Mental Health Access 

Mental health access through the emergency department is a problem throughout the United States. Many people with acute mental health issues seek care through the emergency department simply because they have no other options for immediate help. The proposed bill H.R. 2519, addresses the problem of EDs not having enough resources to manage and treat patients with acute mental health episodes affectively. This bill would provide funding to aid in additional inpatient psychiatric beds, crisis clinics, and emergency psychiatric units for mental health patients with an acute episode. (Actions – H.R.2519 – 116th Congress (2019-2020): Improving Mental Health Access from the Emergency Department Act of 2020, n.d.). The issue remains that people come into the ED for suicidal or homicidal ideation, and we have no inpatient psych beds available to get them the help they desperately need. I have personally witnessed having to hold a suicidal patient in the ED for over 24 hours until either a bed becomes available, or released back on the street, only because we do not have the resources necessary to accommodate them.

I believe there is significant evidence to support this policy and the need for increased resources for mental health patients. Acute mental health patients are seeking emergency care at an alarming rate. One in eight patients in the emergency department seeks help for acute mental health issues or substance abuse. (Laderman et al., 2018). This number continues to grow despite the lack of resources emergency departments have to hand out. Lack of resources for these patients also results in increased wait times and delays in emergency care that they need. Mental health patients simply do not get the treatment they require by sitting in the emergency department for hours or even days. (PR Newswire, 2017). Student 2 AA Discussion

S.4210-115th Congress (2019-2020) desires for the Department of Homeland Security to transfer personal protective equipment or medically necessary equipment to the Department of Health and Human Services (HHS) during a declared public health emergency. These transfers must be made at the request of HHS and on a reimbursable basis (Congress.Gov., 2020). The proposer of this bill was Senator Ron Johnson, Republican party for Wisconsin 112th-116th (2011-present) (Congress.Gov, n.d.).

Before the equipment would be able to be requested, the Secretary of Health and Human Services would need to determine whether the personal protection equipment or medically necessary equipment would be available. The Secretary will also determine whether the personal protection equipment requested is excess equipment (Govtrack, 2020).

Association of State and Territorial Health Officials (2020), explains that the “Public Health Service Act (PHSA) provides the legal authority for the Department of Health and Human Services (HHS), among other things, to respond to public health emergencies”. The act authorizes the HHS secretary to guide federal public health as well as medical response to public health emergencies, determine if a public health emergency exists, and assists in states during their response activities. Seeing the responsibility HHS has in public health emergencies, it would be critical for them to have on hand the supplies needed for any type of declared emergency. Hospitals discussed how there were many challenges with obtaining personal protective equipment (PPE), testing, staffing, supplies and durable equipment, during our recent public health crisis of COVID-19 (U.S. Department of Health and Human Services, 2020). This is an example of where we need to support and have the assistance of all federal entities. The above proposed bill would be a resource much needed when public disasters occur.

There are several locations which can provide a nurse with the opportunity to participate in policy review. One of these is through their own workplace, where the nurse can help influence and change the political action. Another is supporting their nursing professional organization. There is a lack of political awareness and understanding of why there is great importance in being a part of a nursing organization. It also remains challenging to herd up nurses to be involved in a nursing organization, which comes from nurses thinking their primary and sole role is providing direct health care services rather than assisting with policy making (Catallo, C., Spalding, K., & Haghiri-Vijeh, R., 2014). With nurses who have been in the field for many years, this way of thinking can certainly create issues, as the idea of nurses becoming involved in politics is a fairly new module. Challenges that could occur in the work place where a nurse is involved in policy review, could include the possibility of clashing ideas with other coworkers. Whose idea is better? Whose idea will be liked the most over all? There could always be competition with co workers evaluating a policy being reviewed. The best way to overcome this, is by having all members on the team involved at the same time when the policy review takes place. Also, having a statement at the beginning reiterating positivity and respect among all members involved, needs to be taken.

Knowing that nursing organizations work hard to help ensure that the nursing workforce in today’s time is well prepared to meet the demands of extending high-quality health care services in an environment that is always changing (Phillips, J. 2017), is enough of a reason to advocate for nurse’s participation in nursing organizations. Supervisors and healthcare leaders in any type of entity, need to back there staff up, and encourage them to join a membership. One of the best ways to facilitate this, is by personally relating it to the individual. If a nursing organization has financial support, loan replacement, and nursing education and training, as Phillips continues to mention, these can be used as tactics to encourage nurses to get involved.

