Abstract:

During the past quarter century, advances in imaging technology have helped transform scientific fields. As important as the data made available by these new technologies have been, equally important have been the guides provided by existing theories and the converging evidence provided by other methodologies. The field of psychological science is no exception. Neuroimaging is an important new tool in the toolbox of psychological science, but it is most productive when its use is guided by psychological theories and complemented by converging methodologies including (but not limited to) lesion, electrophysiological, computational, and behavioral studies. Based on this approach, the articles in this special issue specify neural mechanisms involved in perception, attention, categorization, memory, recognition, attitudes, social cognition, language, motor coordination, emotional regulation, executive function, decision making, and depression. Understanding the contributions of individual and functionally connected brain regions to these processes benefits psychological theory by suggesting functional representations and processes, constraining these processes, producing means of falsifying hypotheses, and generating new hypotheses. From this work, a view is emerging in which psychological processes represent emergent properties of a widely distributed set of component processes. [ABSTRACT FROM AUTHOR]

Abstract:

Functional magnetic resonance imaging (fMRI) studies of emotion, personality, and social cognition have drawn much attention in recent years, with high-profile studies frequently reporting extremely high (e.g., >.8) correlations between brain activation and personality measures. We show that these correlations are higher than should be expected given the (evidently limited) reliability of both fMRI and personality measures. The high correlations are all the more puzzling because method sections rarely contain much detail about how the correlations were obtained. We surveyed authors of 55 articles that reported findings of this kind to determine a few details on how these correlations were computed. More than half acknowledged using a strategy that computes separate correlations for individual voxels and reports means of only those voxels exceeding chosen thresholds. We show how this nonindependent analysis inflates correlations while yielding reassuring-looking scattergrams. This analysis technique was used to obtain the vast majority of the implausibly high correlations in our survey sample. In addition, we argue that, in some cases, other analysis problems likely created entirely spurious correlations. We outline how the data from these studies could be reanalyzed with unbiased methods to provide accurate estimates of the correlations in question and urge authors to perform such reanalyses. The underlying problems described here appear to be common in fMRI research of many kinds—not just in studies of emotion, personality, and social cognition. Saray Perez, et al. v. Wyeth Laboratories, Inc., et al. (A-16-98) Argued March 2, 1999 — Decided August 9, 1999 O’HERN, J., writing for a majority of the Court. This appeal concerns Norplant, a Food and Drug Administration (FDA)-approved, reversible contraceptive that prevents pregnancy for up to five years. The Norplant contraceptive employs six thin, flexible, closed capsules that contain a synthetic hormone, levonorgestrel.

The capsules are implanted under the skin of a woman’s upper arm during an in-office surgical procedure characterized by the manufacturer as minor. A low, continuous dosage of the hormone diffuses through the capsule walls and into the woman’s bloodstream. Removal occurs during an in-office procedure, similar to the insertion process. Because of the procedural posture of this case, the Supreme Court was required to accept plaintiffs’ version of the facts as true. According to plaintiffs, Wyeth began a massive advertising campaign for Norplant in 1991, which it directed at women rather than at their doctors. Wyeth advertised on television and in women’s magazines, such as Glamour, Mademoiselle, and Cosmopolitan. None of the advertisements warned of any dangers or side effects associated with Norplant, but rather praised its convenience and simplicity. Wyeth also sent a letter to physicians advising that it was about to launch a national advertising program in magazines that the physicians’ patients may read. In 1995, several women began to file lawsuits in various New Jersey counties claiming injuries that resulted from their use of Norplant. Their principal claim was that Wyeth, distributors of Norplant in the United States, failed to warn adequately about the side effects associated with the contraceptive, including weight gain, headaches, dizziness, nausea, diarrhea, acne, vomiting, fatigue, facial hair growth, numbness in the arms and legs, irregular menstruation, hair loss, leg cramps, anxiety and nervousness, vision problems, anemia, mood swings and depression, high blood pressure, and removal complications that resulted in scarring. Class action certification was denied and all New Jersey cases involving Norplant were consolidated in Middlesex County. Following a case management conference, Perez’s counsel sought a determination of whether the “learned intermediary” doctrine applied. The “learned intermediary” doctrine generally relieves a pharmaceutical manufacturer of an independent duty to warn the ultimate user of prescription drugs, so long as it has supplied the physician with information about a drug’s dangerous propensities. Five representative plaintiffs, including Perez, were selected to challenge Wyeth’s motion for summary judgment concerning the “learned intermediary” doctrine. Subsequently, the trial court dismissed those plaintiffs’ complaints, concluding that even when a manufacturer advertises directly to the public, and a woman is influenced by the advertising campaign, a physician nevertheless retains the duty to weigh the benefits and risks associated with a drug before deciding whether the drug is appropriate for the patient. Thus, the trial court held the “learned intermediary” doctrine applicable to plaintiffs’ actions. The trial court was not concerned with the effect that a warning had on the consumer plaintiffs because the Products Liability Act, N.J.S.A. 2A:58C-1 to -11, measures the adequacy of a warning based on the knowledge and characteristics of the health-care provider and not the ultimate consumer. The trial court further found that Perez and the other representative plaintiffs had failed to provide expert testimony to rebut the statutory presumption under the Products Liability Act that the manufacturer’s warning is adequate when it has been approved by the FDA. Finally, the trial court determined that Perez failed to establish that her injuries were proximately caused by any failure on the manufacturer’s part. On appeal, Perez and the other representative plaintiffs challenged the trial court’s failure to hear expert testimony on the adequacy of the warnings and the decision concerning proximate cause, maintaining that “it [had been] specifically agreed that the production of expert testimony would await the outcome of the decision on the issue of the learned intermediary doctrine.” The Appellate Division affirmed the trial court’s grant of summary judgment and its determination that the learned intermediary doctrine applied. The Appellate Division further noted that Section 6(d)(2) of the Restatement (Third) of Torts: Products Liability (1997) (Restatement) may require a warning when the physician or health-care provider has a “diminished role as an evaluator or decision maker,” in which case the manufacturer would have a duty to warn patients directly. However, the court agreed with the trial court that if the warning was legislatively deemed adequate and has been given to the proper party, then no warning defect existed under the Products Liability Law. The Supreme Court granted Perez’s petition for review. HELD: The “learned intermediary” doctrine does not apply to the direct marketing of drugs to consumers; prescription drug manufacturers that market their products directly to consumers should be subject to claims by consumers if their advertising fails to provide an adequate warning of the product’s dangerous propensities.

