E.W.  is a 40-year old African American male, who has had difficulty controlling his HTN lately. He is visiting his primary care provider for a thorough physical examination and to renew a prescription to continue his blood pressure medication.

  • One major episode of major depressive illness caused by the suicide of his wife of 15 years, 5 years ago.

The patient is an air traffic controller at the local airport. He smoked cigarettes for approximately 10 years but stopped smoking when he was diagnosed with HTN.  He drinks “several beers every evening to relax” and does not pay particular attention to the sodium, fat or carbohydrate content of the foods that he eats. He admits to “salting almost everything he eats, sometimes even before tasting it.” He denies ever having dieted. He takes an occasional walk but has no regular daily exercise program.

Option 1: The framers of the Constitution were concerned that everyday citizens would not be able to understand or comprehend the makings of our government. They felt that everyday citizens were uninformed and did not care what was going on in our government. Even today we see where citizens are interested in government affairs seemingly only if our country is in turmoil such as unemployment, recessions, civil unrest, etc. Do you agree with this assessment? Are we uninformed? Do we wait till a crisis happens to voice our opinions?

Option 2: Many experts see the media as biased and more like infotainment. In fact, many people have turned to social networks as an outlet for news instead of CBS, NBC, FOX, CNN and other news stations. Do you see this as an issue? Do you see the news as biased or unbiased? Should there be more restrictions on the news stations?

Option 2

As we learned in week two’s lesson, news media has a “huge impact of public opinion and political behavior” (Chamberlain, 2022). I do agree with the experts and feel that the news can be biased. This is an issue because the people deserve to get the story without things being left out or without options. When the news does this it is called sharing politics. They leave out parts or explain something in a way that leans towards their political side.  Everyone had their own options and working at a certain station could have their own politics involved as to what view they want to have on a piece of news. Mastrine gives a great example, “Our team noted that NPR does not give lots of coverage to current hot-button issues frequently seen in coverage on the Right — such as free speech, abortion, or the Second Amendment — and in that same vein, does not give lots of coverage to hot-button issues often covered by the Left — economic inequality, climate change, or social justice initiatives” (year page). This example is what I feel we all already know. Different media outlets have different approaches and views on how they feel about things. I think the news should be given with as many facts as possible leaving out opinions or have two opposing views review the same story. A lot of times when I am interested in a story, I go to different news outlets to hear different views on the same topic. I do not know about the restrictions that can be enforced with news, but I think people generally know which station has what political view, so they listen or watch the stations they agree with more. To have a well-rounded knowledge base when something interests me, I look at the story from as many different sources that I can find to help get a rounded view. I am then able to decipher through the opinions and facts to have my own view. It would be nice to have a news station that reports the same story with different views back-to-back so you do not have to rewatch the same news story just to try and figure out fact verse opinion and make sure nothing was left out of one news story compared to the other.

There are many mental disorders that occur early in the life course. The DSM-5 describes neurodevelopmental disorders such as intellectual disability and delay, autism spectrum disorder, language and speech disorders, ADHD and specific learning disorder (e.g., dyslexia, difficulty mastering mathematical reasoning). Diagnosis of these various conditions can rarely be made in a single office visit and often requires a comprehensive approach involving multiple stakeholders, including the child, his or her parents, teachers, other significant figures in the child’s life, and medical and mental health professionals, such as psychologists who can conduct comprehensive neuropsychological testing.

The PMHNP must coordinate and integrate several sources of information to arrive at an accurate diagnosis of these disorders. Early and accurate diagnosis is essential to developing an effective treatment plan, which will have the potential to minimize the impact of these disorders on the child’s developmental trajectory. When one considers appropriate diagnosis from this perspective, the importance of diagnostic accuracy becomes quite apparent.

Assignment: Study Guide Forum

Abnormal brain development or damage at an early age can lead to neurodevelopmental disorders. Within this group of disorders, some are resolvable with appropriate and timely interventions, either pharmacological or nonpharmacological, while other disorders are chronic and need to be managed throughout the lifespan.

For this Assignment, you will develop a study guide for an assigned disorder and share it with your colleagues. In sum, these study guides will be a powerful tool in preparing for your certification exam.

