HCM 490 Final Project Rubric
Overview
The final project for this course is the creation of a Strategic Plan Presentation and Professional Reflection. The healthcare industry is constantly changing and
healthcare professionals should be prepared for the challenge. As the final stop in your journey toward your Bachelor of Science in Healthcare Administration,
you will integrate the knowledge and skills you have developed in previous coursework and over the duration of the term into a Capstone Assessment. Your
assessment will consist of two components that will allow you to evidence knowledge, skills, abilities, and dispositional growth you have experienced during your
program.
In the first component, you will select an existing healthcare organization that fits one of the following areas of opportunity and develop a comprehensive
strategic plan for this change.
1. Option 1: Organization is Offering a New Service – Create a strategic plan that will guide the organization in bringing to fruition a new service, such as
offering portable MRIs, starting a new urgent care clinic, opening a new outpatient surgery center, etc. HCM 490 Final Project Rubric
2. Option 2: Organization is Expanding an Existing Service Area – Create a strategic plan that will guide the organization in expanding an existing service
to an underserved population that needs attention in this area. Examples include: population that has limited access to primary care services,
population-to-provider ratio indicating a shortage, groups of persons who face economic, cultural, or linguistic barriers to healthcare in the local area
and reside in a specific geographic area, and so on
The second component of your Capstone assessment is a professional reflection, in which you will reflect upon your capstone, program, and professional
experiences and draw connections and inferences about your growth, ethical practice, and professional goals.
Evaluation of Capstone
This capstone will be assessed somewhat differently than other courses you have taken online at SNHU. There are two (2) separate components which will be
submitted at different times during the course; however, they all operate together to comprise the whole capstone experience and are not assessed separately.
You will be evaluated on both as a unit in determining whether you have demonstrated proficiency in each outcome. Your instructor will guide you through this
process, keeping a running narrative of your strengths and weaknesses in relation to the outcomes as you progress through the class. You must demonstrate
proficiency in all six (6) outcomes to pass this capstone course. Your work is expected to meet the highest professional standards.
The project is divided into five milestones, which will be submitted at various points throughout the course to scaffold learning and ensure quality final
submissions. These milestones will be submitted in Modules One, Two, Three, Four, and Five. The final product will be submitted in Module Seven and Eight.
In this assignment, you will demonstrate your mastery of the following course outcomes: HCM 490 Final Project Rubric

Prompt
After choosing the topic for your course project, you will work to develop the three phases of your strategic plan: the introduction and assessment phase, the
implementation, communication, and management phase, and finally the evaluation phase. The final strategic plan consists of a PowerPoint presentation
including speaker notes and voice over. This first component will also inform your professional reflection, in which you will evaluate your experiences in this
program, capstone experience, and field of interest, and discuss the importance and impact of ethical practice in healthcare.
Capstone Component 1: Strategic Plan
Your strategic plan must be submitted in the form of a comprehensive presentation to the Board of Directors of your selected institution for approval and buyin. You have the flexibility to utilize PowerPoint or another comparable presentation tool as long as your instructor approves the submission file. Your
presentation is expected be professionally crafted to the needs and specifications appropriate for the discipline, area of interest, and type of initiative you are
proposing. Because your strategic plan will be different from others’ in your capstone experience, you will likely include different additional resources, HCM 490 Final Project Rubric
information, visuals, and strategies for progress. At minimum, your presentation must address/include the following:
I. Phase I – Introduction and Assessment — In this section you will give background on your organization, introduce the project that precipitates the
strategic plan and what your assessment tools and approach will be. Your introduction and assessment should include:
a. The mission, vision, values statements, structure, and current community position of the organization.
b. Project topic that is the focus of the strategic plan, including the goal(s) of this project.
c. An appropriate analysis tool to assess the impact of your project on your organization.
d. The effect of internal and external factors on the feasibility and development of the project.
II. Phase II – Implementation and Communication Plans
a. Implementation Plan: Provide a comprehensive plan for implementation that includes:
i. Target area or intended population and establish the goals.

ii. The timeline and implementation milestones of the initiative.
iii. Financial impact of the initiative, including any necessary capital expenditures, necessary fundraising, or potential financial benefits.
iv. Resources needed to effectively implement the initiative, including staffing, equipment, technology needs, and so forth.
v. Compliance with all federal and state regulations relevant to the proposed plan.
vi. Potential ethical challenges and formulate plans for monitoring behavior, practices, procedures, and so forth.
b. Communication and Management Strategy: Your total communication plan should address the strategies and practices you will employ to
communicate to the proper stakeholders at the proper times. In this section you should detail your comprehensive approach for how you will
communicate this change and its impact, how you will keep people informed of the project’s progress, and how you will keep them informed of
the project’s ongoing results.
III. Phase III – Evaluation Strategy: This plan for managing and measuring progress and success of your initiative should include the following:
a. A discussion of the specific goals and milestones of implementation of the initiative. Lay out your goals in a clear, obtainable fashion and to
ensure that progress milestones are clear, reasonable, and consistent.
b. Specific definitions of measures of progress towards obtaining the intended goals and a description of the tools that you would use to measure
progress. What strategies will be used to measure success, and how will adjustments be made should progress towards these goals fall short or
change? HCM 490 Final Project Rubric
c. How do you intend to collect and analyze information to ascertain stakeholder satisfaction with the initiative? What information will be looked
at, and who are the stakeholders that will be considered?
Capstone Component 2: Professional Reflection
For the second and final component of your capstone, you will write an essay in which you discuss the process and outcomes of this project and your overall
Healthcare Administration Program journey and experiences. You will also speak more specifically about how the project relates to your area of
expertise/interest and your desired career path. Writing reflectively allows you to think deeply and consciously about your experiences, which is invaluable to
successful practice in the field of healthcare administration.
You should envision this component as a personal reflection on the capstone and your experience in the Healthcare Administration program as a whole. For
instance, relative to the capstone, you could discuss what you did—or intended to do—and then consider what worked well, what challenges you faced, and
what you would change or do differently to make your experience better. In reflecting on your time here at SNHU, you might discuss where you started, where
you have ended up, where you see yourself going, etc. Note that this component is not about evaluating the capstone itself, but rather, your experience within
the capstone project.
Some of the issues to address in this final component of this capstone may include, but are not limited to:
● Overall, what was your capstone experience like?
● Reflect on the significance of the capstone in relation to your own experience at SNHU.
● How will you relate cultural competence or maintain cultural principles in your profession?
● How does your application of interpersonal skills and collaborative processes relate to your professional goals? HCM 490 Final Project Rubric

