The classical developmental theory that I chose to discuss is the psychosocial theory by Erik Erikson. This theory expands and furthers Freud’s original psychoanalytical theory, which states human functioning is dependent upon the interaction of unconscious drives and forces per individual (McLeod, 2017). Further, Erikson expands the idea of development by noting that there are set stages/milestones in childhood and the stages also advance into adulthood. Moreover, psychosocial theory highlights the importance of culture and socialization on an individual’s social and emotional development. The contemporary theory I chose to discuss is the ethology and evolutionary developmental theory. Traced back to the work of Darwin in evolutionary theory, ethology is a biological perspective concerned with human behavior, and social interaction in regards to survival (Berk, 2018). A major concept coined through research within this theory is that of a “critical period” (Berk, 2018, p. 21). These two theories have similarities in that they both emphasize social and emotional development, and they both postulate that early experiences set a specific course for later human development. An example of this within the psychosocial theory by Erikson is his first developmental stage “basic Trust versus mistrust” (Berk, 2018, p. 15). Within this set stage, infants will learn trust from responsive caregiving. The opposite end of this spectrum would be mistrust due to neglect. In a similar vein, Ethology coined “imprinting” which was originated by observing young birds following and imitating the behavior of their mother for survival purposes. (Berk, 2018 p. 21). Imprinting inspired the concept of a critical period (noted above) which argues that social and emotional behavior is learned at a certain and optimal time. A limitation for both developmental processes is that these theories could be too deterministic. These theories suggest that an individual’s social and emotional processes are determined by our child hood experiences and unconscious thoughts (McLeod, 2017). Discussion: The Contribution of Classical Theorists to Contemporary Developmental Theorists
While all major developmental theories attempt to explain the growth of individuals, each theory has a slightly different perspective. Some theories emphasize environmental (nurture) more than biological (nature) influences. Some theories focus on a particular construct (e.g., cognition), while others emphasize the impact of age range in shaping development. Piaget’s cognitive developmental theory emphasizes fixed stages during which the mind’s capacities allow an individual to learn about the world. Vygotsky’s sociocultural theory, on the other hand, is not stage-based and describes growth as an interaction between the individual and his or her environment.

Contemporary theories (e.g., Langer’s theory of mindfulness) typically build upon the foundation generated by earlier theories. Langer’s theory of mindfulness contains similarities to classical theories, such as Vygotsky’s sociocultural theory, in that they both agree that development is contextual and the organism is an active “mindful” participant. Langer took Vygotsky’s theory to a new level, focusing specifically on education and the learner. Other contemporary theories include neo-Piagetian cognitive developmental theories, which attempt to address the limitations found within Piaget’s classical theory. Robbie Case, Andreas Demetriou, and Kurt Fischer proposed theories that were extensions of Piaget’s theory. These theorists added concepts that expanded on cognitive functioning within the stages of development. ou did a good job in explaining both theories; it was easy for me to understand. I found an interesting article by Nyam and Szymczynska(2016), purporting a link between current dementia activity programs to developmental psychology theories of aging. Facilitating people with dementia’s engagement in meaningful activities is an important feature of their care. They can be encouraged to participate in meaningful activities in both casual and formal settings to help them feel better. Memory cafes and organizations planned for peer support, reading, or singing for the brain’ are examples of this informally, (Nyam & Szymczynska, 2016).

This aspect of an individuals’ life is considered by socio-emotional development theories. The change in an individual’s understanding of, attitudes toward, and behavior toward others over time is referred to as social development, (Susskind, 2005). Erikson’s psychosocial theory suggests that personality is developed is a sequential order through the eight stages of development. The theory suggests that as each conflict is resolved at each stage an individual will acquire the necessary “attitudes” and “skills” to contribute positively to society, (Berk, 2014).

According to Nyam and Szymczynska (2016), spirituality and religion plays an integral role in providing meaning in life for dementia patients. It also aids the person with dementia in mentally preparing for death. Death, according to Erikson’s theory, is the ultimate objective for which people should strive, and that achieving this goal helps create a sense of purpose, with acceptance of death as the end of one’s life story.

