Discussion: Validity in Quantitative Research Designs

NURS 5052/NURS 6052: ESSENTIALS OF EVIDENCE-BASED PRACTICE – Discussion 6 (Grading Rubic and Media Attached)

Discussion: Validity in Quantitative Research Designs

Validity in research refers to the extent researchers can be confident that the cause and effect they identify in their research are in fact causal relationships. If there is low validity in a study, it usually means that the research design is flawed and the results will be of little or no value. Four different aspects of validity should be considered when reviewing a research design: statistical conclusion validity, internal validity, construct validity, and external validity. In this Discussion, you consider the importance of each of these aspects in judging the validity of quantitative research.

To prepare:

Review the information in Chapter 10 of the course text on rigor and validity.

Read the method section of one of the following quasi-experimental studies (also located in this week’s Learning Resources). Identify at least one potential concern that could be raised about the study’s internal validity.

Metheny, N. A., Davis-Jackson, J., & Stewart, B. J. (2010). Effectiveness of an aspiration risk-reduction protocol. Nursing Research, 59(1), 18–25.

Consider strategies that could be used to strengthen the study’s internal validity and how this would impact the three other types of validity.

Think about the consequences of an advanced practice nurse neglecting to consider the validity of a research study when reviewing the research for potential use in developing an evidence-based practice.

Post the title of the study that you selected and your analysis of the potential concerns that could be raised about the study’s internal validity. Propose recommendations to strengthen the internal validity and assess the effect your changes could have with regard to the other three types of validity. Discuss the dangers of failing to consider the validity of a research study.

Read a selection of your colleagues’ responses.

Respond to at least two of your colleagues in one or more of the following ways:

Ask a probing question, substantiated with additional background information, and evidence. Share an insight from having read your colleagues’ postings, synthesizing the information to provide new perspectives.

Offer and support an alternative perspective using readings from the classroom or from your own review of the literature in the Walden Library.

Validate an idea with your own experience and additional sources.

Make a suggestion based on additional evidence drawn from readings or after synthesizing multiple postings.

Expand on your colleagues’ postings by providing additional insights or contrasting perspectives based on readings and evidence.

Job Search, Application, and Interview Processes

Week 11 – Discussion 1

Top of Form

Job Search, Application, and Interview Processes

Throughout your program of study, you have been encouraged to think about your strengths, values, and professional aspirations, as well as the opportunities you have to create positive impact as a nurse leader-manager or nurse informaticist.

How would you most like to leverage your knowledge and expertise going forward? What types of professional positions could you see yourself in, now and further along in your career? What steps could you take to identify a job that appeals to you and to secure a position that you find intellectually challenging and rewarding?

In this Discussion, you examine strategies for conducting an effective job search and assess how you could convey your qualifications for a position of interest to you.

To prepare:

·         Reflect on your strengths, values, and professional aspirations. How will these inform the next stage of your career?

·         Consider what you have learned through your experiences in the practicum about professional roles and functions in your specialty area.

·         Review the information in the Learning Resources, and think about the process of conducting a job search. If possible, speak with individuals in your practicum setting as well as other nursing professionals about how they have identified positions that align with their needs and ambitions. Ask for advice about conducting a successful job search.

·         Search postings of open positions that appeal to you, and identify one for which you would like to apply. If you have difficulty finding such a position, for the purposes of this Discussion you may select one that has already been filled or is in a different region. (Note: If you are not interested in finding a new position, please approach this activity as an opportunity to explore the field. If you are interested in changing positions, please use this activity for that purpose.)

·         What steps would you take to apply for and secure an interview for this position? Write a cover letter and target your résumé or Curriculum Vitae (CV) to this position. (Note: You are not required to complete the job application for this Discussion.)

·         How would you prepare for an interview for this position? What questions do you think would be posed, and what information would you like to learn about the position and the organization? Ask a colleague or friend to conduct a practice interview with you.

·         Consider how you might use your Professional Portfolio (submitted this week) to demonstrate your capabilities during the interview process.

