J.D. is a 70-year-old man with a history of prostate cancer, being treated with palliative hormones and radiation. His admitting diagnosis is adenocarcinoma of the prostate with widespread bone metastasis. J.D. is married and has one grown daughter, who often helps with his care. His chief concern is severe back pain for which the physician has ordered intrathecal morphine sulfate and aspirin 650 mg for pain relief. Antibiotic overuse is another issue that is of concern in our health care system. Improper outpatient use leads to an increase in antibiotic resistance.  Reasons for overuse are things like: the patient expects to get an antibiotic when they visit a provider; patient satisfaction scores, diagnostic uncertainty, and of course, time pressures on the providers. So, an antibiotic is prescribed “just in case.”

            When deciding on curriculum changes/advancements or any new program/change, it is important to focus on both internal and external factors as well as do a needs assessment. These factors can either be positive or negative, and some factors faculty can control while others are out of the faculty’s control. Internal factors are those that are within the institution/clinic etc. and external factors are those that influence the change or development but are external (Keating & DeBoor, 2018). “In any dynamic organization, curriculum change is not a choice but a requirement” (Billings & Halstead, 2016, p. 73). Changes in curriculum or always needed to keep up to date with changes in the community, town, country, and world (Billings & Halstead, 2016).

            This week’s post is going to focus on the external factor: population demographics and trends. Before implementing any changes, it is important to know the characteristics of the population. It is important to know who the potential students will be and who the students will be taking care of once they graduate. Knowing whether there are adult learners or younger learners can help with curriculum advancements. Ethnicity, financial resources, and faculty availability are crucial to curriculum development (Keating & DeBoor, 2018).

            When I worked out west in a college in downtown Calgary, it was very clear based on patient demographics that ethnicity and financial resources were key factors to keep in mind when developing curriculum. Many of the students first language were not English, they had immigrated to Canada, and many had financial struggles. A lot of the students were nurses, pharmacists and even physicians in their home country. Cultural sensitivity will need to be taken into consideration with all changes/developments in the curriculum (Billings & Halstead, 2016). If students can’t afford to go to school or don’t have access to financial resources, they may not perform to their highest standard in school. I had students who came to clinical exhausted and who I had to send home for safety reasons. These students had to work multiple jobs to get by and provide for their family. A lot of these students also needed help with their English, and therefore the college had to take this into mind and make sure the resources were available for English tutoring. Calgary, in general, had a lot of minorities, and therefore these are the patients that the students will be taking care of when they graduate.

            The demographics of our population are rapidly shifting and “by 2030 it is estimated people age 65 and older will represent almost 20% of the total U.S. population” (Billings & Halstead, 2016, p.74). This means nurses need to be trained in chronic care and end of life care (Billings & Halstead, 2016). There will also be an increased need for home care. These statistics help nursing faculty to adjust the curriculum appropriately to make sure the newest generation of nurses is trained to help this population (University of Saint Mary, 2019).

Addressing the issue of workplace violence (WPV) is a significant concern for organizations. WPV of any form (verbal, physical, psychological) increases the chances that nurses will leave the profession permanently, resulting in nursing workforce retention challenges (Marshall  & Broome, 2017). Nurses must maintain environments that allow the profession to be virtuous, allowing for mutual respect, integrity, communication, caring, and benevolence (American Nurses Association (ANA), 2015). Workplace violence does not just include nurses or other healthcare professionals but also patients/ family members being uncivil.

As a new nurse, my preceptor was off the floor for a meeting. The charge nurse did not care for me, and she never had any problems showing it. With two open rooms and already taking care of a DKA patient and prepping another patient for surgery with low hemoglobin. The EMS radio called with a code blue and the eta was 5minutes. There were other rooms and nurses available to take the code, but she placed the patient in my room and offered no assistance, and within minutes of getting the code blue patient she placed yet another patient in the fourth room. 

Feeling overwhelmed and very upset; why would she deliberately try to sabotage a new nurse. My preceptor, nurse manager, and I confronted her and asked why she would intentionally risk patient safety and stack patients on one nurse. Her response was “No one told her to stay in the ED, who is she to try to change the way we do things here, I do not agree with having graduate nurses in the ED, she would be better off on one of the inpatient units.” 

Workforce resources are competing for needs maintaining and implementing policies to combat such behaviors. The consequences of this situation impacted my other three patients and delayed care; the whole situation created a negative attitude towards the rest of the nursing staff, and trusting anyone and asking for help when my preceptor was not available was out of the question. I felt like this situation needed to happen so that management could see the negative culture/attitudes within the department.

 With the looming nursing shortage, leadership may tolerate or entirely ignore WPV.  The charge nurse continued to bully other nurses, and the older nurses felt like, the newer nurses were coming in and changing their department. While professional standards appear specific to WPV, an atmosphere of support is essential to reducing the stigma and reoccurrence often associated with those who are victims of bullying. Standards of professional behavior must be developed and implemented with consistent use across all departments (Fink-Samnick,2015).

As indicated in this week’s Learning Resources, concept analysis is an essential step in research and has significant implications for theory development.
