Research Critique Guidelines – Part II

Obesity is a global issue that has affected individuals of every age. Bariatric Surgery is one of the advanced evidence-based intervention to effectively manage obesity. Psychological nursing interventions provided pre and post-surgery are found to be very effective towards positive outcomes of bariatric surgery.

PICOT Question

Whether subjects undergoing bariatric surgery (P), when subjected to interventions like pre and postoperative information and guidance (I), in comparison to subjects who did not participate in such activities (C), is associated with greater weight reduction (O) over a timeframe of three months (T).Research Critiques and PICOT Statement Final Draft Assignment Paper

Quantitative Studies

Background

The first study selected for this critique is the Examination of the effectiveness of a brief, adapted dialectical behavior therapy skills training group for bariatric surgical candidates. This study suggests that bariatric surgery remains one of the most valuable tools to combat obesity with promising results of reduced BMI. However, the results of bariatric surgery are considered to be temporary for a significant number of candidates as many relapses to their pre-operative weight due to a myriad of reasons. One aspect of this weight gain can be due to some pathological behavior patterns associated with excessive food consumption and therefore leading to weight gain (Delparte, et al., 2019). To tackle this, many researchers have advocated for the establishment of information, guidance, and providing support to patients for lifelong attainment of desired results of bariatric surgery. The rationale of this article was to determine the effectiveness of employing brief and adapted dialect-based therapy in terms of prolonged and persistent reduction of weight among candidates of bariatric surgery. The study hypothesized that people attending and adhering to DBT-ST in their pre-surgical period will result in better outcomes.Research Critiques and PICOT Statement Final Draft Assignment Paper

The second study which was added for quantitative critique is The Exchange of Social Support on Online Bariatric Surgery Discussion Forums: A Mixed-Methods Content Analysis. The study advocates that patient undergoing bariatric surgery can effectively manage psychological stressors through different support groups and psycho-educational interventions (Atwood, et al.,2018). The support groups provide information, guidance, and psychological support to patients to achieve favorable outcomes. This study used various online forums as support group mediums to assess the success of bariatric surgery with prolonged weight management. The rationale of this article was to evaluate the different kinds of support groups regarding bariatric surgery are found online and what is the success of these discussion groups based on the frequency of exchanging them.Research Critiques and PICOT Statement Final Draft Assignment Paper

How do these two articles support the nurse practice issue you chose?

The first study addresses the PICOT question which is focused on the pre and postoperative psycho-education as a mode of informational and educational intervention of the participants undergoing bariatric surgery (Delparte, et al., 2019). A quantitative evaluation of the study is related to the improved outcomes in the case of psychological disorders compensated with bouts of eating and relapses to weight gain.

The second study addresses the PICOT question and the nursing problem as it discusses the effectiveness of online support group among candidates of bariatric surgery and to assess its success by the frequency of exchanges made among the patients seeking support (Atwood, et al.,2018).Research Critiques and PICOT Statement Final Draft Assignment Paper

Method of Study:

1. State the methods of the two articles you are comparing and describe how they are different.

In the first article, Delparte, et al., (2019) recruited 95 Participants from a clinic of preoperative care . The participants were exposed to online intervention after dividing them into two groups based on intervention. The first group was of a dialect-based therapy session, whereas, the other group participants were treated as per usual guidelines (Delparte, et al., 2019). The two groups were assessed post-surgically via eating disorder examination questionnaire 6.0, binge eating scale, emotional eating scale, and clinical impairment assessment questionnaire 3.0. Statistical analysis was carried out via SPSS version 23. The second study by the authors used content analysis to carry out communication evaluation . The study gathered the posted messages in the online support group. The next step of data collection was to organize the data within an organized categorical framework (Atwood, et al.,2018). The third phase of data collection was based on providing explanations for each category to assist in result formation.Research Critiques and PICOT Statement Final Draft Assignment Paper

2. Consider the methods you identified in your chosen articles and state one benefit and one limitation of each method.

The first study used a randomized control trial for the assessment and evaluation of the intervention. The study design is considered to be a higher hierarchy of evidence and provides grounds for establishing clinical intervention. However, the first selected study only recruited 95 participants. The sample size of the recruited population is not large enough to generalize the results (Delparte, et al., 2019). The second study used content analysis. This mode of research enabled the researcher to interpret and record communicable data into categories and patterns and is used to derive purposes behind communicative content. However, unlike randomized control trial, results obtained from the content analysis are not reliable for clinical intervention. The data and results of content analysis are highly prone to errors and form no theoretical base (Atwood, et al.,2018).Research Critiques and PICOT Statement Final Draft Assignment Paper