A study completed in Thailand showed that 72.8 percent of nurses had never experienced policy participation. In order to change this, and for nurses to increase their influence in health policy, they must empower themselves while also increasing their communication skills. This can be done through education and training programs (Wichaikhum, O., et al, 2019). Providing education programs and trainings to nurses on the role and importance of policy development and review, is much needed. Evaluation

Evaluation in any setting is critical to measure the outcome. Policymaking is no different. Being involved in the evaluation of an existing or newly proposed policy is critical in healthcare. It is imperative that nurses know about evaluation principles and implements them in all healthcare programs. (Milstead & Short, 2019, p. 116). There are several opportunities for nurses to become involved in policy evaluation. First, the American Nurses Association is heavily involved in policymaking for healthcare. They also have the opportunity to be active in policy review and analysis of rules, decisions, and regulations affecting our profession. (American Nurses Association, n.d.). This is one reason we must join a professional nursing association and help shape our profession. Another way to participate in policy review is to be active in your workplace. Voicing concerns about newly imposed policies or programs is a great way to evaluate new programs’ implementation. This provides a direct effect on your current nursing practice.

Some challenges exist for active participation in the policy review process. Many nurses are not members of a professional nurses association or do not know the importance of why to get involved for starters. Challenges also exist when nurses do not know the current hospital, federal, state, and local policies or procedures. Or when they are not well educated on the newly proposed policies. Evidence-based research is key to overcome some of these challenges. Researching professional nurses’ associations and what they stand for, newly proposed policies in your hospital, and understanding nurses’ vital role in healthcare policy formation and evaluation is key.

Strategies to build awareness for policy evaluation opportunities starts with good communication.  Effective communication can help motivate others for action and build awareness. (How Do I Advocate for Policy Change?, n.d.). Communicating on social media or networking at conferences is an excellent way to begin to inform nurses of opportunities that exist. Policy evaluation is a critical component of effective policies irrespective of the industry. This is because it allows policymakers to rectify areas of a policy based on the identified problem areas during implementation (Safari et al., 2020). In contemporary healthcare settings, nurses are highly encouraged to participate in policymaking processes during the legislation. This presents one of the opportunities for nurses to also participate in the evaluation of the policies. Nurses’ engagement in the policy evaluation processes will ensure the effectiveness of the policies they advocated for during policymaking.

The second opportunity for nurses to participate in policy evaluation is anchored on their responsibility in shaping and reshaping healthcare. RNs and APRNs can use this opportunity by helping the relevant stakeholders understand the factors involved in the provision of quality, accessible, and cost-effective care to the general population. According to Milstead & Short (2019, p.118), the experiences of RNs and APRNs will significantly inform their understanding of these factors, which they will then use to aid other stakeholders with the aim of advocating for necessary changes.

Several challenges that may hinder RNs and APRNs from acting on such opportunities to participate in policy evaluation exist. First, despite the encouragement of nurses to participate in policymaking processes, only a few nurses take on such opportunities. This makes it difficult for the rest to take part in the policy evaluation processes. The other significant challenge is that many nurses lack relevant knowledge on policy evaluation processes, particularly those that are instituted formally (Milstead & Short, 2019, p.118). Therefore, strategies to overcome these challenges are necessary. The first strategy is creating adequate awareness among nurses of the importance and impact of their participation in policymaking and the subsequent policy evaluation processes. The goal of this strategy is to increase the number of nurses that participate in policy evaluation opportunities (Safari et al., 2020). The second strategy is to carry out education and training programs on policymaking and evaluation processes. The strategy will expand the knowledge of nurses on the same, which, in turn, will motivate them to take up policy evaluation opportunities.

One strategy recommendation to advocate or communicate the existence of such opportunities is the creation of awareness through vibrant and most visited nursing platforms. The creation of awareness will consist of detailed information and updates on policymaking processes, ideas, and evaluations at any given time. Another recommendation is for individual health care facilities to encourage their nurses to participate in such opportunities by informing them of how the evaluation of the policy will affect them and the overall nursing industry (Glasgow et al., 2003). The facilities may use debriefings or in-person meetings as platforms for encouraging their nurses to participate in policy evaluations (Milstead & Short, 2019, p.118).
