In the modern era, there are few professions that do not to some extent rely on data. Stockbrokers rely on market data to advise clients on financial matters. Meteorologists rely on weather data to forecast weather conditions, while realtors rely on data to advise on the purchase and sale of property. In these and other cases, data not only helps solve problems, but adds to the practitioner’s and the discipline’s body of knowledge.

Of course, the nursing profession also relies heavily on data. The field of nursing informatics aims to make sure nurses have access to the appropriate date to solve healthcare problems, make decisions in the interest of patients, and add to knowledge.

In this Discussion, you will consider a scenario that would benefit from access to data and how such access could facilitate both problem-solving and knowledge formation.

To Prepare:

Reflect on the concepts of informatics and knowledge work as presented in the Resources. 

Consider a hypothetical scenario based on your own healthcare practice or organization that would require or benefit from the access/collection and application of data. Your scenario may involve a patient, staff, or management problem or gap.

Post a description of the focus of your scenario. Describe the data that could be used and how the data might be collected and accessed. What knowledge might be derived from that data? How would a nurse leader use clinical reasoning and judgment in the formation of knowledge from this experience?

In a 1000-1,250 word essay, summarize the study, explain the ways in which the findings might be used in nursing practice, and address ethical considerations associated with the conduct of the study.

Qualitative Study

Background of Study:

·         Identify the clinical problem and research problem that led to the study. What was not known about the clinical problem that, if understood, could be used to improve health care delivery or patient outcomes? This gap in knowledge is the research problem.

·         How did the author establish the significance of the study? In other words, why should the reader care about this study? Look for statements about human suffering, costs of treatment, or the number of people affected by the clinical problem.

·         Identify the purpose of the study. An author may clearly state the purpose of the study or may describe the purpose as the study goals, objectives, or aims.

·         List research questions that the study was designed to answer. If the author does not explicitly provide the questions, attempt to infer the questions from the answers.

·         Were the purpose and research questions related to the problem?

Method of Study:

·         Were qualitative methods appropriate to answer the research questions?

·         Did the author identify a specific perspective from which the study was developed? If so, what was it?

·         Did the author cite quantitative and qualitative studies relevant to the focus of the study? What other types of literature did the author include?

·         Are the references current? For qualitative studies, the author may have included studies older than the 5-year limit typically used for quantitative studies. Findings of older qualitative studies may be relevant to a qualitative study.

Case Study 5-6, Develop a Motivation Plan

Jane Crouch is the director of nursing for a 400-bed nonprofit hospital in the Southwest. Susan Smith joined the hospital as a staff nurse three years ago after relocating from the Northeast.  She is 30 years old and has been a staff nurse since graduating from a two-year college nursing program 10 years ago. She is married to a lawyer, and they have two children, ages 6 and 8.

The hospital’s inpatient census has been extremely high because of another hospital’s closing.  The tension on the nursing floors has been running pretty high because of time pressures to discharge patients early, lack of professional staff, and an upcoming accreditation visit from The Joint Commission. Because of time restraints, Jane was unable to complete the staff’s annual performance evaluations. However, all nurses received a 5 percent pay increase. With this increase, the hospital staff is now the highest paid as compared with other hospitals within the region. Jane believes the higher pay compensates the nursing staff for their increased workload and related stress levels.  

Until recently, Jane has been pleased with Susan’s performance. Susan has demonstrated her willingness to work hard and has made very few, if any, patient-care errors. However, over the past three months, Jane has noticed that Susan is not performing at her same productivity level and appears to argue frequently with the treating physicians and other nurses about the patient’s treatment plans. Jane frequently hears Susan complaining that “no one listens to me,” “no one wants to hear my opinion,” and “they don’t pay me enough to do this job.”

Susan was once a highly motivated, productive member of the nursing staff. Jane understands that everyone is experiencing more stress than usual because of the increased workload, but what can be done to motivate Susan to her prior “self”?

Based on these experiences, what would you surmise about the responsibilities of managers and leaders and about the distinctions between these two categories in health care settings?

This week’s Learning Resources classify management and leadership (which are often confused in everyday discussion) and explain their significance for health care organizations. As you advance professionally, it is critical to understand the distinctions between management and leadership and how you can apply this knowledge for increasing effectiveness in your workplace.

To prepare:

  • Review the information in the Learning Resources.
  • Conduct additional research on your own and select at least two current, credible sources that contribute to your understanding of management and leadership.
  • Reflect on how the roles of management and leadership differ in supporting the organization to set and achieve goals.
  • Drawing upon specific examples from a current or previous practice setting, bring to mind someone who seemed to be a leader but not a manager and someone who seemed to be a manager but not a leader (generally speaking, or within a specific circumstance). Be prepared to support your assessment with specific behavioral descriptions found in the literature.

