At various points in your career as a nurse, you have most likely observed significant problems in your practice environment(s). Take a few minutes to think about the problems you have noticed.

Through the DNP degree, candidates develop the tools needed to address practice-related concerns through evidence-based solutions. In this week’s media presentation, Dr. Beechinor and Dr. Stefan discuss this process while sharing their own DNP projects and the needs these projects addressed. Candidates in the PhD program conduct research involving a problem related to practice or another area of professional interest and, as such, often work with DNP-prepared nurses.

To support your doctoral project and growth as a scholar-practitioner, you are required to join a professional nursing organization. Membership in a professional organization offers numerous benefits, including educational endeavors, advocacy opportunities, up-to-date information of interest to the profession, and much more. Some organizations offer scholarships, continuing education units, and a means for disseminating current research findings. In addition, joining a professional organization can help you to become involved in decision making at the organizational level and provide a venue for engaging in social change efforts that benefit your community.

This week’s Assignment serves as your Major Assessment for this course. You identify a DNP project or research topic and select an organization that will facilitate or be useful to you as you engage in your doctoral project (DNP Scholarly project or dissertation research). In addition, the professional nursing organization should be relevant for your current or anticipated professional practice. You then outline plans for participation in the organization and articulate how this could help you meet your professional goals and support your project development.

To prepare:

  • DNP students:
    • Reflect on the DNP projects Dr. Beechinor and Dr. Stefan discussed in this week’s media presentation, as well as the DNP Scholarly Project specifications outlined in the AACN Essentials. Also, review the end-of-program requirements you explored for the Assignment due in Week 5.
    • Consider your professional experiences in light of new information you may have encountered in this course. With this in mind, identify a need or change related to a practice environment. Please note that the objective of this activity is to increase your awareness of potential DNP projects; you are not committing to a project idea at this time.
  • Consider which professional nursing organization you would like to join and how they organization relates to your identified practice need or change or research problem. How might participating in this group support the development of your DNP Scholarly Project or PhD Dissertation?
  • There are numerous types of organizations that qualify as professional nursing organizations, including:
    • An organization that focuses on a specific area of clinical practice (e.g., Association of Operating Room Nursing [AORN], American Holistic Nurses Association [AHNA])
    • A state or national nursing organization (e.g., American Nurses Association [ANA]or Texas Nurses Association [TNA])
    • An area of scholarly emphasis (e.g., American Association of Colleges of Nursing [AACN] or National League for Nursing [NLN])
    • An organization that emphasizes specific groups of nurses (e.g., American Assembly for Men in Nursing [AAMN] or National Association of Hispanic Nurses [NAHN])
  • After evaluating these organizations in light of your most salient professional concerns, goals, and doctoral project select one organization to join. Complete the steps necessary for joining this professional organization.

Discussion: Introduction to the Planning Process

The nursing process provides a cornerstone for care. Engaging in assessment, diagnosis, planning, implementation, and evaluation helps to move the patient from his or her current state toward a desired outcome. How does this correlate with the strategic planning process?

In this Discussion, you examine similarities and differences between the nursing process and strategic planning. You also consider why it is important for a nurse leader-manager to be familiar with the planning hierarchy and to apply business principles to promote strategic change at the organizational or systems level.

To prepare:

  • Review the information related to the nursing process and the strategic planning process addressed in Chapter 4 of the Sare and Ogilvie text and the other Learning Resources. Think about how the nursing process is similar to and different from the strategic planning process.
  • Reflect on the value of nurse leader-managers’ contributions to strategic planning, as discussed by Dr. Huston in this week’s media.
  • Using the Walden library, identify an example from the literature that demonstrates why it is beneficial for nurse leader-managers to be familiar with the planning hierarchy and why they should be engaged in strategic planning in addition to and in contrast with operational planning.

You must use a minimum of four original research articles for your paper.

The purpose is to write a review of the literature on to answer your PICOT question using original research evidence. Discuss how this health problem is applicable in both the United States and a non-USA country, and appraise four original research studies on this topic.

This is a scholarly paper, written as if it was a “Review of the Literature” article published in a nursing journal. Select articles published within the past five years (2014 to present).Do not use a non-peer review journal. Do not use a literature review article or systematic review article or clinical protocols as one of your articles. At least TWO of the four original research report articles MUST BE authored by nurses and published in a peer-reviewed nursing journal.  At least ONE of the two nursing research studies must address nursing practice in a non-USA country.

  • Page 1: Title page Create a title for your paper. Refer to your APA Manual.
  • Page 2: Abstract The abstract should open with a sentence that will serve as an introduction to and purpose of the paper. Then describe what is in each part of the paper. An abstract is usually a brief outline of the contents of the paper that typically does not exceed 250 words.     • Heading: Methods: Discuss the methods used to select the following research studies.Next use the subheading:

This Assignment, due by Day 5 of Week 5, provides an opportunity to deepen your understanding of the conceptual components of a theory. Building on this week’s Discussion, you will build a concept map to express the linkages and interrelationships of the concepts in the middle range theory you have selected.

The purpose of the Course Project is to give the student the opportunity to apply concepts in transcultural nursing by performing a transcultural nursing assessment.