● How will you promote continuous healthcare quality improvement in your professional life?

In assessing patients with anxiety, obsessive-compulsive, and trauma and stressor-related disorders, you will continue the practice of looking to understand chief symptomology in order to develop a diagnosis. With a differential diagnosis in mind, you can then move to a treatment and follow-up plan that may involve both psychopharmacologic and psychotherapeutic approaches.

In this Assignment, you use a case study to develop a focused SOAP note based on evidence-based approaches.

To Prepare

  • Review this week’s Learning Resources. Consider the insights they provide about assessing and diagnosing anxiety, obsessive compulsive, and trauma-related disorders.
  • Review the Focused SOAP Note template, which you will use to complete this Assignment. There is also a Focused SOAP Note Exemplar provided as a guide for Assignment expectations.
  • Review the video, Case Study: Dev Cordoba. You will use this case as the basis of this Assignment. In this video, a Walden faculty member is assessing a mock patient. The patient will be represented onscreen as an avatar. Assignment: Focused SOAP Note For Anxiety, PTSD, And OCD
  • Consider what history would be necessary to collect from this patient.
  • Consider what interview questions you would need to ask this patient.

The Assignment

Develop a Focused SOAP Note, including your differential diagnosis and critical-thinking process to formulate a primary diagnosis. Incorporate the following into your responses in the template:

  • Subjective: What details did the patient provide regarding their chief complaint and symptomology to derive your differential diagnosis? What is the duration and severity of their symptoms? How are their symptoms impacting their functioning in life?
  • Objective: What observations did you make during the psychiatric assessment?
  • Assessment: Discuss the patient’s mental status examination results. What were your differential diagnoses? Provide a minimum of three possible diagnoses with supporting evidence, listed in order from highest priority to lowest priority. Compare the DSM-5-TR diagnostic criteria for each differential diagnosis and explain what DSM-5-TRcriteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case.
  • Plan: What is your plan for psychotherapy? What is your plan for treatment and management, including alternative therapies? Include pharmacologic and nonpharmacologic treatments, alternative therapies, and follow-up parameters, as well as a rationale for this treatment and management plan. Also incorporate one health promotion activity and one patient education strategy.
  • Reflection notes: What would you do differently with this patient if you could conduct the session again? Discuss what your next intervention would be if you could follow up with this patient. Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), health promotion, and disease prevention, taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).
  • Provide at least three evidence-based, peer-reviewed journal articles or evidenced-based guidelines that relate to this case to support your diagnostics and differential diagnoses. Be sure they are current (no more than 5 years old). Assignment: Focused SOAP Note For Anxiety, PTSD, And OCD

INSTRUCTIONS ON HOW TO USE EXEMPLAR AND TEMPLATE—READ CAREFULLY

If you are struggling with the format or remembering what to include, follow the Focused SOAP Note Evaluation Template AND the Rubric as your guide.  It is also helpful to review the rubric in detail in order not to lose points unnecessarily because you missed something required.  After reviewing full details of the rubric, you can use it as a guide.

In the Subjective section, provide:

  • Chief complaint
  • History of present illness (HPI)
  • Past psychiatric history
  • Medication trials and current medications
  • Psychotherapy or previous psychiatric diagnosis
  • Pertinent substance use, family psychiatric/substance use, social, and medical history
  • Allergies
  • ROS

Read rating descriptions to see the grading standards!

In the Objective section, provide:

  • Physical exam documentation of systems pertinent to the chief complaint, HPI, and history
  • Diagnostic results, including any labs, imaging, or other assessments needed to develop the differential diagnoses.

Read rating descriptions to see the grading standards!

In the Assessment section, provide:

  • Results of the mental status examination, presented in paragraph form.
  • At least three differentials with supporting evidence. List them from top priority to least priority. Compare the DSM-5-TR diagnostic criteria for each differential diagnosis and explain what DSM-5-TR criteria rules out the differential diagnosis to find an accurate diagnosis. Explain the critical-thinking process that led you to the primary diagnosis you selected. Include pertinent positives and pertinent negatives for the specific patient case.
  • Read rating descriptions to see the grading standards!

