Gastrointestinal Function

R.H. is a 74-year-old black woman who presents to the family practice clinic for a scheduled appointment. She has complained of feeling bloated and constipated for the past month, sometimes going an entire week with only one bowel movement. Until this episode, she has been very regular all of her life, having a bowel movement every day or every other day. She reports straining most of the time, and it often takes her 10 minutes at a minimum to initiate a bowel movement. Stools have been extremely hard. She denies pain during straining. A recent colonoscopy was negative for tumors or other lesions. She has not yet taken any medications to provide relief for her constipation. Furthermore, she reports frequent heartburn (3–4 times each week), most often occurring soon after retiring to bed. She uses three pillows to keep herself in a more upright position during sleep. On a friend’s advice, she purchased a package of over-the-counter aluminum hydroxide tablets to help relieve the heartburn. She has had some improvement since she began taking the medicine. She reports using naproxen as needed for arthritic pain in her hands and knees. She states that her hands and knees are extremely stiff when she rises in the morning. Because her arthritis has been getting worse, she has stopped taking her daily walks and now gets very little exercise. Gastrointestinal Function

  2. Based on the clinical manifestations in the R.H. case study, name and explain the signs and symptoms presented that are compatible with the constipation diagnosis. Complement your list with signs and symptoms not present in the case study.
  3. Sometimes, as an associate diagnosis and a complication, patients with constipation could have anemia. Would you consider that possibility based on the information provided in the case study?

C.B. is a significantly overweight, 48-year-old woman from the Winnebago Indian tribe who had high blood sugar and cholesterol levels three years ago but did not follow up with a clinical diagnostic work-up. She had participated in the state’s annual health screening program and noticed that her fasting blood sugar was 141 and her cholesterol was 225. However, she felt “perfectly fine at the time” and could not afford any more medications. Except for a number of “female infections,” she has felt fine until recently. Today, she presents to the Indian Hospital general practitioner, complaining that her left foot has been weak and numb for nearly three weeks and that the foot is difficult to flex. She denies any other weakness or numbness at this time. However, she reports that she has been very thirsty lately and gets up more often at night to urinate. She has attributed these symptoms to the extremely warm weather and drinking more water to keep her hydrated. She has gained a total of 65 pounds since her last pregnancy 14 years ago, 15 pounds in the last 6 months alone. Endocrine Function

Health Care Delivery

Health Care Delivery

Health care in the United States is changing. A paradigm shift is occurring as more and more health care organizations are reorganizing existing business models from inpatient to outpatient services. That being said, you have been tasked with developing a proposal for the development of a health care facility that is designed to meet rapidly changing health care needs. Within your proposal, you are required to research and discuss the following:

  • The type of facility you are recommending and the rationale behind your recommendation. For example, if you chose an ambulatory care facility, explain the reasons why this type of facility would be recommended over another type.
  • The type of health care delivery and services that are provided at the facility.
  • The type of staff that will work in the facility. Specify the position categories and/or titles for these positions, and include the credentials or licensures required by these positions in the state in which you live and where your proposed facility would be located.

Step 2: For this discussion, select one of the health care delivery settings discussed in the assigned reading (e.g. home health/hospice, long term care, the VA system, retail/nursing clinics, community health centers) and consider ways in which the setting could incorporate aspects of the PCMH model.  Specifically, address access, quality, and cost. Finally, discuss the nurse’s role in advocating for these changes. If possible, share your own experiences and example of this model (or related principles).

Step 3: Read other students’ posts and respond to at least two of them by Friday at 11:59 pm MT.

Cite any sources in 7th. ed APA format.

Select Reply to join the discussion.  See rubric for grading details. You can find this by clicking the three dots to the top right of this thread.

Response Posts: In your responses to your classmates, contribute to the discussion with your own original professional opinions or interpretation of the course materials. Peer Responses do not require research for this course, so you do not need to include a reference and citation. You have already completed steps 1–4. Make sure you revise this initial submission according to your instructor’s comments.

1. Describe the health problem. Using data and statistics supports your claim that the issue you selected is a problem. What specifically will you address in your proposed health promotion program?

Be sure your proposed outcome is realistic and measurable.

2. Describe the vulnerable population and setting. What are the risk factors that make this a vulnerable population? Use evidence to support the risk factors you have identified.

3. Provide a review of literature from scholarly journals on evidence-based interventions that address the problem. After completing a library search related to effective interventions for your chosen health promotion activity, you will write a review that evaluates the strengths and weaknesses of all the sources you have found. You might consult research texts for information on how to write a review of the literature found in your search.

4. Select an appropriate health promotion/disease prevention theoretical framework or conceptual model that would best serve as the framework guiding the proposal. Provide rationale for your selection, which includes discussion of the concepts of the selected model For this assignment, add criteria 5-8 as detailed below:

5. Propose a health promotion program using an evidence-based intervention found in your literature search to address the problem in the selected population/setting. Include a thorough discussion of the specifics of this intervention, which include the resources necessary, those involved, and feasibility for a nurse in an advanced role. Be certain to include a timeline. (2 to 4 paragraphs). (You may use bullets if appropriate.).

6. Thoroughly describe the intended outcomes. Describe the outcomes in detail in accordance with the SMART goal approach. (1 paragraph).

What is the pathophysiology of Helicobacter pylori?

The liver is a complex organ with many contributions to homeostasis that are often not appreciated until liver function declines. The liver has the capacity to rebound and regenerate after a variety of acute chemically or virally induced insults, but it is vulnerable to chronic chemical or infectious damage.

What blood tests are appropriate for a patient with a suspected acute liver injury?

Explain the rationale for ordering these tests, and patterns of results that you might see in a patient with acute HAV infection.
