            Cognitive behavior therapy among other forms of therapy has proven success in treatment of children and adolescent on individual levels – when apply as an individual cognitive behavior therapy. Although cognitive behavior therapy is effective; however, the Johnson Family Session video from this week’s media, demonstrates that either an individual, group/family can be effective, it all depends on the issues and the type of treatment provided. The video makes it clear, as seen with the girl who had been sexually assaulted previously in a fraternity, she didn’t believe in sharing her past experience, she doesn’t want to talk about it. She doesn’t believe that sharing her experience with her peers, including those with similar past experience will help her. There are some internal issues bothering her. These issues need to be address on the individual basis for her to move forward. At this time the girl is not prepared to participate in any of the group or family therapy session. CM is a 41-year-old female accompanied by her husband to participate in a scheduled one day a week individual cognitive behavioral therapy session and a one day a week family cognitive behavior therapy session. The problem she presented with was alcohol abuse. Patient stated that her husband forced her into attending this session because he felt like she was unable to control her alcohol abused issue. Patient reported that each day she would drink 5 to 7 drinks of alcohol. Patient stated: “It made me lose multiple jobs throughout my career. Open-ended questions were asked during individual cognitive behavior therapy session to obtained situational and cognitive information from the patient. Any question inquiring about her alcohol use provoked her, she got very upset and defensive. This indicates that she was not being honest about her alcohol intake. Other times she wouldn’t provide answers to questions that were being asked of her. On the contrary, she responded appropriately to questions ask during family cognitive behavior therapy session. She opens up about her alcohol intake, and frequency. CM contradicted herself during the family cognitive behavior therapy session. Saying: “my alcohol intake didn’t interfere with my personal life, job, marriage, and health. however, her husband corrected her each time.  Evidence from the literature supports cognitive behavior therapy – individual use alcohol or other substance for behavior reinforcement. This makes the patient to abstain from developing skills desired to reduce substance use or alcohol intake.

Challenges which the psychiatric mental health nurse practitioner might encounter include restructuring thought process or thinking pattern of individual in a family dynamic or different individuals in each session of family and/ individual cognitive behavior therapy session.  Moreover, the practitioner needs to ensure that structural of the session is appropriate for family members from diverse cultural background. For example, eating disorder such as: bulimia nervosa is common among lesbian and gay individuals. However, this population do not seek treatment often because of stigma associated with disproportional sexual minorities group/family. Therefore, practitioners during cognitive behavioral therapy need to structural the session to adapt emotions so that internal family members suffering from bulimia nervosa will share their experiences to improve treatment and function of family dynamic within this sexual minorities’ population (Minaiy, Johnson, Ciochon & Perkins, 2017). Also, practitioner needs to ensure CBT techniques/strategies are applicable and effective for every individual group member.

Therefore, a psychiatrist can reach a more significant number of patients, making it less expensive than in an individual setting. In this setting, the psychiatrist works alone with the client to identify issues and work through them. A study by Hauksson et al. (2017) showed that individual therapy is more effective in the treatment of major depression compared to group therapy. In the individual setting, clients are more likely to develop a therapeutic relationship with the therapist and become more comfortable sharing information than in a group setting. When dealing with children, CBT in the individual setting gives the therapist more time to engage with the child and their families, unlike in a group setting where the therapist has less time to develop treatment plans specifically suited to the child. This treatment flexibility allows the child to have a more personalized experience with larger reported effect sizes. Moreover, since treatment in an individual setting is tailored to the individual needs, the therapist and the child might form a bond in which the youngster feels comfortable disclosing specifics about their fear (Guo et al., 2021). As a result, the therapist can sufficiently address the child’s personal and emotional requirements.

In an individual setting, PMHNPs may encounter a variety of patient behaviors that can obstruct CBT. It may be difficult to work with patients who have been labeled as difficult, unmotivated, or resistant (Heidenreich et al., 2021). Also, the client could have trouble recognizing emotions and thoughts. Clients often experience emotions before recognizing their previous thought (Heidenreich et al., 2021). This problem can challenge the determination of the very thought that caused the emotional response. Also, if the individual is linked to another person, like a spouse or family, the PMHNP can only control the particular individual’s behavior leaving out the other parties (Heidenreich et al., 2021). A case example from the Beck Institute for Cognitive Behavior Therapy (2018) video is where the counselor handles a spouse without their partner.