Myriad characteristics influence the experiences of patients, nurses, and others in any health care setting. In addition to the factors noted above, the mission, vision, and values that guide an organization, as well as an institution’s commitments to patient care, teaching, and/or research, warrant significant consideration.

As a nurse leader–manager, it is helpful to look at all of these aspects, especially as they relate to strategic planning and the sustainability of the organization. To that end, for this Discussion you compare multiple aspects of two organizations and consider the implications for nursing. In addition, you will apply a Strategic Planning tool, such as SWOT or Balanced Scorecard, or Six Sigma principles to establish one realistic goal or objective consistent with the mission of the organization.

To Prepare

  • Review the information in the Learning Resources regarding strategic planning; types of organizations; organizational commitments to patient care, teaching, and/or research; and the mission, vision, and values of organizations.

Overview of the Project

This course project requires you to complete a group task. As a group, you are required to submit a presentation on the theorist assigned to your group. Your final presentation is due in Week 4.

Group Formation

You will be organized into three groups, each of which will be assigned a nursing theorist. The theorists to be studied include, but are not limited to, Madeline Leininger—Transcultural Nursing Theory, Nola Pender; the Health Promotion Model, and Patricia Benner; From Novice to Expert, the Skill Acquisition ModelIf your group would like to study a different theorist, seek permission from your instructor. Each group will conduct a literature review to obtain material related to the assigned theorist and the model. The material should include research conducted in the theory or model, with clinical examples.

Project Deliverables

The Project Deliverables include the following:

1. Packet of Materials

Each group will prepare a packet of materials that will be posted to the DQ section in Week 4 for peer critique and submitted to the W4: Assignment 2 Dropbox. The purpose of this packet is to support your PowerPoint Presentation. The packet should include a detailed outline of the presentation and a one-page summary of each of the five relevant articles on or about the theory. Also include any handouts, such as charts, diagrams, or definitions, which will facilitate understanding of the PowerPoint presentation and the theory itself. This packet should be submitted as a single document that contains the outline, summaries, and any additional handouts, charts, etc.

2. Power Point Presentation

Presentations will be made via PowerPoint and will be uploaded by the assigned due date.

Your presentation should have:

  • A brief overview of the theorist’s background.
  • Discussion of the theorist’s view of the four basic metaparadigms and two concepts unique to the theory.
  • The basic theoretical assertions or propositions.
  • An analysis and a critique of the model with a focus on elaboration of the theory’s implications for nursing practice, nursing education, and nursing research.

Please note: It is expected that each one of you will download and read each presentation, and will include relevant material about each theorist in your individual concept papers.

Working Ahead

In Week 1, your group will be assigned the theorist on whom you have to make the presentation. Your final group presentation is due in Week 4. However, it is recommended that in Week 2 you submit the work you have done on the project up to that point to the facilitator when necessary. The facilitator feedback will help you revise and refine the work already completed and apply the facilitator recommendations to the yet-to-be completed part of the project.

For ease of completion, it is suggested that in Week 1, you begin your literature search on the theorist and provide a brief overview of the theorist’s background. You could also complete a 1-page summary for each  of the five articles related to the theory and theorist, the top of the page should have the complete reference in APA format. In Week 2, you can focus on providing your findings on the theorist’s view of the four basic metaparadigms . In Week 3, you can concentrate on providing the basic theoretical assertions or propositions and you can conduct an analysis and a critique of the model with a focus on elaboration of the theory’s implications for nursing practice, nursing education, and nursing research. Evidence-based practice involves a great deal more than simply reading nursing periodicals on a regular basis. Nurses can take a more proactive approach to evidence-based practice by identifying authentic problems and concerns, and then using that to guide their inquiries into current research. In this way, nurses can connect the results of relevant research studies to their nursing practice.

For the Course Project, you identify and apply relevant research to a specific nursing topic or problem. You begin by formulating an answerable question that is relevant to nursing and evidence-based practice. In later weeks of this course, you continue the course project by conducting a literature review and then determining how the evidence from the literature can be applied to nursing practice.

Course Project: Part 1–Identifying a Researchable Problem

One of the most challenging aspects of EBP is to actually identify the answerable question. —Karen Sue Davies

Formulating a question that targets the goal of your research is a challenging but essential task. The question plays a crucial role in all other aspects of the research, including the determination of the research design and theoretical perspective to be applied, which data will be collected, and which tools will be used for analysis. It is therefore essential to take the time to ensure that the research question addresses what you actually want to study. Doing so will increase the likelihood of obtaining meaningful results.