Final Project Overview: Connecting Real-World Data through Quantitative Analysis

Cassie is a 9-year-old who received the flu vaccine yet was hospitalized after contracting the flu in 2018. Cassie’s parent is torn on whether or not to give her the flu vaccine in 2019. Based on recent findings from the Center for Disease Control, the flu vaccines have reduced hospitalized risk by almost half (47%) in comparison to previous years (Stanglin, 2019). Is this due to the increased effectiveness of the vaccine or has the virulence of flu strains changed? Quantitative analysis can help answer such questions.

As a working professional, you will experience quantitative data in many different ways. These data will contribute to answering research questions, guiding conclusions, shaping decisions, and helping generate more questions. As part of the Final Project for this course, you will do just that. You will gain a better understanding of how quantitative data and their analyses provide an opportunity for you to see how a phenomenon can impact your field/discipline of study.

Your Final Project for this course will consist of a narrative/report in which you demonstrate your learning through a research question related to a dataset, the variables related to the research question, and the data analyzed using specific types of analysis to determine the results from the data.

Informatics is such an important part of healthcare and a great tool to thoroughly coordinate patient care. It is a way to promote patient care and allow patients to share important health information with their providers and vice versa (Snyder et al., 2011). During the current pandemic informatic lead virtual healthcare has become invaluable with the use of telemedicine. Telemedicine allows clinicians to perform evaluations, consultations, and diagnosis remotely (Nagle et al., 2017). Informatics has allowed healthcare to continue to grow and evolve to reach more patients and provide more complete patient care The Application of Data to Problem-Solving Essay.

I work in an outpatient pain management clinic and there are times our patients come in for a steroid injection while they have an active infection or after recently receiving a steroid shot or dose pack from another provider. This causes us to have to cancel the patient’s procedure and reschedule them. If the patient could go online and fill out a preprocedural survey prior to coming into the clinic the nurse could then review their submission and make a clinical decision on whether the patient is appropriate for injection at that time. We would then be able to avoid leaving gaps in our schedule which in turn would allow for a smoother flow through our office. According to Julie Sweeney when referring to such systems, “it is important to not only consider the technology at hand but also the workflow and the data collection process” (Sweeney, 2017). In a clinic such as ours there are times when patients must wait weeks for a procedure, so any gaps left in the schedule is a missed opportunity to provide treatment. Nurse informaticists interact with other healthcare professionals daily to ensure easy and efficient collection and dissemination of information relating to clinical decisions, patient health records, and organizational operations. You made some interesting points about electronic health records(EHR) being able to detect trends and patterns. By detecting these trends, it would help significantly in helping with consistency to provide safe care to patients. I work on a med-surg floor and see a lot of readmissions. To prevent readmissions, nurses rely on technology to keep everything efficient and organized. Nurse informaticists play a massive role in evolving technology (Laureate Education, 2018). As a nurse, this is one of the reasons why interacting with other professionals is important. Nurse informaticists communicate with other healthcare professionals by providing evidence-based clinical decisions ((McGonigle & Mastrian, 2017). It is essential for nurses to understand how technology relates to their patients and their care. The American Nurses Association defines nursing informatics as “the specialty that integrates nursing science with multiple information and analytical sciences to identify, define, manage and communicate data, information, knowledge, and wisdom in nursing practice” (as cited in HIMSS, 2019).  Nursing informatics (NI) is essential to nurses by enhancing their daily practice and ensuring they make sound, evidence-based clinical decisions (McGonigle, 2017).  NI uses modern technology to access data in all facets of the nursing profession, including daily care, administration, and learning and research. Our floor nurses, such as myself, have no contact with facility informaticists or data/technology specialists in my current employment.  I am unaware of the location or contact methods to reach informatics teams for my facility.  We are directed to bring any issues to the nursing executive team. Modern technology is limited in the facility due to the closed and secure correctional environment in which I work.  However, there have been movements toward adopting an electronic health record (EHR) to replace the current paper charts that we continue to use.  Our facility approved the medical department’s use of a mobile hotspot device and Kindle tablet to do telehealth visits for offenders due to the COVID-19 pandemic.  In addition to those, we recently obtained a cell phone and a second tablet to use.

