How competing needs may impact the development of polices to address that issue

For years the healthcare system in the United States has relied on a fee-for-service payment system whereby each medical service and procedure is paid for separately. However, this payment model created several limitations, including fueling health care costs, overtreatment, and overutilization at the same time leaving others underserved and undertreated. In an attempt to counter these perverse incentives, the legislation of payment reforms which focuses on methods that reflect providers’ performances, primarily quality, safety, and patient experience, has been put in place to replace the traditional system of fee-for-service (Palumbo et al., 2017). The new healthcare payment reform has been designed to spur provider efficiency, reducing unnecessary spending subsequently reducing healthcare costs. However, competing needs such as the use of 12-hour shifts as employees’ retention strategy, workforce, and resources have affected the development and adoption of this critical policy following the necessity that surrounds competing needs to align with the agenda of payment reform. Payment reforms such as Medicare, which pushes for bundled payment, whereby patients are required to use only one payment method covering all episodes of care, have to address nursing shortages. With the aging population steadily rising, Medicaid has been burdened with being the primary payer of healthcare for many Americans. Nurses play a critical role in driving system change because of their sheer numbers (Miller et al., 2017). It is estimated that as of 2019, over 3.8 million registered nurses were integrated into the American workforce, a lower number compared to the health care needs of the American people. As a result, the available nursing workforce is forced to register more hours in their workplaces, reducing quality, safety, and quantity of care service (Rivers & Glover, 2008). There are numerous severe ramifications associated with shortages of nurses, such as work overload of available healthcare personnel, which results in burnout and subsequently compromise patient safety and long waiting, which may cause more harm, including preventable deaths.

Manual Muscle Testing: Knee flexion 5/5, pain. Knee extension 5/5, painful. Knee ER 5/5, Knee IR 5/5.

Musculoskeletal Tests: The patient can perform a regular activity such as walking, bending the knee with slight discomfort noted.

Diagnostic tests: Computerized tomography (CT) scan, Bilateral Knee X-ray, CBC, and Magnetic resonance imaging (MRI).

The practitioner must assess for abnormalities during musculosketal examination. The lower extremities must be examined for alignment with the edge between the femur and tibia under 15 degrees (Ball, Dains, Flynn, Solomon, and Stewart, 2015). Range of motion must be assessed in patients with suspected fracture (Ball et al., 2015). McMurray test to rule out tear of the meniscus, Varus-valgus stress test to check for knee stability, and Lachman test for anterior cruciate ligament. The three diagnostic test will help to examine the tissues around the knee, check for patella abnormalities or dislocation, and rule out knee fracture.

ASSESSMENT/Differential Diagnosis:

Priority diagnosis would be Osgood Schlatter Disease: A common cause of knee pain in adolescents. This in an inflammation underneath the knee where the ligament from the patellar tendon attaches to the tibia.

Differential Dx: These include

Osteogenic Sarcoma – This condition is associated with people age 10-25 yrs, and it is characterized by intermittent pain (Ryan, Maryam, & Hue, 2016).
Patellar dislocation. This condition is associated with shifting of the knee patella from its normal positioning and is characterized by pain in the patella (Rudavsky & Cook, 2014)
Patella fracture. The condition is where the kneecap suffers breakage or cracking and result to pain in the knee, accompanied by swelling and bruising around the knee (Reinking, 2016). Though the patient does not have all these symptoms, it is a probability.
Medial meniscus- This condition is associated with knee pain, difficulty flexing, clicking, or catching of the knee movement, just like in the case of the patient (Wu et al. 2018).
Patellar chondromalacia. This is when the cartilage in the kneecap gets worn by age or injury and is accompanied by discomfort of the inner knee and pain that aggravate from the physical activity just like in the case of the patient (Pak, Lee, & Lee, 2013).

Donald Davidson, in his book, American Composition and Rhetoric, discusses the précis as follows:

The précis shears away all elaborations of the thought and gives only what is left, in such a way as to make the summary a complete composition. the chaperones on the class trip recognized the manifestations as an allergic
reaction and called Carlo’s mother. Carlo was taken by his mother to an urgent

history and assessed him.