Results of Study

1. Summarize the key findings of each study in one or two comprehensive paragraphs .

The study showed the frequency and severity of eating disorders after individual undertake DBT presented with higher levels of eating disorders of 30% in comparison to 28% of the control group, however, there was a dramatic reduction in binge eating disorder of 19% in comparison to 27% leading to better outcomes of bariatric surgery (Delparte, et al., 2019). The second study showed that out of a total of 1412 messages posted in online support groups, 84.2% were seeking and providing social support, indicating the effectiveness in providing support to patients. Most of the posts in the pre-surgical period were related to emotional support 49% and information 38.9%. the data collected and analyzed showed that the frequency of exchange of online support groups has provided a forum for emotional support, information, sympathy, and willingness to help that has led to better outcomes of bariatric surgery (Atwood, et al.,2018).Research Critiques and PICOT Statement Final Draft Assignment Paper

2. What are the implications of the two studies you chose in nursing practice?

Both the studies adopted different modes of educational and support interventions for patients undergoing bariatric surgery. The first study showed variable results in the improvement of poor determinants of weight outcomes after bariatric surgery. The researcher suggests that the duration of 8 weeks is optimum for desired outcomes therefore an intervention based on 8 weeks’ education would be suitable before and after bariatric surgery (Delparte, et al., 2019). The second study provided results based on content analysis which remains an unreliable source of testing efficacy but the presence of online support groups can provide sympathy, information, and emotional support (Atwood, et al.,2018). This mode of intervention will not be the basis of any treatment but there remains no harm in joining online mediums as a support group .Research Critiques and PICOT Statement Final Draft Assignment Paper

Outcomes Comparison

1. What are the anticipated outcomes for your PICOT question?

The anticipated outcome for PICOT comparison was better and promising outcomes associated with bariatric surgery patients when exposed to interventions like guidance and informational assistance through psychoeducation and support groups. The question aimed to contrast the interventional group against a control group and study a comparative efficacy.

2. How do the outcomes of your chosen articles compare to your anticipated outcomes?

The first study in comparison to the PICOT question provided variable outcomes. Unlike the PICOT question, the first study found no significant relationship among the two cases and control groups (Delparte, et al., 2019). However, the study suggested a reduction of binge eating disorders in post-surgical patients. The second study used content analysis and suggested that online support groups are highly common among patients seeking emotional support through before and after bariatric surgery (Atwood, et al.,2018) however, due to the nature of the study, the efficacy of this intervention stays undetermine d.Research Critiques and PICOT Statement Final Draft Assignment Paper

Summary of Clinical Issue

Obesity is now presenting itself as an epidemic in both developed and underdeveloped nations. The condition is rising in developing countries despite having a long dates history of malnutrition and undernutrition in many of these states. How obesity imposes health threats is a wide discussion on its own, but the condition is associated with significantly poor health with progressing time, the associated financial burden cannot be ignored, and most important of all, the condition predisposes individual to greater risks of death.Research Critiques and PICOT Statement Final Draft Assignment Paper. The etiology of the disease is vast and is most frequently associated with abnormal eating patterns and a sedentary lifestyle. Other conditions that can lead to obesity are hyperglycemia, hypertension, and other heart diseases. However, the disease prognosis is very unfortunate as many individuals suffering from obesity proceed to life-threatening health conditions. To reduce such consequences, advances in the medical field led to procedures like bariatric surgery to effectively manage obesity. Unlike traditional methods of obesity management by lifestyle training, bariatric surgery results have been more fast and promising. The phenomenon of the surgical procedure relies on surgical alterations of the digestive tracks leading to altered bowel habits and reduced surface area for nutrient uptake. Like other surgical procedures, bariatric surgery requires considerable medical evaluation of an individual to minimize the risk of adverse effects of the procedure. Although bariatric surgery enables an individual to attain desired goals more rapidly, the patient must adhere to healthy lifestyle modifications to sustain these changes for longer.

