Discussion – Week 1
Pregnancy is a crucial time for the development of a human life. There are many changes and growth that is happening during nine months of pregnancy. The pregnant mother must alter her lifestyle to meet the needs of her growing baby. Some of these changes include ending drug and alcohol abuse, eating appropriate foods and avoiding harmful foods, receiving proper prenatal care and staying active to promote a healthy pregnancy.

Patient Safety Problem

I live near an American Indian reservation and I have encountered countless mothers who had late or no prenatal care. In 2017, American Indians were most likely to report late or no prenatal care in 12 percent of births (Late or No Prenatal Care, 2019). The lack of prenatal care poses a risk for both mother and baby. In my experience, when a mother is not receiving prenatal care, they are also involved in reckless activities that are actively harming her and baby. Some of these reckless activities include using drugs and abusing alcohol. Using tobacco and drinking alcohol during pregnancy has been linked to increase the risk of sudden infant death syndrome. Drinking alcohol during pregnancy can also increase the risk of fetal alcohol spectrum disorders, intellectual disabilities, problems with vital organs and having a small head (What is prenatal care and why is it important, 2017). There are countless, long-term effects of children who have been born from a mother who used illegal substances during pregnancy while not receiving proper prenatal care. Change in Practice

A change that could be beneficial for pregnant mothers on the reservation would be completing home visits. Research has shown that a home visiting program for pregnant women and their infants brings about participation and knowledge that will increase positive maternal and infant outcomes (Johnson, 2020). Often, these pregnant mothers do not have the means to go to an appointment or to provide a healthy lifestyle in pregnancy. A home visit would literally meet the expecting mother where she is, she wouldn’t have to try to find a ride to the clinic. Prenatal care and education would be completed, along with supplying necessary vitamins that include folic acid. Enrolling a mother in a drug or alcohol rehab that is safe for pregnancy could improve pregnancy outcomes for both mom and baby.

Research Question

Would home prenatal visits to low-income, high-risk expecting mothers improve pregnancy outcomes for mother and baby?

This question is an example of quantitative research. Common demographic variables utilized in quantitative research are gender and ethnicity (Grove, et al., 2013). Quantitative research contains probable designs, and this research question falls under the quasi-experimental category because it measures the cause and effect of prenatal care via home visits. I think that this type of research would best fit the question posed because quality prenatal care correlates with positive pregnancy outcomes. The other three types of research, descriptive, correlation and explanation, are not useful for this research question simply because they are not focused on the independent variable (prenatal care) that causes change in the dependent variable (pregnant mothers and their baby) (Grove, et al., 2013). Week 1: Overview of Research

What is research, and how does research relate to nursing practice?

Nursing practice is based on outcomes and best practices supported by research findings. That is, issues related to health are examined, and proposed solutions to promote patient safety and health care quality are tested to best determine nursing protocols for professional practice. Consider for example, current nursing protocols to minimize patient falls in health care settings. Without research, extensive testing, and an awareness of how to promote solutions to minimize the risk of falls, there is an increased risk that this very real patient problem would get worse.

This week you explore an overview of the different types of research uses in health care settings. You examine how each type of research is used by nurses in providing quality of care and in promoting positive patient safety outcomes. The week will conclude with an overview of the basic elements of the research process: (a) problem identification, (b) review of literature, (c) implementation of research methods, (d) analyses and presentation of findings, and (e) conclusions about implications for practice.NURS 3150 3151: Foundations of Nursing Research.

Discussion: Choosing the Type of Research for a Research Problem

How do you choose the type of research to conduct to address a research problem? What information should you keep in mind to ensure that your research process will adequately address your research problem?

Understanding the different types of research is a critical skill for the nurse researcher and nursing professional. As a current nursing professional, consider how understanding the different types of research may be conducive to achieving a particular mission in your health care setting, such as developing an intervention to address a quality or patient safety problem. This knowledge can also be a step toward assuming a nursing leadership position. As a critical component of your nursing toolkit, differentiating between the types of research is a fundamental step toward enacting change through the process of research.

