Nursing encompuses more of  high-quality care than knowing diseases. The nurses also act as patient advocates and that requires open communication, effective education, transparent treatment and responsive care – this is how the nurses empower their patients to take control of their own health. They are largely involved in patients affairs, they help patients make informed decisions regarding their health, including helping them navigate a complex medical system, translating medical terms and helping patients make ethical decisions.

The nurses are educated, competent and well informed on matters related to patients wellbeing and safety, and these are the kind of values that they need be an effective nurse advocate and policy players.

The major function of nurses is being a change agent, The APNs have a large influence within their work environment and are able to help create necessary changes. However, these nurses need to embrace the new ideas of change first, before convincing the others of the value of the new approaches.

Example, Consider the following scenario. A pediatric medical attendant whose was my supervisor during my internship program, feels moving a young man to another clinic would additionally damage him and his family, as of now crushed by a flood, so the attendant shouts out for their sake. The doctor thinks about the medical attendant’s contention and concurs. Medical attendants like these supporter to put the individual needs of people and families above clinic schedules and bureaucratic formality. Unit 3 Discussion 1 – Nurse Advocates Nurse Advocates

The APN role of advocacy is implemented at organizations, state, and national level where they work closely with the political leaders as a link and push for change in the health sector, by advocating for patient safety and consideration of the vulnerable. They advocate for their rights in matters related to their pay and the distribution of health resources.

With little evidence base, there is limited credibility in advocacy, the statement is true as it works, reason being,

We contend here that advocacy efforts exist in scholarly clinical duties, to a limited extent since health care academics lend credibility, just as motivation and creative mind to these advantageous interests, however more so in light of the fact that it finishes up an expert commitment to those in our consideration by making an interpretation of evidence based research. Similarly as thorough science constructs the evidence base for change, thorough endeavors and their assessment are important to guarantee that this evidence will be converted into change.

Each journal should be a minimum of 250 words.

The purpose of this reflective journal is self-reflection regarding the role in the process of self-reflection as a PMHNP provider. Through reflective practice, the student will evaluate their own emotional health and recognize one’s own feelings as well as one’s ability to monitor and manage those feelings. The point of the exercise is to learn yourself, your triggers, the types of cases you end up getting overly involved with, and those you’d rather refer to someone else. The idea is to be able to personally reflect on your behaviors/thoughts/decisions and how those impact you in the role of PMHNP.

Chronic diseases–such as heart disease, cancer, and diabetes–are placing a growing burden on the U.S. health care system.  In response, some health care organizations are instituting chronic disease management (CDM) programs to reduce the incidence of preventable hospitalizations and adverse events by more effectively and comprehensively managing the health of patients with chronic conditions. Many of these organizations are implementing health information technology (health IT) to facilitate their chronic disease management programs.Chronic disease management health IT applications may enable the re-distribution of patient management tasks to non-physician personnel. Many health IT solutions for chronic disease management are intended primarily for physician use. However, these systems also can be designed to engage other key members of the health care team in decision-making, such as nurses, case managers and other key healthcare professionals. Use the following information to create a PowerPoint presentation with speaker notes. Include the following information in your final presentation:

  • Identify and explain the key goals of chronic disease management
  • What are the driving forces behind chronic disease management programs?
