health care that enhances the practice of nursing. Fundamental to this concept is the ability to work in teams. No single individual can know everything that is required for practice today, and so the team becomes the focus. The Institute of Medicine emphasizes that teams improve skills, communication, participation, and effectiveness and includes use of inter-professional teams as one of the key five core health care professions competencies. Teams play a critical role in ensuring integration and coordination of care for patients.

Part 1: To begin the EBP quality improvement project group members will select a problem within the group that is common to all students. This one-page topic proposal should include the problem that you are working on, what are you trying to improve? Why did your group select this problem? The goal for the practice improvement project should be to “tell the story” of your improvement project. This is the first page of your group final presentation paper. This assignment should be preparatory work for your practice improvement paper and class presentation (it must be submitted to faculty for approval).

Part 2: This is a formal 10-12 page paper on your EBP practice improvement project. This assignment requires the use of evidence based references. Proper APA format and citation is expected for this assignment (See directions and rubrics).

Part 3: The group presentation will cover the topic of interest chosen for the practice improvement paper. Each group will prepare and present a PowerPoint poster. See the following video for directions “How to Create a Scientific Poster in PowerPoint”

Assignment PracticunJournal Entry:

NURSING

 Assignment PracticunJournal Entry:

Reflect on a patient who presented with cardiovascular or genetic disorders during your Practicum experience. Describe your experience in assessing and managing the patient and his or her family. Include details of your “aha” moment in identifying the patient’s disorder. Then, explain how the experience connected your classroom studies to the real-world clinical setting. If you did not have an opportunity to evaluate a patient with this background during the last 8 weeks, you may select a related case study from a reputable source or reflect on previous clinical experiences.

NOTE:

Describe developmental changes associated with aging which put this patient at risk for injury

Nursing

Scenario:

An 84-year-old male, who fell at home is admitted to the hospital with complaints of confusion, weakness, unsteady gait, and fatigue.  His laboratory results confirm he has a urinary tract infection.  A nursing plan of care has been developed using the NANDA-I label Risk for Injury. The outcome is: The patient will not be injured during the hospital stay.

Initial Discussion Post:

  • Describe developmental changes associated with aging which put this patient at risk for injury
  • Identify three (3) interventions that would help this patient achieve the outcome
  • Include your rationale for each intervention.
  • How would you evaluate whether the outcome has been met?

Base your initial post on your readings and research of this topic. Your initial post must contain a minimum of 250 words. References, citations, and repeating the question do not count towards the 250 word minimum.

Discuss the nature of management, financing, and quality issues related to integration and cooperation in the facilities?

Nursing

Choose two long-term care facilities—one from nursing facilities, assisted living, or subacute care and another from adult day care, home health care, or hospice care—on which you would want to base your research work. Research Internet to read about your chosen long-term care facilities.

Assume you are responsible for the management and administration of the two facilities. You have to orient the newly appointed manager by providing an overview on managing long-term care. You also need to discuss the programs of the two facilities. From this perspective and based on your research about the facilities, prepare a Microsoft PowerPoint presentation of 10–15 slides including the following:

What are the various multidisciplinary departments (teams) included in your facilities?

Who comprise the target population being served by the various programs provided by your chosen facilities?

What are the major staffing and human resource issues faced by your chosen facilities?

What are the significant trends in long-term care likely to impact the operation of the various programs provided by your chosen facilities, and what is your plan of action to overcome them?

What are the various forms of cooperation and integration existing in your chosen facilities? Discuss the nature of management, financing, and quality issues related to integration and cooperation in the facilities?

Support your responses with examples.

Use the Notes section of the slides to provide additional information.

Cite any sources in APA format.

The Impact of Chronic Illness

Nursing

The Impact of Chronic Illness

Identify one person from the illness group you chose in Week 1. The person should not be a patient at the facility in which you work. You can use friends, family members, or coworkers. Do not use the person’s name in the paper but only initials. Administer the questionnaire you created in Week 1 to that person. Compile the data and analyze the responses to better illustrate where this person, his or her family, and friends are in relation to accepting the diagnosis in relation to the standard health or illness definitions. The analysis should also include coping skills, treatment, and support aspects of the illness. Identify how this will direct care plan development for the chosen illness group.

Compile a report of your interview with the questions you created, the responses you received, your analysis, and your interpretation of how it will affect planning care for the group in a 4 page Microsoft Word document.

How would you evaluate your nursing interventions? What criteria would you use to evaluate care?

Nursing

Scenario:

As a community health nurse, you are working in an outpatient clinic in a community that serves mainly elderly clients.

This year, you noted that 85 percent of the visits resulting in hospitalization resulted from bronchitis, asthma, pneumonia, and influenza as compared to last year. At that time; the visits weredue to  colds, sore throats, influenza, bronchitis, and pneumonia, in order of frequency.

Maria Perez, a 73-year-old Cuban American woman has been referred for community health nursing services following her discharge from the hospital. She smokes two packs of cigarettes per day. She was recently hospitalized as a result of a severe shortness of breath due to an asthma attack. The primary care provider prescribed respiratory treatments using Albuterol via jet nebulizer every 4-6 hours as needed. Mrs. Perez lives alone and has three cats to take care of.

Instructions:

  1. Read the scenario above and answer the following questions:
  • What are the biophysical, psychological, physical environmental, sociocultural, behavioral, and health system factors influencing Mrs. Perez’s health?
  • Describe at least three primary, secondary, and tertiary prevention strategies that would be appropriate in resolving Mrs. Perez’s health problems. Why would they be appropriate?
  • What activities related to health promotion and disease prevention would you plan for the late fall?
  • How would you evaluate your nursing interventions? What criteria would you use to evaluate care?

A 19-year-old female college student who is pledging a sorority arrives at the Emergency Department (ED) after participating in a contest to drink the most water. She drank 5 liters of water in about 2 hours. The sorority sisters called 911 when the student reported a headache and was confused. Upon arrival to the ED, a nursing assessment was performed and laboratory tests ordered by the MD were obtained. The patient reports a headache, abdominal cramping, and has orthostatic hypotension. The following laboratory results are reported:

Sodium: 122 mEq/L

Potassium: 3.6 mEq/L

Calcium: 10 mg/dL

Magnesium: 2 mg/dL

Chloride: 100 mEq/L

Phosphate: 2 mEq/L

Initial Discussion Post:

Cluster and analyze the data in the scenario.

  • Determine the electrolyte imbalance.
  • As the RN monitoring this patient, what assessments will you perform for early recognition of potential problems?
  • Identify a nursing diagnosis with related factors, and defining characteristics (if appropriate).
  • Describe the analysis used and rationale for your choice.
