A psychiatric emergency is a situation that if left untreated is likely to cause harm to oneself or others. These acute disturbances of behavior may manifest as suicidal ideation or attempts, violence, agitation, drug overdose, delirium, psychosis, mania, neuroleptic malignant syndrome, or serotonin syndrome. When psychiatric emergencies arise, they can present many challenges to the PMHNP. While many approaches to emergencies are similar when dealing with children and adolescents versus adults, significant differences also exist. This week, you examine psychiatric emergencies and explain the ethical and legal issues surrounding these events. You will also review evidence-based suicide and violence risk assessments that you may use to screen patients.

The diagnosis of psychiatric emergencies can include a wide range of problems—from serious drug reactions to abuse and suicidal ideation/behaviors. Regardless of care setting, the PMHNP must know how to address emergencies, coordinate care with other members of the health care team and law enforcement officials (when indicated), and effectively communicate with family members who are often overwhelmed in emergency situations. In their role, PMHNPs can ensure a smooth transition from emergency mental health care to follow-up care, and also bridge the physical–mental health divide in healthcare.

  1. Choose one of the following three cases from Bette-Jane Crigger’s Cases in Bioethics (full cases and commentaries posted in Moodle): Case 24 (“A Demand to Die”); Case 35 (“Nazi Data: Dissociation from Evil”); Case 48 (“Two Cardiac Arrests, One Medical Team”), or the Cassandra C. Case (“Connecticut Teen Forced to Undergo Chemo: ‘I Feel Like a Prisoner;'” and “Cassandra’s Chemo Fight: ‘This Is My Life and My Body'”). Choosing a case that interests you is a great idea. Remember, the case analysis is an academic assignment, but there is nothing wrong in finding a case that interests you and that you will enjoy writing about. Try to have some fun with the project!
  2. The first step is to summarize the case. What was the specific problem the doctor, patient, healthcare administrator, or medical researcher was facing? What actions did the agent featured in the case take to solve the problem or issue? Try to summarize the case for someone who has never read it before. Be sure to use your own words as you summarize the case! A good method is to read the case several times, and then try to write a “sketch” of the case from memory. At any event, the most important goal is to summarize the case using your own words. Your summary of the case should be approximately one half to three quarters of a page in length.   3. The second step is to summarize the argument provided in one of the commentaries following the case. What does the commentator argue about the case? What recommendation does the commentator make, and what reasons does the commentator provide to support the recommendation? You might want to “sketch” an outline of the argument before beginning the summary (identify the thesis and the reasons that support it). Your summary of the commentator’s position should be approximately one half to three quarters of a page in length.
  4. Quotations from the case should be kept to a minimum. For a case analysis, you should paraphrase material presented in the text as much as possible. Try to present the issues in your own words and formulations. Use citations to indicate the material you paraphrase and summarize. Please use the APA in-text citation format (including page numbers). If you cannot avoid providing a direct quotation, please indicate it by using quotation marks and page numbers (APA in-text citation format) so that the reader may reference it later. Also include a title page and works cited page in APA format.
  5. The final step is to analyze the actions taken by the agent(s) to solve the ethical dilemma in question. Provide your own evaluation of the actions taken by the agents in the case. Did the actions adequately solve the issue from both a medical and an ethical perspective? Is there anything you would have done differently to solve the ethical dilemma? Try to incorporate considerations based upon the ethical theories and concepts examined in class. Your analysis of actions taken, as well as your own recommendations, should be approximately one page in length. The overall length of the project should be approximately 2-2.5 pages in length. You are free to write beyond this length if you desire to do so, but please limit your paper to no more than three pages.

My ultimate goal is to become a highly experienced and knowledgeable nurse manager in the healthcare industry. I am especially intrigued by the idea of working with a team of medical professionals to diagnose and treat illness in coordination with healthcare providers. I am interested in women’s health, so I’m particularly excited about joining a practice specializing in this field. As a nurse manager, I plan to stay up-to-date on medical advancements while providing compassionate care to my patients and effectively managing my staff to meet the patient’s healthcare needs. My enthusiasm for the area will help me continue learning as I progress into my career to provide better care for those in need.

I have the necessary training and education to become a successful, contributing healthcare team member. I recently graduated from an accredited BSN program and started my Master’s degree in nursing (MSN). I am also proficient in computer applications, such as Microsoft Office and electronic medical record systems. My communication skills are strong, which is a necessary part of providing patient care. Mr. and Mrs. Arahan, an older couple in their seventies, have been living with their oldest daughter,

her husband of 15 years, and their two children, ages 12 and 14. They all live in a middle-income

neighborhood in a suburb of a metropolitan city. Mr. and Mrs. Arahan are both college educated and worked full-time while they were in their native country. In addition, Mr. Arahan, the only offspring of wealthy parents, inherited a substantial amount of money and real estate. Their daughter came to the United States as a registered nurse and met her husband, a drug company representative. The older couple moved to the United States when their daughter became a U.S. citizen and petitioned them as immigrants. The Arahans found life in the United States different from that in their home country, but their

adjustment was not as difficult because both were healthy and spoke English fluently. Most of their time was spent taking care of their two grandchildren and the house. As the grandchildren grew older, the older couple found that they had more spare time. The daughter and her husband advanced in their careers and spent a great deal more time at their jobs. There were few family dinners during the week. On weekends, the daughter, her husband, and their children socialized with their own friends. The couple began to feel isolated and longed for a more active life.

Mr. and Mrs. Arahan began to think that perhaps they should return to the home country, where

they still had relatives and friends. However, political and economic issues would have made it

difficult for them to live there. Besides, they had become accustomed to the way of life in the United States with all the modern conveniences and abundance of goods that were difficult to obtain in theircountry. However, they also became concerned that they might not be able to tolerate the winter months and that minor health problems might worsen as they aged. They wondered who would take care of them if they became very frail and where they would live, knowing that their daughter had only saved money for their grandchildren’s college education. They expressed their sentiments to their daughter, who became very concerned about how her parents were feeling.

This older couple had been attending church on a regular basis, but had never been active in other church-related activities. The church bulletin announced the establishment of parish nursing with two retired registered nurses as volunteers. The couple attended the first opening of the parish clinic.

Here, they met one of the registered nurses, who had a short discussion with them about the services offered. The registered nurse had spent a great deal of her working years as a community health nurse. She informed Mr. and Mrs. Arahan of her availability to help them resolve any health-related issues.