With these thoughts in mind:

By tomorrow 5/9/17, post 550 words essay in APA format with at least 3 references from the list below. Apply the level one headings as numbered below:

1)      An explanation of how you would conduct a job search for a position of interest to you, including the resources you would use (and did use to identify the position for this Discussion).

2)      Include a brief description of the position you identified.

3)      Share an application and interview plan for that position, noting how you would convey your qualifications for this position.

Discussion 1: Social Technologies in the Workplace

NURS 6221: MANAGING HUMAN RESOURCES – Discussion 11 -1 (Grading Rubic and Media Attached)

Discussion 1: Social Technologies in the Workplace

As a nurse manager, it is imperative to understand how social technologies can impact productivity, peer-to-peer relationships, and patient safety within the workplace. Collaborating with HR to understand policies and the organization’s expectations related to the use of social technologies allows nurse managers to more effectively and appropriately integrate today’s social platforms while mitigating the occurrence of inappropriate behaviors.

For this week’s Discussion, your Instructor will assign you to one of the scenarios below. You will then use that scenario to investigate the social, ethical, and legal ramifications of social technologies.

Scenario One: You have recently been promoted to charge nurse for the day shift in your department. As a result of this promotion, your former peers are now reporting directly to you. You have been working in your setting for five years and consider many of these peers to be your friends. The way that one of your friends posts on social media sites has always bothered you. Many times, her comments are inappropriate, discussing her negative feelings about “ignorant superiors” and “annoying patients.” You also know that she frequently accesses these sites for extended periods of time while patients are waiting to be helped. Now that she reports directly to you, how do you address this? If your workplace does not have a formal policy on social media use, how can you adapt her behaviors to align with the expectations of nursing professionalism?

Scenario Two: It has been a little over a year since you accepted your nurse management position at a local pediatrics office. Since then, you have observed that many nurses seem uneasy when they work with one of your top physicians. You wonder why so many nurses are hesitant to work with such an experienced physician, particularly since most of your families rave about her dedication and caring nature toward their children. One day, while taking careful observation of your staff, you watch a nurse take out a patient chart and begin to text. The nurse becomes visibly upset as she sees you approach. When you question her about her behavior, she confides that the physician asks all nurses to text patient test results to her. The nurse admits that she feels uncomfortable sending private information via text message and only did so after multiple requests and increasing pressure from the physician. According to ethical and legal guidelines, who is at fault for this error in judgment? Since the nurse is the employee who sent the information, should action be taken against her, against the physician, or against both parties?

Scenario Three: A physician in your setting is an avid user of social media. On many of his personal pages, which include blogs about his various outdoor hobbies, he plasters pictures of himself and his friends out drinking. He also tends to post extreme comments about politics and the economy. Many in your setting joke with him about the intensity of his social life, to which he always comments, “Work hard, play hard.” Though his actions are not hurting the morale of the setting, and his posts are always before or after work hours, should anything be said to this physician? In the future, could your setting experience any ramifications because of his presence in social media?

Scenario Four: The environment in your critical care unit has always been somewhat hectic. However, over the last few months, the morale in your unit has noticeably deteriorated. You learn that nurses have begun to share mass e-mail chains complaining about unfair scheduling treatment. As the nurse manager, you have always tried your best to schedule staff fairly and even allow them to switch their shifts when personal problems arise. Since your effort to talk with staff individually does not seem to be working, you decide to hold a town hall meeting to openly discuss these issues. As you prepare the meeting announcement, you notice e-mails from your hospital administrator, chief operating officer and director of human resources. All are wondering why their inboxes have been inundated with e-mails from upset nurses in your unit. How do you respond to these e-mails? Furthermore, how should you address this situation to improve the morale of your unit?

To prepare

Review this week’s Learning Resources, focusing on the appropriate and inappropriate use of social technologies in the workplace.

Conduct further research on the social, ethical, and legal issues that result from inappropriate use of social technologies both inside and outside work hours.