For this Discussion, please review the following:

  • Think about clinical practice problems you have seen in health care that compromise patient safety and health outcomes. For example, nursing-sensitive indicators reflect high-priority practice problems, which are described in the article by Martinez, Battaglia, Start, Mastal, and Matlock (2015).
  • Choose one of the clinical practice problems you have seen in your current or past job in a health care setting. Consider the possible causes of this problem and how you think it is, or was, affecting patient outcomes.
  • Some patient safety problems are solved by making changes in clinical practice. For example, decreasing catheter-associated urinary tract infections in hospitalized patients often requires changes in clinical practice such as better adherence to preventive measures when catheters are inserted (e.g., use of disposable gloves, maintaining sterile fields, cleansing urethral meatus). For the problem you identified, what specific change in practice do you think is needed?
  • When a practice change is implemented, it is important to verify whether or not the change has improved patient outcomes. One way to determine this is by conducting research. One of the first steps in the research process involves developing a research question that will later serve as the foundation for your study. Using information from Chapter 5 in your textbook and the handout on developing research questions, think about a research question and about the effectiveness of the practice change you proposed in improving patient outcomes.
  • Another step in the research process involves identifying which type of research could be conducted to best answer your research question. In Chapter 5 of your textbook you will find a list of different types of research in the first column in Table 5-2, and Table 5-3. You can learn more about these different types of research in both Chapter 2 and the Glossary in your textbook. To locate even more information, you can also use an Internet search engine for more in-depth descriptions and examples. After learning more about the different types of research, think about which one you think is best for determining how well the clinical practice change you identified will improve patient outcomes.

Describe the reasons why you think this is the best approach and why you would not use the other three types of research.

Andre is a nursing student who has just completed the first 2 weeks in a nursing research course. He has defined a clinical research question for his health care setting and has also determined that the research problem exists in the literature. However, his approach to defining a solution for his research question requires that he receive approval from his health care setting before testing it. What is quantitative research design? What different methods might exist for engaging in quantitative research for nursing practice?

As you have explored in this course, your next step in the research process is to determine what type of research design and methods you will use to engage in research for your research question. While it is possible that many research designs and methods might be appropriate for your particular research question, gaining a firm understanding of different research designs and methods is another useful skill for engaging in nursing research and practice.

This week, you explore the different categories of quantitative research designs used in a clinical research studies. These include experimental, quasi-experimental, descriptive and correlational designs. You will examine the basic characteristics of each design (e.g., causality, control, bias, validity, randomization, probability). You will also examine the basic features of the methods used to conduct most quantitative research designs, such as sample selection, data collection and measurement of variables, as well as data management and analyses.

Discussion: Reliability and Validity of Research Methodology

Selection of the appropriate sample is critical to the reliability and validity of research methodology. The sample must be representative of the population that is being studied. Researchers often report both the inclusion and exclusion criteria for the sample. There are many different methods for selecting a sample and random sampling provides the high control of outside variables but is often difficult to utilize in many studies.

How might a nursing researcher or nursing professional relate their research findings to solutions for improved quality of care and patient safety? Part I: Drawing Conclusions About Research Findings and Their Implications for Clinical Practice

Part I focuses on the last phase of the research process, namely drawing conclusions about the findings and discussing their implications for clinical practice. You will examine how the findings contribute to clinical practice and how the findings might add to new knowledge and the existing literature on a topic. You will consider how the characteristics of well-written conclusions and implications, including consideration of the study’s limitations, recommendations for future research, and variation in the quality of presenting these areas in published research may impact nursing practice. Discussion: Use of Research in Clinical Practice

Throughout this course, you have examined and considered how the research process contributes to the field of clinical nursing practice. More than just an exercise in developing a research question, searching the literature to guide future research, and performing actual data collection/data analysis, the conclusions you draw from your research findings are useful in helping to develop and enact effective clinical practice. In essence, the process of conducting research contributes to positive social change in that the solutions that you might propose as a nurse researcher and professional will likely impact quality of care and patient safety.