Management sent staff emails, which included basic directions on operating both the mobile hotspot and Kindle tablet along with how to use these devices to contact on-call mental health or medical providers after regular business hours.   Management later sent a second email giving further detailed and step-by-step instructions on using these devices after multiple questions arose regarding their instructions.  One of the issues staff has found with these devices is that the provider must be aware they will be receiving a Google Meet request via work email.  Unless providers have prior knowledge to access their email, they do not respond or join the requested Google Meet session.  There has been no instructional email providing directions on when or how to use the new cell phone and second tablet available for staff to use when necessary.  These types of training emails have always been the regular practice that is not due to the COVID-19 pandemic Discussion: Interaction Between Nurse Informaticists and Other Specialists Essay.

The electronic health record that our facility was planning to implement has been postponed indefinitely due to COVID-19. We are unable to ascertain when our statewide facilities will be able to adopt this new technology.  With the paper charts’ current use, patient information is sometimes difficult to find.  Sometimes, looking for the charts themselves is very time-consuming when some staff members do not replace them in the medical records room.  Patient information physically travels between facilities often leading to displaced information and damaged or lost charts.

A fully integrated EHR would be one way to improve communication and collaboration between all multidisciplinary health care team members, resulting in safer care and better outcomes for our patients.  Another benefit of an EHR in my facility would provide nursing staff quicker, more complete access to patient information.  More thorough instructions, including videos, would help my facility give more in-depth and necessary staff training instead of the current method of providing email directions that generally require clarifications for better explanations.  Another strategy to reduce frustration and stress is to have small guidance sessions to ensure all staff fully comprehend new procedures when implementing advanced technology Discussion: Interaction Between Nurse Informaticists and Other Specialists Essay.

Thank you for your post.  I enjoyed reading it.  You are in a challenging situation during this pandemic when technology seems to be rising to its fullest capabilities, but close contact education is struggling.  According to Francis (2017), nurses who do not have the technology to incorporate nursing informatics can not be impacted by its capabilities, nor can patient’s health care be optimized.

We were about ready to switch over to a new EHR program until COVID-19 came along.  That was delayed until our facility’s chief surgeon decided to omit the new program and remain with our current program.  Tyler (2020) included that having a clearly defined process, and an easy-to-understand technology remains simple for a management crisis.  I found this acceptable as switching to a different EHR could create frustration and stress among healthcare workers and compromise patient care during this pandemic.  In your situation, switching from paper to technology would be a whole new ball game.  Your efforts would not be directed towards patient care; instead, it would help learn and implement new technology.  However, introducing the mobile hotspots and tablets for virtual appointments is the first step towards introducing nursing informatics Discussion: Interaction Between Nurse Informaticists and Other Specialists Essay.

Case Study Scenario 2: Cystic Fibrosis

A mother brings her 6-month-old daughter to the HCP for evaluation of possible colic. The mother says the baby has had many episodes of crying after eating and, despite having a good appetite, is not gaining weight. The mother says the baby’s belly “gets all swollen sometimes.” The mother says the baby tastes “salty” when the mother kisses the baby. Further work-up reveals a diagnosis of cystic fibrosis. The mother relates that her 23-month-old son has had multiple episodes of “chest congestion” and was hospitalized once for pneumonia. The mother wants to know what cystic fibrosis is and she also wants to know if she should have any more children NURS 6501: Advanced Pathophysiology Discussion: Alterations in Cellular Processes.

Cystic fibrosis is an autosomal recessive inherited disease. According to McCance & Huether (2019), approximately 1 in 25 whites carry one copy of an allele that can cause cystic fibrosis. Because an individual must be homozygous for a recessive allele to express the disease, the carriers are phenotypically normal (p. 151). The cystic fibrosis gene is located on chromosome 7. More than 2000 different variants of this gene are known to produce cystic fibrosis. The cystic fibrosis transmembrane conductance regulator gene mutation results in the abnormal expression of cystic fibrosis transmembrane conductance regulator protein (McCance & Huether 2019, p.1220). Cystic fibrosis newborn screening is the optimal method to diagnose the disease during the asymptomatic period. The early diagnosis is associated with improved physical development and slower lung deterioration due to the lower frequency of infections (Olszowiec-Chlebna et al., 2021).  NURS 6501: Advanced Pathophysiology Discussion: Alterations in Cellular Processes

In this scenario, the mother believes the baby may be suffering from colic due to crying after eating. With cystic fibrosis presentation of oily-looking stool and abdominal distention are early stages of the disease. According to McCance & Huether (2019), 85% of individuals with cystic fibrosis, present early in life with pancreatic insufficiencies, which causes nutrient malabsorption and failure to thrive in children (p. 1380) NURS 6501: Advanced Pathophysiology Discussion: Alterations in Cellular Processes.