Reflect on this case. Include: Discuss what you learned and what you might do differently. Also include in your reflection a discussion related to legal/ethical considerations (demonstrate critical thinking beyond confidentiality and consent for treatment!), social determinates of health, health promotion and disease prevention taking into consideration patient factors (such as age, ethnic group, etc.), PMH, and other risk factors (e.g., socioeconomic, cultural background, etc.).

(The FOCUSED SOAP psychiatric evaluation is typically the follow-up visit patient note. You will practice writing this type of note in this course. You will be focusing more on the symptoms from your differential diagnosis from the comprehensive psychiatric evaluation narrowing to your diagnostic impression. You will write up what symptoms are present and what symptoms are not present from illnesses to demonstrate you have indeed assessed for illnesses which could be impacting your patient. For example, anxiety symptoms, depressive symptoms, bipolar symptoms, psychosis symptoms, substance use, etc.)  Assignment: Focused SOAP Note For Anxiety, PTSD, And OCD

EXEMPLAR BEGINS HERE

Subjective:

CC (chief complaint): A brief statement identifying why the patient is here. This statement is verbatim of the patient’s own words about why presenting for assessment. For a patient with dementia or other cognitive deficits, this statement can be obtained from a family member.

HPI: Begin this section with patient’s initials, age, race, gender, purpose of evaluation, current medication and referral reason. For example:

N.M. is a 34-year-old Asian male presents for medication management follow up for anxiety. He was initiated sertraline last appt which he finds was effective for two weeks then symptoms began to return.

Or

P.H., a 16-year-old Hispanic female, presents for follow up to discuss previous psychiatric evaluation for concentration difficulty. She is not currently prescribed psychotropic medications as we deferred until further testing and screening was conducted.

Then, this section continues with the symptom analysis for your note. Thorough documentation in this section is essential for patient care, coding, and billing analysis.

Paint a picture of what is wrong with the patient. First what is bringing the patient to your follow up evaluation? Document symptom onset, duration, frequency, severity, and impact. What has worsened or improved since last appointment? What stressors are they facing? Your description here will guide your differential diagnoses into your diagnostic impression. You are seeking symptoms that may align with many DSM-5 diagnoses, narrowing to what aligns with diagnostic criteria for mental health and substance use disorders.

Substance Use History: This section contains any history or current use of caffeine, nicotine, illicit substance (including marijuana), and alcohol. Include the daily amount of use and last known use. Include type of use such as inhales, snorts, IV, etc. Include any histories of withdrawal complications from tremors, Delirium Tremens, or seizures.

Current Medications: Include dosage, frequency, length of time used, and reason for use. Also include OTC or homeopathic products.

Allergies: Include medication, food, and environmental allergies separately. Provide a description of what the allergy is (e.g., angioedema, anaphylaxis). This will help determine a true reaction vs. intolerance. Assignment: Focused SOAP Note For Anxiety, PTSD, And OCD

Reproductive Hx: Menstrual history (date of LMP), Pregnant (yes or no), Nursing/lactating (yes or no), contraceptive use (method used), types of intercourse:  oral, anal, vaginal, other, any sexual concerns

ROS: Cover all body systems that may help you include or rule out a differential diagnosis.  Please note: THIS IS DIFFERENT from a physical examination!

You should list each system as follows: General: Head: EENT: etc. You should list these in bullet format and document the systems in order from head to toe.

Example of Complete ROS:

GENERAL: No weight loss, fever, chills, weakness, or fatigue.

HEENT: Eyes: No visual loss, blurred vision, double vision, or yellow sclerae. Ears, Nose, Throat: No hearing loss, sneezing, congestion, runny nose, or sore throat.

SKIN: No rash or itching.

CARDIOVASCULAR: No chest pain, chest pressure, or chest discomfort. No palpitations or edema.

RESPIRATORY: No shortness of breath, cough, or sputum.

GASTROINTESTINAL: No anorexia, nausea, vomiting, or diarrhea. No abdominal pain or blood.

GENITOURINARY: Burning on urination, urgency, hesitancy, odor, odd color

NEUROLOGICAL: No headache, dizziness, syncope, paralysis, ataxia, numbness, or tingling in the extremities. No change in bowel or bladder control.

MUSCULOSKELETAL: No muscle, back pain, joint pain, or stiffness.

HEMATOLOGIC: No anemia, bleeding, or bruising.

LYMPHATICS: No enlarged nodes. No history of splenectomy.

ENDOCRINOLOGIC: No reports of sweating, cold, or heat intolerance. No polyuria or polydipsia.

Objective:

Diagnostic results: Include any labs, X-rays, or other diagnostics that are needed to develop the differential diagnoses (support with evidenced and guidelines).

Assessment:

Mental Status Examination: For the purposes of your courses, this section must be presented in paragraph form and not use of a checklist! This section you will describe the patient’s appearance, attitude, behavior, mood and affect, speech, thought processes, thought content, perceptions (hallucinations, pseudohallucinations, illusions, etc.)., cognition, insight, judgment, and SI/HI. See an example below. You will modify to include the specifics for your patient on the above elements—DO NOT just copy the example. You may use a preceptor’s way of organizing the information if the MSE is in paragraph form.

