The CCCR model and OPT model are both methods of utilizing a care coordination clinical reasoning framework. The CCCR model and OPT models help guide me in my daily working experience allowing me to think more critically. I have found that individualizing  patient care specific to each patient shows a more effective way in effectively meeting essential care needs. These models both provide a means to think more logically, have good reasoning skills to make better decisions in the clinical setting eliminating the risk of errors. Advanced practice nurses such as nurse practitioners in these roles require additional clinical reasoning skill sets and ways of thinking to manage cases on a day to day basis if they are to practice to the full extent of their education (Kuiper, 2015).

      The CCCR model and OPT model have both played a role in the care I have provided to patients and their family in the clinical setting. Patient satisfaction is my ultimate goal and in respecting the care needs of the patient. The first step is focusing on the patient’s chief complaint, gathering more data by assessing the patient. Taking all the data provided by the patient and or family determining the diagnosis related to the  symptoms. This will help in developing the intervention to implement in providing treatment to the patient more effectively.
