  1. A large number of Americans have access to health care today. While the health policy is largely similar across the country, the implementation process, quality, and cost vary with each state.  It is the responsibility of the state government to fine-tune the health legislations in a way that benefits its citizens.

The healthcare reforms signed in 2008 by Governor Pawlenty is an example of a state attempting to reform the healthcare sector. The policy was meant to significantly reduce the number of obese people in the state of Minnesota as well as reduce the number of tobacco users. An estimated 8,700 more people were to have access to MinnesotaCare coverage by 2011(Aspinall, 2008).

As is the case with many states, politics and the political climate has a role to play in the making of policies. As a largely Democrat state, Minnesota can be perceived as riding the wave of change brought about by the election of president Obama. One of the hallmarks of the Obama administration was the Affordable Care Act, and the state of Minnesota was looking to implement changes of its own in order to have more people covered in their state.

By signing the pieces of legislations into law, the governor put into consideration the recommendations of the Minnesota Department Of Health as well as the governor’s own Transformation Task Force. In fact, MDH was required to develop and implement standards of certification for healthcare homes by July 1, 2009 (Aspinall, 2008). There is no indication however that the professional nursing organizations been involved in this policy issue. 

Among some of the recommendations in the pieces of legislation, it required a workgroup to work on developing recommendations to regulate and educate oral health practitioners (Aspinall, 2008). Nurses have great potential to contribute in developing of such policies. The state of Minnesota and other states could be better working with the various nursing professionals and engage them in health policy activities as their input will result in better legislation that is all inclusive.

The US political space has a long history of directing the public support through mechanisms available in the private sector, and the tax code. This has prompted to the disaffection that has been experienced in the tools and mechanisms used in the implementation of the Affordable Care Act. The implementation mechanisms of the Act used by theUS government make citizens perceive the Affordable Care Act as a system of private insurance, rather than a government program.

It is an apparent fact that professional nursing organizations have been involved in the implementation of the Affordable Care Act. In addition to nurses playing a vital role in ensuring patient safety, quality care, and cost containment, they ensure that they work not only to enhance the individual results, but also the results for health systems in general. The Affordable Care Act is an example of such health systems.  An overview of the Affordable Care Act asserts that it is a reform of the US healthcare system that is aimed at providing more Americans with access to affordable and quality health insurance, and also reduce the US healthcare spending.The points to take from the Affordable Care Act include;

The theory of power as knowing participation in change is derived from Roger’s postulates and principles. This revolves around freedom to act, choices and awareness. Power is all about knowing what to choose to do, feeling free to do it, and doing it intentionally (Smith & Parker, 2015). A person has to be able to participate or be actively involved in the change process. Then, the person has to knowingly participate in the change process. This theory can be applied in a variety of areas in nursing such as caring for people who have self-esteem issues, those experiencing hopelessness, trauma, etc (Smith & Parker, 2015).

The theory of Power as Knowing Participation in Change can be applied in Gloria’s case. Gloria was not ready to consider surgery. She believed that prayers is a better solution to relieving pain. Since she has come to realize that surgery is important for her treatment, the nurses can go ahead and convince her to undergo through it. She is also considering non-traditional and traditional treatments and as such willing to participate in her treatment knowingly as described by the theory. Therefore, Gloria was free to choose the treatment method, feels free to do it and participates in it knowingly which aligns with the Theory of Power as Knowing Participation in Change.

MSN essential I (Background for practice from sciences and humanities) is one of the essentials that most relates to this theory. According to this essential, nurses should use their understanding of nursing and related science in order to design a nursing care (AACN, 2011). Nurses should use their understanding of this theory to develop a plan for care.

Meredith Troutman-Jordan’s Theory of Successful Aging incorporates eight main themes. These are active independence, beneficial contribution to society and others, having comfort resources, freedom, having health, having a positive perspective and coping, a relationship with God, and relationships with other people (Troutman-Jordan & Staples, 2014).  Research has shown that these concepts of successful aging can be present among older adults living in many different types of settings, including assisted living communities (Kozar-Westman, Troutman-Jordan, & Nies, 2013).

This theory is probably the most closely aligned with Masters Essential VIII: Clinical Prevention and Population Health for Improving Health (American Association of Colleges of Nursing, 2011). The elderly encompass a vulnerable population that must be considered in any effort to address the health of populations in the United States, and the Masters Essentials indicates that the health system must provide care that is responsible to the unique needs and values of patients within given populations such as elderly individuals. The concepts of the Theory of Successful Aging and the Masters Essentials can be incorporated into daily practice by assisting elderly patients in many respects. One of the most important ways in which I can help facilitate better patient outcomes and successful aging is to prepare patients for the process of successful aging. This means educating them on healthy lifestyle choices and better nutrition habits that can lead them to a healthier aging process. Another important way to assist patients toward successful aging is to encourage and facilitate healthy relationships with others, including family, friends, and healthcare staff. The third way that I can assist this vulnerable population is to add my voice and serve as their advocate to those lobbying for equitable healthcare specially for the geriatric population, including our elders, to make certain that they have the comfort resources and health resources that they need in order to reach a successful advanced age.