Week 1 Discussion

Discussion Question:Directions:Guidelines: Support your responses with scholarly academic references (added at the end) using APA style format. Assigned course readings and online library resources are preferred. Weekly lecture notes are designed as overviews to the topic for the respective week and should not serve as a citation or reference. In your discussion question response, provide a substantive response that illustrates a well-reasoned and thoughtful response; is factually correct with relevant scholarly citations, references, and examples that demonstrates a clear connection to the readings.Task:Theories are derived from conceptual models and are comprised of concepts and propositions. The only concepts that are common to all nursing theories, in some shape or form, are patient, nurse, health, and environment. These are sometimes referred to as the basic metaparadigms of the nursing domain.Identify two additional concepts that are relevant to your personal practice of nursing and explain how they relate to your practice and why they are important to your practice.Scholarly academic referencesThe Theory EraThe theory era began with a strong emphasis on knowledge development. Although in the previous two decades proponents of nursing theory and nursing theorists had begun to publish their works, it is noteworthy that they denied being theorists when they were introduced as such at the 1978 Nurse Educator Conference in New York with the Nursing Theory theme. There was understanding among those attending the conference that the presenters were theorists, and by the second day, the audience responded to their denials with laughter. This seems strange today, but this was the first time most of the theorists even met each other. Their works had grown out of content organization in nursing education courses, nursing practice administration in large agencies, and structures for the thought and action of practice. It was clear that their works were nursing theoretical structures even before they recognized them as such. The theory era, coupled with the research and graduate education eras, led to understanding of the scientific process beyond production of a scientific product Theory forms the foundation of knowledge. Nursing theories form the foundation of nursing practice, research, and education. Throughout your professional life, you will be applying theory and the knowledge derived from theory in your practice environment regardless of the setting. An understanding of the nature of nursing knowledge from a historical perspective will help you relate better to where nursing theory development is today.TheoryTheory is defined as “an organized, coherent, and systematic articulation of a set of statements related to significant questions in a discipline that are communicated in a meaningful whole; a symbolic depiction of aspects of reality that are discovered or invented for describing, explaining, predicting, or prescribing responses, events, situations, conditions, or relationships” (Meleis, 1997, pp. 8,12)1. Thus, a theory is a coherent set of propositions and statements that describe (factor-isolating), explain (factor-relating), and predict (situation-relating) phenomena as well as prescribe (situation-producing) actions toward goals. (Dickoff et al., 1968)2.Theory development requires perceiving phenomena that are peculiar to nursing and proposing meaningful explanation for these perceptions. The nursing profession identifies four levels of theory—metatheory, grand theory, middle range theory, and practice theory. The theories are classified based on their levels of abstraction or complexity.A complete structure includes a conceptual model, derived theories, and correlated empirical research methods. Each conceptual model and theory is comprised of concepts and propositions. The complete structure forms a hierarchy that is based on levels of abstraction. Most abstract: the conceptual model that provides the context or frame of reference for theory-generating and theory-testing researchIntermediate level: the theory that is generated or testedMost concrete: the empirical research methods used to collect and analyze the data1Meleis, A. I. (1997). Theoretical nursing: development and progress (3rd ed.). Philadelphia: Lippincott.2Dickoff, J., James, P., & Wiedenbach, E. (1968). Theory in a practice discipline Part 1: Practice oriented theory. Nursing Research, 17(5), 415–435.Importance of Identification of theoryEarly knowledge consisted of loosely connected clusters of concepts. Later knowledge developed interrelated statements connecting the concepts. Advanced theory provided a knowledge base for intervention strategies that clusters of concepts could not.Identification of theory is important to understand the characteristics of theory structures. A complete theory of nursing identifies the three elements of context, content, and process; some theorists articulate each element better than others.Context is the environment in which nursing acts occur; the context of a theory describes the nature of the world of nursing and may describe the nature of the patient’s world.Content includes the subject matter of a theory; this comprises the stable components that are acted on or that do the actingProcess implies the action part of the theory, the intervention elements. In addition to these elements, all theories should be examined for certain common factors:Theories should be based on concepts and propositionsTheories should be specific to the nursing contextTheories can be applied to many situationsTheories should be relevant to potential usersTheories should be easy to define it in operational termsTheories should correspond with empirical findingsTheories should demonstrate internal consistency.Florence Nightingale made the first attempts at theory-based nursing during the late 19th and early 20th centuries. She organized a group of women to deliver care under her supervision and that of war surgeons. She established the need for hygiene, with environmental change as the means to enhance healing. For her, the nursing domain was the patient and the environment in which care was offered. Her goals were to expose the unhealthy conditions of soldiers, to gain support for the need for nurses, and to achieve formal education for nurses. She was the first to use data collection and analysis to prove efficacy of nursing actions. The Nursing-Theory ConnectionThe new millennium has witnessed a rapid change in the global healthcare arena. This profession is continually evolving to face the monumental changes that have emerged. In the past, nursing theories were taught strictly from an academic point of view. Those in practice saw little connection or integration of theory and practice. However, the contemporary move toward evidence-based practice has seen nursing professionals applying theory to describe, predict, and prescribe nursing practice.Relevance to Present and FutureTheories are a reflection of the past, present, and future of nursing. Understanding the relevance of theories will enhance the relationship of concept with practice. This will help you comprehend the complex phenomena and interconnectedness between theory and practice and build a new way of thinking, understanding, working, and living.Relevance to ResearchResearch on theories help in the systematic investigation of the domain of knowledge. Research acts as a source of knowledge development. Research studies conducted on nursing theories help highlight the interlinking cyclical connection between theory and practice. Learners get an idea about the interconnectedness between real world clinical issues, theory, and actual research.Nursing theoriesNursing theories are normally derived from conceptual models in which the nursing metaparadigm phenomena are clearly identified (Fawcett, 1983)3. For this reason, the conceptual model is considered a precursor of nursing theory (Peterson, 1977; Fawcet 1983)4. A conceptual model of nursing is a set of abstract concepts and propositions that are integrated into a meaningful configuration and represents an involvement in theoretical formulations by describing nursing phenomena and their interrelationships in abstract terms (Fawcett, 1983).The concepts of a conceptual model are so abstract and general that they often cannot be observed or tested directly and may not even be defined. The concepts and definitions of each conceptual model are often stated in a distinctive vocabulary where the meaning of each term is connected to the unique focus of the conceptual model. Therefore, the same terms may have different meanings in different conceptual models. Each conceptual model provides a different perspective from which to view the phenomena that are within the domain of inquiry of a particular discipline. Most disciplines have more than one conceptual model; each focuses on certain phenomena that are regarded as relevant and other phenomena are ignored because they are deemed less important.3 Fawcett, J. (1983). Hallmarks of success in nursing theory development. In P. L. Chinn (Ed.), Advances in nursing theory development. Aspen, DN: Rockville, p. 3–17.4Peterson, C. J. (1977). Questions frequently asked about the development of a conceptual framework. Journal of Nursing Education, 16( 4 ), 22–32.My autobiography.I am currently in Miami, FL. I have been living here since I came from Cuba 17 years ago. I received my RN license in 2017 with an associate degree, and I obtained my BSN last august. Since I received my RN license in 2017, I have worked in home health, clinical trial clinic, med-Surg floor nurse, ER nurse, and how I’m a hospice clinical liaison. I expect to increase my knowledge of nursing theories and how I can apply these theories to nursing. I’m still undecided between going back to the ER or stay in the hospice field and go for the palliative route. Since I started my nursing career, my goal and objective have always been to obtain my Master’s degree and become a nurse practitioner, and now thanks to this course, fulfilling my goal is getting closer. I’m always up for new challenges, and I think that I can grow within my organization, and I believe that this course can help me along with getting my master’s in science of nursing. I am excited to be a part of this class and good luck to everyone!