For example, what laws protect the privacy and free speech of employees? How can workplaces legally safeguard themselves from various social technology issues such as defamation, misrepresentation, or misuse by individual employees? Of what legal ramifications, such as patient privacy and confidentiality laws, should managers be aware?

Reflect on the scenario to which you have been assigned.

How might employee use of social technologies impact the rest of the setting? How might it impact the care given to patients?

As the nursing manager who oversees the employee(s), what strategies (if any) would you employ to effectively address this situation?

Consider the social technology policies of your past or present workplace.

With regard to employee use of social media, do you believe the workplace policy is effective? Why or why not? As a nurse manager, could you use this policy to effectively address the behaviors in your assigned scenario?

Note: Before you submit your initial post, replace the subject line (“Week 11 Discussion”) with “Review of Case Study ___” identifying the number of the case study you were assigned.

Post an explanation of the possible social, ethical, and/or legal ramifications of your assigned scenario. Explain the policy your current or past workplace has on the use of social technologies and how effective you believe the policy is. If your workplace does not have a policy, explain what the accepted practices or expectations are for your setting. Has lack of policy led to any problems? Support your responses by referencing authentic examples from the workplace and this week’s Learning Resources as appropriate.

Read a selection of your colleagues’ responses.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Ask a probing question, substantiated with additional background information, evidence, or research.

Share an insight from having read a colleague’s posting, synthesizing the information to provide new perspectives.

Validate an idea with your own experience and additional research.

NURS 6241: STRATEGIC PLANNING IN HEALTH CARE ORGANIZATIONS – Discussion 5 (Grading Rubic and Media Attached)

Discussion: Trend Analysis and Forecasting

Green Hills Health System comprises a small community hospital and a primary care practice. Located in a rural area, the nearest large city is approximately 100 miles away. Individuals age 60 and older constitute one of the largest patient populations Green Hills serves. Recently, members of the nursing staff have noticed that the numbers of geriatric patients admitted to the hospital seem to be increasing. Many of these patients have multiple comorbidities and need specialty care that is not available through Green Hills Health System. Some of the patients have commented that they lack the financial resources and transportation required to travel to the large metropolitan health care center that provides a variety of specialty services.

Imagine that you are a nurse leader-manager in the organization described above. How could conducting an internal and external scan of the environment help you and your colleagues to identify opportunities for change moving forward? What literature sources would you consult to learn about trends and emerging issues that are likely to affect the organization? What data would you examine?

In this Discussion, you analyze trends that are likely to have a significant impact on nursing and health care in the coming years. You may use this Discussion to formulate initial thoughts related to trends and unmet needs that you might choose to address through the Course Project, which is introduced this week.

To prepare:

Review the information on forecasting and trends in the Learning Resources.

Conduct a search of the literature to investigate trends and emerging issues in nursing and health care. Evaluate the research evidence in the literature.

Based on your research, identify three trends that are likely to impact nursing and health care in the coming decade.

Select one of these trends on which to focus in greater detail. Then, conduct additional research and analyze evidence-based issues related to this trend.

Based on your analysis, consider whether this trend creates or contributes to an unmet need that currently impacts, or will impact, a specific group, unit, or organization.

Post an analysis of the impact of your selected trend on nursing and health care in the coming decade. Explain how this trend creates or contributes to an unmet need that a specific group, unit, or organization may encounter now or in the coming years.

Read a selection of your colleagues’ responses.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Share an alternative perspective on a trend reported by your colleagues, synthesizing the information to provide a different perspective.

Ask a probing question, substantiated with additional background information or research.

Validate an idea with your own experience and additional resources.

Explain how theoretical frameworks influence advance.

Class 2 Unit 5 ASSIGNMENT

Where Did the Theory Come From

This Assignment addresses this course outcome:

Explain how theoretical frameworks influence advance.

Purpose

The purpose of this Assignment is to explore how a theorist explicates his or her philosophy and thoughts behind a theoretical field. As in other fields of study, nursing has a plethora of theorists and theories. These theorists have developed their paradigm over time enriching it with research and dialogue with other theorists.