The mother makes a comment to the doctor that the baby tasted salty when kissed. This happens due to the dysfunction of the cystic fibrosis transmembrane regulator protein results in altered sodium, chloride, and potassium reabsorption, all of which remain external to the surface of the epithelium membrane with reduced clearance from tubule structures lined by affected epithelia (McCance & Huether, 2019, p. 1380).

This deficiency in chloride ions leads to a salt imbalance that results in secretions of abnormally thick, dehydrated mucus. Some of the digestive organs, particularly the pancreas, becomes obstructed, causing malnutrition, and the lungs become clogged with mucus, making them highly susceptible to bacterial infections, especially Pseudomonas (McCance & Huether, 2019, p. 151). The typical features of cystic fibrosis lung disease are mucus plugging, chronic inflammation, and chronic infection of the small airways (McCance & Huether, 2019, p. 151).

Clinical manifestations involve the respiratory and gastrointestinal systems. Respiratory symptoms include persistent cough or wheeze, excessive sputum production, and recurrent or severe pneumonia. Digital clubbing may appear in the early stages; in the latent stage development of the barrel chest, or persistent crackles occur (McCance & Huether, 2019, p. 1221). Approximately 8% of CF patients will present with haemoptysis during a pulmonary exacerbation, and it is speculated that 1 in 100 CF patients will have a massive haemoptysis episode each year, with an associated two‐year mortality risk ranging from 5.8% to 16% (Gavioli & Aung, 2020) NURS 6501: Advanced Pathophysiology Discussion: Alterations in Cellular Processes.

At its core, pathology is the study of disease. Diseases occur for many reasons. But some, such as cystic fibrosis and Parkinson’s Disease, occur because of alterations that prevent cells from functioning normally.

Understanding of signals and symptoms of alterations in cellular processes is a critical step in diagnosis and treatment of many diseases. For the Advanced Practice Registered Nurse (APRN), this understanding can also help educate patients and guide them through their treatment plans.

I really enjoyed reading your post.  I always found it interesting how one of the findings with CF is a “salty” taste which causes “high

levels of salt in the sweat of patients with cystic fibrosis suggested an abnormality in electrolyte transport from the sweat gland” (Yu, 2021).

I had a coworker who had given birth many years ago long before I became a nurse.  and I remember her telling me her daughter had a

salty taste when she kissed her little girl.  I many years later learned that the little girl had be diagnosed with CF.  Many years ago, before

there was ample research like we have now, the life span of an individual with CF was not ling at all.  Now, with all the

technology  and  research, a person living with CF is expected to live approximately 40 years (Naehrig & Naehrlich, 2017) which still is

terribly young for anyone to lose their life, however,  for this disease is quite an accomplishment from many years ago.

Great post and fanrastic information.  I truly hope at some point research can find a cure for this awful disease.  My son has a

classmate, 14 year old wonderful girl who suffers from this disease.  Her PFT’s are always at a great percentage.  Every now and then she

has to be hospitalized for treatment when she is ill.  She is very active with sports and overall, she has a wonderful happy and active life.

Your post on cystic fibrosis is very informative. Cystic fibrosis is an inherited disease that causes severe injury to the lungs, digestive system, and other organs in the body. According to the May Clinic, the thick and sticky mucus associated with CF clogs the tubes that carry air in and out of your lungs which can cause signs and symptoms such as; persistent cough, wheezing, repeated lung infection, exercise intolerance, and sinusitis. Also, the thick mucus can block tubes that carry digestive enzymes from the pancreas to your small intestine. Without these digestive enzymes, the intestines can’t completely absorb the nutrients in the food one eats which can cause signs and symptoms such as; foul-smelling greasy stools, poor weight gain, intestinal blockage, and chronic or severe constipation (Mayo Clinic, 2020). In the scenario, the mother would like information about CF and if she should have another child. A child will be born with CF only if they inherit one CF gene from each parent. A parent can be a CF carrier, and pass the CF gene on to their child. Once parents have had a child with CF, they have a 1 in 4 chance that each additional child will be born with CF. This means there is a 3 out of 4 chance that additional children won’t have CF. But these children may be carriers of the CF gene (Stanford Children’s Hospital, n.d.) Strep Throat leading to allergic reaction from antibiotics

In the scenario provided, our patient presented with a sore throat, reddened posterior pharynx, and white exudate on tonsils. The patient was tested posted for strep by a rapid strep test. The patient was prescribed amoxicillin, and after the first dose, he immediately had swelling of his tongue and lips with trouble breathing and wheezing.