A client might also agree with the principles but be unable to change their minds. Clients sometimes report an intellectual cognizant of cognitive therapy principles but are unable to implement that understanding in a way that leads to real change. Finally, the client’s motive for change may be limited. Therefore, PMHNPs must establish motivating factors for clients who are not attending therapy of their own volition in the early stages.

I have the five main strengths of Relator, Learner, Discipline, Intellect, and Futuristic. I was surprised by a couple of these strengths but overall these did fit my personality, work ethic, and behaviors. Discipline really stood out to me, I felt drawn to this strength. I love a world where it is predictable and I can make small goals towards a larger goal. I don’t like unexpected changes and prefer plenty of advanced notice of timelines. I enjoy checking off the boxes, always keeping my end goal in sight, and feeling pride in my accomplishments. Relator also stroke a cord in myself. I very much enjoy having a small group of very close friends. I feel out of sorts in large crowds and have to really go against the grain to small talk. However, meeting people and having a network of people to rely on is important to me personally and professionally. Learner is a clear strength as I do find myself always looking for something to help me grow and I enjoy the process of learning. Intellect is interesting to me as I was a little surprised. I actually do not like to have banter with people over various subjects. I prefer to introspectively have a dialog with myself than with others. However, maybe this is a part of the Intellect strength but in a different perspective. Lastly, futuristic is my final strength. I love to plan the future and make goals towards that future I envision. In a leadership study conducted it was found, “the research demonstrated that, for leadership styles, leaders’ communication, emotional intelligence, decision-making, and personal characteristics are important” (Lorber et.al. 2015).  Developing this strength would help me become a transformational leader who develops and inspires others through self awareness of my own communication style and how this affects others perception of me as a leader.

 

The second strength I would like to develop is Arranger. Arranger can see a complex problem and figure out how to rearrange that problem into an intricate and well oiled solution. An Arranger can see the micro and macro elements of a problem. I would like to develop this strength as it would help both at an intimate level of management and as a leader who can not just have a vision of the future, but can see how to rearrange things to make that vision come to life.  It would go hand in hand with my Futuristic strength, as well as my Discipline strength. Discussion 2

According to the Clifton Strength Finder assessment, it is apparent that I am more of a democratic leader or transformational leader (Rath, 2007). I like to include others’ input and make my decisions based on the group’s ideas as a team approach. Other leadership styles, such as strategic and lateral approaches, can help me understand how to network with various groups inside and outside my company (Broome & Marshall, 2021). I tend to focus on the work at hand and forget that others are doing the same job in better ways. I could challenge my co-workers to introduce new ways of producing positive outcomes by intellectual stimulation that can lead to the efficiency of the overall work.

 

Two core values for me are good communication skills and gaining respect from others. I was described best with the achiever theme. I have a great deal of stamina and work hard. I take great satisfaction from being busy and productive only to produce top-notch results. Being the very best at something is quite important to me. I do not tolerate mediocrity, especially about what matters most to me. And because of those strengths, I set very high expectations for myself. With this strength, I can influence people also to work hard. I like to lead by example to increase productivity and competition (Bracamonte & Gonzalez-Argote, 2022).

 

My two main strengths, I would say hard work and high expectations of myself. Typically, I push myself until I reach my goals. I believe that I do this because my second theme was futurist. My vision opens people’s minds to new and wondrous possibilities. Because of my strengths in planning for the future, I think a lot about the coming months or years. I want to leave the job better for the next person behind me. The downside is that I worry about being left behind if what I know and do is no longer valued or needed. My biggest fear is not having a job to provide for my family or leaving a good legacy.

 

Communication was my next strength, and I am an excellent talker. I freely share my ideas or feelings with others, both a blessing and a curse. I am quick to talk as words quickly come to me when I am with people. It is easy to get people to talk about themselves, and I like making them feel like they are the most important person at that time. The verbal give-and-take between human beings is something I value. I will not meet a stranger. The impact of a good nursing leader focuses mainly on patient safety and the satisfaction of other nurses. Hughes (2019) states that the highest job satisfaction was associated with relationship-oriented leadership styles.