There are various trends currently impacting the healthcare industry. These trends are important to consider, as they have the potential to influence many changes for healthcare professionals. Trends can bring about changes in workflow, operations, and approaches to patient care. Some of these current trends include wearable technology and artificial intelligence.

Wearable devices have become a novelty in recent years as consumers are becoming more health-conscious and taking control of their health. These devices can be categorized under the general term – wearable technology – which is used when referring to a group of devices such as fitness trackers and smartwatches (GCF Global, n.d.). In essence, wearables can track number of steps, heart rate, and even sleep patterns to give consumers a better idea of their daily activities (GCF Global, n.d.). Wearable devices collect a tremendous amount of data which can be utilized to improve patient care (VanDenBoom, 2022). The challenge of how to use and manage that data in meaningful ways will fall on HIM professionals. According to the Centers for Disease Control and Prevention (2021), implementing a science-based approach to this data can improve preventive medicine by empowering individuals to take control of their health. Therefore, HIM professionals will need to seek and implement evidence-based approaches to best utilize data from wearable devices. Since data will be transmitted from many points, they will also need to determine how to best streamline the integration of this data. Furthermore, HIM professionals will need to break-down and present the data to healthcare providers in meaningful ways so that they may proceed to incorporate it into their patient’s care. In addition to wearable devices, Artificial Intelligence (AI) is another healthcare trend that can impact HIM professionals. AI can be used to create “enhanced versions of medical devices… such as a blue-tooth operated heart monitor” (University of Central Florida, 2022). AI devices can provide doctors more timely, accurate information. Such real-time information improves patient care by allowing healthcare providers to manage crises remotely when needed (University of Central Florida, 2022). This trend will impact HIM professionals because they will need to manage the data from AI devices and ensure that it is received by the patient’s care team. AI devices will be an additional data-point that HIM professionals will need to streamline. As more places begin to adopt AI technology, more data will be generated and need to be managed by HIM professionals.

Given the trends discussed, HIM professionals will need certain skills to respond to the changes. HIM professionals will need to be tech-savy to incorporate the use of new technology such as AI and wearable devices into healthcare. They’ll also need to become data experts, as these trends will only increase the immense amount of healthcare data that already exists. The challenges that the use of AI and wearables will present to HIM professionals will require them to be analytical and problem-solvers.

The United States Healthcare System has moved into the technology age in the 21st century. Telemedicine and telehealth solutions quickly took over as a new trend at the height of the recent COVID-19 pandemic as one method for rendering care to the population. Another trend that has emerged and will be a bonus to the health informatics and information management (HIMS) for professionals is the Blockchain. Ecosystem.

Blockchain is defined according to Cerchione et al., (2022 ) as “a mechanism to manage access to EHRs stored on the cloud, by using a blockchain it can increase interoperability while maintaining privacy and security of data” Simply put a Blockchain is a digital ledger of data that continuously adds information in chronological order and transactions are maintained by a network of computers in a way that makes it difficult to hack or alter( Cerchione et al., 2022 ).

The blockchain trend will have a great impact on HIT/HIM professionals and patients going forward. For the HIM professional it will be a seamless exchange of health information across health information systems and for the patients; ownership and privacy of data are important issues that blockchain could solve. Furthermore, it is currently debated whether the health care provider or the patient owns health care data relating to a patient (although patients have a definite right to access their personal health information (Gunther et al., 2019). I believe this is an important skill that is needed for the HIM professional to know the infrastructure of Blockchain technology. The HIM professional must have expertise skills in decentralization, security, privacy, breach resistance, and speed of certain features of the internet’s infrastructure (Peng and Clauson, 2019).