Capella FPX4010 assessment 1

For this assessment you will create a 5-10 minute video reflection on an experience in which you collaborated interprofessionally, as well as a brief discussion of an interprofessional collaboration scenario and how it could have been better approached.

Interprofessional collaboration is a critical aspect of a nurse’s work. Through interprofessional collaboration, practitioners and patients share information and consider each other’s perspectives to better understand and address the many factors that contribute to health and well-being (Sullivan et al., 2015). Essentially, by collaborating, health care practitioners and patients can have better health outcomes. Nurses, who are often at the frontlines of interacting with various groups and records, are full partners in this approach to health care.

Reflection is a key part of building interprofessional competence, as it allows you to look critically at experiences and actions through specific lenses. From the standpoint of interprofessional collaboration, reflection can help you consider potential reasons for and causes of people’s actions and behaviors (Saunders et al., 2016). It also can provide opportunities to examine the roles team members adopted in a given situation as well as how the team could have worked more effectively. Capella FPX4010 FULL COURSE

Professional Context

This assessment will help you to become a reflective practitioner. By considering your own successes and shortcomings in interprofessional collaboration, you will increase awareness of your problem-solving abilities. You will create a video of your reflections, including a discussion of best practices of interprofessional collaboration and leadership strategies, cited in the literature.

Scenario

As part of an initiative to build effective collaboration at your Vila Health site, where you are a nurse, you have been asked to reflect on a project or experience in which you collaborated interprofessionally and examine what happened during the collaboration, identifying positive aspects and areas for improvement.

You have also been asked to review a series of events that took place at another Vila Health location and research interprofessional collaboration best practices and use the lessons learned from your experiences to make recommendations for improving interprofessional collaboration among their team. Your task is to create a 5–10 minute video reflection with suggestions for the Vila Health team that can be shared with leadership as well as Vila Health colleagues at your site. Note: If you require the use of assistive technology or alternative communication methods to participate in this activity, please contact DisabilityServices@Capella.edu to request accommodations. If, for some reason, you are unable to record a video, please contact your faculty member as soon as possible to explore options for completing the assessment.

Instructions

Using Kaltura, record a 5–10 minute video reflection on an interprofessional collaboration experience from your personal practice, proposing suggestions on how to improve the collaboration presented in the Vila Health: Collaboration for Change activity.

