Interview Summary

Summarize your interview in this section. Be sure to relate concise information about:

  • the health care organization that was discussed in the interview.
  • the interviewee’s role and duties.
  • the relevant issues at the organization.
  • any relevant comments on the actions taken by the organization or leadership previously and their effectiveness.
  • the organizational culture related to collaboration.
  • any relevant collaboration or interdisciplinary team experience you interviewee has had.

The goal of this section is to convey what was discussed at the interview and identify an issue to pursue throughout the rest of this assessment (and future assessments in this course), as well as have enough information to relate theories, strategies, and approaches to the situation at the organization. Interview and Interdisciplinary Issue Identification

Issue Identification

Identify the issue from the interview for which you consider an evidence-based interdisciplinary approach appropriate. Provide one or more specific reasons why an interdisciplinary approach would be appropriate for the issue.

Change Theories That Could Lead to an Interdisciplinary Solution

For this section, look to the literature and describe one or more change theories that would help to develop an interdisciplinary solution to the identified issue. Also, you should briefly note the relevance and credibility of the sources that you cite.

Another way to think through this section is:

  • What is the change theory?
  • How could it help create an interdisciplinary solution?
  • How relevant is the theory to the specific identified issue?
  • How credible is the source?

Leadership Strategies That Could Lead to an Interdisciplinary Solution

This section is similar to the previous one, except you will be looking to the literature to help you describe one or more leadership strategy that would help you develop an interdisciplinary solution to the identified issue. Also, you should briefly note the relevance and credibility of the sources that you cite.

Another way to think through this section is:

  • What is the leadership strategy?
  • How could it help create an interdisciplinary solution?
  • How relevant is the strategy to the specific identified issue?
  • How credible is the source?

Collaboration Approaches for Interdisciplinary Teams

Again, this section will be similar to the previous once, except that the focus will be on collaboration approaches. Depending on whether or not your interviewee’s organization uses interdisciplinary teams frequently, your focus in this section may be on how to establish interdisciplinary teams and a collaborative foundation or how to improve the collaboration within an already established teams. You should also briefly note the relevance and credibility of the sources that you cite.

Another way to think through this section is:

  • What is the leadership approach?
  • How could it help establish or improve collaboration?
  • How relevant is the approach to the specific identified issue?
  • How credible is the source?

NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD

NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD

Your own experiences might tell you that expectations from family, friends, and work—as well as your own expectations regarding achievement, success, and happiness—can create stress. Stressors are a normal part of life, and stress traditionally has been viewed as an adaptive function with a set of physiological responses to a stressor. In a situation where stress is perceived, the organism is physiologically prepared to attack or flee from the threat. Those with effective fight or flight responses tended to survive long enough to reproduce, so we are descended from those who are genetically hardwired for self-protection. When you experience stress, your biology, emotions, social support, motivation, environment, attitude, immune function, and wellness all feel the ripple effect. NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD

This stress response is an adaptive response the human body has to threats; however, stress can also be difficult to handle and—depending upon the nature and intensity of the stress—can result in anxiety disorders, obsessive-compulsive disorders, or trauma- and stressor-related disorders. This week, you will focus on these disorders and explore strategies to accurately assess and diagnose them. NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD

Learning Objectives – NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD

Students will:

  • Apply concepts, theories, and principles related to patient interviewing, diagnostic reasoning, and recording patient information
  • Formulate differential diagnoses using DSM-5 criteria for patients with anxiety disorders, PTSD, and OCD across the lifespan

Assignment: Assessing and Diagnosing Patients With Anxiety Disorders, PTSD, and OCD

“Fear,” according to the DSM-5, “is the emotional response to real or perceived imminent threat, whereas anxiety is anticipation of future threat” (APA, 2013). All anxiety disorders contain some degree of fear or anxiety symptoms (often in combination with avoidant behaviors), although their causes and severity differ. Trauma-related disorders may also, but not necessarily, contain fear and anxiety symptoms, but their primary distinguishing criterion is exposure to a traumatic event. Trauma can occur at any point in life. It might not surprise you to discover that traumatic events are likely to have a greater effect on children than on adults. Early-life traumatic experiences, such as childhood sexual abuse, may influence the physiology of the developing brain. Later in life, there is a chronic hyperarousal of the stress response, making the individual vulnerable to further stress and stress-related disease. For this Assignment, you practice assessing and diagnosing patients with anxiety disorders, PTSD, and OCD. Review the DSM-5 criteria for the disorders within these classifications before you get started, as you will be asked to justify your differential diagnosis with DSM-5 criteria.