Directions

You are going to explore how a theorist of your choice created his or her theory. Did it develop as an acorn becoming a mighty oak over the decades? Did it arrive as a burst of light like the big bang as some believe created our universe? Was it developed from a blueprint like a spaceship where thousands of scholars worked together to create a rocket to the moon? In point of fact, how is a theory developed? This is what we are exploring.

You will pick a theorist who interests you, it does not have to be a nurse theorist. You may also choose from other disciplines such as: behavioral, leadership, business, education, technology.

You will become the theorist immersing yourself in the writings from the earliest mentioned to the most current.

As you read, look at how you (taking on the persona of the theorist) developed the theory.

In the first stage, theorizing occurs. This is where you, as the theorist, identify the concepts of what nursing is and is not. Perhaps you questioned what concepts were guiding those in nursing practice and then started to question your role. You started asking yourself, “Where am I in nursing, and where the profession is going? Is there some overarching concept that guides the professional in his or her practice?” This is where you recognize that a theory is needed.

In the second stage, syntax is developed. This is where you will define the terms. Look for changing definitions of terms. Consider, for example, Jean Watson. She starts by defining the word “caring.” Within the last ten years she has refined her terminology changing the term caring to caritas. This demonstrates a growth and maturation from decades of research she and other scholars did to produce the theory of caring.

The third stage is theory testing. Defined as the phenomena behind the theory that are exposed through research. The definitions of terms are refined. The theorists and other researchers consider whether this theory helps answer questions that arise in practice. This is where your theory is used in by a widening group of researchers. For example, graduate nursing students request the tool you developed while testing your theory.

The fourth and last stage is evaluation. This is where philosophical debate occurs as the concepts are applied through evidenced based practice in the act of providing nursing care.

Assignment Details

For this Assignment, you are going to write an article for a nursing journal explaining how you developed your theory through the four stages (theorizing, syntax, theory testing, and evaluation). Your paper must be 3 to 5 pages, not including the title and reference pages.

                                Critical Thinking in the Workplace

While there is no clear definition of critical thinking, it can often be associated with the terms “evaluation”, “analysis”, and “higher-thinking” (Horvath & Forte, 2011, pg. 1). Critical thinking can be described as the process in which one uses logic and reason to determine a solution to a problem. As nurses, we develop critical thinking skills through our practices and expansion of knowledge. To critically think one must assess the problem, determine solutions, and apply them to the current situation. Critical thinking is a sought out skill that improves one’s ability to make reasonable decisions in and out of the workplace.

Critical thinking is an essential part in the delivery of high-quality patient care.  In the Emergency Department (ED) for example, patients are triaged and treated based on their level of acuity.  Let’s say that two patients present to the ED. The first patient is a 58 year old female who presents with chest pain and productive cough that started three days ago. The second patient is a 69 year old male who presents with sudden 10/10 “tearing” pain in his upper abdomen and chest. Based on signs and symptoms the nurse would use critical thinking to prioritize and determine which patient needs immediate treatment. In this circumstance, patient number two would be evaluated first; this is due to the high possibility of aortic dissection.  Prioritization and critical thinking are not only used in the ED but as well as other healthcare settings.  Critical thinking skills are essential in the nursing field because they allow nurses to prioritize and make key decisions that can save lives (Nurse Journal, 2005-2018, para. 1).

                                 Strategies to Critical Thinking

In recent years there has been emphasis placed on the quality of care provided by nurses. To ensure the delivery of high quality patient care, it is essential that nurses maintain a high level of competence to respond effectively to complex clinical situations (Carvalho et al., 2017, pg. 103-107).   Two strategies that are often used to encourage the development of critical thinking include problem-based learning (PBL) and simulations.

PBL is a student-centered learning method that focuses on applying theory to real life situations. The use of open-ended questions in PBL allows students to think critically and provide feedback. Simulation labs are mock scenarios that allow health care professionals to practice in a controlled environment. Simulation labs are found to be highly effective as they allow students to critique their skills until competent. While there are several strategies used to develop critical thinking, it is essential to determine which strategies works best for each individual to promote adequate learning.