Be sure that your assessment addresses the following criteria. Please study the scoring guide carefully so you will know what is needed for a distinguished score:

  • Reflect on an interdisciplinary collaboration experience, noting ways in which it was successful and unsuccessful in achieving desired outcomes.
  • Identify how poor collaboration can result in inefficient management of human and financial resources, citing supporting evidence from the literature.
  • Identify best-practice leadership strategies from the literature that would improve an interdisciplinary team’s ability to achieve its goals, citing at least one author from the literature.
  • Identify best-practice interdisciplinary collaboration strategies to help a team achieve its goals and work together, citing the work of at least one author.
  • Communicate in a professional manner, is easily audible, and uses proper grammar. Format reference list in current APA style. Capella FPX4010 FULL COURSE

You will need to relate an experience that you have had collaborating on a project. This could be at your current or former place of practice, or another relevant project that will enable you to address the requirements. In addition to describing your experience, you should explain aspects of the collaboration that helped the team make progress toward relevant goals or outcomes, as well as aspects of the collaboration that could have been improved.

A simplified gap-analysis approach may be useful:

  • What happened?
  • What went well?
  • What did not go well?
    • What should have happened?

After your personal reflection, examine the scenario in the Vila Health activity and discuss the ways in which the interdisciplinary team did not collaborate effectively and the negative implications for the human and financial resources of the interdisciplinary team and the organization as a whole.

Building on this investigation, identify at least one leadership best practice or strategy that you believe would improve the team’s ability to achieve their goals. Be sure to identify the strategy and its source or author and provide a brief rationale for your choice of strategy.

Additionally, identify at least one interdisciplinary collaboration best practice or strategy to help the team achieve its goals and work more effectively together. Again, identify the strategy, its source, and reasons why you think it will be effective.

You are encouraged to integrate lessons learned from your self-reflection to support and enrich your discussion of the Vila Health activity.

You are required to submit an APA-formatted reference list for any sources that you cited specifically in your video or used to inform your presentation. The Example Kaltura Reflection will show you how to cite scholarly sources in the context of an oral presentation.

Refer to the Campus tutorial Using Kaltura [PDF] as needed to record and upload your reflection.

Additional Requirements

  • References:Cite at least 3 professional or scholarly sources of evidence to support the assertions you make in your video. Include additional properly cited references as necessary to support your statements.
  • APA Reference Page:Submit a correctly formatted APA reference page that shows all the sources you used to create and deliver your video.
    • You may wish to refer to the Campus APA Module for more information on applying APA style. Capella FPX4010 FULL COURSE

Capella FPX4010 assessment 2

For this assessment, you will create a 2-4 page report on an interview you have conducted with a health care professional. You will identify an issue from the interview that could be improved with an interdisciplinary approach, and review best practices and evidence to address the issue.

As a baccalaureate-prepared nurse, your participation and leadership in interdisciplinary teams will be vital to the health outcomes for your patients and organization. One way to approach designing an improvement project is to use the Plan-Do-Study-Act (PDSA) cycle. The Institute for Healthcare Improvement describes it thus:

The Plan-Do-Study-Act (PDSA) cycle is shorthand for testing a change in the real work setting—by planning it, trying it, observing the results, and acting on what is learned. This is the scientific method adapted for action-oriented learning…Essentially, the PDSA cycle helps you test out change ideas on a smaller scale before evaluating the results and making adjustments before potentially launching into a somewhat larger scale project (n.d.).

You might also recognize that the PDSA cycle resembles the nursing process. The benefit of gaining experience with this model of project design is that it provides nurses with an opportunity to ideate and lead improvements. For this assessment, you will not be implementing all of the PDSA cycle. Instead, you are being asked to interview a health care professional of your choice to determine what kind of interdisciplinary problem he or she is experiencing or has experienced in the workplace. This interview, in Assessment 2, will inform the research that you will conduct to propose a plan for interdisciplinary collaboration in Assessment 3. Capella FPX4010 FULL COURSE

Professional Context

This assessment will introduce the Plan-Do-Study-Act (PDSA) Model to create change in an organization. By interviewing a colleague of your choice, you will begin gathering information about an interprofessional collaboration problem that your colleague is experiencing or has experienced. You will identify a change theory and leadership strategies to help solve this problem. Capella FPX4010 FULL COURSE.

Scenario

This assessment is the first of three related assessments in which you will gather interview information (Assessment 2); design a proposal for interdisciplinary problem-solving, (Assessment 3); and report on how an interdisciplinary improvement plan could be implemented in a place of practice (Assessment 4). At the end of the course, your interviewee will have a proposal plan based on the PDSA cycle that he or she could present to stakeholders to address an interdisciplinary problem in the workplace.

For this assessment, you will need to interview a health care professional such as a fellow learner, nursing colleague, administrator, business partner, or another appropriate person who could provide you with sufficient information regarding an organizational problem that he or she is experiencing or has experienced, or an area where they are seeking improvements. Consult the Interview Guide [DOCX] for an outline of how to prepare and the types of information you will need to complete this project successfully.

Remember: this is just the first in a series of three assessments.

Instructions

For this assessment, you will report on the information that you collected in your interview, analyzing the interview data and identifying a past or current issue that would benefit from an interdisciplinary approach. This could be an issue that has not been addressed by an interdisciplinary approach or one that could benefit from improvements related to the interdisciplinary approach currently being used. Additionally, you will start laying the foundation for your Interdisciplinary Plan Proposal (Assessment 3) by researching potential change theories, leadership strategies, and collaboration approaches that could be relevant to issue you have identified.

When submitting your plan, use the Interview and Issue Identification Template [DOCX],which will help you to stay organized and concise. As you complete the template, make sure you use APA format for in-text citations for the evidence and best practices that are informing your plan, as well as for the reference list at the end.