As a current or future nursing researcher and professional, your ability to draw reasonable conclusions from research findings will determine the effectiveness of proposed solutions for clinical nursing practice. As you continue to develop your skill set to become a nursing leader, your exposure to a multitude of research designs, research findings, and evidence-based practice will likely inform how you will absorb this knowledge into meaningful clinical practice. Throughout this course and program, your exposure to different research questions, research study designs, and approaches to addressing serious issues in both nursing and health care confirms the ever-changing landscape in which you have elected to practice. While this is in its own right challenging, it also offers several opportunities to create new knowledge, improve upon current nursing practice, and exercise research skills
for positive patient outcomes.

For this Assignment, review the Research Methods and Findings of the Verweij study conducted in 2014 in this week’s resources. The primary purpose of this quantitative research study was to investigate the effectiveness of an intervention to decrease medication errors in a hospital. Client’s pain level is currently a 6 out of 10. The PMHNP questions the client on what would be an acceptable pain level. He states, “I would rather have no pain but don’t think that is possible. I could live with a pain level of 3.” He states that his pain level normally hovers around a 9 out of 10 on most days of the week before the amitriptyline was started. The PMHNP asks what makes the pain on a scale of 1-10 different when comparing a level of 9 to his current level of 6?” The client states, “I’m able to go to the bathroom or to the kitchen without using my crutches all the time. The achiness is less and my toes do not curl as often as they did before.” The client is also asked what would need to happen to get his pain from a current level of 6 to an acceptable level of 3. The change in administration time seemed to help. The client states he is not as groggy in the morning and is able to start his day sooner than before

Client’s current pain level is a 4 out of 10. He states that he is now taking 125 mg of amitriptyline at bedtime.

Client’s has noticed that he is putting on a little weight. When asked, the client states that he has gained 5 pounds since he started taking this medication. He currently weighs in at 162 pounds. He is 5’ 7”. He states that his right leg doesn’t bother him nearly as much as it used to and his toes have only “cramped up” twice in the past month. He states that he is able to get around his apartment without his crutches and that he has even started seeing someone he met at the grocery store. The weight gain seems to bother him a lot and he is asking if there is a way to avoid it

Decision Point Three

Guidance to Student

At this point, the client is almost at his goal pain control and increased functionality. Weight gain is a common side effect with amitriptyline and should be a counseling point at the initiation of therapy. He has a small weight gain of 5 pounds in 8 weeks. A reduction in dose may have an effect on the weight gain but at a considerable cost of pain to the client. This would not be in the best interest of the client at this point. Amitriptyline has a side effect of cardiac arrhythmias. He is not experiencing this at this point. The drug, qsymia contains a product called phentermine which has a history of causing cardiac arrhythmias at higher doses. This product is also only approved for a client with obesity defined as a BMI greater than 30 kg/m2. Your client’s BMI is currently 25.5 kg/m2. He does not meet the definition of obesity but is considered overweight. His best course of action would be to continue the same dose of Elavil, counsel him on good dietary and exercise habits and connect him with a life coach who will help him with this problem in a more meaningful way than a 10 minute counseling session will be able to accomplish.

The Case Study:

BACKGROUND

This week, a 43-year-old white male presents at the office with a chief complaint of pain. He is assisted in his ambulation with a set of crutches. At the beginning of the clinical interview, the client reports that his family doctor sent him for psychiatric assessment because the doctor felt that the pain was “all in his head.” He further reports that his physician believes he is just making stuff up to get “narcotics to get high.”

SUBJECTIVE

The client reports that his pain began about 7 years ago when he sustained a fall at work. He states that he landed on his right hip. Over the years, he has had numerous diagnostic tests done (x-rays, CT scans, and MRIs). He reports that about 4 years ago, it was discovered that the cartilage surrounding his right hip joint was 75% torn (from the 3 o’clock to 12 o’clock position). He reports that none of the surgeons he saw would operate because they felt him too young for a total hip replacement and believed that the tissue would repair with the passage of time. Since then, he reported development of a strange constellation of symptoms including cooling of the extremity (measured by electromyogram). He also reports that he experiences severe cramping of the extremity. He reports that one of the neurologists diagnosed him with complex regional pain syndrome (CRPS), also known as reflex sympathetic dystrophy (RSD). However, the neurologist referred him back to his family doctor for treatment of this condition. He reports that his family doctor said “there is no such thing as RSD, it comes from depression” and this was what prompted the referral to psychiatry. He reports that one specialist he saw a few years ago suggested that he use a wheelchair, to which the client states “I said ‘no,’ there is no need for a wheelchair, I can beat this!”

