For this project, you will take on the role of a nurse practitioner treating a patient with a chronic condition (Coronary Artery Disease) . Your patient is not completely satisfied with the treatments you’ve tried and asks about a specific alternative therapy. Unfamiliar with these sorts of therapies, you decide to do some research before making recommendation. In this case, you get to decide what condition your patient has and which intervention he or she is interested in trying. Your objective is to determine whether or not you would recommend the regular treatment either alone or in combination with alternative treatment. The paper should not be more than 4 pages excluding the title page and references. APA format is required with a professional paper.

Discussion: Urinary Tract Infections

Urinary tract infections (UTIs) are caused by bacteria—most often, Escherichia coli. However, certain viruses, fungi, and parasites can also lead to infection. The infection can affect the lower and upper urinary tract including the urethra, prostate (in males), bladder, ureter, and kidney. Due to the progression of the disease and human anatomy, symptoms present differently among the sexes, as well as among different age groups. It is important to understand how these factors and others impact the pathophysiology of UTIs. Advanced practice nurses must have this foundation in order to properly diagnose patients.

To prepare:

  • Review Chapter 30 in the Huether and McCance text and Chapter 32 of the Arcangelo text. Identify the pathophysiology of lower urinary tract and upper urinary tract infections. Consider the similarities and differences between the two types of infections.
  • Consider one type of drug that would be used to treat symptoms associated with urinary tract infections.
  • Consider gender and one of the following patient factors: genetics, ethnicity, age, or behavior. Consider how gender and the factor you selected might impact the pathophysiology of the urinary tract infections. Then, reflect on how these factors might impact the effects of prescribed drugs, as well as any measures you might take to help reduce any negative side effects.
By Day 3

Post a description of the pathophysiology of lower urinary tract and upper urinary tract infections, including their similarities and differences. Then, describe one type of drug that would be prescribed to treat symptoms associated with urinary tract infections. Finally, explain how gender and another patient factor might impact the pathophysiology of the urinary tract infections and the effects of prescribed drugs. Explain measures you might take to help reduce any negative side effects.

As it happens, there are real-life applications for interaction that we often come across. What is interesting to note is that it’s always beneficial to distinguish between the quantitative interactions with their qualitative counterparts.

So, could you further describe what significance “qualitative” interactions would have in statistics? What about “quantitative” ones? How can you distinguish between the two types of interactions between variables R and S?

Discussion: Diagnosis and Management of Genitourinary Disorders

Many genitourinary (GU) disorders such as kidney disease begin developing during childhood and adolescence (Johns Hopkins Children’s Center, 2010). This early onset of disease makes it essential for you, as the advanced practice nurse caring for pediatric patients, to identify potential signs and symptoms. Although some pediatric GU disorders require long-term treatment and management, other disorders such as bedwetting or urinary tract infections are more common and frequently require only minor interventions. In your role with pediatric patients, you must evaluate symptoms and determine whether to treat patients or refer them for specialized care. For this Discussion, consider potential diagnoses, treatment, and/or referral options for the patients in the following three case studies.

Case Study 1

You see a 3-year-old with a 2-day history of complaints of dysuria with frequent episodes of enuresis despite potty training about 7 months ago. She is afebrile and denies vomiting. Physical examination is normal. Dipstick voided urine analysis reveals: specific gravity 1.015, Protein 1+ non-hemolyzed blood, 1+ nitrites, 1+ leukocytes, and glucose-negative.

Case Study 2

Mark is a 15-year-old with complaint of acute left scrotal pain with nausea. The pain began approximately 6 hours ago as a dull ache and has gradually worsened to where he can no longer stand without doubling over. He is afebrile and in marked pain. Physical exam is negative except for elevation of the left testicle, diffuse scrotal edema, and the presence of a blue dot sign.

Case Study 3

Maya is a 5-year-old who presents for a well-child visit. She is a healthy child with no complaints. Physical examination is normal. Routine urinalysis indicates 2+ proteinuria; specific gravity 1.020; negative for glucose, blood, leukocytes, and nitrites. Her blood pressure is normal, and she is at the 60th percentile for height and weight.

To prepare:

  • Review “Genitourinary Disorders” in the Burns et al. text.
  • Review and select one of the three provided case studies. Analyze the patient information.
  • Consider a differential diagnosis for the patient in the case study you selected. Think about the most likely diagnosis for the patient.
  • Think about a treatment and management plan for the patient. Be sure to consider appropriate dosages for any recommended pharmacologic and/or non-pharmacologic treatments.
  • Consider strategies for educating patients and families on the treatment and management of the genitourinary disorder.

By Day 3

Post an explanation of the differential diagnosis for the patient in the case study you selected. Explain which is the most likely diagnosis for the patient and why. Include an explanation of unique characteristics of the disorder you identified as the primary diagnosis. Then, explain a treatment and management plan for the patient, including appropriate dosages for any recommended treatments. Finally, explain strategies for educating patients and families on the treatment and management of the genitourinary disorder.