To Prepare:

  • Review this week’s Learning Resources and consider the insights they provide about assessing and diagnosing anxiety, obsessive-compulsive, and trauma- and stressor-related disorders.
  • Download the Comprehensive Psychiatric Evaluation Template, which you will use to complete this Assignment. Also review the Comprehensive Psychiatric Evaluation Exemplar to see an example of a completed evaluation document.
  • By Day 1 of this week, select a specific video case study to use for this Assignment from the Video Case Selections choices in the Learning Resources. View your assigned video case and review the additional data for the case in the “Case History Reports” document, keeping the requirements of the evaluation template in mind.
  • Consider what history would be necessary to collect from this patient.
  • Consider what interview questions you would need to ask this patient.
  • Identify at least three possible differential diagnoses for the patient.
By Day 7 of Week 4

Complete and submit your Comprehensive Psychiatric Evaluation, including your differential diagnosis and critical-thinking process to formulate primary diagnosis.

Incorporate the following into your responses in the template:

  • Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
  • Objective: What observations did you make during the psychiatric assessment?
  • Assessment: Discuss the patient’s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses with supporting evidence, listed in order from highest priority to lowest priority. Compare the DSM-5 diagnostic criteria for each differential diagnosis and explain what DSM-5 criteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case. NRNP 6635 Week 4: Anxiety Disorders, PTSD, and OCD
  • Reflection notes: What would you do differently with this client if you could conduct the session over? Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion and disease prevention taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

PICO(T) is a framework that helps breakdown the question into searchable keywords (Davies, 2011). The question framework, PICO(T), standardizes the format for asking questions, which is the motivating force in evidence-based practice (EBP) (Davies, 2011). As nurses, we continuously ask and formulate questions out of curiosity and inquiry to provide the highest quality of care to our patients (Melnyk et al., 2009).

The clinical issue I am interested in is fall prevention. Working in a telemetry-neuro unit, we have many incidents of falls. These falls can lead to a longer stay in the hospital or cause complications, such as fractures or hemorrhages.

(P): Inpatient adults ages 40 and older

(I): Rigorous fall assessment and early ambulation

(C): bed alarms and fall education

(O): reduction of falls in Telemetry and Medical-Surgical floors

(T): during the hospital stay

Database Research

The two databases I used were PubMed and MEDLINE. It was a great start to my interest in the topic. The database allows me to advance my research by allowing me to choose the article type, change the publication date range, and search with Boolean operators. These features helped narrow down my search to the specific article I am looking for specifically with the Boolean operators. Boolean operators allow me to search combinations of keywords or phrases in a specified order (Library of Congress, n.d.). Having a question framework such as PICO(T) maximizes the effectiveness of the search as it helps guide the search results for the specific topic. You will complete the template as if you are the project manager. The project charter will serve as a statement of work for the case study you selected in Chapter 8 of the textbook.

Case model I will use -Nurse Manager

Ruth S., a newly graduated MSN, is the nurse manager who reports to Becky and as a new nurse manager is required to learn how to plan and schedule staff to adequately cover a dynamic and frequently changing work environment. She has also been asked to manage the budget for the unit, something she will need to learn, as she has not done that before. Ruth has had an introduction to the basic skills in her master’s program but not to the extent she will need to perform her role functions well. There are other competencies she will need to learn as a manager and, just as her predecessor, will take the same two PM courses at the university where she will develop skills such as developing communication plans, learning how to communicate with organizational leadership through report writing, and attending meetings where she will learn more about the group process (Chapter 4, Planning: Project Management—Phase 2). She will learn to develop change management plans and by doing so will learn how to utilize the change process to become more knowledgeable in team building and managing conflict, as well as provide resolutions and manage resources; she became more computer literate as well (Chapter 3, Design/Initiation: Project Management—Phase 1, and Chapter 4, Planning: Project Management—Phase 2).Ruth is not well versed in finance so Becky, who promoted her, initially will take on this task until Ruth becomes more conformable with the process (Chapter 4, Planning: Project Management—Phase 2). With oversight from Becky, Ruth will need to review and validate how to assign tasks, as well as delegate, deal with conflict, and assess and prioritize timelines (Chapter 4, Planning: Project Management—Phase 2). Becky also suggested that Ruth work with other peers who have expertise in data collection and analysis, understand how to collect data, analyze and prepare reports for the CNO and NE, define the metrics that leadership will require, and review outcomes (Chapter 3, Design/Initiation: Project Management—Phase 1, and Chapter 4, Planning: Project Management—Phase 2). Ruth will learn how to do other tasks as she takes the courses in project management. Other tasks include the ability to:■Develop objectives for both short- and long-term goals (Chapter 4, Planning: Project Management—Phase 2)■Develop and plan “a” and “b” options (Chapter 4, Planning: Project Management—Phase 2)■Take action and know when to delegate (Chapter 5, Implementation/Execution—Phase 3)■Define what processes need to be in place (Chapter 3, Design/Initiation: Project Management—Phase 1, and Chapter 4, Planning: Project Management—Phase 2)■Determine how to monitor and control (Chapter 6, Monitoring and Controlling: Project Management—Phase 4)■Define the different types of project closing (Chapter 7, Closing the Project—Phase 5)■Develop, utilize all tools developed, and successfully close a project (Chapter 7, Closing the Project—Phase 5)Ruth has also been told that the ABC Medical Center will be implementing a new system to document and track nursing standards. She will need to work with IT to implement the system and ensure the system has implementation of standards in place, including a method for documenting employees exceeding or failing to meet standards (Chapter 4, Planning: Project Management—Phase 2, and Chapter 5, Implementation/Execution—Phase 3).Ruth realizes that she will be very busy at least for her first 6 months in the new job and works with Becky to establish a timeline for all of the new tasks including her schoolwork (Chapter 4, Planning: Project Management—Phase 2).