The client reports that he used to be a machinist where he made “pretty good money.” He was engaged to be married, but his fiancé got “sick and tired of putting up with me and my pain, she thought I was just turning into a junkie.” The assignment; Assessing and Treating Clients With Complex Regional Pain Disorder

He reports that he does get “down in the dumps” from time to time when he sees how his life has turned out, but emphatically denies depression. He states “you can’t let yourself get depressed… you can drive yourself crazy if you do. I’m not really sure what’s wrong with me, but I know I can beat it.”

During the client interview, the client states “oh! It’s happening, let me show you!” this prompts him to stand with the assistance of the corner of your desk, he pulls off his shoe and shows you his right leg. His leg is turning purple from the knee down, and his foot is clearly in a visible cramp as the toes are curled inward and his foot looks like it is folding in on itself. “It will last about a minute or two, then it will let up” he reports. Sure enough, after about two minutes, the color begins to return and the cramping in the foot/toes appears to be releasing. The client states “if there is anything you can do to help me with this pain, I would really appreciate it.” He does report that his family doctor has been giving him hydrocodone, but he states that he uses is “sparingly” because he does not like the side effects of feeling “sleepy” and constipation. He also reports that the medication makes him “loopy” and doesn’t really do anything for the pain.

MENTAL STATUS EXAM

The client is alert, oriented to person, place, time, and event. He is dressed appropriately for the weather and time of year. He makes good eye contact. Speech is clear, coherent, goal directed, and spontaneous. His self-reported mood is euthymic. Affect consistent to self-reported mood and content of conversation. He denies visual/auditory hallucinations. No overt delusional or paranoid thought processes appreciated. Judgment, insight, and reality contact are all intact. CORPORATE RESPONSIBILITY 3

Personal professional development is an integral portion of the practicum experience. In this post, I will discuss my professional development objectives, my progress towards achieving them, how my practicum activities are contributing to my professional growth, and the means I will utilize to ensure I am achieving my objectives. I will also discuss the challenges I have experienced in meeting my objectives, two unexpected opportunities that have arisen, and the knowledge I have acquired during this experience.

Practicum Professional Development Objectives

I developed three professional development objectives for this portion of the practicum. During the eleven-week practicum, I will observe and discuss senior leadership involvement in executive level meetings, be able to compare and discuss how three nursing leaders adjust their leadership styles situationally, and evaluate the effectiveness of my leadership skills in a variety of situations. I have chosen to observe our Chief Nursing Officer (CNO), Chief Operating Officer (COO), and Director of Professional Development (DPD) in a variety of settings to determine how and when each adjusts their leadership style situationally. Cope and Murray (2017) indicated that nurses can develop new leadership styles through modeling and observing other leaders. My practicum activities thus far have supported my development.

Practicum Activities Contributing to Professional Development

Several opportunities have been arranged which have allowed direct observation of the CNO, COO, and DPD. The most enlightening was a mandatory educational session on colleague relations following the positive union vote at the closest tertiary hospital to our organization. The educator is new to our corporation; she was previously a union representative who was difficult to negotiate with when we reopened a bankrupt critical access hospital as a freestanding emergency department. Our Director of Human Resources introduced the educator as a previous adversary who is now an ally.

The DPD initially was engaged in the education, asked clarifying questions, and provided examples which supported the ongoing education. Her coaching leadership style provided two-way communication and increased the participation of other team members (Cardosos, Ramos, & D’Innocenzo, 2011). However, at one point during the discussion, the educator indicated unionized organizations could not gain Magnet status. She also stated Magnet status would not be renewed if the organization became unionized. Our DPD became defensive, moved from her coaching leadership style, and moved into a transactional leader. She provided evidence from the Magnet organization manual that differed from the education being provided. Manning (2016) indicated transactional leaders focus on regulations and facts. Our COO was the CEO of the bankrupt critical care hospital; she had previously attempted to negotiate contract changes with the educator to keep the hospital open. Our COO is self-aware and emotionally intelligent; she chose to interact minimally with the educator in this situation (Delmatoff & Lazarus, 2014).

