Assignment: Off-Label Drug Use in Pediatrics

Assignment: Off-Label Drug Use in Pediatrics

The unapproved use of approved drugs, also called off-label use, with children is quite common. This is because pediatric dosage guidelines are typically unavailable, since very few drugs have been specifically researched and tested with children.

When treating children, prescribers often adjust dosages approved for adults to accommodate a child’s weight. However, children are not just “smaller” adults. Adults and children process and respond to drugs differently in their absorption, distribution, metabolism, and excretion.

Children even respond differently during stages from infancy to adolescence. This poses potential safety concerns when prescribing drugs to pediatric patients. As an advanced practice nurse, you have to be aware of safety implications of the off-label use of drugs with this patient group.

To Prepare

Review the interactive media piece in this week’s Resources and reflect on the types of drugs used to treat pediatric patients with mood disorders. Assignment: Off-Label Drug Use in Pediatrics

Reflect on situations in which children should be prescribed drugs for off-label use.

Think about strategies to make the off-label use and dosage of drugs safer for children from infancy to adolescence. Consider specific off-label drugs that you think require extra care and attention when used in pediatrics.

By Day 5 of Week 11

Write a 1-page narrative in APA format that addresses the following:

Explain the circumstances under which children should be prescribed drugs for off-label use. Be specific and provide examples.

Describe strategies to make the off-label use and dosage of drugs safer for children from infancy to adolescence. Include descriptions and names of off-label drugs that require extra care and attention when used in pediatrics.

Reminder: The College of Nursing requires that all papers submitted include a title page, introduction, summary, and references. The Sample Paper provided at the Walden Writing Center offers an example of those required elements (available at http://writingcenter.waldenu.edu/57.htm). All papers submitted must use this formatting. Assignment: Off-Label Drug Use in Pediatrics

1. Mobilize resources and stakeholders to take care of the selected health problem in your community, determining mission and vision of the resulted coalition, defining partners, their roles and meeting plans.

2. Asses the problem, including a realistic long-term goal, how you may collect data to determine your needs and priorities logically organized

3. Plan objectives and steps to achieve them. Consider opportunities for interventions with broad reach and impact. How may you measure your progress? What is expected to change, by how much, and by when? Choose objectives that are challenging yet realistic.

4. Implement. Create a detailed work plan that includes concrete action steps assigned to specific people with clear deadlines and/or timelines. Share responsibilities across coalition members but consider having a single point of contact to manage the process to ensure that things get done. Theoretical and conceptual frameworks

Theoretical and conceptual frameworks are terms in which are used interchangeably in research. Green (2014) mentions that some research methods do not use a theoretical or conceptual framework in their designs. All of my articles state no types of framework used nor does it use any specific theories to back up or test. The research articles pertaining to my research question have two different approaches to conclude the effectiveness of the teach-back method to reduce readmissions within 30 days of discharge.

One approach has a common theme of researchers comparing a treatment group to a control group. The treatment group was taught specific information using the teach-back method along with disease specific education from trained professional nurses with a background in cardiology compared to standard discharge teaching. These were mainly qualitative studies, which involved questionnaires, interviews and observing readmission rates.

Another approach that researchers took had also a common theme, which was to compare and contrast multiple studies on the effectiveness of the teach-back method in relation to readmission rates. They conducted a systematic review analysis in order to show how effective the teach-back method is from different aspects of research. In one study, the researchers analyzed 15 different research studies where as the researchers in another study analyzed 4 different studies. The researchers in these studies were able to extract the common problem in all of the research articles they analyzed to support the effectiveness of the teach-back method. After reading through both articles and going through the study I am researching, I found that my research proposal holds both theoretical and conceptual theories. The articles stated that conceptual theories pull from different theories to determine the direction of the research, whereas theoretical research is centered around a single theory (Greene, 2014). The theories that guide research are an important aspect of research. The article stated that these theories can be used in different ways, and if done correctly can predict the results of the research (Connelly , 2014). The concept map that I designed shows the different concepts and that lead to nurse burnout, and how research supports those concepts. Attached you will find my concept map.