Genetics role

Certain people are predisposed to have allergies. If this is the case for type I allergies, it is called atopic. If our male patient’s mother or father has an allergy, then our patient has about a 40% chance of developing allergies. If both parents have allergies, then he has around an 80% chance of developing allergies. If our patient is atopic, he possibly produces higher amounts of IgE and could have more Fc receptors to IgE on his mast cells. This could make his airways and skin more responsive as well. What caused the symptoms

The patient possibly had a type I allergic hypersensitivity related to amoxicillin. Most of the symptoms of a type I reaction are due to the effects of histamine. The tissues that tend to be most affected contain a more significant number of mast cells and are more sensitive to histamine. The primary symptom being shortness of breath and wheezing could be because the throat was the main portal of entry for the allergen if it is the antibiotic. Effects of the allergy to the respiratory tract include vasodilation, inflammation, swelling, and edema of the larynx (McCance & Huether, 2018).

Physiologic response

Our patient had swelling of the tongue and lips with difficulty breathing and audible wheezes. The reaction to the medication caused an allergic reaction that produced a massive histamine response. This led to swelling, bronchoconstriction, and vasodilation. Since the cause of inflammation was due to histamine, it was an immediate response. Cells involved

In the inflammatory response, there are many cells present. As the histamine enters the body, it causes vasodilation as the capillaries are dilated. The mast cells are activated by the allergen and release histamine. There are chemokines involved that act as messengers. Other cells involved could include phagocytes as the first responders, such as neutrophils. The body turns into a battlefield with everyone showing up, including B cells and T cells (Khan Academy, 2010).

Things that could change my response

If the kid’s genetics played a different factor, he might not be allergic to the antibiotic, and therefore there would be no allergic reaction. My genes have blessed me that, to my knowledge, I have no known drug allergies. I am also blessed to be able to eat peanut butter and shellfish. Our patient here may be atopic, making him predisposed to allergies with a more sensitive system.

Thankfully, I don’t have any allergies. Even last spring, I did not have any symptoms of sneezing or a runny nose.

But I believe I’m not yet on the safe side. According to Eaddy Norton & Broyles, drug-induced anaphylaxis is most frequent in females, but there is no clear sex predominance (Eaddy Norton, Broyles, 2019).

In addition, anaphylaxis by all causes has been reported with higher frequency in adult females than in males, and this predominance appears after puberty. However, there is no sex difference in rates of fatal anaphylaxis (Turner e. al, 2017)

With all this information given, as a female we should still be cautious about what we take and eat because it may not be evident in our later years but might come out in the future.

Why is the patient presenting with these symptoms?