Additionally, be sure to address the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so you understand what is needed for a distinguished score.

· Summarize an interview focused on past or current issues at a health care organization.

· Identify an issue from an interview for which an evidence-based interdisciplinary approach would be appropriate.

· Describe potential change theories and a leadership strategies that could inform an interdisciplinary solution to an organizational issue.

· Describe collaboration approaches from the literature that could facilitate establishing or improving an interdisciplinary team to address an organizational issue.

· Communicate with writing that is clear, logically organized, and professional, with correct grammar and spelling, and using current APA style. Capella FPX4010 FULL COURSE

Additional Requirements

· Length of submission:Use the provided template. Most submissions will be 2 to 4 pages in length. Be sure to include a reference page at the end of the plan.

· Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your central ideas. Resources should be no more than 5 years old.

· APA formatting: Make sure that in-text citations and reference list follow current APA style.

Capella FPX4010 assessment 3

For this assessment you will create a 2-4 page plan proposal for an interprofessional team to collaborate and work toward driving improvements in the organizational issue you identified in the second assessment.

The health care industry is always striving to improve patient outcomes and attain organizational goals. Nurses can play a critical role in achieving these goals; one way to encourage nurse participation in larger organizational efforts is to create a culture of ownership and shared responsibility (Berkow et al., 2012). Participation in interdisciplinary teams can also offer nurses opportunities to share their expertise and leadership skills, fostering a sense of ownership and collegiality.

Demonstration of Proficiency

  • Competency 1: Explain strategies for managing human and financial resources to promote organizational health.
    • Explain organizational resources, including a financial budget, needed for the plan to be a success and the impacts on those resources if nothing is done, related to the improvements sought by the plan.
  • Competency 2: Explain how interdisciplinary collaboration can be used to achieve desired patient and systems outcomes.
    • Describe an objective and predictions for an evidence-based interdisciplinary plan to achieve a specific objective related to improving patient or organizational outcomes.
    • Explain the collaboration needed by an interdisciplinary team to improve the likelihood of achieving the plan’s objective. Include best practices of interdisciplinary collaboration from the literature.
  • Competency 4: Explain how change management theories and leadership strategies can enable interdisciplinary teams to achieve specific organizational goals.
    • Explain a change theory and a leadership strategy, supported by relevant evidence, that are most likely to help an interdisciplinary team succeed in collaborating and implementing, or creating buy-in for, the project plan.
  • Competency 5: Apply professional, scholarly, evidence-based communication strategies to impact patient, interdisciplinary team, and systems outcomes.
    • Communicate the interdisciplinary plan with writing that is clear, logically organized, and professional, with correct grammar and spelling, using current APA style. Capella FPX4010 FULL COURSE

Scenario

Having reviewed the information gleaned from your professional interview and identified the issue, you will determine and present an objective for an interdisciplinary intervention to address the issue.

Note: You will not be expected to implement the plan during this course. However, the plan should be evidence-based and realistic within the context of the issue and your interviewee’s organization.

Instructions

For this assessment, use the context of the organization where you conducted your interview to develop a viable plan for an interdisciplinary team to address the issue you identified. Define a specific patient or organizational outcome or objective based on the information gathered in your interview.

The goal of this assessment is to clearly lay out the improvement objective for your planned interdisciplinary intervention of the issue you identified. Additionally, be sure to further build on the leadership, change, and collaboration research you completed in the previous assessment. Look for specific, real-world ways in which those strategies and best practices could be applied to encourage buy-in for the plan or facilitate the implementation of the plan for the best possible outcome.

Using the Interdisciplinary Plan Proposal Template [DOCX] will help you stay organized and concise. As you complete each section of the template, make sure you apply APA format to in-text citations for the evidence and best practices that inform your plan, as well as the reference list at the end.

Additionally, be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so you understand what is needed for a distinguished score.

  • Describe an objective and predictions for an evidence-based interdisciplinary plan to achieve a specific goal related to improving patient or organizational outcomes.
  • Explain a change theory and a leadership strategy, supported by relevant evidence, that is most likely to help an interdisciplinary team succeed in collaborating and implementing, or creating buy-in for, the project plan.
  • Explain the collaboration needed by an interdisciplinary team to improve the likelihood of achieving the plan’s objective. Include best practices of interdisciplinary collaboration from the literature.
  • Explain organizational resources, including a financial budget, needed for the plan to succeed and the impacts on those resources if the improvements described in the plan are not made.
  • Communicate the interdisciplinary plan, with writing that is clear, logically organized, and professional, with correct grammar and spelling, using current APA style.

Additional Requirements

  • Length of submission:Use the provided template. Remember that part of this assessment is to make the plan easy to understand and use, so it is critical that you are clear and concise. Most submissions will be 2 to 4 pages in length. Be sure to include a reference page at the end of the plan.
  • Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your central ideas. Resources should be no more than 5 years old.
  • APA formatting: Make sure that in-text citations and reference list follow current APA style.

