Should laws governing nursing homes be changed?

Although the nursing homes are filled with the elderly, should they be made to relingquish all of their property to pay for nursing home. Is the system failing the elderly?

The Federal Government should not allow this to happen to the elderly. What are they doing to resolve this matter.

Nursing home care is undeniable expensive, what happens when you’ve worked all your life and you plan to have a nice nest egg stored away for and your loved ones only to sell it in order for you to live out the rest of your life in a nursing facility?

There should be laws in place for any individual that is experiencing a medical disease or can no longer care for themselves and is under doctor’s order must transistioned into a nursing home. The nursing homes all have different prices that they charge. Usually payments will be made through Medicare, Medicaid, private insurances or personal monies that have been saved. When I was completing the paperwork for my mother to be admitted into a nursing facility, the admission office kindly told me that her Medicare will not cover her stay because she was diagnosed with Dementia which means her level of care will more than likely remain the same for the duration of her stay at the facility so therefore is best that the paperwork be drawn up so that Medicaid which is a state/federal health insurance program for low income patients. Which the nursing home care is covered through Medicaid and plus the facility is garuanteed to get paid.

Over all, 31.5 percent of Medicaid’s $400 billion in shared federal and state spending goes to long-term care for the elderly and the disabled. According to the U.S. Bureau of the Census, slightly over 5 percent of the 65+ population occupy nursing homes, congregate care, assisted living, and board-and-care homes, and about 4.2 percent are in nursing homes at any given time.

With more than 70 million people enrolled in Medicaid, the program certainly faces long-term financial challenges. Federal Medicaid spending is projected to grow 6 percent a year on average, rising to $650 billion in 2027 from $389 billion this year, according to the Congressional Budget Office.

Skilled nursing staff consisting of RNs, LPNs, and certified nurses’ aides (CNAs) are available to provide 24-hour medical attention. Skilled nursing facilities are commonly used for short-term rehabilitative stays. Extending stays into longer-term care must be medically necessary for the treatment of a serious medical condition and is largely dependent on state and federal benefit regulations. We are definitely under staffed and paid. There hasn’t been any raises in three years. Unfortunately a lot of our nurses due to child care issues will have to switch off shifts just so they’ll be able to work. Upper management doesn’t address the problems at hand instead they keep patching things up around here and nothing is being done about it. Who’s in charge of decision-making and withthe end-of-life care. The staff has lack of resources so that we may perform our job duties as needed. We have made suggestions to the management team that maybe they should bring in some animals to get the residents involved more and to cheer them up. Have more Family and Friends Day instead of once a year. Get the youth/community attention about our aging residents. Give the residents more to eat so they’re not unhappy throughout the evening. We need to adopt a better system with dealing with the residence monthly Patient Allowance Exchange-give them more than $35.00 a month to spend on personal items.

No matter what, I will still be part of the care team. I will still be my mother’s advocate. I will give much of my attention and my life to help her. I have learned to accept and respect reality. I try hard to have a life of my own so that I don’t get burned out. That’s a wining situation for both sides.

As the nurse manager of the MICU, you are used to the rumor mill. You have a million things on your plate, and this item goes on your very long TO DO list. You make a note to yourself that when you have time, you will speak with Dr. Snuggles in private. You will tell him that while no one has formally complained, his behavior is bordering on sexual harassment and people are saying things.

You go home after a long day and fall asleep as soon as your head hits the pillow.

At 3 am, the phone rings. The caller’s voice is slurry, as if he’s drunk.

“Dr. Snuggles is having an affair with Bob.”

You are half asleep and say, “Who is this?”

“Dr. Snuggles is GAY.”

The caller hangs up. You check the caller ID. The number is blocked.

Now you are wide awake. Bob is an RN who reports to you in the MICU. An excellent nurse, Bob works alongside Dr. Snuggles every day. Ridiculous. Dr. Snuggles is married. Bob is a consummate professional, well liked, respected by his peers. What an outrageous phone call.

When you arrive at work the next day at 7 am, the morning nursing report is abuzz. Several other people received the same anonymous phone calls. Bob is not on that shift, nor is Dr. Snuggles, so there’s no way to address this matter right away. You get to work on the pile of papers on your desk. Late that day, you open your interoffice mail and a find a piece of paper that appears to have been printed on a computer. The large blocky font spells out: “Bob and Dr. Snuggles are GAY.”

You call Human Resources and get their voicemail recording. The entire department is at an offsite retreat for the day. Callers should try back tomorrow. You get busy with other burning issues and set the Dr. Snuggles matter aside.

At the end of your 10-hour day, you decide to close your office door and stay late to research the matter. You consult the Riverfront Medical Center policy manual. It does not forbid consensual sexual relations between coworkers. However, it does state that if a romantic relationship exists and one of the workers supervises the other, the couple must declare the relationship and find a way to move into different roles. Dr. Snuggles does not supervise Bob. You are Bob’s supervisor. The policy manual is silent on same-sex relationships, except where it applies to benefits for domestic partners. Purpose

The purpose of this Assignment is to explore how a theorist explicates his or her philosophy and thoughts behind a theoretical field. As in other fields of study, nursing has a plethora of theorists and theories. These theorists have developed their paradigm over time enriching it with research and dialogue with other theorists.

Directions

You are going to explore how a theorist of your choice created his or her theory. Did it develop as an acorn becoming a mighty oak over the decades? Did it arrive as a burst of light like the big bang as some believe created our universe? Was it developed from a blueprint like a spaceship where thousands of scholars worked together to create a rocket to the moon? In point of fact, how is a theory developed? This is what we are exploring.

You will pick a theorist who interests you, it does not have to be a nurse theorist. You may also choose from other disciplines such as: behavioral, leadership, business, education, technology.

You will become the theorist immersing yourself in the writings from the earliest mentioned to the most current.

As you read, look at how you (taking on the persona of the theorist) developed the theory.

In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed.

In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring.

The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care.