Description

Write a 4-6 page annotated bibliography where you identify peer-reviewed publications that promote the use of a selected technology to enhance quality and safety standards in nursing.

Before you begin to develop the assessment you are encouraged to complete the Annotated Bibliography Formative Assessment. Completing this activity will help you succeed with the assessment and counts towards course engagement. NURS FXP 4040 Managing Health Info & Tech Annotated Bibliography

Rapid changes in information technology go hand-in-hand with progress in quality health care delivery, nursing practice, and interdisciplinary team collaboration. The following are only a few examples of how the health care field uses technology to provide care to patients across multiple settings:

  • Patient monitoring devices.
  • Robotics.
  • Electronic medical records.
  • Data management resources.
  • Ready access to current science.

Technology is essential to the advancement of the nursing profession, maintaining quality care outcomes, patient safety, and research.

This assessment will give you the opportunity to deepen your knowledge of how technology can enhance quality and safety standards in nursing. You will prepare an annotated bibliography on technology in nursing. A well-prepared annotated bibliography is a comprehensive commentary on the content of scholarly publications and other sources of evidence about a selected nursing-related technology. A bibliography of this type provides a vehicle for workplace discussion to address gaps in nursing practice and to improve patient care outcomes. As nurses become more accountable in their practice, they are being called upon to expand their role of caregiver and advocate to include fostering research and scholarship to advance nursing practice. An annotated bibliography stimulates innovative thinking to find solutions and approaches to effectively and efficiently address these issues.

DEMONSTRATION OF PROFICIENCY

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

  • Competency 3: Evaluate the impact of patient care technologies on desired outcomes.
    • Analyze current evidence on the impact of a selected patient care technology on patient safety, quality of care, and the interdisciplinary team. NURS FXP 4040 Managing Health Info & Tech Annotated Bibliography
    • Integrate current evidence about the impact of a selected patient care technology on patient safety, quality of care, and the interdisciplinary team into a recommendation.
  • Competency 4: Recommend the use of a technology to enhance quality and safety standards for patients.
    • Describe organizational factors influencing the selection of a technology in the health care setting.
    • Justify the implementation and use of a selected technology in a health care setting.
  • Competency 5: Apply professional, scholarly communication to facilitate use of health information and patient care technologies.
    • Create a clear, well-organized, and professional annotated bibliography that is generally free from errors in grammar, punctuation, and spelling.
    • Follow APA style and formatting guidelines for all bibliographic entries.

PREPARATION

To successfully complete this assessment, perform the following preparatory activities:

  • Select a single direct or indirect patient care technology that is relevant to your current practice or of interest to you. Direct patient care technologies require an interaction, or direct contact, between the nurse and patient. Nurses use direct patient care technologies every day when delivering care to patients. Electronic thermometers or pulse oximeters are examples of direct patient care technologies. Indirect patient care technologies, on the other hand, are those employed on behalf of the patient. They do not require interaction, or direct contact, between the nurse and patient. A handheld device for patient documentation is an example of an indirect patient care technology. Examples of topics to consider for your annotated bibliography include:
    • Delivery robots.
    • Electronic medication administration with barcoding.
    • Electronic clinical documentation with clinical decision support. NURS FXP 4040 Managing Health Info & Tech Annotated Bibliography
    • Patient sensor devices/wireless communication solutions.
    • Real-time location systems.
    • Telehealth.
    • Workflow management systems.
  • Conduct a library search using the various electronic databases available through the Capella University Library.
    • Consult the BSN Program Library Research Guide for help in identifying scholarly and/or authoritative sources.
    • Access the NHS Learner Success Lab, linked in the courseroom navigation menu, for additional resources.
  • Scan the search results related to your chosen technology.
  • Select four peer-reviewed publications focused on your selected topic that are the most interesting to you.
  • Evaluate the impact of patient care technologies on desired outcomes.
    • Analyze current evidence on the impact of a selected patient care technology on patient safety, quality of care, and the interdisciplinary team.
    • Integrate current evidence about the impact of a selected patient care technology on patient safety, quality of care, and the interdisciplinary team into a recommendation.
Notes
  • Publications may be research studies or review articles from a professional source. Newspapers, magazines, and blogs are not considered professional sources.
  • Your selections need to be current—within the last five years.

ANNOTATED BIBLIOGRAPHY