Our CNO routinely demonstrates transformational leadership. She is inspiring, champions change, and motivates others to follow (Arnold, Connelly, Walsh, Martin-Ginis, & Ginis, 2015). She engaged her leaders and identified improving colleague engagement as the outcome of the session. The CNO redirected challenging portions of the conversation, supported the DPDM in the Magnet/union discussion, and provided positive examples of how her team promoted positive colleague relations.

Temperature regulation can be challenging in the room chosen for the education. The educator and several colleagues were altering the temperature. The educator wanted the room cooler; local leaders were visibly cold and wanted the room warmer. Two colleagues excused themselves and came back with jackets. The educator told one colleague loudly enough for others to hear “we need to compromise; if you are cold, you need to move.” The CNO demonstrated her transactional leadership ability by regulating the thermostat herself; no one moved it after that point (Manning, 2016).

Achieving Objectives

The above is one example of how I am compare how three nursing officers can flex their leadership styles situationally. The research and observations made have encouraged me to reflect and compare my leadership style. I have had opportunities to flex my leadership style situationally. I have been transactional with the architects designing a family friendly viewing room outside our morgue. I have educated and coached a team leader who delayed the transfer of a critically ill patient from a referring critical access hospital. I have demonstrated transformational leadership with human resources planning a patient care technician recruitment blitz. Delmatoff and Lazarus (2014) indicated emotionally intelligent leaders are both self and socially aware. My practicum experiences have thus far strengthened both.

Challenges Experienced and Anticipated

I have experienced some challenges and anticipate others during my practicum. Our inpatient census has been significantly higher than average. Average daily census for this time of year is 122; we have been experiencing census between 150 and 175. During a high census, resources are reallocated and meetings are cancelled. Several of the meetings I had planned to observe have been postponed. My preceptor and I are seeking other learning opportunities. I anticipate ongoing challenges while the census remains well above average.

Unexpected Opportunities

I have had two unexpected opportunities arise this far in my practicum. I was one of three colleagues chosen to participate in a four hour 5-Why Cause Mapping in-service. Similar to Root Cause Analysis, 5-Why Cause Mapping exposes the underlying system and process issues rather than place blame on individuals working within the existing processes. This type of cause mapping will be utilized in the Transfer Peer Review Process being developed. The second opportunity is to attend the corporate design event for our new electronic staffing and scheduling solution. This event will develop key design and functionality metrics for the solution.

Acquired Knowledge

Thus far I have learned ways nursing leaders can flex their leadership style to meet the current situation. This has created introspection and the realization the I also flex my leadership style situationally. The external and internal views have altered my perspectives regarding my interactions with colleagues. As the manager of the internal float pool, it can be challenging to connect frequently with my colleagues as their assignments are throughout the organization. Manning (2016) indicated transformational leaders need to connect and communicate frequently with their colleagues to inspire and support them. I am developing a colleague rounding plan with protected time on my calendar to strengthen these connections.

Summary

During the first five weeks of the practicum, my experiences have supported my professional development and are meeting my objectives. Two unexpected opportunities have been offered to support my professional development and practicum project. There have been some challenges with high census and meeting cancellations. My preceptor and I have discussed ways to overcome these challenges.

Please interview a family friend or relative outside your immediate family (i.e., not parents and siblings) who has experience with nurses, clinical care, and/or medical settings. Your interviewee should be of legal age (18 or older) and should be fully aware of the purpose of the interview and the fact that it is a course assignment. Grandparents, aunts, and uncles are great candidates for an interview. Interviewing classmates or close friends may not yield particularly interesting or fruitful results.

The purpose is for you to try out some of your interview skills as discussed in Coulehan and Block (2001) and Bickley & Szilagyi (2003) (e.g., such as active listening, empathy, open invitations, and prompts). It is best to conduct the interview face to face, but a phone interview is acceptable. Please avoid email interviews.

Please ask questions related to each of the following areas. Remember to ask open-ended questions, avoiding “yes or no” questions. Before the interview, you should brainstorm a few questions under each area.
