The Affordable Care Act

The Affordable Care Act

The Patient Protection and Affordable Care Act (ACA, also known as ObamaCare) became law on May 23, 2010. It strives to reform the health insurance and health care industries in an effort to cut health care costs and provide Americans with affordable health insurance. The ACA also strives to expand Medicare and Medicaid to assist those Americans who cannot currently afford health insurance. As a result of the reforms, there may be a significant influx of patients into the health care system. In addition, there will be a greater focus on integrating health care services, particularly for individuals with physical and mental health conditions. It is expected that under the ACA, social workers will take a more active role in the coordination of care for existing and new members of the health care system. Social workers will also provide additional direct, frontline services to patients. The Affordable Care Act

Post a description of the findings of your research on the debate over ACA. Explain some of the misconceptions or misunderstandings the general population might have about the ACA. Explain how the ACA may or may not increase affordability, health insurance coverage, and access to health care services for the population you selected. Explain the potential or documented influence of the ACA on medical social work practice. Describe the medical social worker’s role in clarifying the ACA to patients and families. Assignment: Antipsychotic Medications and the Treatment of Psychotic Disorders

Assignment: Antipsychotic Medications and the Treatment of Psychotic Disorders

Imagine that you have been seeing a client for a year. Initially, treatment was tailored to address a diagnosis of major depressive disorder. Over the course of your counseling sessions, your client has begun to develop psychotic symptoms. She becomes mildly psychotic during a session and explains that the voices are telling her that she is wasting her time and she will never be well. Your client is surprised to have heard these voices, but is cognizant of the fact that they are coming from within her, rather than some external, supranatural source. She understands that they are psychotic symptoms. Assignment: Antipsychotic Medications and the Treatment of Psychotic Disorders

Often, counselors encounter ethical and clinical dilemmas in deciding the best course of treatment for their clients. Considering the possibility that a client might wish to avoid going on antipsychotic medication, could you support this choice? Does the client’s awareness and understanding of the voices mean no further action is required on the part of the counselor? Since there appears to be no immediate danger to herself or others, could the counselor disregard the symptoms?

The Application Assignment contains a creative component, in which you integrate DSM-5 sourced and other scholarly material to create a case presentation. As hallucinations and delusions can vary from those that were seemingly borne of logic to the truly bizarre, this is an opportunity to fully engage your most imaginative thoughts.

For this Assignment, review the Learning Resources. Reflect on the development of drugs to treat psychosis over time. Consider the side effects of antipsychotics and their interactions with neurotransmitter function. Explore the ethical concerns and challenges of treating psychotic disorders. Develop a presentation of a client who would benefit from antipsychotic medications.

By Day 7
In a 3- to 5-page, APA-formatted paper, include the following based on the concepts in the treatment of psychotic disorders:

An overview of the major developments in the treatment of psychosis including major drug classes
An explanation of three severe side effects of antipsychotic medications and potential alternative treatments Assignment: Antipsychotic Medications and the Treatment of Psychotic Disorders
An explanation of the neurotransmitters involved with and affected by these medications
An explanation of the ethical concerns related to the treatment of psychotic disorders and how you would address them
An explanation of the challenges mental health professionals face in treating psychotic disorders and how you would address them
Then, based on what you know about the treatment of psychotic disorders provide an original and hypothetical case study illustrating a client that would benefit from antipsychotic medications and a brief explanation of an appropriate treatment plan.

Malingering and Addiction in the Treatment of Sleep Disorders

Malingering and Addiction in the Treatment of Sleep Disorders

According to the National Sleep Foundation (2013), about 30–40% of the general population reports some level of insomnia during their lives, and 10–15% experience significant, chronic insomnia. For these individuals, medications to help induce and sustain sleep may be helpful. On the other hand, sleep aids pose potential concerns, namely abuse. Some people exceed recommended doses, and some continue taking medications even after symptoms are no longer present. Others obtain medications under false pretenses, which is one form of malingering. Malingering occurs when clients make up or exaggerate symptoms for some personal gain. Although mental health professionals may not be directly implicated in the client’s deceit, their unique position to receive more accurate and honest information than other medical professionals presents ethical concerns. What is the mental health professional’s role in these instances? In which instances would it be appropriate to break confidentiality due to a concern of malingering? How could the malingering potentiality be planned for and avoided? Malingering and Addiction in the Treatment of Sleep Disorders

Addressing Ethical Implications in the Treatment of ADHD

Addressing Ethical Implications in the Treatment of ADHD

Assignment: Addressing Ethical Implications in the Treatment of ADHD
Generally, mental health professionals attempt to empower clients to care for their own problems by supporting client agency. However, in some cases clients may be stifled in their efforts to self-advocate. In other instances, clients may make dangerous, potentially lethal decisions out of misinformation or attempts to get high. Mental health professionals should be prepared to identify warning signs that might indicate a need to protect a client beyond asking that she or he take the necessary steps. HIPAA and Electronic Medical Records

HIPAA and Electronic Medical Records

Discussion 2: HIPAA and Electronic Medical Records
An electronic health or medical record (EMR) is a digital version of a paper medical chart that contains a patient’s medical history. EMRs offer clinicians and patients increased access to information and assist in the coordination of care. In hospitals, EMRs are particularly useful because each member of the health care team is able to monitor a patient’s progress across disciplines. A social worker, for example, is able to read, in real time, the note of every physician, nurse, or therapist who has seen a patient. To maintain HIPAA compliance, institutions have strict policies that restrict access to EMRs. These policies require computers and networks to be secure, ensuring that only authorized users can access protected health information. HIPAA and Electronic Medical Records

Chronic Fatigue

Chronic Fatigue

A 52-year-old male patient who is a house painter presents to the office reporting chronic fatigue and “mild” chest pain. When he is painting, chest pain is relieved after taking a break. He reports that the pain usually lasts 5 minutes or less and occasionally spreads to his left arm before subsiding. The patient was last seen 3 years ago by you, and you recommended diet changes to manage mild hyperlipidemia, but the patient has gained 30 pounds since that time. The patient’s medical history includes anxiety, vasectomy, cholecystectomy, and mild hyperlipidemia. The patient does not smoke or use other tobacco or nicotine products. The patient cares for his wife, who has multiple sclerosis and requires 24-hour care. His daughter and grandson also live with the patient. His daughter assists with the care of his wife, and his job is the major source of income for the family. The initial vital signs are: blood pressure 158/78, heart rate 87, respiratory rate 20, and body mass index 32. As part of the diagnostic work-up, an ECG, lipid levels, cardiac enzymes, and C-reactive protein (CRP) are ordered. The patient reports that he does not have time to “be sick” and says that he needs to take care of everything during this visit so he can return to work and care for his wife. Discuss the following: Chronic Fatigue