Critical Thinking, Nursing Practice, and Scholarship

Critical thinking is an essential part of one’s nursing practice as it helps to build confidence and competency in the delivery of patient-directed care. With continued education and practical application nurses can develop skills needed to practice at a scholarly level. To continue to advance in one’s nursing practice nurses must evaluate strategies essential for the development of critical thinking skills.

From my Prof. This week’s assignments build off of last week and further the foundation for your capstone project.  They will help you to:

  1. Build a thorough literature review based on your rapid appraisal from last week.  Use the literature review sample format questions provided in the course materials and
  2. Choose and describe a nursing (or other discipline’s) theory that supports your intervention.  A common error with this assignment is to write about your proposal as if it were a theory (hypothesis).  Please review the readings for this week so that you can choose a nursing theory that will make the most sense to support your proposal. (Examples:  The theory of planned behavior, the health promotion model, organizational change model)

Learning Online

Discussion : Learning Online

Completing courses online can present some challenges. Yet, it is possible—and advisable—to apply the skills you use to be successful at work in this unique learning environment. Consider, for instance, what you have learned throughout your career about the value of careful time management, planning, and effective use of technology. How might these skills help you to accomplish your academic goals while fulfilling responsibilities in other areas of your life?

This Discussion builds upon last week’s introduction to your chosen doctoral degree, and asks you to evaluate the skills that can help you to be a successful online learner.

To prepare:

  • Review Section 1 in the Essential Guide to Online Learning in this week’s Learning Resources.
  • Then review the strategies and tips in the documents linked in this week’s Learning Resources, such as “Technical Tips for Learning at Walden,” “Citing a Discussion Posting in APA Style,” and those presented in the Essential Guide to Online Learning. Look for strategies related to time management, planning, studying, and using technology that you think would contribute to your success as an online learner in each of these areas.
  • Which of these strategies have you employed in the past (academically or professionally)? Of those you have not tried, which do you think would be most valuable? Why do you think so?

By tomorrow 6/6/17, post 550 words essay in APA format with 3 references from the list below, that include the level one heading as numbered below:

Post a cohesive response that addresses the following:

1)       Describe one strategy that you have used in the past (academically or professionally) that has empowered you in terms of one or more of the following skill sets: planning, time management, studying, and effective technology use. Explain how this strategy has been advantageous.

2)       Evaluate a strategy that you have yet to try but believe would be beneficial given your strengths and weaknesses.

Write a 3–5 page safety score improvement plan for mitigating concerns, addressing a specific patient-safety goal that is relevant to quality patient care.

Determine what a best evidence-based practice is and design a plan for resolving issues resulting from not maintaining patient safety.

Quality improvement and patient safety are health care industry imperatives (Institute of Medicine’s Committee on Quality of Health Care in America, 2001). Effective quality improvement results in system and organizational change. This ultimately contributes to the creation of a patient safety culture.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

  • Competency 2: Apply systems theory and systems thinking to facilitate health care delivery and patient outcomes.
    • (IMPORTANT) Apply systems thinking to explain how current policies and procedures may affect an identified safety issue.
    • (IMPORTANT) Explain an evidence-based strategy to collect information about, and improve, an identified safety concern.
    • (IMPORTANT) Explain a plan to implement a recommendation and monitor outcomes.
  • Competency 4: Evaluate how power relates to health care organizational structure, behavior, and leadership.
    • 9IMPORTANT) Describe the influence of nursing leadership in driving needed changes.
  • Competency 5: Communicate in a manner that is consistent with the expectations of a nursing professional.
    • (IMPORTANT) Write content clearly and logically, with correct use of grammar, punctuation, mechanics, and current APA style.

This is what need to be covered, resumed:

-Explain an evidence-based strategy to collect information about, and improve, an identified safety concern.

-Apply systems thinking to explain how current policies and procedures may affect an identified safety issue.