Research Critique Guidelines – Part I

Qualitative Studies

Background of Study

Study 1:

Article 1, I chose for this critique is the only chance of normal weight life: a qualitative study of online forum discussions about bariatric surgery . This article elaborates that bariatric surgery is the only effective treatment to lose weight for severe obesity. Roux-en-Y gastric bypass is one of the most common methods used in bariatric surgery. Even if lifelong lifestyle changes are required, patients still have many unrealistic expectations from the bariatric surgery and expect a miracle cure. One of the major problems is that most patients get information about their bariatric surgery through online platforms but do not seek education from nurses and contact those who have had the surgery to get information(Taube et al., 2016). This study aims to demonstrate shared feelings, thoughts, and values of patients attained who wants to undergo bariatric surgery through a web-based forum. The research question addressed in this study is: What are the thoughts and experiences of bariatric surgery subjects collected via online platforms? The outcome is the variable response of bariatric surgery seen in different people, suggesting mixed lifestyle improvement.Research Critiques and PICOT Statement Final Draft Assignment Paper

Study 2:

The second study I choose is “Bridging the gap: Patient experiences following the transfer of care from a pediatric obesity management program to an adult bariatric surgery program,” in which themes are identified to check the experience of a base of young adults whose age ranges from 18 years 24 years. These are the adults who transfer their care from a pediatric management program of obesity to an adult bariatric surgery program. This study aims to understand the developmental needs of obese adults and how it impacts their experiences of the transition process(Willmer & Salzmann-Erikson, 2018). The research question of this article discusses the efficiency of support group-based programs in the psychological condition of obese subjects undergoing bariatric surgery and how it can be associated with an improved lifestyle. The outcome of the study is the group settings that lead to healthy lifestyle promotion among subjects.Research Critiques and PICOT Statement Final Draft Assignment Paper

How do these two articles support the nurse practice issue you chose?

1. The nurse practice issue I choose is the pre and post stages of bariatric surgery . Article 1 is in accordance with the selected nursing issue as it addresses both the pre and postoperative stage of bariatric surgery and online discussion.

2. My second article is about the nurse practice issue about checking the efficiency of support group. This evidence from the study reviewed the efficiency of support groups among the patient of bariatric surgery and its relationship with reduced weight and a better lifestyle, therefore, meets the research goal of the selected nursing problem.

Method of Study:

1. In the first article, a cross-sectional observational method was used for qualitative analysis of the contents. In this study, online discussion forums were used to collect the information about bariatric surgery experiences with 498 posts. These posts were saved in a document and were available to the participants for reading. One of the benefits of a cross-sectional observational study is that it is easy to conduct and can measure the prevalence of all the required factors. The limitation of this method is that no evidence of a temporal relationship exists between outcomes and exposures as both parameters are assessed simultaneously. The other limitation was that the patient’s age, gender, and occupation were not known through google forms.

2. In the second article , seven young adults were selected who transformed their management care program of obesity from pediatric to adult healthcare. Semi-structured interviews were conducted with these bariatric patients. The ground theory approach was used, and analysis was done by constant comparative analysis. The benefit of a semi-structured interview is that questions can be prepared beforehand, and the participants are allowed for open-ended responses. The limitation is that it is very time-consuming to conduct open-ended interviews.Research Critiques and PICOT Statement Final Draft Assignment Paper

Results of Study

1. Study 1 showed that the patients who underwent surgery had very unrealistic and high expectations and believe that surgery will change their lives. Key findings are that Many participants expected a new and healthier life after surgery without making lifestyle modifications. Nurses can educate the patient before the conduction of surgery about the real scenarios so that they do not expect unrealistic outcomes. The nurse also provides information to the patient about a healthy lifestyle and changes they need to adapt to achieve it.

2. The second study showed that a support group program could empower subjects in making healthy lifestyle choices resulting in reduced weight and prolonged health. It also demonstrates that the adults regarding the transition process experience negative and positive aspects. Key findings are that a support group program can empower subjects in making healthy lifestyle choices resulting in reduced weight and prolonged health. Nurses can conduct programs in providing health education for a healthy lifestyle. Nurses also help to eliminate the negative thoughts of patients regarding the transition process.Research Critiques and PICOT Statement Final Draft Assignment Paper

Ethical Considerations

1. While carrying out qualitative research, some important ethical concerns should be taken into consideration. First of all, informed consent should be provided to the participants, and the person who is conducting the research should follow these rules. Secondly, the privacy and confidentiality of the participant should be respected(Arifin, 2018).

2. In the first study, the ethical considerations are followed properly as no personal information such as occupation, personal identities were uncovered. Moreover, the online Google forum was designed in such a way that participants felt very comfortable expressing their feelings and thoughts. In the second study, participants were ensured that their personal information would be kept safe, and permission was taken from them before recording their interview.