The patient was unresponsive due to an overdose. Narcan, an opioid antagonist used to reverse the actions of opioid overdose, was given in the field. Opioids are known for depressing the respiratory system thus leading to respiratory failure or distress. The patient mos t likely experienced respiratory failure and passed out (McCane & Huether, 2019). The necrotic tissue is most likely the result of a fall after passing out and could represent cellular damage and perhaps even a broken bone. Necrosis might also represent damage from lying or an undetermined amount of time in one position, in this case, the left hip and forearm (McCance & Huether, 2019). Necrosis is tissue death caused by lack of blood and oxygen and can be caused through or because of Rhabdomyolysis (Cabral, Edding, Portacarrero, & Lerma, 2020). This could have also caused the high level of potassium (Chatzizisis, Misirli, Hatzitolios, & Giannoglou, 2020). In this case, gender would not play a significant role. The clinical presentation of drug overdose in male versus female are not greatly different as shown in the study by (Syse, et al., 2019). Age would play a difference only in that it usually takes less of a drug taken by mouth to cause an overdose in polder adults due to decreased metabolism and excretion (Vu, Beselman, Monolakis, Wag, & Rastegar, 2018). Moreover, injections in a nonsterile environment and practices such as reusing needles or sharing used equipment facilitate local soft-tissue bacterial inoculation, with subsequent risk of serious complications such as sepsis, gangrene, amputation, and death. Great post. Overdoses are becoming more common and Narcan is one of those drugs we always keep handy especially in the hospital setting. Naloxone reverses opiate-related cases in and out the hospital without the addictive effects (Wojcicki, 2019). In most opiate OD cases it most time not intentional. Depending on the drug of choice the symptoms vary, but eventually respiratory distress begins, and the users is not far from becoming unresponsive, like the patient was in the scenario. An opioid overdose is caused by the overly excessive stimulation of the opiate pathways leading to respiratory issues and maybe even death (Schiller, 2021). I agree that your patient may have lost consciousness and fell to the ground on that necrotic side and may have possible broken something causing another systemic response. Furthermore, effective communication skills cannot be emphasized enough, especially when treating younger patients and adolescents.  If developed during formative years, effective communication skills, a core competency for practitioners, can help students and future practitioners alike (Choudhary & Gupta, 2015).  However, clinical communication skills with the pediatric and adolescent populations specifically, pose a distinct challenge for health care providers, as research in this area is complex and limited, (Frost et al., 2015). I agree with your techniques of avoiding medical jargon, actively listening, and expressing empathy.  I would add to the questionnaire more focused inquiries regarding this patient’s mental health status. Thank you for such a detailed post. Your approach in communication techniques with this teenager is professional and definitely empathic and establishing trust is very important. Adolescence is a critical developmental period associated with increased vulnerability for depressive symptoms and gender differences in depression generally emergence during adolescence (Lorenzo-Blanco et al., 2012). Although the reasons and details of the visit are not indicated, it would be important to inquire about thoughts of wanting to hurt himself or anyone else.  (Caplan, 2019) reports that the rate of self-reported mental illness for Latinos are similar to or slightly less than those of the population as a whole, which is a very large population NURS 6512 Discussions and Assignments: Advanced Health Assessment and Diagnostic Reasoning. May 2012, Alice Randall wrote an article for The New York Times on the cultural factors that encouraged black women to maintain a weight above what is considered healthy. Randall explained—from her observations and her personal experience as a black woman—that many African-American communities and cultures consider women who are overweight to be more beautiful and desirable than women at a healthier weight. Randall’s statements sparked a great deal of controversy and debate; however, they emphasize an underlying reality in the healthcare field: different populations, cultures, and groups have diverse beliefs and practices that impact their health. My patient is a 40-year-old black recent immigrant from Africa without health insurance. The challenge here lies in the fact that this patient’s English fluency is not indicated. I would begin my interview by offering a friendly smile to make the patient feel comfortable and introduce myself.  I would then ask some basic questions such as name, age, and where the patient resides to assess the patient’s language competency.  If necessary, I may require the assistance of an interpreter, perhaps a software tool to help with translating if an interpreter is not available.  Whereas some African nations do communicate in English, this is not the case for all African nations.  Thus, a language assessment is imperative in obtaining this patient’s comprehensive health history 40-year-old black recent immigrant from Africa without health insurance.

After addressing any communication challenges, I would focus on obtaining basic information such as age, gender, marital status, and occupation.  Thereafter, I would proceed towards obtaining a thorough health assessment by asking specific questions.  The aim of obtaining a high-quality comprehensive patient health history is to identify symptoms that represent a range of pathologies; even by itself, the assessment is a powerful instrument (Diamond-Fox, 2021).  Specifically, I would ask the below five basic questions:

  1. What is the reason for your visit today? By asking this, I would assess the patient’s unexpressed fear or concern.
  2. When did you first begin to feel your symptoms? This question is asked to assess the chronological order of events, as I assess the patient’s most recent state of health prior to the current condition.
  3. Can you describe your symptoms, giving the location, duration, aggravating and alleviating factors, onset, and duration? This inquiry is for symptom analysis.
  4. Do you have any medical conditions, or are you taking medications for any condition?  Perhaps here I can also ask of any childhood illnesses or injuries that may be pertinent to the patient’s chief complaint. Assessing the patient’s past history helps practitioners assess the present issue and also formulizes a risk assessment of their health.
  5. Towards the end, I would like to summarize the data collected with my patient by repeating the information and asking “Am I missing anything? or “Is this information correct?”  This is to correct any errors and ensure that I have in fact obtained the most accurate health history for the patient that can now be established in his chart/profile 40-year-old black recent immigrant from Africa without health insurance.

