Patient states his primary care physican started him on a new blood pressure medication last week. Upon checking the nurse discovered that the patient was started on hydrchlorothiazide, a diuretic.

A: Upon assessment, the patient is noted to have 1 + pitting edema in bilateral lower extremeties, BP is 156/100, T 99.9, RR 18, O2 97% RA. Patient is complaining of unilateral lower left flank pain. Patient is complaining of nausea and has stated he has vomited three times since this morning. Patient also complains of a severe headache.

R: I recommend the patients kidney function be checked immediately, and an ultrasound of the kidney be done to check for inflammation. I suspect the patients kidneys might be rejecting the new medication. A IV line should be started and fluids should be started pending lab results. Healthcare Financing Issues from the Viewpoint of the Nurse Leader: An Economic Perspective

Like in other countries, the United States also experiences issues to do with healthcare financing. This is in contrast with what many may believe since the United States is considered superior in terms of its gross domestic product (GDP). When the ACA was signed into law it expressly sought to shift the focus of universal healthcare from quantity to quality. This was to be achieved through a set of measures and regulations in the law. Several years down the line, the return on investment of this policy decision is evident as more Americans are benefiting from the coverage by getting quality care (Cai et al., 2020).

Retraining of healthcare employees is an expense that many healthcare organizations find hard to avoid. However, this is a necessity now with the emergence of novel healthcare technologies that require nurses and other healthcare staff to upskill. In all organizations, financial resources are scarce. Lastly, healthcare organizations just like hotels make a choice about location based on the purchasing power of the expected clientele in its locality. This is a pricing strategy that is tied to the calibre of services the institution expects to offer. The strategy also factors in the matter of demand and supply through the carrying out market research to establish demand (Dawson, 2019).

As a solution to the cost of upskilling staff, for instance, organizations should incorporate a training fund into its strategic plan as a policy directive. Nurse Leader as Economist. This would remove the fiscal difficulty of always trying to look for areas of surplus to get funding.

Three strategies are especially helpful. First, the organization needs to introduce new technologies gradually to allow for adaptation and training. Second, upskilling should be made a policy initiative that features in the organization’s goals. Third and last, upskilling should be properly budgeted for like any other expense if it is to succeed (Fralic, 2013; Rimmer, 2018; Teague, 2019). Under no circumstances should upskilling be considered as an afterthought. These strategies impact my organization positively in that they are geared towards giving the organization a competitive advantage. On the negative side, however, budgeting means that this strategy will consume resources that the organization must set aside.