 

The relator theme was my next strength. I believe this goes along with the communication theme. I love to mentor or teach without being overwhelmed with too many details. I bond and work well with co-workers who tell me what they want to accomplish in life. I have close companions who frequently seek my guidance. I want everyone to like me or find me valuable.

 

The activator theme described me very well. Especially the part that said these types of people are often impatient. This would be one characteristic that I would like to strengthen. I am assertive and want certain people to like me and do what I tell them to do. I like to get everyone to concentrate on performing one task at a time to keep it simple. Another characteristic I would like to build on would be to stay calm in a storm. I get easily overwhelmed if I have many jobs to complete simultaneously. I have had to learn to focus on one thing at a time. But as soon as I make up my mind on what I want to do, I am eager to get started. People who interfere with my progress can irritate me.

 

I could improve on these five strengths as a leader by understanding that not everyone is the same type of person. By nature, I want them all to be, but dealing with different personalities and ethics makes it a challenge to get the same results from everyone. Finding each person’s strengths and building on them can help me know who to assign specific tasks to and who might need extra motivation to succeed. I tend to be disappointed when those results are not what they should be. Setting different goals or better expectations depending on the situation can be something I work on by having more patience and understanding that we can do all things with time and more effort.

There are various strategies I will utilize to increase rigor and effectiveness when searching regarding the PICOT question. The first strategy is to choose the most significant keywords and I will also utilize various Boolean operators such as “OR” and “NOT.” Another strategy is to limit the search to peer-reviewed articles to ensure that the information obtained is relevant and effective to avoid research errors. It is also crucial to limit the search to articles produced within the last five years to ensure the information obtained is relevant and represents current issues. If the materials are from the past, they might not be effective because the research is on a recent clinical issue. Finally, the significant words must be in the title of the research materials. Utilizing the strategies will ensure that the information about the PICOT question is adequate.

Two Core Values

To be an effective leader, it is important that you are aware of your core values and strive to be an authentic leader. Two Strengths

My top two strengths are developer and a believer. I am currently in a unit manager position, and I believe it is important to give others the tools and guidance that they need to be successful because I believe everyone has potential. I try to be a support person and mentor to staff, especially those that work on my unit, so they know that they can come to me for help and for words of encouragement when needed (Brunt & Bogdan, 2021). I feel my other value, belief, explains me so accurately. I am very family-oriented, strive for success, feel a sense of meaning and satisfaction in my life, and value responsibility and ethics in myself and others. Because of this core value, I feel that my team members feel that they can trust me and that I am dependable.

Two Weaknesses

I believe two areas that I could improve upon would be the core values of activator and command. I struggle as an activator in my personal life because I oftentimes lack the energy or motivation that is needed to get things done. I have not always been this way, but recently feel that I sometimes push things off until the last minute possible. I think establishing a good routine and sticking to it and setting personal goals for myself will help me to become a better activator. I struggle with command in my work life because I do not like to come off as bossy, confrontational, or insensitive, however, there are many situations where I must take control and make difficult decisions (Lead Through Strengths, 2021). I believe because my core values include belief and harmony, that it makes the value of command difficult for me.

Two Core Values

The two core values of utmost importance to me are discipline and consistency.  Discipline keeps me grounded and focused on the right choices. Discipline steers me in the right direction in life. Two Characteristics to Strengthen

The first characteristic to strengthen would be relator. Although I form deep relationships with people I know, as a leader I want to be vulnerable and approachable. Although the relator theme states it’s a positive due to building strong teams, I feel it would be hard if one does not come across as approachable, to begin with. The second characteristic I would like to strengthen would be an achievement. While this is usually a positive, there are times the need for achievement can almost become like a competition. This is where I would like to strengthen this theme. Achievers tend to never shut off and tend to be judgmental (literacymn, 2010). I would definitely want to strengthen this area of this theme for effective leadership.