 

As a provider, I must remain aware of cultural sensitivities to ensure my actions or wording will not be misinterpreted.  Immigrants from Africa in particular faces challenges from the US healthcare system, including the lack of culturally sensitive care and hostile provider attitudes that reinforce their lack of trust in the healthcare system (Omenka et al., 2020). For this reason, it is crucial that I remain empathetic, courteous, and ensure comfort in order to develop a bond of trust. A useful tool that can be applied here is I.C.E. (ideas, concerns, expectations.) This ensures that the provider listens actively, is aware of proper body language, empathizes with the patient, and focuses on the patient’s ideas, concerns, and expectations of healthcare by directly asking the patient “what are your concerns?” (Desai, MD, 2018).

The fact that my patient is an immigrant places him at a greater risk of psychological health problems due to both pre-migration risk factors such as previous traumatic experiences along with post-migration factors such as culture shock, low socioeconomic status, and a possible void in the emotional family/support system (Adu-Boahene et al., 2017). And for this reason, I would add a mental health component in my interview to check for any signs of depression, anxiety, trauma, sleep disturbance, eating disorder, or substance abuse.  There are many tools for mental health that can be utilized such as the PTSD checklist for DSM-5, or an anxiety checklist that gives the provider a general idea of the stress level patient is currently experiencing. 40-year-old black recent immigrant from Africa without health insurance

As a provider, I would take all such factors into consideration when developing an appropriate care plan.  An added degree of empathy is required in the assessment of this patient due to the fact that the patient is in a foreign land, dealing with culture shock, communication barriers, and not having health insurance.  This assessment will most likely be a bit longer than the usual admitting patient because of these factors, and this patient will need to be referred to a social worker for assistance in obtaining proper care although uninsured.  In the end, I would hope to have provided comfort and empathy and developed a bond of trust with this patient, in addition to providing proper medical care 40-year-old black recent immigrant from Africa without health insurance.

I agree with your assertion that you should offer a friendly smile to make the patient feel comfortable and introduce yourself to warm up the encounter before engaging in any further action(s). According to Omenka et al. (2020), many African immigrants already have substantial health vulnerabilities when they arrive in the United States, which will only worsen if they don’t have access to good treatment. For this reason(s), the practitioner must determine the patient’s needs, establish a suitable treatment plan, obtain reliable information via history collection and a physical examination (Ball et al., 2019). Furthermore, Communication is a two-way process to exchange meaning. As clinicians, we must evaluate the patient’s appearance, exposure or experience, language skills, tone of voice, listening ability, cultural practices/beliefs, and body language to collect the necessary information or create a therapeutic alliance. Furthermore, patient-centered care based on the patient’s knowledge necessitates the use of a unique technique. In an immigrant, the communication tactics should focus on chronic illness, migratory trauma, and socioeconomics to the best of their knowledge. The provider must address the language barrier if applicable in the case of the immigrant and any access to available resources. Although some African countries use English, the majority speak French or dialects. Conscious awareness and ongoing examination of all communication process components can help improve diagnostic abilities and patient compliance 40-year-old black recent immigrant from Africa without health insurance.

According to Ball et al. (2019), Structuring a health history is critical for accurately diagnosing and treating patients. Firstly, I will agree with you to evaluate the patient’s mastery of English and a language interpreter used in a language barrier to facilitate this encounter and after that.  Healthcare practitioners must accept patient variations or backgrounds (considering that this immigrant’s values may vary from U.S. values) and views to make this process comfortable and trustworthy. Also, the patient may be suffering the usual obstacles of cross-continental or geographical travel, such as not finding work. The practitioner, in this instance, must assess the patient’s emotional and psychological well-being as adaptation has been linked to mental illness. Isasi et al. You made so many great points and hit so many things, I can already tell you will be a very empathetic practitioner. I like how you included ICE (ideas, concerns, and expectations) in your assessment. I also like your five questions. I haven’t really thought about all the issues that can occur with a patient who is an immigrant where language barriers might be a factor and also when they do not have insurance. It is very sad that some people are unable to receive the same care that others are afforded. Suzanne et al. (2019) agree that ethnic minorities, and those from other countries, are vulnerable and are at a social disadvantage. Many scams are also out there with healthcare letting some believe they will cover things and getting them hooked into paying for insurance that they can’t afford or don’t need. Dagmar Dzúrová et al. I can see how uninsured patients a problematic issue might be so I would make sure to have pamphlets available for the patients about other insurance options. I would also see the need to address as many issues as possible in one visit for the patient because due to the lack of insurance, they might be as willing to return to the office. Radoslaw et al. (2018) point out that those with no insurance or high deductibles have a lower life expectancy than those with insurance. I would try to get as good of a health history as possible so I could see determining factors leading up to the reason for the visit. If possible, I would suggest that the patient get bloodwork to ensure they are healthy. Everything costs money though, and the patient may just need an acute need met rather than a comprehensive exam 40-year-old black recent immigrant from Africa without health insurance.