-Explain a plan to implement a recommendation and monitor outcomes.

-Describe the influence of nursing leadership in driving needed changes.

-Write content clearly and logically, with correct use of grammar, punctuation, mechanics, and current APA style.

Preparation

Refer to the Capella library and the Internet for supplemental resources to help you complete this assessment, a safety score improvement plan.

Consider the hospital-acquired conditions that are not reimbursed for under Medicare/Medicaid. Among these conditions are specific safety issues such as infections, falls, medication errors, and other safety concerns that could have been prevented or alleviated with the use of evidence-based guidelines.

Hospital Safety Score, an independent nonprofit organization, uses national performance measures to determine the safety score for hospitals in the United States. The Hospital Safety Score Web site and other online resources provide hospital safety scores to the public.

Read the scenario below:Scenario As the manager of a unit, you have been advised by the patient safety office of an alarming increase in the hospital safety score for your unit. This is a very serious public relations matter because patient safety data is public information. It is also a financial crisis because the organization stands to lose a significant amount of reimbursement money from Medicare and Medicaid unless the source of the problem can be identified and corrected. You are required to submit a safety score improvement plan to the organization’s leadership and the patient safety office.

Select a specific patient safety goal that has been identified by an organization, or one that is widely regarded in the nursing profession as relevant to quality patient care delivery, such as patient falls, infection rates, catheter-induced urinary infections, IV infections, et cetera.

Deliverable: Safety Score Improvement Plan

Develop a 3–5 page safety score improvement plan.

  • Identify the health care setting and nursing unit of your choice in the title of the mitigation plan. For example, “Safety Score Improvement Plan for XYZ Rehabilitation Center.”
  • You may choose to use information on a patient safety issue for the organization in which you currently work, or search for information from a setting you are familiar with, perhaps from your clinical work.
    • Demonstrate systems theory and systems thinking as you develop your recommendations.

Organize your report with these headings:

Study of Factors

  • Identify a patient safety issue.
  • Describe the influence of nursing leadership in driving the needed changes.
  • Apply systems thinking to explain how current policies and procedures may affect a safety issue.

Recommendations

  • Recommend an evidence-based strategy to improve the safety issue.
  • Explain a strategy to collect information about the safety concern.
    • How would you determine the sources of the problem?
  • Explain a plan to implement a recommendation and monitor outcomes.
    • What quality indicators will you use?
    • How will you monitor outcomes?
    • Will policies or procedures need to be changed?
    • Will nursing staff need training?
    • What tools will you need to do this?

Additional Requirements

  • Written communication: Written communication should be free of errors that detract from the overall message.
  • APA formatting: Resources and in-text citations should be formatted according to current APA style and formatting.
  • Length: The plan should be 3–5 pages.
  • Font and font size: Times New Roman, 12 point, double-spaced.
  • Number of resources: Use a minimum of three peer-reviewed resources.

Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.

Topic 3

System Admin

Max Points: 20.0

Use the following Case Scenario, Subjective Data, and Objective Data to answer the Critical Thinking Questions.

Case Scenario

Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40 years and has refused to quit. Three days ago, she had an onset of flu with fever, pharyngitis, and malaise. She has not taken her antihypertensive medications or her medications to control her heart failure for 4 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure.

Subjective Data

  1. Is very anxious and asks whether she is going to die.
  2. Denies pain but says she feels like she cannot get enough air.
  3. Says her heart feels like it is “running away.”
  4. Reports that she is so exhausted she cannot eat or drink by herself.

Objective Data

  1. Height 175 cm; Weight 95.5 kg
  2. Vital signs: T 37.6 C, HR 118 and irregular, RR 34, BP 90/58
  3. Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint; all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation
  4. Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%
  5. Gastrointestinal: BS present: hepatomegaly 4 cm below costal margin

Critical Thinking Questions

What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?

  1. IV furosemide (Lasix)
  2. Enalapril (Vasotec)
  3. Metoprolol (Lopressor)
  4. IV morphine sulphate (Morphine)

Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.

Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.