Capella FPX4010 assessment 4

For this assessment you will create an 8-12 slide PowerPoint presentation for one or more stakeholder or leadership groups to generate interest and buy-in for the plan proposal you developed for the third assessment. Capella FPX4010 FULL COURSE

As a current or future nurse leader, you may be called upon to present to stakeholders and leadership about projects that you have been involved in or wish to implement. The ability to communicate a plan—and potential implications of not pursuing such a plan—to stakeholders effectively can be critically important in creating awareness and buy-in, as well as building your personal and professional brand in your organization. It is equally important that you know how to create compelling presentations for others’ delivery and ensure that they convey the same content you would deliver if you were the presenter.

Demonstration of Proficiency

  • Competency 1: Explain strategies for managing human and financial resources to promote organizational health.
    • Explain how the interdisciplinary plan could be implemented and how the human and financial resources would be managed.
  • Competency 2: Explain how interdisciplinary collaboration can be used to achieve desired patient and systems outcomes.
    • Explain an organizational or patient issue for which a collaborative interdisciplinary team approach would help achieve a specific improvement goal.
  • Competency 3: Describe ways to incorporate evidence-based practice within an interdisciplinary team.
    • Summarize an evidence-based interdisciplinary plan to address an organizational or patient issue.
  • Propose evidence-based criteria that could be used to evaluate the degree to which the project was successful in achieving the improvement goal.
    • Competency 5: Apply professional, scholarly, evidence-based communication strategies to impact patient, interdisciplinary team, and systems outcomes.
  • Communicate the PowerPoint presentation of the interdisciplinary improvement plan to stakeholders in a professional, respectful manner, with writing that is clear, logically organized, with correct grammar and spelling, using current APA style.

Professional Context

This assessment will provide you with an opportunity to sharpen your ability to create a professional presentation to stakeholders. In this presentation, you will explain the Plan-Do-Study-Act cycle and how it can be used to introduce the plan (P), implement the plan (D), study the effectiveness of the plan (S), and act on what is learned (A) to drive continuous improvement. By using this cycle, the stakeholders will have a tool and a proposal to expand on these ideas to drive workplace change and create improved processes to solve an interprofessional collaboration problem.

Scenario

In addition to summarizing the key points of Assessments 2 and 3, you will provide stakeholders and/or leadership with an overview of project specifics as well as how success would be evaluated—you will essentially be presenting a discussion of the Plan, Do, and Study parts of the PDSA cycle. Again, you will not be expected to execute the project, so you will not have any results to study. However, by carefully examining the ways in which your plan could be carried out and evaluated, you will get some of the experience of the thinking required for PDSA.

When creating your PowerPoint for this assessment, it is important to keep in mind the target audience: your interviewee’s organizational leadership. The overall goal of this assessment is to create a presentation that your interviewee could potentially give in his or her organization.

Instructions

Please follow the Capella Guidelines for Effective PowerPoint Presentations [PPTX]. If you need technical information on using PowerPoint, refer to Capella University Library: PowerPoint Presentations. Capella FPX4010 FULL COURSE

Be sure that your plan addresses the following, which corresponds to the grading criteria in the scoring guide. Please study the scoring guide carefully so you understand what is needed for a distinguished score.

  • Explain an organizational or patient issue for which a collaborative interdisciplinary team approach would help achieve a specific improvement goal.
  • Summarize an evidence-based interdisciplinary plan to address an organizational or patient issue.
  • Explain how the interdisciplinary plan could be implemented and how the human and financial resources would be managed.
  • Propose evidence-based criteria that could be used to evaluate the degree to which the project was successful in achieving the improvement goal.
  • Communicate the PowerPoint presentation of the interdisciplinary improvement plan to stakeholders in a professional manner, with writing that is clear, logically organized, and respectful with correct grammar and spelling using current APA style.

There are various ways to structure your presentation; following is one example:

  • Part 1: Organizational or Patient Issue.
    • What is the issue that you are trying to solve or improve?
    • Why should the audience care about solving it?
  • Part 2: Relevance of an Interdisciplinary Team Approach.
    • Why is using an interdisciplinary team relevant, or the best approach, to addressing the issue?
    • How will it help to achieve improved outcomes or reach a goal?
  • Part 3: Interdisciplinary Plan Summary.
    • What is the objective?
    • How likely is it to work?
    • What will the interdisciplinary team do?
  • Part 4: Implementation and Resource Management.
    • How could the plan be implemented to ensure effective use of resources?
    • How could the plan be managed to ensure that resources were not wasted?
    • How does the plan justify the resource expenditure?
  • Part 5: Evaluation.
    • What would a successful outcome of the project look like?
    • What are the criteria that could be used to measure that success?
      • How could this be used to show the degree of success?

Again, keep in mind that your audience for this presentation is a specific group (or groups) at your interviewee’s organization and tailor your language and messaging accordingly. Remember, also, that another person will ultimately be giving the presentation. Include thorough speaker’s notes that flesh out the bullet points on each slide.

Additional Requirements

  • Number of slides:Plan on using one or two slides for each part of your presentation as needed, so the content of your presentation will be 8–12 slides in length. Remember that slides should contain concise talking points, and you will use presenter’s notes to go into detail. Be sure to include a reference slide as the last slide of your presentation.
  • Number of references: Cite a minimum of 3 sources of scholarly or professional evidence that support your central ideas. Resources should be no more than five years old.
  • APA formatting: Make sure that in-text citations on your slides and in your notes pages and reference slide reflect current APA Style and Format. Capella FPX4010 FULL COURSE