To Prepare

  • Your Instructor will assign you to a specific neurodevelopmental disorder from the DSM-5.
  • Research your assigned disorder using the Walden Library. Then, develop an organizational scheme for the important information about the disorder.

The Assignment

Create a study guide for your assigned disorder. Your study guide should be in the form of an outline with references, and you should incorporate visual elements such as concept maps, charts, diagrams, images, color coding, mnemonics, and/or flashcards. Be creative! It should not be in the format of an APA paper. Your guide should be informed by the DSM-5 but also supported by at least three other scholarly resources. A democracy is a type of government in which the people have political power, the head of state’s power is limited, the separation of powers between governmental bodies is ensured, and natural rights and civil freedoms are protected (Achen et al., 2017). The term democracy is derived from the Greek terms “people” (dmos) and “rule” (rule). However, developing and maintaining a people-centered government—a “popular” government—is significantly more complex than the concept’s semantic simplicity suggests.

Economic development and affluence do not always imply democracy. However, higher wealth levels and the presence of a solid middle class can prevent a return to authoritarianism after democracy develops (Lawson-Remer, 2013). It expounds on the study of some researchers who suggest that high GDP does not accelerate democratic changes but rather forecasts whether democracy, however it emerged, would be sustained. It is not to say that democracy needs a particular level of economic prosperity before seeking political liberty. Even if greater wealth levels aid the sustainability of democracy, democracy in low-income nations is possible. Indeed, various researchers have recently argued that democracy may lead to the fulfillment of other developmental areas, such as economic progress. Nevertheless, the more nations can promote equitable economic growth and develop a large middle class, the greater their chances of maintaining democratic gains (Lawson-Remer, 2013). Considerably under the best of circumstances, this is a challenging assignment; it is made even more difficult by the transitional nations are experiencing.

Those interested in fostering democracy across the world should keep in mind that, while economic growth only is insufficient to provide political freedom, the existence of an extensive and thriving middle class significantly improves the possibilities of successful democratization.

BACKGROUND

The client is a 34-year-old Pakistani female who moved to the United States in her late teens/early 20s. She is currently in an “arranged” marriage (her husband was selected for her when she was 9 years old). She presents following a 21-day hospitalization for what was diagnosed as “brief psychotic disorder.” She was given this diagnosis as her symptoms have persisted for less than 1 month. Prior to admission, she was reporting visions of Allah, and over the course of a week, she believed that she was the prophet Mohammad. She believed that she would deliver the world from sin. Her husband became concerned about her behavior to the point that he was afraid of leaving their 4 children with her. One evening, she was “out of control,” which resulted in his calling the police and her subsequent admission to an inpatient psych unit.

During today’s assessment, she appears quite calm and insists that the entire incident was “blown out of proportion.” She denies that she believed herself to be the prophet Mohammad and states that her husband was just out to get her because he never loved her and wanted an “American wife” instead of her. She currently weighs 140 lbs., and she is 5’ 5.

SUBJECTIVE

Client reports that her mood i s “good.” She denies auditory/visual hallucinations but believes that the television talks to her. She believes that Allah sends her messages through the TV. At times throughout the clinical interview, she becomes hostile towards you but then calms down.

A review of her hospital records shows that she received a medical workup from physician, who reported her to be in overall good health. Lab studies were all within normal limits.

Client admits that she stopped taking her Risperdal about a week after she got out of the hospital because she thinks her husband is going to poison her so that he can marry an American woman.

MENTAL STATUS EXAM

The client is alert and oriented to person, place, time, and event. She is dressed appropriately for the weather and time of year. She demonstrates no noteworthy mannerisms, gestures, or tics. Her speech is slow and, at times, interrupted by periods of silence. Self-reported mood is euthymic. Affect is constricted. Although the client denies visual or auditory hallucinations, she appears to be “listening” to something. Delusional and paranoid thought processes as described above. Insight and judgment are impaired. She is currently denying suicidal or homicidal ideation.

You administer the PANSS which reveals the following scores:

-40 for the positive symptoms scale

-20 for the negative symptom scale

-60 for general psychopathology scale