In this first component of the course project, you formulate questions to address a particular nursing issue or problem. You use the PICOT model—patient/population, intervention/issue, comparison, and outcome—outlined in the weekly Learning Resources to design your questions.

To prepare:

  • Review the article, “Formulating the Evidence Based Practice Question: A Review of the Frameworks,” found in the Learning Resources for Week 2. Focus on the PICOT model for guiding the development of research questions.
  • Review the section beginning on page of the course text, 75 titled, “Developing and Refining Research Problems” in the course text, which focuses on analyzing the feasibility of a research problem.
  • Reflect on an issue or problem that you have noticed in your nursing practice. Consider the significance of this issue or problem.
  • Generate at least 5 questions that relate to the issue that you have identified. Use the criteria in your course text to select one question that would be most appropriate in terms of significance, feasibility, and interest. Be prepared to explain your rationale.
  • Formulate a preliminary PICOT question—one that is answerable—based on your analysis. What are the PICOT variables (patient/population, intervention/issue, comparison, and outcome) for this question? Note: Not all of these variables may be appropriate to every question. Be sure to analyze which are and are not relevant to your specific question.
  • Using the PICOT variables that you determined for your question, develop a list of at least 10 keywords that could be used when conducting a literature search to investigate current research pertaining to the question.

To complete:

Write a 3- to 4-page paper that includes the following:

  • A summary of your area of interest, an identification of the problem that you have selected, and an explanation of the significance of this problem for nursing practice
  • The 5 questions that you have generated, and a description of how you analyzed them for feasibility  
  • Your preliminary PICOT question and a description of each PICOT variable relevant to your question  
  • At least 10 possible keywords that could be used when conducting a literature search for your PICOT question and a rationale for your selections

Your rough draft should include all of the research paper elements of a final draft, which are listed below. Based on the reading assignment (McEwen & Wills, Theoretical Basis for Nursing, Unit II: Nursing Theories, chapters 6–9), select a grand nursing theory.

Think about your Practicum Project. Which groups would find your project findings, conclusions, and recommendations most valuable? Why? How might your focus differ if you were delivering a presentation at a professional conference versus at your practicum site? Ask which organizations or journals they would recommend you consider for dissemination based on your Practicum Project topic.
  • Consider the following questions related to dissemination:
    • Who is your target audience? Who would be most interested in your project findings, conclusions, and recommendations? Consider how the needs or dispositions of the intended audience may inform your choice of dissemination method.
    • Which method of dissemination would you choose to convey the findings, conclusions, or recommendations from your Practicum Project? What are the relative benefits and challenges of giving a presentation, sharing your findings through a poster session, and/or publication?
    • Consider various nursing, health care, informatics, and leadership and management organizations and journals. What is one organization or journal you would select for your presentation or publication?
    • What are some potential challenges? Evaluate what you could do to promote the possibility that your presentation, poster, or manuscript would be accepted.
  • Conduct a search of the literature using the Walden Library and other reliable sources. Locate two or more articles published in peer-reviewed journals within the last five years that relate to your Practicum Project. These articles should demonstrate the significance of the topic for nursing leadership and management or nursing informatics. Note: These articles may also help you determine if your project findings are related to an area that will contribute to a more comprehensive literature base. (See attached PDF)

Research Project

Students this project will allow you to formulate and hypothetically develop your own research project. The purpose of this project is for the student to complete an abstract submission to a specific nursing journal. The assignment must be strictly following APA guidelines, points will be deducted if otherwise (See Rubric).

An abstract is a concise summary of a larger project (a thesis, research report, performance, service project, etc.) that concisely describes the content and scope of the project and identifies the project’s objective, its methodology and its findings, conclusions, or intended results.

Abstract Outline:

-Title of Project

-Problem Statement: what is the problem that needs fixing?

-Purpose of the Project

-Research Question(s)

-Hypothesis

-Methodology (Qualitative vs. Quantitative)

            -Steps in implementing your project

-Results (Pretend results)

-Conclusion

Follow the following guidelines:

1-Student will think about a problem in their nursing career that needs fixing (Example, Increase number of falls in delirious patients in ICU). 

2- Student will select a nursing journal (not medical journal, ex ANA) and review the abstract submission guidelines for that specific journal.

3- Paper: The paper will consist on three sections

1-Introduction: in which the student will reveal the selected journal and reasoning for selecting such journal. And a brief section on the submitting criteria for the journal (DO NOT PLAGARIZE).

2-Research steps: The student will follow the journal’s abstract guidelines and complete the abstract outline steps based on the hypothetically selected problem as above. If the selected journal does not require the steps highlighted, and then follow the highlighted steps.

3-Explain how your hypothetical project will impact the nursing career.

4-Follow APA format, meaning Title page, Abstract, Body and Reference page.