The purpose of Milestone 1 is to identify an adult who is not a relative who is willing to participate in the transcultural assessment and to secure the instructor’s approval for the assessment. Note: The actual assessment does not occur until Milestone 3. It is not a purpose for this assignment.

3. Identify a potential individual who has been living in a culture other than the one of origin for at least 2 years. This individual must be born outside the United States. The individual can be a patient, friend, or colleague, such as a physician, or acquaintance, but cannot be a relative. The individual must be living in your community and must be currently receiving or have previously received healthcare services in your community. The individual’s race, ethnicity, language, religion, and culturally based beliefs about healthcare and illness should be quite different from yours. The individual must speak your language. An interpreter is not permitted for the actual assessment, which occurs in Milestone 3 (Week 6). Note: This is the same individual for whom you will develop verbal assessment questions in Milestone 2 and whom you will ask those questions in Milestone 3 and then reflect on your learning about the Course Project and the assessment.

4. Individuals can come from a variety of settings. Recent immigrants, migrant workers, exchange students, international students, long-term care residents, refugees, coworkers, physicians, patients, employees at local ethnic businesses, religious venues, community centers, and individuals who have recently moved to your community are some suggestions. The individual does not need to be a healthcare worker but must have received healthcare services in the country of origin as well as since arriving in the United States. Work with your instructor prior to submitting Milestone 1. Approval must be provided by your instructor before submitting Milestone 1 (this assignment).

PE:

Hypothyroidism is six times more common in women than men, and is most common in older women (Carson, 2009). A thorough physical assessment should be completed. Clinical signs and symptoms may include paleness, brittle appearing hair and skin, elevated blood pressure, and bradycardia (Carson, 2009). The patient may have a “puffy” appearance to her face, irregular periods, and report sustained fatigue (Roberts et al, 2009).

           TSH – Will be elevated in Hypothyroid

           Free T4 – Result will be low in Hypothyroid

           Cholesterol – Often elevated with Hypothyroid

           CBC – To rule out anemia

           CMP – To monitor other electrolytes

           EKG – to assess for any blocks, prolonged QRS, or electrolyte abnormalities

Treatment

           Patients with symptomatic hypothyroidism should be treated to prevent long-term complications (Roberts et al, 2004) Depending on the results of her TSH & T4 I would initiate a daily regimen of Levothyroxine. 4-6 weeks after the initiation of Levothyroxine I would recheck the patients TSH. After the TSH has reached a therapeutic level – I would recheck it again in 6 months.

  1. What additional questions should you ask the patient and why?

Some questions include:

  • How much weight has been gained? What kinds of meals/foods do you typically take? Do you exercise?
  • Quantifying the amount of weight provides perspective. A gain of 5 lbs does not carry the urgency that a 20 lb weight gain does. Asking about her lifestyle habits can offer some insight into factors that can aggravate what sounds like hypothyroidism. This creates teachable opportunities for improving lifestyle habits.
  • Are you still menstruating and if so, how regularly?
    • This can rule out pregnancy or hormone changes that precipitate menopause. Also, with hypothyroidism, this condition can disrupt a normal menstrual cycle. For a woman in menopause, hypothyroid symptoms can be masked or ignored when it is assumed that it is a lack of ooestrogen that is causing her concerns (Baisier, Hertoghe, & Eeckhaut, 2000).
  • Bowel habits, specifically any problem with constipation?
    • (Chaker, Bianco, Jonklaas, & Peeters, 2017)
  • Any hx of depression?
    • Her reported complaints are common findings for hypothyroid but they can also be related to depression. Though the rate of depression in hypothyroid patients is >60% (Bathla & Singh, 2016), the patient should be screened for depression. Her symptoms could be psychosomatic.
  1. What should be included in the physical examination at this visit?
  • Included items to address are skin for dryness, hair for thinning or irregular growth pattern, eyes for exopthalmus, neck/throat and thyroid for possible goiter, cardiac sounds for bradyarrhythmias, and also for peripheral manifestations like delayed relaxation of deep tendon reflexes (Chaker, et al., 2017).

  • Hypothyroidism

  • TSH and free T3 and T4

  • A depression screen can be done in office

Hypothyroidism can increase lipids and alter cardiac function (Chaker, et al., 2017). EKG may reveal cardiac abnormalities. CBC can reveal anemia. The metabolic panel can reveal diabetes or problems with hepatic or renal function. Hypothyroidism continues to be researched as far as the specific link to renal and hepatic dysfunction (Chaker, et al., 2017). The depression screen serves, like the other lab orders, to exclude causes of her symptoms. The most obvious test is a thyroid panel. To assess the circulating amount of hormone in the body is to judge her thyroid function.

  1. How should this patient be managed?
  • Pending the diagnosis, the patient should be encouraged to complete all lab work in a timely fashion. Since these labs can be resulted within 24hrs, if not same day, that would be my biggest priority for completion. For hypothyroidism, pending the thyroid results, the patient should start on hormone replacement with a drug like levothyroxine. Often, levothyroxine 50-100mcg is a starting dose (Dunphy, Winland-Brown, Porter, & Thomas, 2015), it should be taken daily, on an empty stomach, in the morning. She should return in about 1 mo to reassess symptoms and lab value. If the patient is difficult to manage, due to comorbid conditions or lack of therapeutic response, endocrine may be consulted.