In the end, you brought some good points at the challenges in helping those that are unaware or unable of the best type of healthcare needed. This is something that will not go away and we will always have challenges how to help all those that need care. To help the patient feel comfortable, I would hope that there is a family member or friend that could come to the appointment to help with translation if needed. I also realize it might be a good idea to invest in a good translation app as language barriers might happen often. I truly believe mental health could be a potential problem with some immigrants depending on the support they have so I think asking mental health questions, or including a mental health questionnaire to be answered if possible would be a great option as well.

Thanks for your post, it opened my eyes and brought many things to light that I didn’t think about before when giving healthcare. I live in Arizona and just started working as a nurse at a prison and I didn’t realize how many people are in the prison that do not speak English. The prison has invested in an interpreter service that we can call with a telephone that really helps when delivering healthcare.

I enjoyed reading your post. As I read through your post, I can identify as that 40-year-old immigrant, and I can correctly validate some of the ideas here in your post. You did a great job by stressing the importance of obtaining a health history and health assessment. You also touched on mental health, which in most African countries, residents still see mental illness as an omen or taboo.

Africa is a continent that is reached in culture. I agree with you 100% that as a provider, you must remain aware of cultural sensitivities to ensure your actions or wording will not be misinterpreted 40-year-old black recent immigrant from Africa without health insurance.

Adequate or appropriate assessment of an African patients would be dependent on how culturally competent and sensitive the provider is.

The provider might be the brightest of all providers, but if not culturally aware of Africans, there would always be a barrier in communication. Most countries in Africa are either Anglophone (English speaking) or francophone (French-speaking) countries, depending on the colonized country in Europe. A 40-year-old from an English-speaking country should have at least a high school diploma. The ability to understand the patient’s accent and be understood by the patient is a big challenge. Sometimes the interpreter is not the solution.

The interview technique would entail speaking slowly, and sometimes, when words are written out, it helps to understand some phonetics.

Smiling at a new patient may mean different things in different cultures. A handshake is always a welcoming and friendly gesture rather than a smile. Lack of eye contact during the interviewing process is not derogatory or bad for this 40-year-old man. Asking about what brought a forty-year-old man to the US is irrelevant to him, but he would appreciate you asking about his goals in American and whom he left behind. This question would guide the provider to recommend the proper health promotion for this immigrant man. These questions are not intrusive, but rather, they help the man open up and tell you more stories about his life and living conditions.

The United States has done so well in health history taking and documentation for the future. The new technological systems have made it easier to keep patient records from birth to old age. Unlike most countries in Africa, there are gaps in medical journals if at all they are found. The life expectancy of a Nigerian male is 60 years compared to a US male of 78-year-old (Varrella, 2021). Social determinants of health are rampant in most African families, such as poor housing conditions, food scarcity, and lack of healthcare. A provider must assess and address which factors impact the patient’s health (Ball et al., 2019). Family health history would be necessary to make a presumptive diagnosis for diabetes, hypertension, and heart disease.

The Center for Disease Control and Prevention (CDC) has a screening tool for new refugee immigrants for providers and clinics to use. This tool is termed the National Standards for Culturally and Linguistically Appropriate Services in Health and Health Care (CLAS Standards) (Center for Disease Control and Prevention CDC, 2020). It is recommended that care providers obtain this tool to appropriately provide that clinical care, address language and cultural barriers that patients may experience (CDC, 2020) 40-year-old black recent immigrant from Africa without health insurance.

It is crucial to assess immunization. Ask questions about the last time the patient saw a health care provider and for what reason.
